Claims (1)
- 【特許請求の範囲】 1.管腔と長手方向軸線と長さL1の融除表面領域とを有する一次アンテナと、 一次アンテナが組織を通して導入されるときに一次アンテナ内に位置決めされて いる二次アンテナと、を含み、二次アンテナ遠位端部が、選択組織部位で一次ア ンテナ管腔から長手方向軸線に対して横方向に展延される多重アンテナ装置と 二次アンテナ遠位端部の外側に、かつ、二次アンテナ遠位端部に沿って一次 アンテナから距離L2のところに、少なくとも部分的に位置決めされているセン サと、を含み、L2は1/3L1に少なくとも等しく、 一次アンテナに結合されたエネルギ源をさらに含む、 ことを特徴とする融着処置装置。 2.L2は1/2L1に少なくとも等しい、 ことを特徴とする請求の範囲第1項に記載の装置。 3.L2はL1に少なくとも等しい、 ことを特徴とする請求の範囲第1項に記載の装置。 4.センサに結合されたセンサ監視装置を更に含む、 ことを特徴とする請求の範囲第1項に記載の装置。 5.センサ監視装置は温度監視装置である、 ことを特徴とする請求の範囲第4項に記載の装置。 6.エネルギ源及びセンサに結合され、センサからの検出特性に応答して一次ア ンテナに対しエネルギ源からの融除エネルギの供給出力を付与するフィードバッ ク制御装置を更に含む、 ことを特徴とする請求の範囲第1項に記載の装置。 7.二次アンテナは不能動電極であり、一次アンテナは能動電極である、 ことを特徴とする請求の範囲第1項に記載の装置。 8.二次アンテナ遠位端部は、一次アンテナよりも構造的に剛性が小さい、 ことを特徴とする請求の範囲第1項に記載の装置。 9.装置がパイポーラ動作とモノポーラ動作との間で切り替え可能である、 ことを特徴とする請求の範囲第1項に記載の装置。 10.一次アンテナが、注入媒体を受け入れるため、中空であり、注入媒体源に結 合されている、 ことを特徴とする請求の範囲第1項に記載の装置。 11.一次アンテナが、二次アンテナの融除面の長さの少なくとも20%の長さの 融除面を有する、 ことを特徴とする請求の範囲第1項に記載の装置。 12.一次アンテナが、二次アンテナの融除面の長さの少なくとも1/3の長さの 融除面を有する、 ことを特徴とする請求の範囲第1項に記載の装置。 13.一次アンテナが、二次アンテナの融除面の長さの少なくとも1/2の長さの 融除面を有する、 ことを特徴とする請求の範囲1項に記載の装置。 14.二本の二次電極が設けられて一次アンテナから横方向に展延し、一次アンテ ナ及び二次アンテナのそれぞれが、融除面の間に融除領域を生成するように融除 面を有する、 ことを特徴とする請求の範囲第1項に記載の装置。 15.三本の二次電極が設けられて一次アンテナから横方向に展延し、一次アンテ ナ及び二次アンテナのそれぞれが、融除面の間に融除領域を生成するように融除 面を有する、 ことを特徴とする請求の範囲第1項に記載の装置。 16.一次アンテナの外側の少なくとも一部に該部分を取り囲むようにして位置決 めされた絶縁スリーブを更に含む、 ことを特徴とする請求の範囲第1項に記載の装置。 17.絶縁スリーブが、一次アンテナの外側に沿って調節可能に移動可能である、 ことを特徴とする請求の範囲第16項に記載の装置。 18.二次アンテナの外側の少なくとも一部に該部分を取り囲むようにして位置決 めされた絶縁スリーブ、を更に含む、 ことを特徴とする請求の範囲第1項に記載の装置。 19.絶縁スリーブが、一次アンテナの外側に沿って調節可能に移動可能である、 ことを特徴とする請求の範囲第13項に記載の装置。 20.更に、接地パッド電極を備え、 一次及び二次アンテナがモノポーラモードで動作する、 ことを特徴とする請求の範囲第1項に記載の装置。 21.一次アンテナ及び二次アンテナが、高周波アンテナである、 ことを特徴とする請求の範囲第1項に記載の装置。 22.一次アンテナ及び二次アンテナが、マイクロ波アンテナである、 ことを特徴とする請求項の範囲第1項に記載の装置。 23.装置がパイポーラ動作とモノポーラ動作との間で切り替え可能である、 ことを特徴とする請求の範囲第1項に記載の装置。 24.一次アンテナが、注入媒体を受け入れるため、中空であり、注入媒体源に結 合されている、 ことを特徴とする請求の範囲第1項に記載の装置。 25.二次アンテナ遠位端部は、一次アンテナよりも構造的に剛性が小さい、 ことを特徴とする請求の範囲第1項に記載の装置。 