Description

本発明は癌治療の分野に関する。さらに詳しくは、本発明は、癌の治療に有用な超安定リポソーム、および超安定リポソームを用いた癌の治療方法に関する。超安定リポソームからの有糸分裂阻害剤放出の極度の延長が、標的化可能な癌細胞の割合を増加させることによって癌の治療における有効性を改善することが発見された。超安定リポソームからの有糸分裂阻害剤の遅い放出は、インビトロおよびインビボでの癌細胞の死滅と相関する。1つの例では、単回投与の超安定リポソームBI 2536で処置した異種移植マウスは、処置マウスの20%において、12日間持続する腫瘍体積の減少および完全寛解を経験した。1週間に2回の投与で処置すると、治療率は処置されたマウスの75%に増加した。
以下の実施例を参照して本発明を説明するが、これらの実施例は説明のために提供され、決して本発明を限定することを意図するものではない。当該技術分野において周知の標準的な技術または以下に具体的に記載される技術を利用した。
リポソーム調製:
1,2-ジステアロイル-sn-グリセロ-3-ホスホエタノールアミン-N-[メトキシ(ポリエチレングリコール)-2000](DSPE-PEG2000)および水素添加卵L-α-ホスファチジルコリン(HEPC)は、Lipoidから購入し、Cholesterol(Chol)は、Sigma-Aldrichから購入した。モル比50:45:5のHEPC:Chol:DSPE-PEG2000の脂質混合物をクロロホルム(Takara)に溶解した。混合物を回転蒸発下(Eyela NVC-2200/N-1100)、丸底フラスコ中で薄い脂質膜になるまで乾燥し、さらに、高真空下で一晩乾燥した後、リポソームの内部環境として所望の塩溶液で水和した。得られた100mM脂質懸濁液をバスソニケーター(S30H Elmasonic)で1時間超音波処理し、続いて、Lipex Thermobarrel Extruder(Northern Lipids)を用いて二重積層100nm Nucleporeフィルター(Whatman)を通して10回押し出して、単一の単層ベシクル(SUV)を形成した。
これらのSUVを、24時間以内に新鮮なスクロース溶液を3回交換しながら4℃で300mMのスクロース(Sigma)中で透析して、リポソームの外部環境を交換した。意図した使用までリポソームをガラス管内で4℃にて保存する。
pH勾配法を介してBI 2536(Axon)をリポソームに能動的にローディングした。必要なBI 2536を最初にエタノール中に溶解し、続いて回転蒸発下で乾燥することによりシンチレーションバイアル中で薄膜としてコーティングした。BI 2536薄膜をさらに、少なくとも24時間真空乾燥した。リポソームを、3:1の脂質:薬剤濃度でローディングし、最終濃度約50~約70mMの脂質になるように水で希釈した。次いで、ローディングを容易にするために、混合物を70℃のウォーターバス中でインキュベートし、続いて、300mMのスクロース中で少なくとも36時間透析して封入されていないBI 2536を除去した。透析後、リポソームを、使用するまでガラス管内に保存し、スクロース透析液の一部を下流の封入効率測定のために4℃で保存した。
リポソームにローディングされたBI 2536の量を、直接計算(インビトロ研究)または逆算(動物研究について)によって決定した。直接計算のために、1μlのリポソームを20μlのエタノールで希釈し、360nm励起および470nm発光を用いる蛍光測定により読み取った(Tecan Infinite M200)。BI 2536の量を、標準曲線との比較により決定した。逆算のために、100μlの1-ノナノール(Merck)を使用して、1時間ボルテックスすることによって、1.5mlの透析液から未封入BI 2536を抽出した。ノナノールおよびスクロースを短時間の遠心分離によって相分離させ、20nmのノナノール層を330nmの励起および370nmの発光を用いる蛍光強度を測定した(Tecan Infinite M200)。透析液中のBI 2536の濃度を標準曲線と比較することによって決定し、封入効率を式:(A-B)/A × [BI 2536]初期

によって計算した。
HCT116(CCL-247、ヒト大腸癌腫)をAmerican Type Culture Collection(ATCC)から購入し、10%ウシ胎児血清(Thermo Scientific)を添加したMcCoyの5A培地(Life Technology)を用いて培養した。細胞を37℃、5%CO2でインキュベートし、コンフルエンスが~80%に達したときに2~3日ごとに継代した。
