Claims (1)
- 【特許請求の範囲】 1.組織を縫合するための自動縫合器具であって、 (a) 近位端と、縫合部位の近傍に位置づけされる遠位端を有するハウジングと 、 (b) 前記ハウジングに穿設された少くとも1つの開口に連通した、縫合糸を挿 通するための通路を前記組織を貫通して形成するために該組織を突き通すための 、該ハウジングの前記遠位端に設けられた突き刺し手段と、 (c) 前記縫合糸を前記通路の開口の近傍で該開口に整合させる部位で前記ハウ ジングに設けられた、該縫合糸を該通路に挿通するための糸挿通手段と、 (d) 前記縫合糸を緊張させるために前記ハウジングに設けられており、前記通 路の外部で該縫合糸に連結された糸緊張手段と、 (e) 前記組織に近接したところに完成した縫い目を形成するように前記通路の 開口のところで前記縫合糸の、該組織への進入部分と該組織からの退出部分を互 いに溶接接合するために該通路の開口の近傍で前記ハウジングに設けられた溶接 手段と、 から成る自動縫合器具。 2.前記突き刺し手段は、中空孔又は半中空孔を有する針から成り、該針は 、前記組織を抜差し自在に突き通すことによって前記通路を形成するようになさ れていることを特徴とする請求の範囲第1項に記載の自動縫合器具。 3.前記突き刺し手段は、回動自在に取り付けられた2本の対向した引込め 自在の湾曲針から成り、該2本の湾曲針は、作動されると、前記組織を湾曲した 態様に突き通して該組織内で又は該組織の周りで相接し、それによって該組織を 貫通して又は該組織の周りに前記通路を形成するようになされていることを特徴 とする請求の範囲第2項に記載の自動縫合器具。 4.前記溶接手段は、超音波溶接手段であることを特徴とする請求の範囲第 1項に記載の自動縫合器具。 5.信頼性及び反復精度の高い溶接部を形成するために前記縫合糸の端部に 導入すべきエネルギーの大きさ及びエネルギーを導入する時間の長さを制御する マイクロプロセッサ制御器を備えていることを特徴とする請求の範囲第1項に記 載の自動縫合器具。 6.前記溶接領域に第1開口を有し、該第1開口から離れたところに第2開 口を有するシャフトが設けられており、該第2開口は、前記溶接部を乾燥させる ために該シャフトを通して供給されるガスを受け入れるように配置され構成され ていることを特徴とする請求の範囲第1項に記載の自動縫合器具。 7.前記縫合糸を切断するための切断手段が設けられており、該切断手段と 前記溶接手段とは、前記完成された縫い目にほつれた自由端を残さないように互 いに整合されていることを特徴とする請求の範囲第1項に記載の自動縫合器具。 8.縫合糸の連続供給源が設けられており、該供給源は、1つの縫い目が完 成した後直ちに前記糸挿通手段へ縫合糸を供給するように配置され構成されてい ることを特徴とする請求の範囲第1項に記載の自動縫合器具。 9.該器具は、人間及び動物のアクセスしにくい内部部位を探索するのに適 した細長いシャフトを構成しており、前記前記突き刺し手段は、該細長いシャフ トの遠位先端部に連結されており、前記糸緊張手段及び溶接手段は、該シャフト 内を通して該先端部にまで延長していることを特徴とする請求の範囲第1項に記 載の自動縫合器具。 10.前記細長いシャフトは、関節連結されていることを特徴とする請求の範 囲第9項に記載の自動縫合器具。 11.該器具は、前記組織に対して位置づけし直す必要なしに完成した縫い目 を形成することを特徴とする請求の範囲第1項に記載の自動縫合器具。 12.