本発明の態様は、本発明の局所用ナロキソン組成物を対象に適用するための方法も含む。上述のように、局所とは、ナロキソンが皮膚にわたって送達される投与経路を指し、例えば皮下、真皮、ならびに角質層、胚芽層、棘層、および基底層を含む表皮のうちの1つ以上に送達される。したがって、方法は、主題のナロキソン組成物を、腕、脚、臀部、肩、腰、太もも、腹部、背中、首、陰嚢、顔、耳の後ろなどの皮膚部位に適用することを含み得る。いくつかの実施形態では、方法は、本発明の組成物を健康な無傷の皮膚に適用することを含む。他の実施形態では、方法は、1つ以上の層(例えば、角質層、胚芽層、棘層、基底層など)が疾患状態、炎症状態、または完全に無傷ではない状態であってもよい皮膚、例えば疾患、炎症または他の状態に起因して角質層などの1つ以上の層に欠陥があるバリア特性(例えば穿孔)を含む皮膚に、主題の組成物を適用することを含む。例えば、方法は、皮膚が1つ以上の層に、局所用ナロキソン組成物が適用される表面領域の累積的に0.01%以上、例えば局所用ナロキソン組成物が適用される表面領域の0.05%以上、例えば0.1%以上、例えば0.5%以上、例えば1%以上、例えば2%以上、例えば3%以上、例えば5%以上、例えば10%以上、例えば25%以上、例えば50%以上、例えば75%以上、例えば90%以上、例えば95%以上、例えば97%以上(99%以上を含む)である欠陥があるバリア特性(例えば穿孔)を含む対象の皮膚表面に適用することを含み得る。
本明細書に記載のある特定の方法の実践において使用するためのキットもまた提供される。ある特定の実施形態では、キットは、例えば上述のような1つ以上の局所用ナロキソン組成物を含む。ある特定の実施形態では、キットは、スポイトまたは計量ディスペンサなどの分与デバイスを含む。いくつかの場合において、局所用ナロキソン組成物は、搾り出し可能なチューブ、例えば軟膏を保持するように構成された搾り出し可能なチューブの中に存在する。キットはまた、スポンジ、ブラシ、ドクターブレード、拡散デバイス、平滑化デバイス、またはそれらの組み合わせなどのアプリケータを含み得る。ある特定の実施形態では、キットは、局所用組成物の適用部位を覆うために使用され得る防水接着性オーバーレイなどのオーバーレイをさらに含む。
A.分解生成物の検出
まず、PEG1450を除くすべての成分(例えば、低MWのPEG、NLX、および他の添加剤、例えば抗酸化物質、グリセロール、クエン酸など)を室温で添加および攪拌することによって、以下でさらに詳述される製剤を調製した。次いで、PEG1450を添加し、すべてのPEG1450が溶解するまで、約50℃に加熱した。均質な混合物が形成されたら、それをチューブ(Albeaからの0.305インチのオリフィスを有する1インチのラミネートチューブ)に充填し、箔で密封した。ラミネートの内層は、線状低密度ポリエチレン(LLDPE)であった。充填したチューブを室温まで冷却して、チューブ内に不透明な軟膏を形成した。
注目される分解生成物を同定するために大量のPEG材料を分離する必要があることからかなりの時間および労力を要する多段階液体クロマトグラフィー法を使用して、未知の関連物質を単離した。PEG300中の約200gの1%ナロキソン遊離塩基軟膏を、湿度チャンバ内でエージングし、単離に使用した。6.2mMの1-オクタンスルホン酸ナトリウム水溶液を0.1%リン酸で洗浄し、次いで40%メタノール溶液で溶出するC18カートリッジにより、注目される分解生成物を製剤試料中で濃縮した。関連物質を含有する画分を合わせ、凍結乾燥した。分解物を、Shimadzu Prominence Preparative HPLCシステムを使用して、調製HPLCにより単離した。Security Guard PREP C18カートリッジ(21.2×15mm)を備えるPhenomenex Luna C18カラム(5μm、21.2×150mm)を使用した。
