Description
本出願は、2019年7月8日に作成され、63,211バイトを含むファイル10452WO01-配列としてコンピュータ可読形式で提出された配列表を参照することにより組み込む。
本明細書で用いられる「CD3」という表現は、多分子T細胞受容体(TCR)の一部としてT細胞上に発現され、4つの受容体鎖すなわちCD3-イプシロン、CD3-デルタ、CD3-ゼータ、CD3-ガンマのうち2つの会合から形成されるホモ二量体またはヘテロ二量体からなる抗原を指す。ヒトCD3-イプシロンは、配列番号116に記載のアミノ酸配列を含み、ヒトCD3-デルタは、配列番号117に記載のアミノ酸配列を含み、ヒトCD3-ゼータは、配列番号118に記載のアミノ酸配列を含み、CD3-ガンマは、配列番号119に記載のアミノ酸配列を含む。本明細書におけるタンパク質、ポリペプチド、およびタンパク質断片についてのすべての言及は、非ヒト種由来であることが明確に特定されない限り、それぞれのタンパク質、ポリペプチド、またはタンパク質断片のヒト型を指すことを意図している。したがって、表現「CD3」は、非ヒト種、例えば「マウスCD3」、「サルCD3」などに由来するものとして特定されない限り、ヒトCD3を意味する。

本明細書で使用される場合、抗体、免疫グロブリン、抗体結合断片、またはFc含有タンパク質のいずれかが、例えば、細胞表面タンパク質またはその断片などの所定の抗原へ結合する文脈における用語「結合」は、典型的には、最低2つの実体または抗体-抗原相互作用などの分子構造間の相互作用または会合を指す。
本発明の抗体は、単一特異性、二重特異性または多重特異性であり得る。多重特異性抗体は、1つの標的ポリペプチドの異なるエピトープに特異的であり得るか、または2つ以上の標的ポリペプチドに特異的な抗原結合ドメインを含有し得る。例えば、Tutt et al.,1991,J.Immunol.147:60-69、Kufer et al.,2004,Trends Biotechnol.22:238-244を参照されたい。本発明の抗BCMA単一特異性抗体または抗BCMA×抗CD3二重特異性抗体は、別の機能性分子、例えば、別のペプチドまたはタンパク質に連結するか、またはそれと同時発現させることができる。例えば、抗体またはその断片は、別の抗体または抗体断片などの1つ以上の他の分子実体に(例えば、化学結合、遺伝的融合、非共有結合性会合などにより)機能的に連結されて、第2のまたは追加の結合特異性を有する二重特異性または多重特異性抗体を生成することができる。

本発明は、pH依存性の結合特性を有する、抗BCMA抗体、および抗BCMA×抗CD3二重特異性抗原結合分子を含む。例えば、本発明の抗BCMA抗体は、中性pHと比較して、酸性pHでBCMAへの結合の低下を示し得る。あるいは、本発明の抗BCMA抗体は、中性pHと比較して、酸性pHでBCMAへの増強された結合を示し得る。「酸性pH」という表現は、約6.2未満、例えば、約6.0、5.95、5、9、5.85、5.8、5.75、5.7、5.65、5.6、5.55、5.5、5.45、5.4、5.35、5.3、5.25、5.2、5.15、5.1、5.05、5.0以下のpH値を含む。本明細書で使用される場合、「中性pH」という表現は、約7.0~約7.4のpHを意味する。「中性pH」という表現は、約7.0、7.05、7.1、7.15、7.2、7.25、7.3、7.35、および7.4のpH値を含む。
本発明のある特定の実施形態によると、例えば、中性pHと比較して、酸性pHで、FcRn受容体への抗体結合を増強または減少させる1つ以上の変異を含むFcドメインを含む、抗BCMA抗体、および抗BCMA×抗CD3二重特異性抗原結合分子が、提供される。例えば、本発明は、FcドメインのCH2またはCH3領域に変異を含む抗体を含み、変異(複数可)は、酸性環境においてFcRnに対するFcドメインの親和性を増加させる(例えば、pHが約5.5~約6.0の範囲のエンドソームにおいて)。そのような変異は、動物に投与したときに抗体の血清半減期の増加をもたらし得る。そのようなFc修飾の非限定的な例には、例えば、250位(例えば、EまたはQ)、250位および428位(例えば、LまたはF)、252位(例えば、L/Y/F/WまたはT)、254位(例えば、SまたはT)、および256位(例えば、S/R/Q/E/DまたはT)での修飾、または428位および/または433位(例えば、H/L/R/S/P/QまたはK)および/または434位(例えば、H/FまたはY)での修飾、あるいは250位および/または428位の修飾、あるいは307位もしくは308位(例えば、308F、V308F)、ならびに434位での修飾が含まれる。一実施形態において、修飾は428L(例えば、M428L)および434S(例えば、N434S)の修飾、428L、259I(例えば、V259I)、および308F(例えば、V308F)の修飾、433K(例えば、H433K)および434(例えば、434Y)の修飾、252、254、および256(例えば、252Y、254T、および256E)の修飾、250Qおよび428Lの修飾(例えば、T250QおよびM428L)、307および/または308の修飾(例えば、308Fまたは308P)を含む。
本発明は、高い親和性(例えば、ナノモル以下のKD値)を有するヒトBCMAに結合する抗体およびその抗原結合断片を含む。