組織を縫合するための自動縫合器具であって、 (a) ハウジングと、 (b) 該ハウジング内に固定されて該ハウジングから延長しており、縫合すべき 組織又は結紮すべき脈管又は管路に接触する先端部を有する管状部材と、 (c) 前記先端部を損傷しないように保護するために該先端部を摺動自在に被う 筒状鞘と、 (d) 前記組織の縫合すべき開口の両側に、又は、前記脈管又は管路に跨がって 位置づけされるように互いに対向した2本の、全長に亙ってU字形チャンネルを 有する湾曲針と、 (e) 前記管状部材内に固定され、前記各針の近位端に連結されたリンク機構と 、 (f) 前記リンク機構に連結されており、前記2本の針を前記組織内へ、又は、 前記脈管又は管路の周りに押し込み、該2本の針が湾曲状に該組織を突き通すか 、又は、該脈管又は管路の周りを取り囲み、該2本の針が相接してチャンネルを 設定するように該リンク機構を作動せるための駆動手段と、 (g) 前記ハウジング内に固定されており、縫合糸を前記チャンネルへ挿通する ために縫合糸を前記管状部材を通して前記先端部へ供給するための縫合糸供給源 と、 (h) 前記ハウジング内に固定されており、縫合糸を前記チャンネルを通して押 進め縫合糸の完全なループを形成するように、縫合糸に接触して縫合糸を該チャ ンネルへ案内するように配置され構成された糸駆動手段と、 (i) 前記先端部内に取り付けられており、一方の面と他方の面を有し、一方の 面を前記組織の上に載せ、他方の面上で縫合糸をオーバーラップさせるように、 前記ループ内へ進退自在に位置づけすることができるアンビルと、 (j) 前記管状部材に移動自在に取り付けられて該管状部材内を通って延長して おり、縫合糸を組織内に残して前記針を組織から抜き取ることができるように、 縫合糸を挟んで前記アンビルに圧接し該縫合糸を該アンビルに固定するための第 1クランプ部材と、 (k) 前記ハウジングに固定されており、前記縫合糸を握持する機械的手段と、 該機械的手段により該縫合糸を引張って前記ループを緊張させ、それによって前 記組織の開口を閉鎖するために該機械的手段を作動させる作動手段と、 (l) 前記機械的手段に連結されており、前記縫合糸の張力を測定し、該張力が 所定値に等しくなるまで前記作動手段を付勢して縫合糸をぴんと引張るように構 成された張力制御手段と、 (m) 前記管状部材に移動自在に取り付けられて該管状部材内を通って延長して おり、作動されたとき前記縫合糸のループの両端のオーバーラップ部分をクラン プして固定するための第2クランプ部材と、 (n) 前記先端部内に取り付けられており、前記縫合糸の前記オーバーラップ部 分を前記第1クランプ部材と第2クランプ部材の間に挟みつけるための溶接用ホ ーンと、 (o) 前記第2クランプ部材の進路に干渉するように該進路に近接して固定され ており、前記溶接用ホーンによって接触され該溶接用ホーンとの間で前記縫合糸 を縫合糸供給源から剪断するように配置されている刃部材と、 (p) 前記縫合糸の前記オーバーラップ部分に近接して配置されており、該オー バーラップ部分を乾燥させるための乾燥用ガスを前記管状部材を通して受取り、 該オーバーラップ部分へ供給するための乾燥用ガス供給源及び調整手段と、 (q) 超音波エネルギーによって前記縫合糸をそのオーバーラップ部分で溶接し て前記組織に近接したところに閉ループ縫い目を形成するように、前記溶接用ホ ーンに接続された超音波発生器とその超音波エネルギーを該溶接用ホーンを通し て該縫合糸のオーバーラップ部分へ伝達するための手段と、 (r) 信頼性の高い溶接部が形成されるように前記縫合糸に投入される超音波エ ネルギーを制御するように、かつ、該縫合糸をその溶接部が固化するまで保持す るように前記第1及び第2クランプ部材を制御するためのコンピュータ及びプロ グラム手段と、 (s) 前記アンビルを後退させて前記閉ループ縫い目を該縫合器具から解放する ために該アンビルに連結された解放手段と、 から成る自動縫合器具。 13.組織の開口を縫合するための縫合方法であって、 (a) 縫合糸を挿通するための通路を前記組織を貫通して形成するために前記開 口の両側から同時に該組織を突き通す工程と、 (b) 前記縫合糸を前記通路を通して挿通する工程と、 (c) 前記縫合糸を緊張させる工程と、 (d) 前記縫合糸を切断する工程と、 (e) 前記組織に近接したところで前記縫合糸の両端部分を溶接接合して完成し た縫い目を形成する溶接工程とから成り、該溶接工程において、信頼性及び反復 精度の高い溶接部を形成するために該縫合糸に導入される前記エネルギーを制御 することを特徴とする縫合方法。 