約7x103個のHCT116細胞を96ウェルプレートに播種し、一晩インキュベートした後、~50%のコンフルエンスに達した。ウェル内の培地を、遊離BI 2536またはリポソームBI 2536のいずれかを補足した新鮮な培地と交換した。使用したBI 2536の濃度は、1μMのBI 2536を連続的に希釈することによって生成された。培地のみを含むウェルをブランク対照として使用した。製剤化のためにそれぞれ少なくとも3回反復を実施した。細胞生存の尺度としてSYBR Green I(Life Technologies)を使用してDNAを定量した。これは、最初に細胞を50μlの0.2%ドデシル硫酸ナトリウムと共に37℃で2時間インキュベートして、細胞を溶解させることによって行った。次いで、150μLのSYBRグリーン溶液(水中1:750希釈)を各ウェルに添加し、Tecanプレートリーダーを使用して蛍光強度(Ex:497nm/Em:520nm)を測定した。蛍光強度値をGraphPad Prism V5に入力した。ロジスティックス回帰曲線およびEC50を、最高蛍光値を100%生存として、そして最低蛍光値を0%生存として設定することによって決定した。
すべての動物実験は、Temasek Life Sciences Laboratoryの動物実験委員会およびシンガポール国立大学(NUS)によって承認された。雌性NCrヌードマウス(5~8週齢)を(Singapore/InVivos)から購入し、HCT116細胞を用いて皮下異種移植した。HCT116細胞を600cm2の皿(Corning)中で上記のように増殖させ、コンフルエンスが、~80%に達したときに各皿を使用して5匹のマウスに移植した。
遊離BI 2536(0.1N HCl、生理食塩水に溶解)または指示されたリポソームBI 2536製剤を、HCT116を移植した7日後に緩徐な尾静脈注射により投与した。腫瘍体積は、少なくとも150mm3であり、長さ×幅2×0.5を用いて計算された。すべての測定は、ノギスを用いて行われ、マウスの体重は、一日おきに測定された。マウスが完全に水分補給されることを確実にするために、治療後5日間、毎日マウスに1mlのハートマン溶液で皮下的に水分補給した。
HCT116異種移植片を有するマウスを、指示された遊離BI 2536またはリポソームBI 2536製剤で処置し、処置後の指示された時点で安楽死させて、心臓、腫瘍、筋肉、腎臓、肝臓および脾臓を採取した。臓器を秤量し、組織処理の前に-80℃で保存した。組織を、カオトロピック8M尿素(Vivantis)中に浸漬し、そして直径0.5mmのジルコニアビーズ(Biospec)を使用してBertin Homogenizer中で均質化することによって処理した。均質化した組織をベンチトップ遠心分離機にて最高速度で1時間回転させ、100μlのノナノールを用いて1時間穏やかに回転させて、BI 2536の抽出のために800μlの上清を回収した。ノナノールおよびスクロースを短時間の遠心分離によって相分離させ、20μlのノナノール層を、Tecanプレートリーダーを用いて、蛍光測定(Ex:330nm/Em:370nm)によって読み取った。標準曲線と比較することによってBI 2536を定量し、次いで、組織の重量に対して正規化した。
処置後の指示された日に、腫瘍組織収集のためにマウスを屠殺した。腫瘍組織をOCT培地(Sakura Finetek)中で凍結し、切片化の前に-80℃で保存した。CM3050Sクライオスタット(Leica)を用いて、10μmの腫瘍組織切片を得た。切片化した組織をメタノール中で固定し、そして、直ちにヘマトキシリンおよびエオシン(H&E)で染色した。H&E染色を行うために、最初に腫瘍切片をろ過したハリス溶液(Sigma)で過剰染色し、流水(水道水)で洗浄し、酸-アルコール(1%塩酸、70%エタノール)に浸し、さらに水道水で洗浄した。次いで、組織切片を、青味が出るまで0.2%アンモニア水(Sigma)に浸した。水道水で10分間洗浄した後、組織切片をエオシン-フロキシン(それぞれSigmaおよびMerck)で染色し、95%エタノールに浸して過剰の染色を洗い流した。組織切片を一晩乾燥させた後、Permount(Fisher)でマウントした。すべてのH&E染色切片を観察し、DXM 1200Fカメラ(Nikon)および63倍対物レンズと組み合わせたAxioplan 2顕微鏡(Carl Zeiss、Inc)を使用して明視野画像を取得した。