本発明の抗原結合分子が結合するCD3および/またはBCMA上のエピトープは、CD3またはBCMAタンパク質の3つ以上(例えば、3、4、5、6、7、8、9、10、11、12、13、14、15、16、17、18、19、20、またはそれ以上)のアミノ酸の単一連続配列からなってもよい。あるいは、エピトープは、CD3またはBCMAの複数の非連続アミノ酸(またはアミノ酸配列)からなってもよい。本発明の抗体は、単一のCD3鎖内に含まれるアミノ酸(例えば、CD3-イプシロン、CD3-デルタ、またはCD3-ガンマ)と相互作用し得るか、または2つ以上の異なるCD3鎖上のアミノ酸と相互作用し得る。本発明で使用する場合、「エピトープ」という用語は、パラトープとして既知の抗体分子の可変領域における特異的抗原結合部位と相互作用する抗原決定基を指す。単一の抗原は2つ以上のエピトープを有してもよい。したがって、異なる抗体は、抗原上の異なる領域へ結合し得、異なる生物学的効果を有し得る。エピトープは、立体配座または線状のいずれかであり得る。立体配座エピトープは、直鎖状ポリペプチド鎖の異なるセグメントから空間的に並置されたアミノ酸によって産生される。直鎖状エピトープは、ポリペプチド鎖中の隣接するアミノ酸残基によって産生されるものである。ある特定の状況において、エピトープは、抗原上の糖類、ホスホリル基、またはスルホニル基の部分を含み得る。
特定の抗原に特異的な抗原結合ドメインは、当技術分野で既知の任意の抗体産生技術によって調製することができる。いったん得られれば、2つの異なる抗原(例えば、CD3およびBCMA)に特異的な2つの異なる抗原結合ドメインを互いに適切に配置して、常法を使用して本発明の二重特異性抗原結合分子を産生することができる。(本発明の二重特異性抗原結合分子を構築するために使用できる例示的な二重特異性抗体フォーマットの考察は、本明細書の他所に提供される)。ある特定の実施形態において、本発明の多重特異性抗原結合分子の1つ以上の個々の構成要素(例えば、重鎖および軽鎖)は、キメラ抗体、ヒト化抗体または完全ヒト型抗体に由来する。そのような抗体を作製するための方法は当技術分野において周知である。例えば、本発明の二重特異性抗原結合分子の1つ以上の重鎖および/または軽鎖は、VELOCIMMUNE(商標)技術を使用して調製することができる。VELOCIMMUNE(商標)技術(または任意の他のヒト抗体生成技術)を使用して、特定の抗原(例えば、CD3またはBCMA)に対する高親和性キメラ抗体が、ヒト可変領域およびマウス定常領域を有して最初に単離される。抗体を、親和性、選択性、エピトープなどを含む望ましい特徴について特徴付けし選択する。マウス定常領域は、所望のヒト定常領域と置換されて、本発明の二重特異性抗原結合分子に組み込まれ得る完全ヒト型重鎖および/または軽鎖を生成する。
本発明は、本明細書に開示される例示的な分子のものとは異なるが、CD3および/またはBCMAに結合する能力を保持するアミノ酸配列を有する抗原結合分子を包含する。このような変異体抗体は、親配列と比較してアミノ酸の1つ以上の付加、欠失、または置換を含むが、説明された二重特異性抗原結合分子の生物学的活性とは本質的に等価である生物学的活性を示す。
本発明の特定の実施形態によると、ヒトCD3に結合するが他の種由来のCD3には結合しない抗原結合分子が提供される。ヒトBCMAに結合するが、他種由来のBCMAには結合しない抗原結合分子もまた、提供される。本発明はまた、ヒトCD3および1つ以上の非ヒト種由来のCD3に結合する抗原結合分子、および/またはヒトBCMAおよび1つ以上の非ヒト種由来のBCMAに結合する抗原結合分子を含む。

本発明は、抗BCMA抗体もしくはその抗原結合断片、またはCD3およびBCMAに特異的に結合する二重特異性抗原結合分子を含む治療用組成物を、それを必要とする対象に投与することを含む方法を含む。治療用組成物は、本明細書に開示される抗体または二重特異性抗原結合分子のいずれかと薬学的に許容される担体または希釈剤とを含み得る。本明細書で使用される場合、「それを必要とする対象」という表現は、癌の1つ以上の症状または徴候を示すヒトまたは非ヒト動物(例えば、腫瘍を発現する対象または本明細書で後述される癌のうちのいずれかに罹患している対象)、あるいは、そうでなければBCMA活性の阻害もしくは減少またはBCMA+細胞(例えば、多発性骨髄腫細胞)の枯渇から利益を得る者を意味する。
本発明は、本明細書に記載の例示的な抗体および二重特異性抗原結合分子のいずれかを含む薬学的組成物を1つ以上の追加の治療剤と組み合わせて投与することを含む方法を提供する。本発明の抗原結合分子と組み合わせて、または組み合わせて投与することができる例示的な追加の治療剤は、例えば、抗腫瘍剤(例えば、メルファラン、ビンクリスチン(オンコビン)、シクロホスファミド(シトキサン)、エトポシド(VP-16)、ドキソルビシン(アドリアマイシン)、リポソームドキソルビシン(ドキシル)、オベンダムスチン(Treanda)を含む化学療法剤、または対象における形質細胞腫瘍の治療に有効であることが知られている任意の他のもの)を含む。いくつかの実施形態において、第2の治療剤は、ステロイドを含む。いくつかの実施形態において、第2の治療剤は、サリドマイド、レナリドマイド、およびボルテゾミブを含む標的療法を含み、これらは、新たに診断された患者を治療するために承認された療法である。