Description
操作されたT細胞及びex vivoで増殖(expanded)又は選択されたT細胞を用いる療法を包含する、腫瘍特異的T細胞に基づく免疫療法が、抗腫瘍治療について研究されている。いくつかの場合において、このような療法に使用されるT細胞は、十分長い期間、in vivoで活性を維持しない。いくつかの場合において、T細胞の腫瘍特異性は比較的低い。いくつかの場合において、操作されたT細胞は腫瘍へのアクセスが不十分である。従って、より効果的な抗腫瘍機能を有する腫瘍特異的癌治療のための当該分野での必要性が存在する。
本明細書に記載されるのは、T細胞(例えば、CAR T細胞、腫瘍浸潤リンパ球(tumor infiltrating lymphocytes)(「TIL」)、TCR操作されたT細胞、又はT細胞クローン)を含む組成物を患者の脳脊髄液(「CSF」)に投与することによって中枢神経系の悪性腫瘍を治療する方法である。T細胞には、例えば、所望のレセプターを発現する核酸分子の導入により、単離されたもしくは遺伝子改変されたT細胞のex vivoでの増殖(expansion)により、又は患者もしくはドナーから得られたT細胞のサブセットのex vivo選択により、又はこれらの技術の2つ以上の組み合わせにより、操作されたT細胞が含まれる。CNSへの投与は、例えば、脳室系(ventricular system)又は脊柱の中心空洞への投与によって達成することができる。当該用語が本明細書中で使用される場合、CNSへの投与は、腫瘍内投与(腫瘍自体への注射又は注入)及び腫瘍の切除によって作製された空腔への投与の両方とは異なる。しかし、本明細書に記載のCNS投与方法は、腫瘍内(intraturmoral)投与及び/又は切除後の腔内投与と組み合わせることができる。

以下に、種々のCAR T細胞の構造、構築及び特徴付け、及び中枢神経系の癌の治療におけるそれらの使用について説明する。キメラ抗原(CAR)は、最低でも、細胞外認識ドメイン、膜貫通領域、及び細胞内シグナル伝達ドメインを含有する組換え生体分子である。したがって、用語「抗原」は、抗体に結合する分子に限定されずに、標的に特異的に結合することができるいずれの分子に限定される(but to)。例えば、CARは、細胞表面レセプターに特異的に結合するリガンドを包含することができる。細胞外認識ドメイン(細胞外ドメイン又は単にそれが含有する認識要素とも呼ばれる)は、標的細胞の細胞表面上に存在する分子に特異的に結合する認識要素を含む。膜貫通領域は膜中にCARを固定する。細胞内シグナル伝達ドメインは、ヒトCD3複合体のζ鎖由来のシグナル伝達ドメインを含み、必要に応じて1つ以上の共刺激シグナル伝達ドメインを含む。CARは、MHC制限とは両方とも無関係に、抗原に結合し、且つ、T細胞活性化を形質導入することができる。したがって、CARは、そのHLA遺伝子型にかかわらず、抗原陽性腫瘍を有する患者集団を治療することができる「普遍的な(universal)」免疫レセプターである。腫瘍特異的CARを発現するTリンパ球を用いた養子免疫療法は、癌治療のための強力な治療戦略となり得る。
有用なIL13Rα2特異的CARの構造を以下に記載する。コドン最適化CAR配列は、IL13Rα1、Fcレセプター媒介認識モデルを大きく減少させる2つの突然変異(L235E; N297Q)を含有するIgG4 Fcスペーサー、CD4膜貫通ドメイン、共刺激4-1BB細胞質シグナル伝達ドメイン、及びCD3ζ細胞質シグナル伝達ドメイン、への潜在的結合を低減するために、単一部位(E13Y)で突然変異した膜テザード(tethered)IL-13リガンド、を含有する。T2Aリボソームスキップ配列は、このIL13(EQ)BBζCAR配列を、不活性で非免疫原性の細胞表面検出/選択マーカーであるCD19tから分離する。このT2A結合は、単一の転写産物からのIL13(EQ)BBζ及びCD19tの両方の同調発現をもたらす。図1Aは、IL13(EQ)BBZ-T2ACD19t構築物をコードする2670ヌクレオチドのオープンリーディングフレームの概略図である。