同化作用のためのIGF−1およびIGFBPの組合わせ本発明は哺乳類に、同
化または成長促進状態をもたらすことに関する。さらに詳しくは、本発明は、身
体全体および骨の成長の促進を初め、同化状態を引き起こすための、IGF−1
と、該物質が結合する1またはそれ以上のタンパク質との複合物の使用に関する
。
循環系において、他の体液内で、そして培養細胞によって調整された培地内で、
ソマトメジン(IGF−IおよびIGF−T I)は、IGF作用のモジュレー
タ−と示唆されている、特異的に高親和性の担体タンパク質に結合する。
IGF結合タンパク質(BP類)の歴史は、最初に血清中に特異的ソマトメジン
結合タンパク質が存在することが発見された1984年にさかのぼる。[ハイン

ローン化され、配列決定されており、さらに、幾つかの他の、まだ完全には特性

れたクラス番号にあてはまることが分かった。これらBP類の現状を明確にする
ために、1989年6月にカナダのバンク−バーでIGF結合タンパク質に関す
るワークショップが開催され、配列決定された結合タンパク質類についてIGF
BP−1、IGFBP−2およびIGFBP−3の名称が提案された。バラード

ショップで、他の特性化が不完全なIGFBP類はそれが配列決定されるまで、

は234アミノ酸を含有し、分子量は25.3kDと予測された。リーら(Le

開)。IGFBP−1は5DS−PAGE上で還元の状態に応じて28−35k
Dの位置に泳動する。IGFBP−1は血清中に少量ある結合タンパク質であっ
て、不飽和血清IGF結合部位を含有している。血清濃度は、インシュリン逆依
存性であり、日々で著しく変化し、その濃度は早朝に最も高い。これらの濃度は
妊娠中に数100μg/Iに増大し、羊水中の濃度は血清中濃度の1000倍と
いう高さになる。
IGFBP−2クラスの担体タンパク質はヒト胎児肝臓およびラットおよびウシ
細胞系から単離された。ピンカートら(Binkert et al、 ) 、

GE上での還元状態に応じて3l−40kDである。ヒトにおいて、脳を髄液に
高濃度のIGFBP−2が認められている。胎児組織にこのタンパク質が豊富で
あることは、それが発達の制御に加担していることを示している。IGFBP−
2はIGF−IIに優先的に結合する。
血清IGF類の大多数は2部分からなるBP類と結合して分子量125−150
kDの複合体を形成する。IGFBP−3はこの複合体におけるIGF結合サブ

る消極的な役割と、IGF−1活性のプロモーターとしての積極的な役割がある

ヒト胎盤および肝がんcDNAライブラリーから得た28−kDのIGFBPを
IGF−1およびIGF−11,または他の成長因子と一緒に投与するか、ある
いは局所投与のための、傷または骨の治癒に有用な治療手段(device)あ
るいは骨粗鬆症の治療のための治療手段(device)に適用される通常の組
成物として製剤化すれば、そのような手段からのソマトメジンの安定で制御され