レナリドミド、ポマリドミド、ボルテゾミブ、カルフィルゾミブ、パノビノスタット、イキサゾミブ、エロツズマブ、およびダラツムマブは、再発性骨髄腫を治療するために有効な第2の治療剤の例である。ある特定の実施形態において、第2の治療剤は、放射線療法または幹細胞移植を含むレジメンである。ある特定の実施形態において、第2の治療剤は、免疫調節剤であり得る。ある特定の実施形態において、第2の治療剤は、ボルテゾミブ(Velcade)、カルフィルゾミブ(Kyprolis)、イキサゾミブ(Ninlaro)を含むプロテアソーム阻害剤であり得る。ある特定の実施形態において、第2の治療剤は、パノビノスタット(Farydak)などのヒストンデアセチラーゼ阻害剤であり得る。ある特定の実施形態において、第2の治療剤は、モノクローナル抗体、抗体薬物コンジュゲート、抗腫瘍剤にコンジュゲートした二重特異性抗体、チェックポイント阻害剤、またはそれらの組み合わせであり得る。本発明の抗原結合分子と組み合わせて有益に投与され得る他の薬剤には、小分子サイトカイン阻害剤およびIL-1、IL-2、IL-3、IL-4、IL-5、IL-6、IL-8、IL-9、IL-11、IL-12、IL-13、IL-17、IL-18などのサイトカインに結合する抗体、またはそれらのそれぞれの受容体を含むサイトカイン阻害剤が含まれる。本発明の薬学的組成物(例えば、本明細書に開示される、抗BCMA×抗CD3二重特異性抗原結合分子を含む薬学的組成物)はまた、形質細胞表面上の異なる抗原と相互作用し得る本明細書に記載の抗体以外のモノクローナル抗体、腫瘍細胞表面上の抗原に結合する一方のアームおよびT細胞上の抗原に結合する他方のアームを有する二重特異性抗体、抗体薬物コンジュゲート、抗腫瘍剤にコンジュゲートした二重特異性抗体、チェックポイント阻害剤、例えば、PD-1またはCTLA-4を標的化するもの、またはそれらの組み合わせ、から選択される1つ以上の治療的組み合わせを含む治療レジメンの一部として投与され得る。ある特定の実施形態において、チェックポイント阻害剤は、ペンブロリズマブ(Keytruda)、ニボルマブ(Opdivo)、またはセミプリマブ(REGN2810)などのPD-1阻害剤から選択され得る。ある特定の実施形態において、チェックポイント阻害剤は、アテゾリズマブ(Tecentriq)、アベルマブ(Bavencio)、またはデュルバルマブ(Imfinzi)などのPD-L1阻害剤から選択され得る。ある特定の実施形態において、チェックポイント阻害剤は、イピリムマブ(Yervoy)などのCTLA-4阻害剤から選択され得る。本発明の抗体と組み合わせて使用することができる他の組み合わせは、上に記載されている。
本発明のある特定の実施形態によると、複数回用量の抗原結合分子(例えば、BCMAおよびCD3に特異的に結合する抗BCMA抗体または二重特異性抗原結合分子)を、所定の期間にわたって対象に投与することができる。本発明のこの態様による方法は、本発明の抗原結合分子の複数回用量を対象へ連続的に投与することを含む。本明細書で使用する場合、「連続的に投与すること」は、抗原結合分子の各用量が、異なる時点、例えば、所定の間隔(例えば、数時間、数日間、数週間または数か月間)によって分けられた異なる日に対象へ投与されることを意味する。本発明には、抗原結合分子の単回の一次用量と、それに続く抗原結合分子の1回以上の二次用量、次いで場合により抗原結合分子の1回以上の三次用量を患者へ連続的に投与することを含む方法が含まれる。
本発明の抗BCMA抗体はまた、例えば診断目的のために、サンプル中のBCMA、またはBCMA発現細胞を検出および/または測定するために使用してもよい。例えば、抗BCMA抗体またはその断片を使用して、BCMAの異常な発現(例えば、過剰発現、過少発現、発現欠如など)を特徴とする病態または疾患を診断してよい。BCMAについての例示的な診断アッセイは、例えば、患者から得られたサンプルを本発明の抗BCMA抗体と接触させることを含んでよく、抗BCMA抗体は検出可能な標識またはレポーター分子で標識されている。あるいは、標識されていない抗BCMA抗体は、それ自体が検出可能に標識された二次抗体と組み合わせて診断用途において使用することができる。検出可能な標識またはレポーター分子は、3H、14C、32P、35Sもしくは125Iなどの放射性同位体、フルオレセインイソチオシアナートもしくはローダミンなどの蛍光部分もしくは化学発光部分、またはアルカリホスファターゼ、β-ガラクトシダーゼ、セイヨウワサビペルオキシダーゼ、もしくはルシフェラーゼなどの酵素であり得る。本発明の抗BCMA抗体の別の例示的な診断用途は、89Zr-デスフェリオキサミン標識などの、対象において腫瘍細胞の非侵襲的識別および追跡の目的のための89Zr-標識抗体(例えば、ポジトロン放出断層撮影(PET)イメージング)を含む。(例えば、Tavare,R.et al.Cancer Res.2016 Jan 1;76(1):73-82、およびAzad,BB.et al.Oncotarget.2016 Mar 15;7(11):12344-58を参照されたい。)サンプル中のBCMAを検出または測定するために使用することができる特定の例示的アッセイには、酵素結合免疫吸着検定法(ELISA)、ラジオイムノアッセイ(RIA)、および蛍光活性化細胞選別(FACS)が含まれる。