26.選択組織対象物内に融除領域を形成する方法であって、 電磁エネルギ供給面を有する一次アンテナと、遠位端部を有する二次アンテ ナと、二次アンテナ遠位端部の外側に部分的に位置決めされたセンサと、一次ア ンテナに結合されたエネルギ源と、を有する融除装置を準備し、 二次アンテナを一次アンテナ管腔内に位置決めした状態で、一次アンテナを 選択組織対象物内に挿入し、 二次アンテナの遠位端部を、一次アンテナ管腔から一次アンテナの長手方向 軸線に対して横方向に選択組織対象物内に前進させ、 一次アンテナ融除面から選択組織対象物に電磁エネルギを供給し、 センサで温度を測定し、 選択組織対象物内に融除領域を生成する、 ことを特徴とする方法。 27.二次アンテナは不活性アンテナである、 ことを特徴とする請求の範囲第26項に記載の方法。 28.二次アンテナ遠位端部は、電磁エネルギ供給面である、 ことを特徴とする請求の範囲第26項に記載の方法。 29.二次エンテナ遠位端部を一次アンテナ遠位端部から前進させる、 ことを特徴とする請求の範囲第26項に記載の方法。 30.融除装置が、モノポーラモードで動作する、 ことを特徴とする請求の範囲第26項に記載の方法。 31.融除装置が、バイポーラモードで動作する、 ことを特徴とする請求の範囲第26項に記載の方法。 32.融除装置がパイポーラ動作とモノポーラ動作との間で切り替え可能である、 ことを特徴とする請求の範囲第26項に記載の方法。 33.センサが温度を測定する、 ことを特徴とする請求の範囲第26項に記載の方法。 34.電磁エネルギ源と、 その長手方向軸線に沿って延びる中空の管腔及び遠位端部を有するトロカー ルと、 三本以上のアンテナを有する多重アンテナ融除装置と、を含み、アンテナは 、トロカールを組織に挿入するときに、トロカール管腔内に位置決めされており 、アンテナは、選択組織対象物でトロカール管腔から長手方向軸線に対して横方 向に展延可能であり、展延されたアンテナはそれぞれ、10乃至50ワットの電 磁エネルギを電磁エネルギ供給源から多重アンテナ融除装置に供給したときに、 展延されたアンテナを阻害することなく展延されたアンテナ間に体積融除部を生 成するのに十分な大きさの電磁エネルギ供給面を有し、 多重アンテナ融除装置を電磁エネルギ源に結合さするなくとも一本のケーブ ルを、更に含む、 ことを特徴とする融除処置装置。 35.展延されたアンテナはそれぞれ、10乃至30ワットの電磁エネルギを電磁 エネルギ供給源から多重アンテナ融除装置に供給したときに、展延されたアンテ ナを阻害することなく展延されたアンテナ間に体積融除部を生成するのに十分な 大きさの電磁エネルギ供給面を有する、 ことを特徴とする請求の範囲第34項に記載の装置。 36.展延されたアンテナはそれぞれ、10乃至15ワットの電磁エネルギを電磁 エネルギ供給源から多重アンテナ融除装置に供給したときに、展延されたアンテ ナを阻害することなく展延されたアンテナ間で体積融除部を形成するのに十分な 大きさの電磁エネルギ供給面を有する、 ことを特徴とする請求の範囲第34項に記載の装置。 37.アンテナの各々は電磁エネルギ源に結合されている、 ことを特徴とする請求の範囲第34項に記載の装置。 38.トロカールは、13ゲージ以下の外形を有する、 ことを特徴とする請求の範囲第34項に記載の装置。 39.トロカールは、14ゲージ以下の外形を有する、 ことを特徴とする請求の範囲第34項に記載の装置。 40.トロカールは、15ゲージ以下の外形を有する、 ことを特徴とする請求の範囲第34項に記載の装置。 41.四本のアンテナが選択組織対象物でトロカールから展延される、 ことを特徴とする請求の範囲第34項に記載の装置。 42.五本のアンテナが選択組織対象物でトロカールから展延される、 ことを特徴とする請求の範囲第34項に記載の装置。 43.六本のアンテナが選択組織対象物でトロカールから展延される、 ことを特徴とする請求の範囲第34項に記載の装置。 44.アンテナのうち少なくとも二本がトロカール遠位端部から展延される、 ことを特徴とする請求の範囲第34項に記載の装置。 45.トロカールが、該トロカールの本体に形成された一つ以上の側方ポートを含 む、 ことを特徴とする請求の範囲第34項に記載の装置。 46.