Abstract
Description
したがって、この発明の目的は、先行技術の利点を克服し、特に、動脈瘤の継続的な成長を回避でき、特に、血栓形成材料を容易に、かつ患者に対する危険性なく用いることができる、内蔵式人工器官、送給装置および送給方法を提供することである。
表面上の或る場所に血栓形成要素を取付けて、血栓形成要素が本体部品の軸方向縁部を越えて延在しないように血栓形成要素のサイズを選択することは有利である。一旦内蔵式人工器官が移植されると血栓形成要素が血流に延在することが、それによって防がれ得る。
この発明の他の特徴、目的および利点は、純粋に例示的および非限定的であり、添付の図面を参照して読まれることになっている以下の記載から明らかになる。
詳細な記載
図1は、たとえば、典型的にはポリエステルから形成された管状のシースに縫合された1つ以上の部品におけるステントから構成されるタイプであり、典型的には血管または動脈における動脈瘤の処置のために用いられる、脈管内人工器官1の例を示す。さらに、それはたとえば心臓弁タイプの人工器官であり得、より一般的には、内蔵式人工器官が適所に取付けられた後に、それの外側に血栓形成効果を得ることが望ましい、任意の内蔵式人工器官であり得る。
示されるような内蔵式人工器官1は、近位端21と遠位端22との間に延在し、ステントおよびそのシースの組合せによって形成される、管状本体2からなる。
この図では、脈管4、たとえば動脈は、脈管の近位部分41と遠位部分42との間に位置する動脈瘤43と共に示される。
この実施の形態では、内蔵式人工器官は、それ自体公知の態様で、2つの部品、すなわち主要部品および取付けられる脚部からなる本体2を伴う二叉の内蔵式人工器官である。
Claims (25)
- 少なくとも1つの本体部品(2、24、25)を含む、特に脈管ステントまたは心臓ステントである内蔵式人工器官(1)であって、前記少なくとも1つの本体部品(2、24、25)の外側表面の少なくとも1つの領域、好ましくは外側表面全体は、前記外側表面の少なくとも1つの領域、好ましくは前記外側表面全体にわたって、実質的に一様に分布される血栓形成要素(3)を設けられることにおいて特徴付けられる、内蔵式人工器官(1)。
- 前記血栓形成要素(3)は長尺にされる、請求項1に記載の内蔵式人工器官。
- 各前記血栓形成要素(3)は前記血栓形成要素(3)の少なくとも1つの係留点(31)で前記本体部品(2、24、25)に取付けられる、請求項1または2に記載の内蔵式人工器官。
- 特に請求項1〜3の1つに記載の内蔵式人工器官(1)であって、血栓形成要素(3)が、前記人工器官の少なくとも1つの本体部品(2、24、25)の外側表面上に少なくとも部分的に配置され、前記内蔵式人工器官は前記本体部品の表面の近くで前記血栓形成要素を保持するための少なくとも1つの選択的に非作動化可能な保持手段(33)を設けられる、内蔵式人工器官(1)。
- 前記血栓形成要素(3)は、少なくとも1つのループ(34)、特に係留点(31)に対向する自由端上に実質的にあるループを有する、請求項1〜4の1つに記載の内蔵式人工器官。
- 前記ループは前記血栓形成要素の実質的に全長を形成し、長尺の保持要素(33)が好ましくは2つの要素の少なくとも2つのループのカバーゾーンを通過する、請求項5に記載の内蔵式人工器官。
- 少なくとも1つの選択的に非作動化可能な保持手段(33)が、前記血栓形成要素の前記少なくとも1つのループ(34)と作動的接続にあることによって、前記血栓形成要素(3)を保持する、請求項5または6に記載の内蔵式人工器官。
- 1つの選択的に非作動化可能な保持手段(33)が、前記血栓形成要素の前記少なくとも1つのループ(34)と作動的接続にあることによって、少なくとも2つの血栓形成要素、好ましくはすべての血栓形成要素を保持する、請求項7に記載の内蔵式人工器官。
- 少なくとも1つの選択的に非作動化可能な保持手段は、特に送給システムの近位端に向かって引かれることによって非作動化することができる、請求項4〜8の1つに記載の内蔵式人工器官。
- 少なくとも1つの選択的に非作動化可能な保持手段は再吸収可能な材料から形成され、再吸収を介して非作動化可能である、請求項4〜8の1つに記載の内蔵式人工器官。

- 前記血栓形成要素(3)は、前記表面において或る場所に取付けられ、前記血栓形成要素(3)は、前記血栓形成要素(3)が前記本体部品(2、24、25)の軸方向縁部を越えて延在しないように選択されるサイズを有する、請求項1〜15のいずれかに記載の内蔵式人工器官。
- 前記内蔵式人工器官は、2つの本体部品、特に主要部品(24)および取付けられるかまたは取付け可能な脚部(25)を含む、二叉の内蔵式人工器官として構築される、請求項1〜16の1つに記載の内蔵式人工器官。
- 前記内蔵式人工器官は、3つの本体部品(24)、特に主要部品および2つの取付けられるかまたは取付け可能な脚部(25)を含む、三叉の内蔵式人工器官として構築される、請求項1〜16の1つに記載の内蔵式人工器官。
- 前記主本体部品および/または前記脚部は、好ましくは、前記主本体部品(24)に対する前記脚部(25)の取付けのための、先端を切られた取付け部分(27、28、46)を有する、請求項17または18の1つに記載の内蔵式人工器官。
- 前記取付け部分は、血栓形成要素がない縁部領域を有する、請求項19に記載の内蔵式人工器官。
- 前記血栓形成要素は前記主要部品(24)上に設けられ、前記脚部(25)は好ましくは血栓形成要素がない、請求項17〜20のいずれかに記載の内蔵式人工器官。
- 少なくとも1つのステントおよび弁を含み、先在する心臓弁の領域に配置されることができ、弁機能の再建を可能にする弁タイプのものであって、好ましくは前記弁の上流部分において、前記血栓形成要素によって引起される血栓形成効果が、弁周囲の漏洩における低減を可能にする、請求項1〜21の1つに記載の内蔵式人工器官。
- 請求項1〜22の1つに記載の内蔵式人工器官を含む送給装置であって、前記送給装置は、好ましくは、送給の間において前記内蔵式人工器官を圧縮しておくように構成される少なくとも1つのシースを含み、前記内蔵式人工器官は、前記シースと前記内蔵式人工器官との間の相対的移動を介して解放されるように配される、送給装置。
- 前記装置は前記内蔵式人工器官の近くで前記血栓形成要素を保持するための保持要素を非作動化するための作動化機構を設けられる、請求項23に記載の送給装置。
- 特に、動脈瘤を有する脈管部位において、または生来の心臓弁において、請求項1〜22の1つに記載の内蔵式人工器官を移植する方法であって、
動脈瘤を血流から分離するように前記部位で、または生来の弁の近隣の部位で、前記内蔵式人工器官を位置決めするステップと、
前記内蔵式人工器官の前記血栓形成要素が前記内蔵式人工器官から或る距離を離れて動脈瘤内に延在するように、前記内蔵式人工器官の前記血栓形成要素を解放するステップとを含む、方法。