抗BCMA抗体は、遺伝子改変マウスをヒトBCMA抗原(例えば、hBCMA、配列番号115)で免疫することによって、またはヒト免疫グロブリン重鎖およびカッパ軽鎖可変領域をコードするDNAを含む遺伝子操作マウスをヒトBCMA抗原で免疫することによって得られた。
抗CD3抗体は、参照により本明細書に組み込まれるWO2017/053856に記載されているように生成された。2つのそのような抗CD3抗体は、本発明による二重特異性抗BCMA×抗CD3抗体の産生から選択された。表3は、選択された抗CD3抗体の重鎖可変領域および軽鎖可変領域ならびにCDRのアミノ酸配列識別子を示す。対応する核酸配列識別子を表4に示す。本発明による二重特異性抗体の調製に使用するための他の抗CD3抗体は、例えば、WO2014/047231に見出すことができる。
本発明は、CD3およびBCMAに結合する二重特異性抗原結合分子を提供し、そのような二重特異性抗原結合分子はまた、本明細書において「抗BCMA×抗CD3または抗CD3×BCMAまたは抗BCMA×抗CD3二重特異性分子」とも称される。抗BCMA×抗CD3二重特異性分子の抗BCMA部分は、BCMA(CD269としても知られている)を発現する腫瘍細胞を標的化するのに有用であり、二重特異性分子の抗CD3部分は、T細胞を活性化するのに有用である。腫瘍細胞上のBCMAおよびT細胞上のCD3の同時結合は、活性化T細胞によって標的腫瘍細胞の直接死滅(細胞溶解)を促進する。
精製された抗BCMA mAbおよび抗BCMA×抗CD3二重特異性mAbに結合するhBCMA.mmh(配列番号106)の平衡解離定数(KD値)は、Biacore4000装置を使用するリアルタイム表面プラズモン共鳴バイオセンサーを使用して決定された。CM5 Biacoreセンサー表面を、モノクローナルマウス抗ヒトFc抗体(GE、#BR-1008-39)とのアミンカップリングによって誘導体化して、精製された抗BCMA mAbおよび抗BCMA×抗CD3二重特異性mAbを捕捉した。すべてのBiacore結合試験は、0.01M HEPES pH7.4、0.15M NaCl、3mM EDTA、0.05体積/体積%界面活性剤P20(HBS-ETランニング緩衝液)で構成される緩衝液中で実施した。単量体親和性については、C末端myc-myc-ヘキサヒスチジンタグ(ヒトBCMA-MMH;配列番号106)で発現されたヒトBCMAまたはC末端myc-myc-ヘキサヒスチジンタグ(サルBCMA-MMH;配列番号110)で発現されたサルBCMAの異なる濃度の細胞外ドメインを、HBS-ETランニング緩衝液(90~1.11nM、3倍希釈)中で調製した。二量体親和性については、C末端mFcタグ(ヒトBCMA-MFC;配列番号108)で発現されたヒトBCMAまたはC末端mFcタグ(サルBCMA-MFC;配列番号112)で発現されたサルBCMAの異なる濃度の細胞外ドメインを、HBS-ETランニング緩衝液(30~0.37nM、3倍希釈)中で調製したか、またはC末端mFcタグ(マウスBCMA-MFC;配列番号114)で発現された30nMのBCMAを、調製した。次いで、抗原サンプルを、30μL/分の流量で表面に捕捉された抗BCMAおよび抗BCMA×抗CD3二重特異性mAbにわたって注入した。HBS-ETランニング緩衝液中での解離を10分間モニタリングしながら、抗体-試薬結合物を5分間モニタリングした。すべての結合速度論実験を、25℃で実施した。Scrubber 2.0c曲線適合ソフトウェアを使用してリアルタイムセンサーグラムを1:1結合モデルへあてはめることによって、動的結合(ka)速度定数および動的解離(kd)速度定数を決定した。結合解離平衡定数(KD)および解離半減期(t1/2)を、動的速度定数から以下のように計算した:
フローサイトメトリー分析を利用して、ヒトおよびカニクイザルCD3(Jurkat細胞、mfCD3操作Jurkat細胞、一次ヒトCD8+およびカニクイザルCD8+T細胞)へのBCMA×CD3二重特異性抗体の結合を決定した。簡言すれば、1e05細胞/ウェルを、BCMA×CD3および対照抗体の連続希釈法を伴うブロック(PBS+1%濾過FBS+5%マウス血清)を用いたFACS洗浄の存在下で氷上で30分間インキュベートした。インキュベーション後、細胞を冷FACS洗浄(PBS+1%濾過FBS)で2回洗浄し、氷上でさらに30分間、Alexa647でコンジュゲートした抗ヒト二次抗体でインキュベートすることによって、結合抗体を検出した。抗体なしまたは二次抗体のみを含有するウェルを対照として使用した。サルおよびヒトT細胞を検出するために、CD4、CD8、およびCD16に対するヒトおよびカニクイザルの交差反応性抗体のカクテルを抗ヒト二次抗体に添加した。インキュベーション後、細胞を洗浄し、1%濾過FBSを含有する200μLの冷PBSに再懸濁し、BD FACS Canto IIでフローサイトメトリーによって分析した。細胞を、FSC-HによってFSC-Aによってゲーティングし、シングレットイベントを選択し、続いて側面散乱および前方散乱がライブイベントを選択した。サルT細胞については、CD8+/CD16-細胞で追加のゲーティングを実施した。