最初にBI 2536をエタノールに溶解し、次いで、スピン乾燥により1.5mlの微量遠心管上にコーティングした。次に、BI 2636でコーティングしたチューブを高真空下で一晩さらに乾燥させた後、指示された塩溶液に再懸濁して、最終濃度500μMを達成した。コーティングされたBI 2636の完全な溶解を確実にするために、チューブを短時間ボルテックスし、特徴決定に使用する前に1分間浴超音波処理に付した。
1mlの指示された緩衝液から、100μlの1-ヘキサノール(Merck)で、1時間の短時間の振盪によりBI 2536を抽出した。ヘキサノールおよび緩衝液を短時間の遠心分離によって分離させ、20μlのヘキサノール層を、Tecanプレートリーダーを用いて、蛍光測定(Ex:330nm/Em:370nm)ついて分析した。
漏出したBI 2536の蛍光の脱消光を、リポソーム不安定性の尺度として使用した。すべての蛍光読み取りは、Tecanプレートリーダー(Ex:280nm、Em:385nm)を用いて行った。リポソームからBI 2536を完全に放出させて、0.2%の最終濃度を達成するために、Triton-X100(シグマ)を添加し、蛍光の読み取りを再度行った。放出されたBI 2536の割合を計算するために、トリトン添加前の蛍光測定値をトリトン添加後の蛍光測定値で割った。安定性測定を行うために、リポソーム製剤を水または600mMスクロース溶液のいずれかで50倍希釈し、37℃でのインキュベートの開始時および12時間後に、Tecanプレートリーダーを用いて蛍光を測定した。長期安定性を決定するために、リポソームを水または600mMのスクロースで希釈した後に、37℃のインキュベーター内に保存し、指示された日に同様の方法で蛍光測定を行った。
アニオンの同一性および濃度がBI 2536の物理化学的性質に影響を与える範囲は、pH3に調整された以下の溶液を用いて研究された:クエン酸ナトリウム(C)、酢酸ナトリウム(A)、リン酸ナトリウム(P)、2-(N-モルホリノ)エタンスルホン酸(M)および塩酸(H)。これらの溶液からBI 2536がヘキサノールに分配する傾向を、さまざまな濃度で測定した(図1)。アニオンの同一性がヘキサノール抽出の効率に影響を及ぼし、そして、この効率がアニオン濃度依存的に増加(P、A)または減少(M、C、H)することも観察された(図2)。さらに、これらのアニオンのペアにした組み合わせを使用することによって、これらのヘキサノール-抽出効率曲線の多様性をさらに高めることができることが観察された(図3)。これらの結果から、これらのアニオンを組み合わせて使用して、放出速度が異なるリポソーム製剤のライブラリーを作製することができると推論された。
放出速度が最も遅い2つのアニオンは、クエン酸塩単独とクエン酸塩とリン酸塩の組み合わせであるため(図4)、クエン酸塩:リン酸塩比を変化させることを研究して、それが有効性において実質的効果を有するかどうかを決定した。0:1、1:3、1:1、3:1および1:0の比を包含する製剤を、放出速度およびEC50について以前と同じ方法で試験した。これらの変数が細胞傷害性アッセイの第3日または第8日のどちらで測定されたかにかかわらず、放出速度およびEC50に関して同じ傾向が一貫して観察された(図5A)。興味深いことに、最も遅い放出は、クエン酸塩:リン酸塩比1:3で達成され、この組み合わせ比は、相乗的であって、単なるクエン酸塩単独とリン酸塩単独の平均された結果ではないことを明らかにすることが注目された。この結果はさらに、実際の固形腫瘍におけるインビボでの有効性を最大にするために、最適なクエン酸塩:リン酸塩比を特定することが重要であることを示唆した。この比を特定するために、確立されたヒト大腸癌異種移植片を有するマウスを、クエン酸塩:リン酸塩比1:7、1:4、1:3、1:2および1:0.5の範囲を網羅するリポソームで処理した。以前に観察された1:3の比と一致して、1:3および1:4のクエン酸塩:リン酸塩比で最良のインビボ有効性が観察された(図5B)。重要なことには、これらの比に対する腫瘍体積の減少は、2週間にわたって持続し、この観察はこれらの超安定リポソームからの予想される遅い放出と一致する。1:4の比は、1:3より大きい抗腫瘍効果をもたらしたが、それはまた、より多い重量減少をもたらした。したがって、1:3が、その後の実験のための最適比として採用された。このように封入されたBI 2536は「超安定」と呼ばれる。