近くに、局所的に製造されるIGF−■を濃縮させるために、IGFBP類はあ
らゆる組織で局所的に製造されており、BP類に結合したIGF−1と循環系の
IGF−1の活性な役割を減少しているという見解もある。ルサクソンら(Is
aksson et al、 ) 、 Endocrine Reviews、
8:426−438 (1987)。例えば、IGF−1はGHにより、局所
的に骨内で製造されている[エルシンら(Nilsson et al、) 、
5cience、233:571−574 (1986)]そしてGH受容体
が軟骨細胞に発見された[エルシン(Nilsson et al、 )、J、
Endocr、122+69−77 (1989)]。
さらに、コノバー[αtnover、 72nd Annual Meetin
g of Endocrine 5ociety、 Prog、 Abstra
ct 1860une 1990)コは最近の仕事で、インビトロでのIGFB
P類のIGF−1活性の促進における活性がBPのみに暴露された細胞に依存す
ることを示した。あらかじめ混合しておいたBPおよびIGF−1と一緒にイン
キュベートした細胞はIGF−1になんら応答しなかった。BPのみを細胞とイ
ンキュベートし、次いでIGF−1を加えると、加えたIGF−1の活性は増大
した。これらのデータはIGF−1およびIGFBP類の同時混合および複合体
(コンプレックス)の同時注入はIGF−1活性の促進に至らないことを示唆し
ている。
本発明は1GF−1とIGFBPとの複合体(コンプレックス)を皮下注射で投
与することにより哺乳類での成長を促進する特異的な方法を提供することを目的
とする。
また本発明は、低血糖の危惧なく、患者に大量のIGF−Iを投与する方法を提
供することを目的とする。
これらおよび他の目的は当業者に明らかであろう。
発明の要約
従って、本発明は哺乳類における同化状態を引き起こす方法を提供するものであ
って、当量のIGF−1を単独で投与した時に比べて、哺乳類内で大きい同化状
態を引き起こすことができるよう、哺乳類に有効量のIGFBP−1とIGF−
1とを、IGFBPのIGF−[に対するモル比的0.5:1から約3・1で、
ポーラス皮下注射により共−投与(co−administration)する
ことを含む方法を提供するものである(ここに、哺乳類に成長ホルモンを共に投
与することはない)。
好ましくは、IGFBPはIGFBP−3である。IGFiとIGFBP−3の
共−投与はIGFI単独の3倍以上大きい成長応答をもたらした。誰かの理論を
借らずとも、この効果が薬物動力学または半減期事象である、あるいはIGFB
P−3が、結合しているにもかかわらず、IGI”lに同化効果を発揮せしめt
:と信じられる。TGFBPはIGFiの低血糖作用を阻害するが同化作用を阻
害しないので、急性低血糖の恐第1なしにIGF−Iの大量投与ができる。IG
FBPと結合したIGF−1の皮下注射が成長期の骨に効果的であり、軟骨プレ
ート幅を増大し、全面的な同化作用を有することが分かった。逆にIGF−Iそ
れ自身は皮下注射の場合には比較的不活性である。データが、全体重量の増加、
および器官の重量の増加を初め、全哺乳類で普遍的な同化作用を示しているとい
う事実は、同化作用が他の状況′、例えば栄養ストレス状況においても観察され
るてあろうことを示唆している。

本明細書中、rlGl−IJとは、IGFBPの適当な部位に結合することを条
件として、ウノ、ヒツジ、ブタ、ウマおよび好ましくはヒト等の任意の種由来の
、天然の配列または変異した形の配列の、そして天然、合成、または組換え等の
任意の供給源からのインシュリン様成長因子−■を指す。本発明において動物に
用いるには、処置される個々の動物種から得られたIGF−1が好ましい。例え
ば、ブタを処置するにはブタIGI−1,ヒツジを処置するにはヒツジIGF−
1,ウシを処置するにはウシIGI−I等々である。本発明においてヒトに用い
るのに好ましいのは、ヒト天然配列、成熟IGF−1,より好ましくはN−末端
メチオニンのないもので、例えばEP230.869 (1987年8月5日公
開)、EP128.733 (1984年12月19日公開)、またはEP28
8,451 (1988年10月26日公開)に記載されている方法で調製され
るものである。より好ましいのは、組換え法で製造され、ジェネンテク(Gen
entech、 Inc、 5outh San Francisco、 CA
)から臨床研究のために入手可能な、組換え的に製造された天然配列IGF−1
である。また、カビゲン(KabiGen AB、 Stockholm、Sw
eden)から入手可能な、胎盤膜を用いるラジオレセプターアッセイによる測
定で約14.000.11位/mgの比活性を有する■GF−1も好ましい。
最も好ましいTGF−1変異体はPCTWO87101038(1987年2月
26日公開)およびPCTWO89105822(1989年1月29日公開)
に記載のものであって、少なくとも、成熟分子のN−末端から第3位のグルタミ
ン酸残基を欠くか、N−末端に5アミノ酸までの欠失を有する分子である。最も
好ましい変異体はN−末端から最初の3アミノ酸が欠失されているものである(