抗BCMA×CD3抗体、mAb25442Dが、BCMA陽性多発性骨髄腫(NCI-H929、MM.1S、OPM-2、およびRPMI-8226)、BCMA陽性リンパ腫(RajiおよびDaudi)、ならびにBCMA陰性(HEK293)細胞の表面に結合する能力を、フローサイトメトリーで決定した。細胞解離緩衝液(Millipore、カタログ番号S-004-C)を使用してフラスコから細胞を回収し、染色緩衝液(PBS、カルシウムおよびマグネシウムを含まない(Irving 9240)+2%FBS(ATCC 30-2020)中に96ウェルV字底プレートのウェルあたり500,000個の細胞の密度で播種した。細胞を、4℃で30分間、Alexa647コンジュゲート抗BCMA×CD3抗体(mAb25442D-A647)または無関連腫瘍標的アーム(アイソタイプ-A647)と対となる同じCD3結合アームを有するAlexa647コンジュゲートアイソタイプ対照の2倍段階希釈で、4℃で30分間染色した。細胞を、染色緩衝液で2回洗浄し、製造業者の取扱説明書に従ってLIVE/DEAD(商標)Fixable Green死細胞染色キット(Invitrogen,L34970)で標識して、生細胞と死細胞を区別した。次いで、細胞を洗浄し、PBSで希釈したBD Cytofix(BD、カタログ番号554655)の50%溶液を使用して、4℃で25分間固定した。サンプルは、Accuri C6フローサイトメーター(BD Biosciences)で実行され、Flowjo 10.2(Tree Star)で分析された。生細胞および単一細胞についてゲーティングした後、平均蛍光強度(MFI)を決定し、MFI値は、EC50を計算するために10ポイントの応答曲線上に4パラメータロジスティック方程式を使用して、Graphpad Prismにプロットした。各用量反応曲線のゼロ条件も、2倍連続希釈の継続として分析に含まれ、最低用量として表される。信号対雑音比(S/N)は、mAb25442D-A647 MFIとIsotype-A647 MFIの比率をとることによって決定される。(表13)。mAb25442D-A647のS/Nは、2~470の範囲であり、EC50値は、27~83nMの範囲であった。HEK293細胞では、検出可能な結合は観察されなかった。
抗BCMA×抗CD3二重特異性抗体の活性は、BCMA表面発現のレベルが異なるいくつかの細胞株を利用したJurkat/NFATLucレポーターバイオアッセイで評価された。Jurkat細胞は、NFAT-ルシフェラーゼレポーター(Jurkat/NFATLuc.3C7)を発現するように設計され、50,000個のJurkatレポーター細胞が、50ulのアッセイ培地(10%FBSおよび1%P/S/Gを含むRPMI培地)中の、50,000個のBCMA陽性(Daudi、MM1-S、NCI-H929、OPM-2、RPMI-8226、MOLP-8、もしくはRaji)またはThermo Nunclonデルタ96ウェル白色マイクロウェルプレート(Thermo Scientific、カタログ番号136102)中のBCMA陰性(HEK293)細胞と混合した。BCMA×CD3二重特異性抗体(mAb25441DもしくはmAb25442D)または二価抗BCMA抗体(mAb21581)の3倍連続希釈液を、50uLのアッセイ緩衝液中に直ちに添加した。プレートを穏やかに撹拌し、37℃、5%CO2インキュベーター中で4~6時間インキュベートした。NFAT-ルシフェラーゼ活性は、Promega One-Glo(カタログ番号E6130)およびPerkin Elmer Envisionプレートリーダーを使用して決定した。RLUは、EC50値を計算するために、12ポイントの応答曲線上で4パラメータロジスティック方程式を使用して、GraphPad Prismにプロットした。各用量応答曲線の抗体を用いない治療条件も、3倍連続希釈の継続として分析に含まれ、最低用量として表される。信号対雑音比(S:N)は、曲線上の最高のRLUと最低のRLUの比率をとることによって決定される。
市販の抗ヒトBCMA抗体(クローン19F2)の抗体結合能(ABC)は、Quantum Simply Cellular抗ヒトIgGキットを使用し、製造業者の取扱説明書(Bangs Laboratories)に従って、多発性骨髄腫細胞株のパネル上で決定した。
フローサイトメトリーによって多発性骨髄腫細胞の特異的死滅をモニタリングするために、多発性骨髄腫患者からの骨髄単核細胞(BMMC)をヒト間質細胞(HS5)に播種し、37℃で一晩休ませた。別に、適合した患者の末梢血単核細胞(PBMC)を、解凍し、付着細胞を枯渇させることによってリンパ球を濃縮するために、1×106細胞/mLで補充RPMI培地中で37℃で一晩培養した。翌日、BMMCは、間質細胞(HS5)上の付着細胞枯渇ナイーブPBMCおよびBCMA×CD3二重特異性または1アームCD3アイソタイプ対照(開始濃度66.7nM)の連続10倍希釈液を用いて37℃で共培養した。3、4、または7日目に、細胞を細胞培養プレートから取り出し、FACSにより分析した。死滅の特異性を評価するために、多発性骨髄腫細胞を、単一、生、CD90陰性(間質細胞を除く)、CD2陰性、CD56陽性としてゲーティングした。CD45は、MM455を除くほとんどのサンプルで多発性骨髄腫細胞において低かった。調整された生存率の計算のために、生標的細胞の割合を、以下のように報告した:調整された生存率=(R1/R2)*100(式中、R1=抗体の存在下での生標的細胞の割合(%)、およびR2=試験抗体の不在下での生標的細胞の割合(%)。