5N塩酸、5N水酸化ナトリウム、または3%過酸化水素のいずれかの存在下で、18時間または60℃で2週間、0.5%ナロキソン原薬(Siegfried、Batch 1530F001)の強制分解を対照として試験した。過酸化水素処理を除いて、対照および他の処理で検出された関連物質の合計は、総面積に対してピーク面積の1%未満であった。対照的に、3%の過酸化水素では、18時間後に、3つの分解生成物(0.66RRTで1.75%、0.72RRTで1.59%、および0.94RRTで20.42%)が観察された。0.72におけるピークは、ナロキソンN-オキシドとして同定された0.79RRTにおける関連物質のピークに対応した。0.94RRTにおけるピークは、ナロキソンピークと共溶出した。この強制分解は、他の応力と比較して、ナロキソンと過酸化物との反応性および酸化による分解の可能性を明確に示している。
PEGベースナロキソン軟膏の製剤開発中に遭遇した主な問題は、酸化によるナロキソンとPEGとの間の相互作用に起因する化学的不安定性である。この安定性の問題に対処するために、以下のようないくつかの異なるアプローチが調査された。
・抗酸化物質(クエン酸、BHT、トコフェロール、およびメタ重亜硫酸ナトリウム)の使用
・不活性ガス(アルゴンガスまたは窒素ガス)による製造加工および貯蔵管理
・異なるPEGの使用(PEG300対PEG400)
・異なるPEG源(Dow、Croda、BASF、EMD Millipore、およびClariantからのPEG)の使用
1.未開封の容器からCrodaのsuper refined PEG400を使用する
2.混合プロセス中にアルゴン保護を実行する
3.充填前に溶かした軟膏にアルゴンを吹き込む
4.チューブまたは容器のヘッドスペースにアルゴンブランケットを使用して、軟膏が空気に触れないようにする
最初に室温でPEG1450を除くすべての成分を添加して攪拌することによって、製剤を調製した。次いで、PEG1450を添加し、すべてのPEG1450が溶解するまで、約50℃に加熱した。均質な混合物が形成されたら、それをチューブに充填してキャップし、室温まで冷却して不透明な軟膏を形成した。抗酸化剤として、トコフェロールおよび/またはクエン酸を1%PEGベースナロキソン(NLX)軟膏に添加した。PEG300およびPEG400の両方を、25℃で3ヶ月にわたり安定性の差について調べた。結果を以下の表2に示す。
1%(w/w)のナロキソン塩基軟膏を、異なる供給源(Dow、Croda、BASF、EMD Millipore、およびClariant)からのPEG300またはPEG400で調製した。使用した低分子量PEGの初期過酸化物価、およびナロキソンN-オキシド分解物の成長に反映されるナロキソンの安定性について、異なる供給源からのPEGを調べた。これらの軟膏は、以下の必須プロセスを使用して調製した。
1.未開封の容器からPEG300またはPEG400を使用する
2.混合プロセス中にアルゴン保護を実行する
3.充填前に溶かした軟膏にアルゴンを吹き込む
4.チューブまたは容器のヘッドスペースにアルゴンブランケットを使用して、軟膏が空気に触れないようにする
1.0%ナロキソン遊離塩基(NLX)PEGベース軟膏を、溶融軟膏へのアルゴンの吹き込みおよびチューブのブランケットあり、ならびになしで、Croda super refined(低過酸化物)PEG400(60%)およびPEG1450(39%)を用いて調製した。また、過酸化物が低くはないPEG400、および追加のアルゴン処理を用いてこの同じ製剤を調製した。6ヶ月間、40℃/75%RHでの加速安定性試験において、アルゴン処理の有無にかかわらず、低過酸化物PEG400(SR)製剤は、低濃度の分解物1(ナロキソンN-オキシド)を示し、より高い過酸化物のPEG400製剤は、この分解物のより高い濃度を示した(以下の表6を参照されたい)。より高い過酸化物のPEG400(Hi)製剤について、より長い時間では、この効果は、実際にはナロキソンN-オキシドが他の分解物にさらに分解することにより、部分的に不明瞭であることに留意されたい。PEG400源の過酸化物のレベルは重大な影響を及ぼすが、アルゴン処理は、分解速度に明らかな改善をもたらすことはない。
1.勾配アッセイ法
以前の研究に使用されたHPLC法(「旧式」の方法)では、N-オキシドと共溶出した別の不純物が疑われた。この潜在的に新しい不純物の定量化を提供するために、より長い実行時間を有する「新規」の勾配法を開発した。加えて、「旧式」の方法は、100μg/mLの試料濃度を有し、これは、より大きな定量限界(LOQ)および検出限界(LOD)をもたらす。「新規」の方法は、試料濃度を500μg/mLに増加させることにより、より高い感度を提供した。「新規」の不純物法のLOQは約0.03%であり、LODは0.02%である。比較のためのアッセイ方法として、「旧式」の不純物法を同時に利用した。「新規」の方法は、以下の実施例で使用される。