本明細書中、rlGFBPJとは上記のごとく、1989年6月にカナダのバン
ク−バーで開かれたIGF結合タンパク質に関するワークショップで定義されバ
ラードら[(Ballard et al、 ) 、^cta Endocri
nol、(Copenh)、 121: 751−752 (1989)]の報
告にあるように、循環系において、他の体液内で、そして培養細胞によって調整
された培地内で、IGF−1と結合するタンパク質を指す。この語句にはIGF
BP−1、IGFBP−2、IGFBP−3、IGFBP−4、IGFBP−5
およびIGFBP−6が含まれ、さらにあらゆる既知のIGF結合タンパク質に

本明細書中、「哺乳類」とはヒトならびに動物を指す。治療候補としての哺乳類
には、ウシ、ヒツジおよびブタなどの商業上重要な動物が含まれる。本発明に好
ましい哺乳動物はヒトである。
本明細書中、「同化状態をもたらす」という語句は全体重増加を促進すると同時
に、正常な成長曲線、即ち、骨芽細胞によって引き起こされる骨プレー]・の直
線的な製造、によって示される個人の幼少、子供および青年期に経験した身長の
伸びの動力学、ならびに骨の様々な部分から導かれる骨芽細胞の増殖を指す。正
常な成長パターンを回復することにより、患者はより満足のいく成長曲線に近付
く。GHに比1較的抵抗性であるが同化作用の誘導治療が必要な患者の例として
、ターナ−症候群、GH治療には殆ど応笛しないGI4欠損小児、彼らの成長プ
L−−−1−が閉じる約2.3年前に正常な成長曲線が鈍化または遅くなったと
いう経験のあるP供、1−れらの子供はGH投与のみではもはや子供の成長を増
大できない、いわゆる正ギな免除フ)低い子供、およびGHへのIGF−1応答
が科学的に遮匪さA−1,た廚ち(即ちr(ルアコルチーコイド治療6Jより)

さらに、本発明方法(よ異化状態にあり、そして/ブヨt−は上置の減、少苓経
験1、°7−いる妊婦、骨粗髭症の女性患者の治療および骨修復に有用である。
本発明を実施するための形聾
本発明方法は、I G F −、−1とI G F !I Pとイ、約()、5
・1から:3:lのモルL’r皮下(SC)ポーラス注射する、二i::+::
より共−投Jjすることを色む。治療(5,用いるIGF−1,、!=IGFB
Pとの混合物を製剤化し、個々の患者の臨床状態(IGF−1単独使用の場合に
了解されたまたは予測されるあらゆる副作用または減少同化作用は−含め)、是
正すべき特定の成長の阻害または異化状態、用いるべき特定のIGFBP、混合
物を到達させるべき部位、および医師に既知の他の因子、を考慮に入れ良い医療
の実施に適合する方法で投与される。本発明にとって、IGFiおよびIGFB
Pの「有効量」とは、2つの薬物が投与前に、IGFBPのIGF−1に対する
モル比にして約0.5:1から約31の範囲で、あらかじめ混合されることを理
解し、並びに投与された量が治療された叡者内で同じ方法、同じ処方および経路
で、同量のIGF−I、ただしIGFBPをも投与することはない、を投与した
場合に得られる同化作用以上の同化状態を引き起こすことを理解した上で、考察
し決定される。
IGFBPは1個のIGFBPであっても、2またはそれ以上のIGFBPの混
合物であってもよい。用いられる好ましいIGFBPはIGFBP−3である。
IGFBP−3はまた、混合物の第3の成分としてALSを、I GFBP−3
に対するALSのモル比的1=1で伴っていてもよい。ALSが存在することで
IGF−1を血液中に自然に担持する125−150kDの3成分複合体を形成
することが出来る。
投与はGH1即ち、あらゆる種の成長ホルモンまたは組換えh lIGのような
成長ホルモ゛/変異体、例えば、米国特許No、4.755.465(1,98
8年7月5日発効)およびゲ・リプルら(Geddel et al、、 Na
ture、 282:544 (1979))に記載の組換えメチオニルhHG