少なくとも一本のアンテナがトロカール遠位端部から選択組織対象物内に展 延され、少なくとも一本のアンテナが側方ポートから選択組織対象物内に展延さ れる、 ことを特徴とする請求の範囲第34項に記載の装置。 47.アンテナは高周波電極であり、電磁エネルギ源は高周波エネルギ源である、 ことを特徴とする請求の範囲第34項に記載の装置。 48.多重アンテナ装置が、モノポーラモードで動作する、 ことを特徴とする請求の範囲第34項に記載の装置。 49.多重アンテナ装置が、バイポーラモードで動作する、 ことを特徴とする請求の範囲第34項に記載の方法。 50.装置がパイポーラ動作とモノポーラ動作との間で切り替え可能である、 ことを特徴とする請求の範囲第34項に記載の装置。 51.各アンテナの遠位端部の少なくとも一部分はトロカールよりも構造的に剛性 が小さい、 ことを特徴とする請求の範囲第34項に記載の装置。 52.アンテナの外側に少なくとも部分的に位置決めされたセンサを更に含む、 ことを特徴とする請求の範囲第34項に記載の装置。 53.トロカールの外側の少なくとも一部分を取り囲むように位置決めされた絶縁 層を更に含む、 ことを特徴とする請求の範囲第34項に記載の装置。 54.絶縁層の遠位端部がトロカールの遠位端部から移動され、トロカールの遠位 端部に電磁エネルギ供給面を形成する、 ことを特徴とする請求の範囲第53項に記載の装置。 55.トロカール管腔は、注入媒体を受け入れるため注入媒体源に結合されている 、 ことを特徴とする請求の範囲第34項に記載の装置。 56.電磁エネルギ源と、 その長手方向軸線に沿って延びる中空の管腔及び遠位端部を有するトロカー ルと、 複数のアンテナを有する多重アンテナ融除装置と、を含み、アンテナは、ト ロカールを組織に挿入するときに、トロカール管腔内に位置決めされており、複 数のアンテナは、選択組織対象物でトロカール管腔から長手方向軸線に対して横 方向に展延可能であり、複数のアンテナは、10乃至50ワットの電磁エネルギ を電磁エネルギ供給源から多重アンテナ融除装置に供給したときに、展 延されたアンテナを阻害することなく選択組織部位でアンテナ間に体積融除部を 生成するのに十分な大きさの電磁エネルギ供給面を有し、 多重アンテナ融除装置を電磁エネルギ源に結合する少なくとも一本のケーブ ルを、更に含む、 ことを特徴とする融除処置装置。 57.トロカールは、15ゲージ以下の外形を有する、 ことを特徴とする請求の範囲第56項に記載の装置。 58.多重アンテナ融除装置は、高周波装置である、 ことを特徴とする請求の範囲第56項に記載の装置。 59.多重アンテナ融除装置は、モノポーラモードで作動する、 ことを特徴とする請求の範囲第58項に記載の装置。 60.多重アンテナ融除装置は、バイポーラモードで作動する、 ことを特徴とする請求の範囲第58項に記載の装置。 61.多重アンテナ融除装置は、パイポーラ動作とモノポーラ動作との間で切り替 え可能である、 ことを特徴とする請求の範囲第58項に記載の装置。 62.選択組織対象物内に融除領域を形成する方法であって、 トロカール管腔を有するトロカールと、前記管腔から展延可能な複数のアン テナと、前記複数のアンテナに結合された電磁エネルギ源と、を含む多重アンテ ナ融除装置を準備し、 複数のアンテナをトロカール管腔内に位置決めした状態で、トロカールを選 択組織対象物内に挿入し、 複数のアンテナを、トロカール管腔からトロカールの長手方向軸線に対して 横方向に選択組織対象物内に前進させ、 複数のアンテナの1つのアンテナを阻害することなく10乃至50ワットの の電磁エネルギを電磁エネルギ供給源から多重アンテナ融除装置に供給し、 選択組織対象物内に融除領域を生成する、 ことを特徴とする方法。 63.融除領域はスフェロイド形である、 ことを特徴とする請求の範囲第62項に記載の方法。 64.融除領域は球形である、 ことを特徴とする請求の範囲第62項に記載の方法。 65.融除領域は部分球形である、 ことを特徴とする請求の範囲第62項に記載の方法。 66.融除領域を複数の隣接して展延されたアンテナ間に連続的に生成する、 ことを特徴とする請求の範囲第 67.トロカールは、15ゲージ以下の外形を有する、 ことを特徴とする請求の範囲第67項に記載の方法。 68.複数のアンテナの各々は、10乃至50ワットの電磁エネルギを電磁エネル ギ供給源から受け取るのに十分な大きさの電磁エネルギ供給面を有する、 ことを特徴とする請求の範囲第62項に記載の方法。
