【産業上の利用分野】本発明は,生体内のダクト,特に
頚静脈に通じている肝臓内門脈系統シャント,の直径を
減縮する減縮ステント,頚静脈に通じている肝臓内門脈
系統シャントに取り付ける装置並びに減縮ステント及び
該装置を使用する方法に関する。

【従来の技術】肝臓障害,特に肝硬変,が生じた場合,
門脈から肝臓を通って肝静脈へ至る静脈血流が減少せし
められる。これによって門脈側において血圧の増大が生
じ,その結果食道の範囲に静脈瘤が生じる。静脈瘤が破
裂すると,患者が出血死する危険がある。血圧を低下さ
せるために,門脈吻合,中間静脈吻合あるいは脾臓静脈
吻合を行うこと,つまり門脈基部を腸の下大静脈と吻合
すること,あるいは脾臓静脈を腎臓静脈と吻合すること
は公知である。これによって門脈高血圧の際の血圧の低
下が行われる。
門脈から肝臓を通って肝静脈へ至る静脈血流が減少せし
められる。これによって門脈側において血圧の増大が生

【0003】このような吻合を行わずに,肝臓組織内に
たんにこのような接続部(シャント)を形成しておく場
合には,この接続部は再び閉じることがある。シャント
口径の正確な大きさは手術の際にあらかじめ明確に決め
ることができない。したがって現在では,シャントは,
円筒形のステント,有利には曲げることのできる格子状

たれる。緊急の場合には,門脈の高血圧を迅速に低下さ
せるために,著しく大きな直径のシャントを門脈と肝臓
静脈との間に形成して,血圧を迅速にかつ充分に低下さ
せ,静脈瘤出血を停止させる。もちろんこのシャントを
貫流する血液は清浄にされることなしに肝臓を通過して
脳に流れ,血液から取り除かれなかったアンモニア,ま
たアミン及びフェノール体のような物質が肝性脳疾患を
生ぜしめ,患者の脳機能を著しく阻害する。また適当な
ステント直径の大きさをあらかじめ決めることは原則的
に不可能である。なぜなら,門脈高血圧を回避するため
の比較的に大きな直径と,できるだけわずかな清浄にさ
れなかった血液が肝臓を通り,できるだけ多くの血液が
肝臓による清浄化作用を受けるようにするための減縮さ
れた直径との間の正しい値は,種々の因子例えば血液の
粘度に関連しており,簡単に決定できないからである。
たんにこのような接続部(シャント)を形成しておく場
合には,この接続部は再び閉じることがある。シャント
口径の正確な大きさは手術の際にあらかじめ明確に決め
ることができない。したがって現在では,シャントは,
円筒形のステント,有利には曲げることのできる格子状
ステントを挿入することによって,確実に開放状態に保
たれる。緊急の場合には,門脈の高血圧を迅速に低下さ
せるために,著しく大きな直径のシャントを門脈と肝臓
静脈との間に形成して,血圧を迅速にかつ充分に低下さ
せ,静脈瘤出血を停止させる。もちろんこのシャントを
貫流する血液は清浄にされることなしに肝臓を通過して
脳に流れ,血液から取り除かれなかったアンモニア,ま

【課題を解決するための手段】この課題を解決するため
に,本発明の構成では,生体内のダクト,特に頚静脈に
通じている肝臓内門脈系統シャント(TIPS),の直
径を減縮する減縮ステントにおいて,スリーブ状部分を
備え,このスリーブ状部分は,パーフォレーションを備
えた壁体と,拡大された両端部と,くびれて直径を減縮
されている中間範囲とを有しており,スリーブ状部分の
外側には拡大されている両端部の間にトロンボゲンの糸
が設けられているようにした。

にした。更に本発明によれば,前述の構成の減縮ステン
トを,頚静脈に通じている肝臓内門脈系統シャントの直
径を減縮するために使用し,また,比較的に直径の大き
いほぼ円筒形のステントと前述の構成の減縮ステントと
を,直径を適合せしめられた減縮通路の形成のために使
用する。

を開いた状態に保つために相応する直径のほぼ円筒形の
シャントステントと,シャントステントの直径を減縮す
るための前述の構成の減縮ステントとを有しているよう
にした。更に本発明によれば,前述の構成の減縮ステン
トを,頚静脈に通じている肝臓内門脈系統シャントの直
径を減縮するために使用し,また,比較的に直径の大き
いほぼ円筒形のステントと前述の構成の減縮ステントと
を,直径を適合せしめられた減縮通路の形成のために使
用する。

ャントの直径を減縮することができる。それはトロンボ
ゲンの糸に凝血塊が形成され,シャントの口径が減縮ス
テントの中央範囲の直径にまで効果的に減縮されるから

状を有していて,カテーテル内に挿入することができる
が,体温状態に加熱されると,自動的に拡張して,前述
の拡開した形状になるようにするのである。温度が高め
られた使用状態の形状は複円すい体あるいは双曲線体に

ーション3を有する格子状のスリーブ状部分2から成っ
ている。このスリーブ状部分2は,金属薄板に互いに前
後にかつ横方向に並べて,最終状態のパーフォレーショ
ン3の相互間隔に相当する相互間隔をおいて,短いスリ
ットを切り込み,この金属薄板をエキスパンデッドメタ
ルの形式で拡張することによって製作することができ
る。この場合横方向に並んでいるスリット列のスリット
は,その長さの半分だけずらされている。このようにし
て製作された減縮ステント1は次のように前処理され
る。すなわち,減縮ステント1が図1に示すように,体
温状態若しくは使用状態に加熱されると,自動的に拡張
して,複円すい体若しくは双曲線体の形状になり,その
両端部が拡張して,比較的に大きな直径を有しており,
中央範囲においてはくびれていて,比較的に小さな直径
を有しているようにする。減縮ステント1の両端部には
尖端6があり,これらの尖端6によって減縮ステント1
が,あらかじめシャント14(図3)内に挿入されてい
るシャントステント7に引っ掛かって,固定される(図
1)。

ントステント7が図3に示した位置に取り付けられる。

ル−チタン合金(nitinol)から成っている場合は,シ
ャントステント7の高温位置である。このようにして患
者の急性出血死の危険が回避された後に,同じようにし
てカテーテル及びスライダによって減縮ステント1を,
既に挿入されているシャントステント7内に押し込ん
で,肝性脳障害を引き起こす大きな流動横断面を効果的
に減縮することができる。

- 【請求項1】 スリーブ状部分(2)を備え,このスリ
ーブ状部分(2)は,パーフォレーションを備えた壁体
と,拡大された両端部と,くびれて直径を減縮されてい
る中間範囲とを有しており,スリーブ状部分(2)の外
側には拡大されている両端部の間にトロンボゲンの糸
(8)が設けられていることを特徴とする,生体内のダ
クトの直径を減縮する減縮ステント。 - 【請求項2】 スリーブ状部分(2)の温度が上昇する
と,スリーブ状部分(2)が自動的に拡張するようにし
たことを特徴とする,請求項1記載の減縮ステント。 - 【請求項3】 スリーブ状部分(2)がニッケル−チタ