者に入手可能なr h G I−T、およびイーライ・リリー(Eli Li1
ly)から購入可能なrhGH等のあらゆる成長ホルモンの不在下で行われる。
一般的な説としで、sc:投与におけるIGF−1の用量あたりの総薬剤的有効
量は患者の体tあメ:l)、約1μg/kg/日から100w+g/kg/日の
範囲であるが、」二重のごとく、これは大いに治療時の裁量に任せられており、
IGF−1単独の場合の低血糖効果の可能性により、IGF−1の最大投与量は
それを単独で用いる場合よりも少ないであろう。好ましくは、この用量は少なく
とも0 、1 mg/kg/日であり、さらに好ましくは約1−20−g/kg
/日である。最も好ましくは、IGF−1/IGFBP複合体またはIGFBP
が分解した場合、遊離で残った過剰のIGF−1が患者にとって決定的でないよ
う、それ自身では患者に低血糖を起こさない量とする。
SC投与におけるIGFBPの用量あたりの総薬剤的有効量は一般的に、IGF
BPとIGF−1のモル比が約0.5:1から3=1、好ましくは約0.81か
ら1.5+1である。もしもモル比が0.5:1以下になるとIGFBPによっ
て結合されないIGF−1の過剰が望ましくない低血糖作用を有し得るが、比率
が約3・1より大きくなるとIGF−1の活性がかなり減少され得る。
適当な用量の選択におけるキー因子は、副作用、特に低血糖、を最適に最小にす
る1方で、効果を最大にするという成果である。有効結果は、医師が適当と考え
るような、本明細書で定義する、体重の増加、低脂肪量、骨の成長、または正常
な範囲に近い満足いく成長により、あるいは哺乳類の同化状態を測定する他の基

IGF−1とIGFBPの混合物は、IGF−1がBPと複合体を形成している

通常、皮下投与のためには、所望の純度のGF−1とIGFBPとを一緒に混合
し、さらに製剤的に許容される担体を同時にまたは追加して、1回投与注射剤形
(溶液、懸濁液または乳化剤)とする。製剤的に許容される担体とは、投与され
る量および用いる濃度において受容体に無毒であり、製剤の他の成分と適合し得
るものである。例えば、製剤は、酸化剤やその他のポリペプチドに対して分解性
であることが既知の化合物を含有しないことが好ましい。また、混合物はIGF
−1とIGFBPとの混合製剤が貯蔵容器内であるいは注射後に分解しないよう
、アプロチニンのようなプロテアーゼインヒビター等の保存剤を含有することが
好ましい。
一般に、製剤はIGF−1およびIGFBPを均一に、そして緊密に液体担体と
混合することで製造することが好ましい。そのような担体ベヒクルの例として、
水、食塩水、リンゲル溶液、およびデキストロース溶液が挙げられる。
固定オイル(fixed oil)やオレイン酸エチル等の非水性ベヒクルも、
リポソームと同様、本発明に有用である。
担体は等強性や化学的安定性を増大する物質等の添加物を少量含有することが適
当である。そのような物質は用いられる用量および濃度で受容者に無毒であり、
以下のものが含まれる。りん酸、クエン酸、コハク酸および酢酸等のバッファー
および他の有機酸またはその塩のバッファー(完全長さIGF−Iにはクエン酸

