Claims (1)
- 【特許請求の範囲】 1.心臓内の心筋層の所定点に隣接した位置にカテーテルを配置するステップ と、 前記心筋層に必ずしも接触せずに前記所定点に隣接の約1mm乃至3mm内で 前記心筋層から局部制限された心臓信号のみを検出するステップと、 前記心臓内の電気生理学的活性化の正確かつ個別のマッピングが達成されるよ うに、前記カテーテルを配置するステップと前記心筋層内の複数の異なる所定点 から前記局部制限された心臓信号のみを検出するステップを繰り返すことを特徴 とする心臓の心筋層内の電気生理学的活性化のマッピング方法。 2.隣接の心筋層からの前記局部制限された心臓信号のみを検出する前記ステ ップは、前記カテーテルにかつ前記心筋層内の前記所定点近くに配置された少な くとも一対の直交電極の前記心臓信号を検出するステップを有することを特徴と する請求項1記載の心臓の心筋層内の電気生理学的活性化のマッピング方法。 3.前記検出するステップは、前記隣接の心筋層に接触せずに行われることを 特徴とする請求項1記載の心臓の心筋層内の電気生理学的活性化のマッピング方 法。 4.前記検出するステップは、前記隣接の心筋層に接触せずに行われることを 特徴とする請求項2記載の心臓の心筋層内の電気生理学的活性化のマッピング方 法。 5.前記直交電極の前記心臓信号を検出する前記ステップは、前記直交電極の 間で差信号を発生するために前記直交電極の各々で受信された心臓信号の差を取 るステップを有し、この差信号は、前記局部制限された心臓の活動を示すことを 特徴とする請求項2記載の心臓の心筋層内の電気生理学的活性化のマッピング方 法。 6.刺激されるまさにその心筋層を検出することを含む、前記隣接の心筋層か らの局部制限された心臓信号のみを検出する前記ステップを行いながら、前記隣 接の心筋層を同時に刺激するステップを更に有することを特徴とする請求項1記 載の心臓の心筋層内の電気生理学的活性化のマッピング方法。 7.前記刺激するステップは、前記カテーテルの遠隔端に設けられた刺激チッ プとこの刺激チップから近くのリング電極とを介して行われ、前記同時に検出す るステップは、少なくとも一対の直交検出電極によって前記刺激チップの近くの 前記心筋層内の点で行われることを特徴とする請求項6記載の心臓の心筋層内の 電気生理学的活性化のマッピング方法。 8.前記検出するステップは、前記刺激チップから約1mm乃至5mm離れた 前記心筋層内の前記点において前記直交電極により行われることを特徴とする請 求項7記載の心臓の心筋層内の電気生理学的活性化のマッピング方法。 9.前記検出するステップは、前記カテーテルの長さに沿って長手方向に隣接 する前記心筋層内の複数の点で行われることを特徴とする請求項1記載の心臓の 心筋層内の電気生理学的活性化のマッピング方法。 10.前記検出するステップは、前記隣接心筋層に接触せずに行われることを 特徴とする請求項9記載の心臓の心筋層内の電気生理学的活性化のマッピング方 法。 11.前記カテーテルに隣接して長手方向に配置された前記心筋層内の前記複 数の点に沿って検出するステップは、前記カテーテルに沿って前記心筋層内の前 記対応点の各々における局部化制限された信号を示す複数の差信号を発生するた めに、対応の複数の直交電極により検出するステップと、前記複数の直交電極の 各々により受信された前記心臓信号の差を取るステップとを有することを特徴と する請求項9記載の心臓の心筋層内の電気生理学的活性化のマッピング方法。 12.前記他の所定点において心筋層の切除のために他の所定点に前記カテー テルを介してエネルギを同時に加えるステップを更に有することを特徴とする請 求項1記載の心臓の心筋層内の電気生理学的活性化のマッピング方法。 13.エネルギを加える前記ステップは、前記カテーテルの遠隔端における切 除チップを介して無線周波切除エネルギを加えることにより行われることを特徴 とする請求項12記載のマッピング方法。 14.前記心筋層内の前記所定点での前記検出するステップは、切除される前 記心筋層内の前記同一の所定点において切除エネルギを加える前記ステップと同 時に行われることを特徴とする請求項12記載の心臓の心筋層内の電気生理学的 活性化のマッピング方法。 15.人間の心臓内に配置されて遠隔端を有するカテーテル本体、 前記カテーテル本体に配置されて前記心筋層内の所定点から約1mm乃至3m m内の前記心筋層内の局部制限化領域からのみ心電信号を受信するための少なく とも一対の直交電極、及び、 前記所定点から約1mm乃至3mm内にのみ局部制限された心臓活動を示す電 気グラフィック信号を発生するために前記少なくとも一対の直交電極により受信 された心臓信号どうしの差をとるための回路手段を有し、 カテーテルが前記心筋層内において特に空間的に電気生理学的活動をマッピン グするために設けられていることを特徴とする人間の心臓内の心筋層の電気生理 学的活動をマッピングするためのカテーテル。 16.前記カテーテルの前記遠隔端に配置されたカテーテルチップ電極を有し 、このカテーテルチップ電極は、前記心臓のペーシングを行うための刺激パルス を送るためのものであり、前記直交電極と前記回路手段は、前記所定点から約1 mm内でのみ局部制限された心臓活動を示す前記電気グラフィック信号を発生す るために前記局部制限された心臓活動のみを同時に検出することを特徴とする請 求項15記載の人間の心臓内の心筋層の電気生理学的活動をマッピングするため のカテーテル。 