BCMA×CD3二重特異性抗体(Ab)のインビボでの有効性を決定するために、異種腫瘍研究を実施した。免疫不全NOD.Cg-PrkdcscidIl2rgtm1Wjl/SzJ(NSG)マウスに、10×106個のBCMA発現NCI-H929多発性骨髄腫細胞と正常ドナーから単離された0.5×106個のヒト末梢血単核細胞(PBMC)との混合物を皮下に移植した。マウス(群あたりn=7)に、PBSビヒクル対照、無関連抗FelD1二価アイソタイプ対照Ab(REGN2759)、CD3結合対照二重特異性Ab(mAb17664D)、BCMA×CD3(G;REGN5458)二重特異性Ab、またはBCMA×CD3(G20;REGN5459)二重特異性抗体を4mg/kgの用量で直ちに投与した。マウスに、Abを週に2回、合計3週間投与し、腫瘍の成長を40日間にわたって評価した。BCMA+腫瘍は、ビヒクル、アイソタイプ対照、およびCD3結合対照治療マウスで同様に成長したが、試験されたBCMA×CD3 Abは両方とも、インビボでの腫瘍の成長を防止した。
抗BCMAx抗CD3二重特異性抗体(Ab)のインビボでの有効性を決定するために、異種腫瘍研究を実施した。免疫不全NOD.Cg-PrkdcscidIl2rgtm1Wjl/SzJ(NSG)マウスに、10×106個のBCMA発現NCI-H929ヒト多発性骨髄腫細胞と正常な健常ドナーから単離された0.5×106個のヒト末梢血単核細胞(PBMC)との混合物を皮下に移植した。次いで、マウス(群あたりn=7)に、PBSビヒクル対照、CD3結合対照二重特異性Ab(G;mAb17664D)を4mg/kgの用量で、CD3結合対照二重特異性Ab(G20;REGN4460)を4mg/kgの用量で、BCMA×CD3(G;REGN5458)二重特異性Abを4mg/kg、0.4mg/kg、もしくは0.04mg/kgのいずれかの用量で、またはBCMA×CD3(G20;REGN5459)二重特異性Abを4mg/kg、0.4mg/kg、もしくは0.04mg/kgのいずれかの用量で直ちに投与した。マウスに、これらのAbを週に2回、合計7回の用量を投与し、腫瘍の成長を60日間にわたって評価した。BCMA+NCI-H929腫瘍は、ビヒクルおよびCD3結合対照で治療されたマウスで同様に成長したが、試験された抗BCMA×抗CD3 Abは両方とも、インビボでの用量依存的様式で腫瘍の成長を防止した。

抗BCMA×抗CD3二重特異性抗体(Ab)のインビボでの有効性を決定するために、異種腫瘍研究を実施した。11日目に、免疫不全NOD.Cg-PrkdcscidIl2rgtm1Wjl/SzJ(NSG)マウスに、正常な健常ドナーからの4×106個のヒト末梢血単核細胞(PBMC)を腹腔内に注射した。0日目に、マウスに、ホタルルシフェラーゼ(MOLP-8-ルシフェラーゼ細胞)も発現するように操作された2×106個のBCMA+MOLP-8ヒト多発性骨髄腫腫瘍細胞を静脈内投与した。次いで、マウス(群あたりn=5)に、CD3結合対照二重特異性Ab(G;mAb17664D)を4mg/kgの用量で、またはBCMA×CD3(G;REGN5458)二重特異性Abを4mg/kgの用量で直ちに投与した。マウスに、これらのAbを3および7日目に2回以上、合計3回の用量を投与した。麻酔した動物の腫瘍生物発光(BLI)を測定することによって、48日間にわたって腫瘍の成長を評価した。陽性対照として、マウス群(n=5)には、MOLP-8-ルシフェラーゼ細胞のみを投与し、PBMCまたは抗体は投与しなかった。バックグラウンドBLIレベルを測定するために、マウス群(n=5)は治療せず、腫瘍、PBMC、または抗体を投与しなかった。BCMA+MOLP-8-ルシフェラーゼ腫瘍は、CD3結合対照治療マウスで進行的に成長したが、REGN5458によるBCMA×CD3Ab治療は、インビボで腫瘍の成長を遅らせた。
抗BCMA×抗CD3二重特異性抗体(Ab)のインビボでの有効性を決定するために、異種腫瘍研究を実施した。0日目に、免疫不全NOD.Cg-PrkdcscidIl2rgtm1Wjl/SzJ(NSG)マウスに、ホタルルシフェラーゼ(OPM-2-ルシフェラーゼ細胞)も発現するように設計された2×106個のBCMA+OPM-2ヒト多発性骨髄腫腫瘍細胞を静脈内投与した。10日目に、マウスに、正常な健常ドナーからの4×106個のヒト末梢血単核細胞(PBMC)を腹腔内に注射した。21日目に、マウス(群あたりn=5)に、CD3結合対照二重特異性Ab(G;mAb17664D)を0.4mg/kgの用量で、BCMA×CD3(G;REGN5458)二重特異性Abを0.4mg/kgで、またはBCMA×CD3(G20;REGN5459)二重特異性Abを0.4mg/kgで投与した。マウスに、これらのAbを25および28日目に2回以上、合計3回の用量を投与した。麻酔した動物の腫瘍生物発光(BLI)を測定することによって、61日間にわたって腫瘍の成長を評価した。陽性対照として、マウス群(n=5)には、OPM-2-ルシフェラーゼ細胞のみを投与し、PBMCまたは抗体は投与しなかった。バックグラウンドBLIレベルを測定するために、マウス群(n=5)は治療せず、腫瘍、PBMC、または抗体を投与しなかった。BCMA+OPM-2-ルシフェラーゼ腫瘍は、CD3結合対照で治療されたマウスで徐々に成長したが、REGN5458およびREGN5459によるBCMA×CD3 Ab治療は、動物の大多数で腫瘍負荷をバックグラウンドレベルまで減少させた。