典型的には、IGF−1およびIGFBPを約01諺g/++1から100諺g
/璽1、好ましくは1−10mg/mlの濃度で、pH約45−8においてその
ようなベヒクルで注射用に製剤化する。完全長さのIGFIは通常約p H6を
越さないpHで安定である。上記の賦形剤、担体、またはある安定剤等の使用で
!GF−1/IGFBP複合体の安定性を促進することが理解されよう。
治療目的に用いられるIGF−1およびIGFBPは無菌でなければならない。
滅菌は滅菌ろ過膜(例えば、0.2ミクロン膜)を通すことで容易に行える。治
療用のIGF−IおよびIGFBP組成物は通n、滅菌を行っ口を有する単一ま
たは複数投与容器、例えば、皮下注射用の剣で突き刺すことができる栓を有する
密封アンプル、バッグ、バイアルに入れて保存する。組成1物は水溶液としであ
るいは再構成するための凍結乾燥品として保存される。凍結乾燥製剤の例として
、10s】バイアルに滅菌濾過したIGF−1およびNGFBPの1%(w/v
)水溶液を入わ、得られた混合物を凍結乾燥する。凍結乾燥した1GF−1を注
射用滅菌水で再構成することで注射溶液を調製する。
以下の実施例は、本発明の実施にあたり、既知の1つの態様を示すだめのもので
あり、本発明はこれらの実施例に限定されると考えられるべきでない。本明細書
に記載のあらゆる学術および特許文献をここに引用する。
実施例I
IGFBPの血中グルコース濃度への効果2つの別々の研究でIGI”lをそれ
のみであるいは様々な用量のIGFBP−3と一緒にし、そして共−投与し、た
。基準血液試料と、タンパク質注射後の血液試料を採取した。血中グルコースを
別の血漿で測定した。IGF−1はそれ自身から予測される血中グルコースの減
少をもたらした。用量依存性で■GFBP−3はIGF−1の血中グルコース低
下作用を阻害した。最初の実験で何効と思われた量の1GFBN”−3を用いた
反復実験でこの観察を確認した。
IGFBP−3それ自身は非活性であった。
2つの別々の実験で雌性矯小ラット(70−70日令、100−140g)をケ
タミン/キシラジンで麻酔し、血液試料採取のために1力s−,,j、1ノを移
植した。血液の採取は、12頭のラットから同時に採取できるよう、自動血液採
取装置で行った。3つ(実験1)または2つ(実験2)の基準血液試料各100
μmを5分間隔て採取した。次いで、試験物質を類カテーテルを通してポーラス
注射によって投与した。血液試料を注射から40分間は10分間隔で、以後12
0分までは20分間隔で採取した。試料を遠心し血漿を保持し、Monarch
2000血液化学アナライザーでグルコースを分析した。
組換λヒ1−IGF−1は、EP230,869 (1987年8月5日公開)
に記載の一般的手法によりE、coliにより産律されるヒ1−IGF−I、ま
たはカビゲン(KabiGen AB、 Stoekholm、Sweden)
から市販品を入手可能な、胎盤膜を用いるラジオレセブターアッ(τイによる測
定で約14.000単位/I1gの比活性を有するヒトIGF−I、またはジェ
ネンテク(Genenteeb、 Inc、 5outh San Frane
isco、 CA)から臨床研究のために入手可能なヒトIGF−Iをクエン酸