HCT116異種移植腫瘍を有するヌードマウスを、超安定リポソームBI 2536、またはアニオンとしてクエン酸塩もしくはリン酸塩のみのいずれかを含むリポソームBI 2536の単回静脈内注射で処置した。遊離のカプセル化されていないBI 2536を対照として使用した。超安定リポソームで処置された異種移植片は、12日間にわたって体積が減少し、我々の以前の動物実験で観察された長期の治療効果を再現した(図6A)。比較すると、クエン酸塩またはリン酸塩のみを使用するリポソームは、遊離の薬剤と区別することができず、アニオンの組み合わせが、どちらのアニオンだけでは説明されない相乗効果をもたらすことを実証した。超安定BI 2536で処置されたマウスは、処置後の体重が重い傾向にあり、これは薬剤放出の延長と共に予想される毒性よりも毒性が低いことと一致する傾向である。重要なことには、超安定BI 2536は、マウスの生存を有意に改善し、10匹中2匹のマウスで完全寛解が観察された(表1)。他の実験群では、完全寛解は観察されなかった。
超安定リポソームで観察された改善された有効性についての合理的な説明は、通常のペグ化リポソームと比較して薬剤の半減期が改善されることである。超安定リポソームの単回投与で処置されたヌードマウスは、クエン酸塩またはリン酸塩単独のいずれかを含有する対照リポソームと比較して、BI 2536の腫瘍中濃度が有意に高かった(図7A;表2)。この傾向は、腫瘍体積の減少が組織加工を非実用的にした後、測定の9.5日間全体にわたって持続した。超安定リポソームBI 2536への腫瘍曝露(曲線下面積で測定)は、クエン酸塩リポソームと比較して5倍高く、リン酸塩リポソームと比較して3倍高かった。健康な組織(脾臓、筋肉、腎臓、心臓、肝臓)中の薬剤濃度は、超安定リポソームについても同様に上昇したが、この傾向は10時間後には統計的に有意ではなかった(図7A;表2)。これらのより高い組織濃度にもかかわらず、超安定リポソームは、対照リポソームよりも毒性が低く、これは、健康な組織を循環している間、超安定リポソーム中のBI 2536の大部分が安全に封入されたままであることを示唆する。曲線下面積の一般的な増加と合わせて考慮すると、データから、超安定カプセル化は、BI 2536の循環半減期を増加させ、結果として、腫瘍区画内でのBI 2536の灌流および保持を増強させることが示唆される(図7B)。BI 2536(または任意の抗有糸分裂化学療法)の特徴は、腫瘍における有糸分裂を阻害するその能力にある。BI 2536の、より高いバイオアベイラビリティが超安定リポソームと対照との間の差異を説明することができるかどうかという問題を調べるために、単回投与の1.5および5.5日後の異種移植片上の腫瘍サンプルの組織学的分析を行った(図7Cおよび7D)。第1.5日では、封入されたBI 2536のすべてのリポソーム製剤および遊離BI 2536が、組織学的切片中の約25%の腫瘍核において観察される有糸分裂像と関連があった。しかしながら、第5.5日では、超安定リポソームBI 2536が、対照リポソームおよび遊離薬剤と比較して、有糸分裂停止細胞の有意に高い割合と関連があった。この延長された一時的なバイオアベイラビリティは、超安定的に封入されたBI 2536の改善された有効性を説明すると考えられる。
本発明はまた、以下の態様および実施態様を含む。
[1] 封入された抗有糸分裂剤が、リポソームが600mMスクロース中に懸濁されている場合、12時間で0.6%未満または8日間で5%未満である遅い速度で放出される、内部環境に封入された抗有糸分裂剤、1つ以上のアニオンおよび1つ以上のカチオンを含む、膜によって外部環境から分離された内部環境を含む超安定リポソーム。
[2] 1つ以上のアニオンが、クエン酸塩、酢酸塩、リン酸塩、2-(N-モルホリノ)エタンスルホン酸塩、塩化物、クエン酸塩と酢酸塩、クエン酸塩と2-(N-モルホリノ)エタンスルホン酸塩、クエン酸塩と塩化物、酢酸塩とリン酸塩、酢酸塩と2-(N-モルホリノ)エタンスルホン酸塩、酢酸塩と塩化物、リン酸塩と2-(N-モルホリノ)エタンスルホン酸塩、リン酸塩と塩化物、2-(N-モルホリノ)エタンスルホン酸塩と塩化物である、[1]に記載の超安定リポソーム。
[3] 2つのアニオンが、約1:7~約7:1の比で存在する、[1]もしくは[2]に記載の超安定リポソーム。
[4] 1つ以上のアニオンが、クエン酸塩とリン酸塩またはクエン酸塩と酢酸塩または酢酸塩とリン酸塩である、[3]に記載の超安定リポソーム。
[5] 1つ以上のカチオンが、ナトリウム、アンモニウム、トリエチルアンモニウム、銅、マグネシウム、亜鉛または鉄である、[1]~[4]のいずれか1つに記載の超安定リポソーム。