2つの異なるメーカー(DowおよびBASF)からのCroda super refined(低過酸化物)PEG400(60%)およびPEG1450(39%)を用いて、1.0%ナロキソン遊離塩基(NLX)PEGベース軟膏を調製した。アルゴンを流しながら両方の軟膏を調製した。表9において、これら5つのRRTにおける不純物の成長が、特に40℃で貯蔵した場合、DOW PEG1450において一貫してより大きかったことが明らかである。換言すれば、BASF PEG1450とCroda PEG400との使用は、より安定していた。
この同じより安定な製剤、すなわちCroda PEG400を含むBASF PEG1450もまた、ポリエチレングリコールに可溶性である選択された抗酸化剤、BHTおよびガラクトプロピルを添加して調製した。
マウス(HOS:HR-1ヘアレス)に特別な飼料の食事10を提供することによって、アトピー性皮膚炎の皮膚症状を誘発した。すべての群を、34日目から開始して3週間連続で、各マウスの背中全体にわたり毎日0.1ml/日の軟膏で治療した。マウスを、投与前(ベースライン活性)に30分間ビデオ記録し、適用後34、35、38、41、45、48、および55日目に、1時間の期間にわたり引っ掻きについて観察した。局所用ナロキソン活性剤組成物を1時間乾燥させた後、引っ掻き挙動のビデオ記録を行った。引っ掻き挙動は、5分毎に1時間監視し、スクラッチバウトの数は、後ろ足を上げる、背中を引っ掻く、または脚を床に下げるの動作のうちの1つ以上を含む。引っ掻き挙動の累積期間もまた決定した。また、34日目、48日目、55日目、20日目(フェーズ2)、および33日目および55日目(フェーズ3)に測定される経皮水分損失(TEWL)も測定した。群に応じた治療用の製剤を、以下の表16に示す。

Claims (13)
- ナロキソン遊離塩基と、2つ以上の異なるポリエチレングリコールを含む非水性ビヒクルとを含む、 貯蔵安定性非水性局所用組成物であって、
ナロキソンN-オキシドを含まず、ナロキソンを対象の皮膚表面を通して送達するように製剤化される、 前記組成物。 - 前記2つ以上の異なるポリエチレングリコールが、異なる平均分子量の2つ以上の異なるポリエチレングリコールを含む、請求項1に記載の貯蔵安定性非水性局所用組成物。
- 前記2つ以上のポリエチレングリコールが、1000g/mol以下の平均分子量を有する第1のポリエチレングリコールと、前記第1のポリエチレングリコールより高い平均分子量を有する第2のポリエチレングリコールとを含む、請求項2に記載の貯蔵安定性非水性局所用組成物。
- 前記第2のポリエチレングリコールが、1000g/mol以上の平均分子量を有する、請求項3に記載の貯蔵安定性非水性局所用組成物。
- 前記第1のポリエチレングリコールが、600g/mol以下の平均分子量を有し、前記第2のポリエチレングリコールが、1350g/mol以上の平均分子量を有する、請求項4に記載の貯蔵安定性非水性局所用組成物。
- 前記第2のポリエチレングリコールよりも多くの前記第1のポリエチレングリコールを含む、請求項5に記載の貯蔵安定性非水性局所用組成物。
- 調製された時点で、少なくとも前記第1のポリエチレングリコールが、20ppm以下の過酸化物を含む、請求項3~6のいずれか一項に記載の貯蔵安定性非水性局所用組成物。
- 1重量%のナロキソン遊離塩基と、50~70重量%の300~400g/molの平均分子量を有する前記第1のポリエチレングリコールと、30~45重量%の1450g/molの平均分子量を有する前記第2のポリエチレングリコールとを含む、請求項3~7のいずれか一項に記載の貯蔵安定性非水性局所用組成物。
- 1つ以上の追加の不純物を含まない、請求項1~8のいずれか一項に記載の貯蔵安定性非水性局所用組成物。
- 1つ以上の抗酸化物質を含む、請求項1~9のいずれか一項に記載の貯蔵安定性非水性局所用組成物。
- 前記ナロキソン遊離塩基を局所的に送達するように製剤化される、請求項1~10のいずれか一項に記載の貯蔵安定性非水性局所用組成物。
- クリーム、ゲル、ローションまたは軟膏である、請求項1~11のいずれか一項に記載の貯蔵安定性非水性局所用組成物。