実験I
IGF−IG50μg/mlに希釈しこの溶液0.5w1lをラブt−(25μ
g/ラット)に与えた。様々な濃度のIGFBP−3をIGF−1(50μg/
諒1)とITJlインキsべ hした。IC;FBP−3111度が50.10
1.及び2001zg/mlとなるよう、IGI−125μgと一緒に25.5
0および100 ItgのIGFBP−3を共−投与した。
1)3匹の動物が25 t−t gのIGFiを投与さ第1た。
2)3匹の動物が25μgのIGI−1と2571gのIGFBI−1−3を投
与された。
3)3匹の動物が25μgのICF−1と5011gの1GI43P−3を投与
された。
4)3匹の動物が25μgのIGF−1と100μgのi G F B P −
3を投上記のようにしてIGF−1およびIGFBP−3を調製し、投与した。
この実験では、ただ1つの用量でIGFBP−3をIGF−I組み合わせ、IG
FBP−3を単独で投与した。
6)4匹の動物が25μgのIGF−Jを投与された。
7)4匹の動物が5GFgのIGFBP−3を投与された。
8)4匹の動物が25μgのIGF−1と5GFgのIGFBP−3を投与実験
1の全結果を図1に示す。データは試験物質の注射5分前の血液試料カベ100
%の値になるよう正常化されている。注射前には血中グルコースカ犬安定である
ことが分かる。IGF−Iは血中グルコースの53±】3%の減少(平均値±S
D)を惹起した。IGFBP−3の投与量が増大するにつれ、この応答は階段状
に阻害された。
図2は実験1および2の両方の初期血中グルコースに対して正常化されtこ20
分間血中グルコース値を示す(上記の1群から4群、および6群力)ら8群)。
実験2では、IGF−1は20分て初期値の58±5%の血中グツ1ノコース減
少をもたらした。これは実験1で誘導されたレベルと変わらなし1゜実験2でf
ilGFBP−3のみが血中グルコースに影響せず(初期値の99±4%)、2
0分目では、IC;FBP−3+IGF−1が血中グルコース初期値の93±5
%をもたらした。
実施例lI
20匹の85−105gの下垂体切除ラット(タコニック(Taconic)
、 Germantown、 New York)を、成長スターシス(sta
sfsX T g以内の体重の増加または減少)の達成のために10日間にわた
って4回体重測定した。それらにペレットラブ(pelleted 1ab)を
摂取させ、水は任意にとらせた。それらを光と温度を調節した部屋にグループ飼
育(5匹/ケージ)した。次いでその初期体重に基いて無作為に3群に分けた。
ラットをケタミン/キシラジンで麻酔し、2個の浸透圧ミニポンプ(2001、
デリバリ−速度1μl/時/ポンプ)を皮下に配した。毎日ラットを秤量し7日
後に殺し、様々な器官を摘出した。
この実験にはIGF−1として実施例1で用いたクエン酸バッファー中5+g/
m1のICF−1を用いた。用いたIGFBP−3は実施例jに記載のものであ
る。
各実験群が以下の通りになるよう、ポンプは賦形剤(クエン酸バッファー)また
はIGF−1、またはlG1−1プラスIGFBP−3を含有していた。
1)賦形剤ポンプ(n=8)
2)IGF−1デリバリ−(0、3mg/ kg/日) (n、=8)3)TG
F−1デリバリ−(0、3+*g/kg/日)プラスIGFBP−3デリバリ−
(0、9mg/ kg/日) (n=6)結果
図3は7日間の体重増加を示す。賦形剤−処置下垂体切除ラットは実験器官中、
予測どおりの体重増加の不足を示した。用いた用量(0,3謹g/kg/日)の
IGF−1はこの用量の皮下注射投与に予測された体重増加を示した。ICF−
1をIGFBP−3と1:1のモル比で結合させたデリバリ−ではIGF−I誘
導体重増加は有意に減少されなかった。
図4はIGF−1処置ラツトにおける器官の重さ応答へのIGFBP−3への効
果を示す。胸腺、肺臓、腎臓はすべてIGF−1処ヲの後、予測された湿重量の
有意な増加を示した。しかしながら、これらの1GF−I感受性器官はIGFB
P−3の存在下では異なる反応を示した。IGF−1とIGFBP−3とが一緒
にデリバリ−さ4]ると、肺臓は有意に成長したが、胸腺と腎臓は有意な成長応
答を示さなかった。
図3および4の両者で、ダンカンマルチプルlノンジテスト(Duncan M
ultipleRange Te5t)によるフォローアツプ比較を用い、相違
を統計学的に分析(ANOVA)した。0.05以下のp値を有意と考えた。す
べてのデータは群あたり6−8匹の動物に関する平均値±SDとして表されてい
る。アステリクスは賦形剤処置対照に比べて統計学的に有意であることを意味す
る。(※p<0.05、※※p<0.01)
実施例lll
5c注射によるデリバリ−
予試験では、1日2回、3日間(6注射)皮下注射投与されたIGF−1(実施
例■およびIIに記載)はXGF−1の皮下ミニポンプ注入のはるかに太きL%
効果に比べ、下垂体切除ラットの体重増加に非常に小さい効果しか及ぼさな力\
つた。図5は3つの異なる用量でIGF−Iを皮下注射した場合の体重増加を示
す。
IGFBP−3のIGFBP−3皮下注射への効果を試験するための新しい実験
では、26匹の雌性ラブh (85−105g) (Taeonie、 Ger
mantovn、 New York)を1紀のごとく体重スタンスを確立する
ために10日間にわたって4回体重測定した。そわらにペレットラブ(pell
eted tab)を摂取させ、水は任意にとらせた。それらを光と温度を調節
した部屋でグループ飼育(5匹/ケージ)した。次いでその初期体重に基づいて
無作為に3群に分けた。う・ソトに0.1および2日の午前8時と午後5時の1
日2回(合計6注射) 、0.1mlづつ皮下注射した。午前の注射に際して体