抗BCMA×抗CD3二重特異性抗体(Ab)のインビボでの有効性を決定するために、ヒトCD3を発現するマウスにおいて同系腫瘍研究を実施した。マウスCD3deg(CD3-ヒト化マウス)の代わりにヒトCD3degを発現するC57BL/6マウスに、完全長ヒトBCMA(B16/BCMA細胞)を発現するように操作された0.5×106個のB16メラノーマ細胞または完全長ヒトBCMA(MC38/BCMA)を発現するように操作された1×106個のMC38結腸癌細胞のいずれかを皮下に移植した。次いで、マウス(群あたりn=7)に、CD3結合対照二重特異性Ab(G;mAb17664D)を0.4mg/kgの用量で、またはBCMA×CD3(G;REGN5458)二重特異性Abを0.4mg/kgもしくは0.04mg/kgの用量で直ちに投与した。マウスに、これらのAbを4および7日目に2回以上、合計3回の用量を投与し、腫瘍成長を、実験を通して評価した。B16/BCMA腫瘍およびMC38/BCMA腫瘍は、CD3結合対照治療マウスで成長したが、BCMA×CD3 REGN5458は、インビボで用量依存的様式で両方の腫瘍株の成長を抑制することができた。
質量分析(HDX-MS)によるH/D交換エピトープマッピングを実施して、REGN5458(BCMA×CD3二重特異性抗体)と相互作用するBCMA(組換えヒトBCMA、配列番号115のアミノ酸配列)のアミノ酸残基を決定した。H/D交換法の一般的な説明は、例えば、Ehring(1999)Analytical Biochemistry 267(2):252-259、およびEngen and Smith(2001)Anal.Chem.73:256A-265Aに記載されている。
フローサイトメトリー分析を利用して、多発性骨髄腫細胞株と抗BCMA抗体との一晩のインキュベーションが表面BCMAのレベルに及ぼす影響を決定した。MM細胞株(H929、Molp8、U266、およびMM1.S)を2回洗浄し、66.7または667nMの抗BCMA抗体、を含むR10培地(RPMI+10%FBS+pen/strep/glut)、DAPT(ガンマセクレターゼ阻害剤)、または培地のみで37℃で培養した。18時間後、ウェルを、冷FACS洗浄(PBS+1%濾過FBS)で洗浄し、氷上で30分間、冷染色緩衝液(Miltenyi 130-091-221)中の667nMの同じ抗BCMA抗体に再懸濁した。インキュベーション後、細胞を、冷FACS洗浄(PBS+1%濾過FBS)で2回洗浄し、氷上で適切な抗ヒト二次抗体(抗hIgGまたは抗HIS)とさらに30~45分間インキュベートすることによって、結合した抗体を検出した。インキュベーション後、細胞を洗浄し、1%濾過FBSを含有する200μLの冷PBSに再懸濁し、BD FACS Canto IIでフローサイトメトリーによって分析した。染色の倍増は、BCMA abまたはDAPTでこれまでに一晩インキュベートした染色細胞のMFIを、培地のみで一晩インキュベートした染色細胞のMFIで除することによって計算された。
刺激されていない自己T細胞による濃縮CD138+ヒトまたはカニクイザル形質細胞の特異的死滅は、フローサイトメトリーによって評価された。ヒトまたはカニクイザルの骨髄穿刺液および血液は、採取から24時間以内に提供された。CD138+形質細胞は、製造業者の取扱説明書に従って、EasySep Human CD138+陽性選択キットを使用した陽性選択によって、骨髄から濃縮された。全血からのPBMCは、密度分離によって分離された。PBMCは、1μMのVybrant CFDA-SE蛍光追跡色素で標識された。標識後、1×104個の富化CD138+形質細胞は、10:1のE:T比で丸底96ウェルプレートにVybrant CFDA-SE標識PBMC、およびREGN5458、CD3結合対照bsAb、またはBCMA結合対照mAbの連続希釈で、完全培地中で37℃で72時間播種した。培養の最後に、生存しているCD138+形質細胞を、固定可能なLIVE/DEAD色素および形質細胞特異的細胞表面マーカーを利用してフローサイトメトリーによって分析した。生存率は、対照条件(PBMCのみの存在下の形質細胞)に対して正規化された。T細胞の活性化は、フローサイトメトリーによって評価された。活性化は、CD25を発現するCD2+/CD4+またはCD2+/CD8+/CD16-T細胞の割合として報告される。T細胞活性化の割合は、対照条件(PBMCのみの存在下の形質細胞)に対して正規化された。
BCMA×CD3二重特異性抗体(Ab)がPD-1遮断と相乗作用して、インビボで優れた抗腫瘍効果を提供するかどうかを決定するために、ヒトCD3を発現するマウスにおいて同系腫瘍研究を実施した。結果は、REGN5458とPD-1遮断の組み合わせが、REGN5458またはPD-1遮断単独よりも優れた抗腫瘍効果を提供することを示す。
群あたりのマウスの数が10であり、BCMAxCD3 REGN5458の高用量が0.4mg/kgであったことを除いて、実施例20において上記で論じられたものと同一である、類似の結果が、第2の実験で得られた。第2の実験における特定の治療群を、以下の表34に示す。
Claims (17)
- 単離された二重特異性抗原結合分子であって、
(a)第1の抗原結合ドメイン配列番号68、70、72のアミノ酸配列をそれぞれ含む、HCDR1、HCDR2、HCDR3ドメインを含む重鎖可変領域(HCVR)、および、配列番号84、86、88のアミノ酸配列をそれぞれ含む、LCDR1、LCDR2、LCDR3ドメインを含む軽鎖可変領域(LCVR)を含み、ヒトB細胞成熟抗原(BCMA)に特異的に結合する、第1の抗原結合ドメイン;ならびに