17.前記直交電極は、前記刺激するカテーテルチップ電極に隣接して配置さ れて、刺激される前記心筋層内の前記まさしくその同一の所定点において電気生 理学的活動を同時に検出することを特徴とする請求項16記載の人間の心臓内の 心筋層の電気生理学的活動をマッピングするためのカテーテル。 18.前記直交電極は、前記刺激するカテーテルチップ電極から1mm乃至5 mmの範囲内に配置されていることを特徴とする請求項17記載の人間の心臓内 の心筋層の電気生理学的活動をマッピングするためのカテーテル。 19.前記カテーテルに配置されて前記切除電極と接触して心筋層を根絶する ため前記切除電極を介してエネルギを加えるように構成された切除電極を更に有 することを特徴とする請求項15記載の人間の心臓内の心筋層の電気生理学的活 動をマッピングするためのカテーテル。 20.前記直交電極は、前記切除電極に隣接配置されて前記同一の心筋層の電 気生理学的活性化をこの心筋層の根絶中に同時に検出することを特徴とする請求 項19記載の人間の心臓内の心筋層の電気生理学的活動をマッピングするための カテーテル。 21.前記直交電極の内の最も接近したものは、前記切除電極から1mm乃至 5mmの範囲内に配置されていることを特徴とする請求項20記載の人間の心臓 内の心筋層の電気生理学的活動をマッピングするためのカテーテル。 22.人間の心臓内に配置されるカテーテル本体、 前記カテーテル本体に配置されて、前記心筋層内の所定点に1mm乃至3mm 内で隣接して心筋層においてのみ局部制限された心臓活動を検出するための少な くとも一対の直交電極、 前記少なくとも一対の直交電極に接続されて、前記心筋層内における前記所定 点から1mm乃至3mm内においてのみ局部制限された心臓の活動のみを示す電 気グラフィック信号を発生するために前記少なくとも一対の直交電極により検出 される心臓信号どうしの差をとるための手段、及び 前記カテーテルの前記遠隔端に配置されて、前記心臓のペーシングのための刺 激パルスを供給するためのカテーテルチップ電極を有し、 前記直交電極と前記心臓信号どうしの差をとる手段とは、前記カテーテルチッ プ電極への前記刺激パルスの供給中に前記局部制限された心臓活動を同時に検出 するように構成され、 前記カテーテルは、前記所定点に直接隣接した心筋層からのみ局部制限された 心臓信号に応答して前記心臓へ剌激を供給するようにしてあることを特徴とする 人間の心臓のペーシングのために心筋層を刺激するカテーテル。 23.前記直交電極のうちの最も接近したものは、前記カテーテルチップ電極 から1mm乃至5mm内に配置されてることを特徴とする請求項22記載の人間 の心臓のペーシングのために心筋層を刺激するカテーテル。 24.人間の心臓内に配置されるカテーテル本体、 前記カテーテル本体に配置されて、前記心筋層内の所定点に1mm乃至3mm 内で隣接して心筋層においてのみ局部制限された心臓活動を検出するための少な くとも一対の直交電極、 前記少なくとも一対の直交電極に接続されて、前記心筋層内における前記所定 点から1mm乃至3mm内においてのみ局部制限された心臓の活動のみを示す電 気グラフィック信号を発生するために前記少なくとも一対の直交電極により検出 される心臓信号どうしの差をとるための手段、及び 前記カテーテルに配置された切除電極を有し、この切除電極は、この切除電極 と接触した心筋層を根絶するために前記切除電極を介してエネルギを加えるよう にされ、 前記直交電極は、前記切除電極に隣接配置され、前記直交電極と前記心臓信号 どうしの差を取る手段とは、前記切除電極と接触した前記心筋層が根絶されてい る間に前記所定点において前記心筋層の前記局部制限された心臓活動を同時に検 出するように構成され、 前記カテーテルは、心筋層の特定の点からの局部制限された心臓信号に応答し て前記心臓内の選択された心筋層を切除するために設けられていることを特徴と する人間の心臓の心筋層を切除するカテーテル。 25.前記直交電極のうち最も接近したものは、前記切除電極から1mm乃至 5mm内に配置されてることを特徴とする請求項24記載の人間の心臓の心筋層 を切除するカテーテル。 26.人間の心臓内に配置されるカテーテル本体、 前記カテーテル本体に配置されて、前記心臓の心筋層内の複数の点に約1mm 乃至3mm内で隣接して前記心筋層からのみ局部制限された心臓活動を検出する ための前記複数の点に対応する複数の直交電極対であって、前記直交電極対の各 々が約5mm以下だけ隣接の直交電極対から離れ、各直交電極対が前記心筋層内 の前記複数の点の内の異なる点を示す識別可能な信号を受信するようにした複数 の直交電極対、及び 前記複数の所定点に隣接した心筋層においてのみ局部制限された心臓活動のみ を示す電気グラフィック信号を発生するために前記複数の直交電極により受信さ れた心臓信号どうしの差をとるための手段を有し、この差をとるための手段は、 前記複数の直交電極に接続され、 これにより、前記カテーテルは、前記複数の所定点にすぐ隣接した心筋層から の局部制限された心臓信号にのみ応答して前記心臓をマッピングするために設け られていることを特徴とする人間の心臓内の心筋層の電気生理学的活動をマッピ ングするカテーテル。