重測定を行った。3日目(6回目の注射から約16時間後)にラットを殺し、様
々な器官を摘出した。IGF−1およびIGFBP−3は実施例IおよびIIで
用いたものであった。実験群は以下の通りである。
1)賦形剤注射
2)IGI−1デリバリ−(0,3mg/kg、7日)(15メzg/日を2回
注射)3)IGI−1デリバリ−(0゜9mg/ka/R)(45//g/Dを
2回注射)4)IGF−Iデリバリ−(0,3礒g/kg/日)プラス1GFB
P−3デリバリ−(0、8mg/kg/日)(1日あたり151Igの1GF−
Iと40μgの頁GFBP−3を2回注射)
結果
賦形剤−処置下垂体切除ラブ(・は実験期間中、わずかな体重増加を示した。
低投与量(0,3+g/kg/日)のIGF−1は体重を増加しなかったが、高
投与量(0,9■g/kg/日)はわずかな体重増加を示した。驚いたことに、
それ自身では体重増加をもたらさなかった低投与量IGF−1が、IGFBP−
3との結合物のデリバリ−では大きい体重増加を誘導した。図6はこのデータを
示す。統計学的な比較は、ANOVA、次いでDuncanの試験により行った
。※奈※は賦形剤及び高および低投与量のIGF−1に対してp<0.001、
※は賦形剤に対してp<0.05を表す。
殺した時、脛骨を摘出しその周囲の領域をホルマリンで固定化し、脱灰し、長手
方向にそって切片化し、染色し、貴地ブレー1−幅をマイクロメーターを用い光
学顕微鏡で検査した。図7は結果を示す。統計学的な比較は、Duncaniの
試験に従い、ついでANOVAにより、行った。図7においてシンボルは賦形剤
処理対照に対する統計学的有意性を示す(高投与IのIGF−1に対して、※※
※はr)<0.001または#※はp<Q、01)。賦形剤処理した、下垂体切
除対照における平均プレート幅は歴史的に得られたデータと一致して228ミク
ロンであった。低投与量のIGFiはプレート幅を有意に増加しなか。
たが、高投与量のIGFiは有意な青酸#(平均291ミクロン)を与えた。
再び、IGF−IとIGFBP−3との組み合わせは非常に大きい骨成長を41
えた。
器官の重さ、絶対量および相対量のいずれも、は肝臓、胛臓、胸腺および心臓に
関して不変であった。腎臓の絶対重量は不変であったが、相対腎臓重量は増加し
た(IGF−IプラスIGFBP−3処理ラツトと賦形剤および低投与量IGF
−1群との比較(p<0.05))。
実施例I−実施例IIIの結論
IGF−Iの有意な同化特性は、遊離のIGF−1が遅い注入によってデリバリ
−された場合にのみ認められた。例えば、IGF−Iの複数注射、1日2回、は
同化応答の誘導に比較的無効である。IGF−IとIGFBP−3との皮下注入
による共−投与はIGF−1の効果を増加しなかった。IGF−1の成長促進活
性の強化が観察されたのは、IGF−IとIGFBP−3とのポーラス皮下注射
の時のみであった。IGFBP−3が大量のIGF−1によって誘導された低血
中グルコースを阻害することが分かる。かくして、IGFBPと結合したポーラ
ス皮下注射によるIGF−1のデリバリ−によって、短期間代謝応答(低血糖)
が最小になるので、より広範な治療インデックスを伴い、低い頻度の注射が可能
となる。
IGFBP−3と結合したIGF−1はインビボ半減期が長く、この方法による
IGF−1のデリバリ−は循環血液中IGF−1を維持するであろう。かくして
IGFBP類はIGF−1の皮下注射による大量投与のための、潜在的放出制御
系となるであろう。さらに、血液中で循環しているIGF−1は同化作用を有し
ており、このことは、IGF−Iのデリバリ−がIGF−1の同化性を強化する
はずであり、それにより、IGF−iの短期間代謝効果対長期間同化特性の比を
改善することを意味する。IGF−1の好ましい投与形はIGFBP−3と分離
したとき、他のIGFBP類と結合せず、完全長さのIGF−1よりもより活性
であると予測されるdeslGF−1である。
実施例IV
併用治療のための2つの臨床シナリオ
疑い無< IGFBP、好ましくはIGFBP−3とIGF−1の随伴投与の利
点を持つ2つの適切な臨床シナリオ(筋書)を以下に記載する。
1)少なくとも12力月のGH投与の後、遅い成長を示す患者天然(以前に治療
を受けていない)または以前に治療を受けた患者(GH投与の中断の後)のいず
れも、成長速度の第2年低下を示すことは、小児内分泌学者によく認められてい
る。この状況はGH欠損型低身長(例えば、特発性、器官性、中隔−視覚形成異
常(S−OD) 、ターナ−(Turner)等)等の病因とは独立である。図
8参照。
即ち、成長速度が遅くなっている期間中、ICF−1とIGFBPにより治療す
ることで、この第2年の無応答を補償して年間速度を増大する。
2)最大に効果的なGH投与のための時間が殆ど無い患者患者がGH欠損と診断
された時点で年令が過ぎていた場合、かれらの低身長を訂正するための時間が殆
ど無い。これは図9に示されており、この図は7つの年令グループにおける患者
についての年間成長速度を報告したものである。
例えば、彼らの成長プレートが閉じるまで2−3年しか残っていない年令の大き
い患者はかさらに直線的な成長を示すことはありそうもない。これらの患者は彼
らの成長速度を最適にするためにIGF−1とIGFBP−3との併用で治療さ
れ得る。
考察とまとめ
本明細書に示した結果はI GF”−1治療が用いられるあらゆる状況において
、医師および農業者にとって意義深い。IGF−1とIGFBPとの併用治療の
レジンによれば、IGF−1単独で治療する場合よりも低用量(少なくとも約9
倍少ない)IGF−Iで、IGF−1単独の場合と同等の応答を得ることができ
る。これはIGF−1の副作用(即ち、低血糖)を最小にする等の状況で特に重
要である。