(b)配列番号92、94、96のアミノ酸配列をそれぞれ含む、HCDR1、HCDR2、HCDR3ドメインを含むHCVR、および、配列番号84、86、88のアミノ酸配列をそれぞれ含む、LCDR1、LCDR2、LCDR3ドメインを含むLCVRを含み、ヒトCD3に特異的に結合する、第2の抗原結合ドメイン、
を含む、前記単離された二重特異性抗原結合分子。 - 単離された二重特異性抗原結合分子であって、
(a)第1の抗原結合ドメイン配列番号68、70、72のアミノ酸配列をそれぞれ含む、HCDR1、HCDR2、HCDR3ドメインを含む重鎖可変領域(HCVR)、および、配列番号84、86、88のアミノ酸配列をそれぞれ含む、LCDR1、LCDR2、LCDR3ドメインを含む軽鎖可変領域(LCVR)を含み、ヒトB細胞成熟抗原(BCMA)に特異的に結合する、第1の抗原結合ドメイン;ならびに
(b)配列番号100、102、104のアミノ酸配列をそれぞれ含む、HCDR1、HCDR2、HCDR3ドメインを含むHCVR、および、配列番号84、86、88のアミノ酸配列をそれぞれ含む、LCDR1、LCDR2、LCDR3ドメインを含むLCVRを含み、ヒトCD3に特異的に結合する、第2の抗原結合ドメイン、
を含む、前記単離された二重特異性抗原結合分子。 - (a)配列番号66のアミノ酸配列を含むHCVR、および、配列番号82のアミノ酸配列を含むLCVRを含む、第1の抗原結合ドメイン;ならびに
(b)配列番号90のアミノ酸配列を含むHCVR、および、配列番号82のアミノ酸配列を含むLCVRを含む、第2の抗原結合ドメイン、
を含む、請求項1に記載の単離された二重特異性抗原結合分子。 - (a)配列番号66のアミノ酸配列を含むHCVR、および、配列番号82のアミノ酸配列を含むLCVRを含む、第1の抗原結合ドメイン;ならびに
(b)配列番号98のアミノ酸配列を含むHCVR、および、配列番号82のアミノ酸配列を含むLCVRを含む、第2の抗原結合ドメイン、
を含む、請求項2に記載の単離された二重特異性抗原結合分子。 - 二重特異性抗体である、請求項1~4のいずれか一項に記載の単離された二重特異性抗原結合分子。
- (a)前記二重特異性抗体が、ヒトIgG重鎖定常領域を含み;場合により、前記ヒトIgG重鎖定常領域が、アイソタイプIgG1もしくはIgG4である;および/または
(b)前記二重特異性抗体が、同じアイソタイプの野生型ヒンジと比較して、Fcγ受容体結合を減少させるキメラヒンジを含む、
請求項5に記載の単離された二重特異性抗原結合分子。 - 請求項1~6のいずれか一項に記載の二重特異性抗原結合分子、および、薬学的に許容される担体または希釈剤を含む、薬学的組成物。
- 請求項1~6のいずれか一項に記載の二重特異性抗原結合分子をコードするヌクレオチド配列を含む、核酸分子、または、請求項1~6のいずれか一項に記載の二重特異性抗原結合分子の、それぞれ、第一の抗原結合ドメインのHCVR、第二の抗原結合ドメインのHCVR、ならびに、第一および第二の抗原結合ドメインのLCVRをコードするヌクレオチド配列を含む、核酸分子のグループ。
- 請求項8に記載の核酸分子を含む発現ベクター、または、それぞれ、請求項8に記載の核酸分子のグループを含む、発現ベクターのグループ。
- 請求項1~6のいずれか一項に記載の二重特異性抗原結合分子、請求項8に記載の核酸分子もしくは核酸分子のグループ、または、請求項9に記載の発現ベクターもしくは発現ベクターのグループを含む、宿主細胞。
- 対象における形質細胞腫瘍の成長を阻害するための、請求項7に記載の薬学的組成物。
- 前記形質細胞腫瘍が、多発性骨髄腫である、請求項11に記載の薬学的組成物。
- 多発性骨髄腫、または別のBCMA発現B細胞悪性腫瘍に罹患している患者を治療するための、請求項7に記載の薬学的組成物。
- 前記BCMA発現B細胞悪性腫瘍が、ワルデンストレームマクログロブリン血症、バーキットリンパ腫、びまん性大細胞型B細胞リンパ腫、非ホジキンリンパ腫、慢性リンパ性白血病、濾胞性リンパ腫、マントル細胞リンパ腫、辺縁帯リンパ腫、リンパ形質細胞性リンパ腫、およびホジキンリンパ腫からなる群から選択される、請求項13に記載の薬学的組成物。
- 第2の治療剤または治療レジメンと組み合わせて使用するための、請求項11~14のいずれか一項に記載の薬学的組成物であって;場合により、前記第2の治療剤または治療レジメンが、化学療法薬、DNAアルキル化剤、免疫調節剤、プロテアソーム阻害剤、ヒストンデアセチラーゼ阻害剤、放射線療法、幹細胞移植、異なる腫瘍細胞表面抗原およ
びT細胞または免疫細胞抗原と相互作用する異なる二重特異性抗体、抗体薬物コンジュゲート、抗腫瘍剤にコンジュゲートした二重特異性抗体、PD-1、PD-L1、またはCTLA-4チェックポイント阻害剤、またはそれらの組み合わせを含む、前記薬学的組成物。 - 抗PD-1抗体またはその抗原結合断片と組み合わせて、BCMA発現腫瘍に罹患している患者を治療するための、請求項7に記載の薬学的組成物。
- 前記抗PD-1抗体または抗原結合断片が、抗PD-1抗体であり;場合により、前記抗PD-1抗体が、セミプリマブ(REGN2810)である、請求項16に記載の薬学的組成物。