14.脈管のための自動結紮器具であって、 (a) 前記脈管の結紮糸を巻きつけるべき部位に位置づけすべき遠位端を有する ハウジングと、 (b) 該ハウジングの前記遠位端に設けられており、前記結紮糸を挿通するため の、該ハウジングに穿設された少くとも1つの開口に連通した通路を有し、前記 脈管を取り巻くための手段と、 (c) 前記結紮糸を前記通路の開口の近傍で該開口に整合させる部位で前記ハウ ジングに設けられた、該結紮糸を該通路に挿通するための糸挿通手段と、 (d) 前記結紮糸を前記通路の開口の近傍で該開口に整合させる部位で前記ハウ ジングに設けられた、該結紮糸を緊張させるための糸緊張手段と、 (e) 前記組織に近接したところに完成した縫い目を形成するように前記通路の 開口のところで前記結紮糸の、該組織への進入部分と該組織からの退出部分を互 いに溶接接合するために該通路の開口の近傍で前記ハウジングに設けられた溶接 手段と、 から成る自動結紮器具。 15.脈管を取り巻くための前記手段は、中空孔又は半中空孔を有する針から 成り、該針は、前記脈管を取り巻いたとき前記通路を形成するようになされてい ることを特徴とする請求の範囲第14項に記載の自動結紮器具。 16.脈管を取り巻くための前記手段は、回動自在に取り付けられた2本の対 向した引込め自在の湾曲針から成り、該2本の湾曲針は、作動されると、前記脈 管を湾曲した態様で取り巻いて連続した通路を形成するようになされていること を特徴とする請求の範囲第14項に記載の自動結紮器具。 17.前記溶接手段は、超音波溶接手段であることを特徴とする請求の範囲第 14項に記載の自動結紮器具。 18.信頼性及び反復精度の高い溶接部を形成するために前記結紮糸の端部に 導入すべきエネルギーの大きさ及びエネルギーを導入する時間の長さを制御する マイクロプロセッサ制御器を備えていることを特徴とする請求の範囲第14項に 記載の自動結紮器具。 19.前記溶接領域に第1開口を有し、該第1開口から離れたところに第2開 口を有するシャフトが設けられており、該第2開口は、前記溶接部を乾燥させる ために該シャフトを通して供給されるガスを受け入れるように配置され構成され ていることを特徴とする請求の範囲第14項に記載の自動結紮器具。 20.前記結紮糸を切断するための切断手段が前記溶接手段に連結されており 、該切断手段と前記溶接手段とは、前記完成された縫い目にほつれた自由端を残 さないように互いに整合されていることを特徴とする請求の範囲第14項に記載 の自動結紮器具。 21.結紮糸の連続供給源が設けられており、該供給源は、1つの縫い目が完 成した後直ちに前記糸挿通手段へ結紮糸を供給するように配置され構成されてい ることを特徴とする請求の範囲第14項に記載の自動結紮器具。 22.該器具は、人間及び動物のアクセスしにくい内部部位を探索するのに適 した細長いシャフトを構成されていることを特徴とする請求の範囲第14項に記 載の自動結紮器具。 23.前記細長いシャフトは、関節連結されていることを特徴とする請求の範 囲第22項に記載の自動結紮器具。 24.該器具は、位置づけし直す必要なしに完成した結紮を形成することを特 徴とする請求の範囲第14項に記載の自動結紮器具。 25.脈管を結紮するための結紮方法であって、 (a) 結紮糸を挿通するための通路を前記脈管の周りに形成するために前記脈管 の両側から同時に該脈管を取り巻く工程と、 (b) 前記結紮糸を前記通路を通して挿通する工程と、 (c) 前記結紮糸を緊張させる工程と、 (d) 前記結紮糸を切断する工程と、 (e) 前記脈管の全周を取り巻く前記結紮糸の両端部分を該脈管に近接したとこ ろで溶接接合して完成した結紮を形成する溶接工程とから成り、該溶接工程にお いて、信頼性及び反復精度の高い溶接部を形成するために該結紮糸に導入される 前記エネルギーを制御することを特徴とする結紮方法。