[6] 抗有糸分裂剤が、BI 2536、ON01910、GSK 461364、HMN 214またはBI 6727などのポロ様キナーゼ阻害剤;イスピネシブ(SB 715992)、SB 743921、MK 0731またはARRY 520などのキネシンスピンドル阻害剤;またはMK 0457 (VX 680)、AZD 1152、PHA 680632、PHA 739358、MLN8054、MLN8237、R763、AT9283、SNS 314、SU 6668、ENMD 2076、BI 811283、CYC116、ENMD 981693またはMKC 1693などのオーロラキナーゼ阻害剤である、[1]~[5]のいずれか1つに記載の超安定リポソーム。
[7] 抗有糸分裂剤が、BI 2536またはイスピネシブである、[6]に記載の超安定リポソーム。
[8] 膜が、1,2-ジステアロイル-sn-グリセロ-3-ホスホエタノールアミン-N-[メトキシ(ポリエチレングリコール)-2000](DSPE-PEG2000)、水素化卵L-α-ホスファチジルコリン(HEPC)およびコレステロール(Chol)を含む、[1]~[7]のいずれか1つに記載の超安定リポソーム。
[9] 膜が、50:45:5のモル比でHEPC:Chol:DSPE-PEG2000を含む、[8]に記載の超安定リポソーム。
[10] 水性媒体中に、[1]~[9]のいずれか1つに記載の超安定リポソームを含む、リポソーム組成物。
[11] [1]~[9]のいずれか1つに記載の超安定リポソームを含む、医薬組成物。
[12] 組成物が、少なくとも1つの薬学的に許容される賦形剤および/または担体をさらに含む、[11]に記載の医薬組成物。
[13] 治療有効量の[1]~[9]のいずれか1つに記載の超安定リポソーム、または[10]に記載のリポソーム組成物、または[11]もしくは[12]に記載の医薬組成物を、治療を必要とする対象に投与することを含む、対象の癌を治療する方法。
[14] 対象における癌を治療するための医薬の製造のための、[1]~[9]のいずれか1つに記載の超安定リポソームの使用。
[15] 対象における癌を治療するための、治療有効量の[1]~[9]のいずれか1つに記載の超安定リポソーム、または[10]に記載のリポソーム組成物、または[11]もしくは[12]に記載の医薬組成物の使用。
[16] 対象における癌を治療するための医薬の製造における使用のための、[1]~[9]のいずれか1つに記載の超安定リポソーム。
[17] 対象における癌の治療における使用のための[1]~[9]のいずれか1つに記載の超安定リポソーム、または[10]に記載のリポソーム組成物、または[11]もしくは[12]に記載の医薬組成物。

Claims (12)
- 膜によって外部環境から分離された内部環境を含む超安定リポソームであって、
内部環境が、BI 2536またはイスピネシブである抗有糸分裂剤;1:3~1:7の比のクエン酸塩とリン酸塩、または1:3~3:1の比のクエン酸塩と酢酸塩より選択される2つのアニオン;および1つ以上のカチオンを封入し、
膜が、水素化卵L-α-ホスファチジルコリン(HEPC)、コレステロール(Chol)および1,2-ジステアロイル-sn-グリセロール-3-ホスホエタノールアミン-N-[メトキシ(ポリエチレングリコール)-2000](DSPE-PEG2000)を含み、HEPC:Chol:DSPE-PEG2000のモル比が50:45:5であり、
リポソームが、37℃にて600mMスクロース中に懸濁させ、インビトロでインキュベートしたとき、封入された抗有糸分裂剤がリポソームから12時間で0.6%未満または8日間で5%未満放出される遅い放出物性を有する、超安定リポソーム。 - 2つのアニオンが、クエン酸塩とリン酸塩である、請求項1に記載の超安定リポソーム。
- 1つ以上のカチオンが、ナトリウム、アンモニウム、トリエチルアンモニウム、銅、マグネシウム、亜鉛または鉄である、請求項1または2に記載の超安定リポソーム。
- 抗有糸分裂剤が、BI 2536である、請求項1~3のいずれか1つに記載の超安定リポソーム。
- 抗有糸分裂剤が、イスピネシブである、請求項1~3のいずれか1つに記載の超安定リポソーム。
- 2つのアニオンが、1:3の比のクエン酸塩とリン酸塩である、請求項1~5のいずれか1つに記載の超安定リポソーム。
- 水性媒体中に、請求項1~6のいずれか1つに記載の超安定リポソームを含む、リポソーム組成物。
- 請求項1~6のいずれか1つに記載の超安定リポソームを含む、医薬組成物。