Claims (19)
- 1.哺乳類における同化状態を引き起こす方法を提供するものであって、当量の IGF−Iを単独で投与した時に得られる同化作用よりも大きい同化作用を哺乳 類で引き起こすことができるよう、IGFBPのIGF−Iに対するモル比約0 .5:1から約3:1で、有効量のIGFBPとIGF−Iとを、哺乳類にボー ラス皮下注射によって共−投与することを含む方法(ただし、成長ホルモンをも 哺乳類に投与することはない)。
- 2.哺乳類か動物である請求項1の方法。
- 3.IGF−IおよびIGFBPがそれぞれ、ウシ、ヒツジまたばブタIGF− IおよびIGFBPであり、動物がウシ、ヒツジ、またはブタである請求項2の 方法。
- 4.哺乳類がヒトである請求項1の方法。
- 5.IGFBPがIGFBP−3である請求項1の方法。
- 6.IGF−Iがヒト天然配列、成熟IGF−Iであり、IGFBPがヒトヒト IGFBP−3である請求項4の方法。
- 7.IGF−Iがクエン酸バッファー、pH6を含有する滅菌した、等張性溶液 中に含有されている請求項6の方法。
- 8.IGFBP−3のIGF−Iに対するモル比が約0.8:1から1.5:1 である請求項6の方法。
- 9.混合物がさらにヒトALSを含んでいる請求項6の方法。
- 10.IGF−Iか、ヒト天然配列IGF−Iの3位グルタミン酸が他のアミノ 酸で置換されているか、欠失しているヒト天然配列IGF−I同族体である請求 項4の方法。
- 11.IGF−Iがdes(1−3)−IGF−Iである請求項10の方法。
- 12.des(1−3)−IGF−Iが酢酸、pH3.2−4.5を含有する滅 菌した、等張性溶液中に含有されている請求項11の方法。

- 16.治療されるべきヒトが、過去の成長ホルモン単独による治療で最大年間成 長レベルに達した後、減少したヒトである請求項4の方法。
- 17.治療されるべきヒトが、そのヒトの成長プレートが閉じる2−3年前の年 令である請求項4の方法。
- 18.IGF−Iの有効量が、最大成長応答を与えるIGF−I単独の量より少 ない、請求項4の方法。
- 19.治療されるべきヒトが妊婦である請求項4の方法。