この図において、前記IL13(EQ)BBZ CARの、IL13Rα2特異的リガンドIL13(E13Y)、IgG4(EQ)Fc、CD4膜貫通、4-1BB細胞質シグナル伝達、3-グリシンリンカー、及びCD3ζ細胞質シグナル伝達ドメイン、並びにT2Aリボソームスキップ及び短縮型CD19配列はすべて示されている。ヒトGM-CSFレセプターα、及びIL13(EQ)BBZ CAR及びCD19tの表面発現を駆動するCD19シグナル配列も示される。従って、IL13(EQ)BBZ-T2ACD19t構築物は、IL13Rα2特異的ヒンジ最適化共刺激キメラ免疫レセプター配列(IL13(EQ)BBZと呼ばれる)、リボソームスキップT2A配列及びCD19t配列を包含する。
pHIV7プラスミドは親プラスミドである。当該親プラスミドから、臨床ベクターIL13(EQ)BBZ-T2A-CD19t_epHIV7がCity of Hope(COH)のT細胞治療研究研究所(TCTRL)において得られたCARの発現に用いたepHIV7ベクターは、pHIV7ベクターから作製した。重要なことに、このベクターは、CARの発現を駆動するためヒトEF1プロモーターを使用する。ベクターの5’及び3’配列は、以前はHXBc2プロウイルス由来のpv653RSN由来であった。ポリプリントラクトDNAフラップ配列(cPPT)は、NIH AIDS Reagent RepositoryからのHIV-1 pNL4-3株由来であった。ウッドチャック転写後調節エレメント(WPRE)配列は以前に記載されている。
ステップ1では、5’LTRからrev応答エレメント(RRE)までの配列がp5’HIV-1 51を作成し、次いで前記5’LTRが、TATAボックスの上流の配列を除去することによって改変され、及び最初にCMVエンハンサーに、次いでSV40複製起点にライゲーションした(p5’HIV-2)。ステップ2では、p3’HIV-1を作製するため3’LTRをpBluescriptにクローニングした後、HIV U3中のcis調節エレメントを除去し、p3’HIV-2を形成するため、3’LTRエンハンサー/プロモーターにおける400bpの欠失を行った。ステップ3では、pHIV-3を作成するため、p5’HIV-3及びp3’HIV-2から単離された断片をライゲーションした。ステップ4では、前記p3’HIV-3を生成するために、余分な上流のHIV配列を除去することによりp3’HIV-2をさらに改変し、p3’HIV-4を作成するため、WPREを含有する600bpのBamHI-SalIフラグメントをp3’-3に添加した。ステップ5では、pHIV-3RREをPCRによりサイズを減少させ、及び、pHIV-6を作製するため、pHIV-3由来の5’断片に(図示せず)、及びp3’HIV-4にライゲーションした。ステップ6では、HIV-1 pNL4-3(55)由来のcPPT DNAフラップ配列を含有する190bpのBglII-BamHI断片をpNL4-3から増幅し、pHIV-7を作製するため、pHIV6におけるRRE配列とWPRE配列との間に配置した。この親プラスミドpHIV7-GFP(GFP、グリーン蛍光タンパク質(green fluorescent protein))は4-プラスミド系を用いて親ベクターをパッケージングするのに使用された。
1)pCgpはウイルスベクター構築に必要なgag/polタンパク質をコードする;
2)pCMV-Rev2は、効率的なパッケージングのためにウイルスゲノムの輸送を助けるためにRRE配列に作用するRevタンパク質をコードする;
3)pCMV-Gは、ウイルスベクターの感染性のために必要とされる水疱性口内炎ウイルス(vesiculo-stomatitis virus)(VSV)の糖タンパク質をコードする。
pCgpヘルパープラスミドのgag/pol配列中に位置する約600ヌクレオチドのパッケージングシグナル領域;
全部で3つのヘルパープラスミド中のCMVプロモーター配列;及び
ヘルパープラスミドpCgp中のRRE配列;
を包含する。複数の組換え事象を必要とするため、これらの領域における相同性に起因して複製能力のある組換えウイルスが生成される可能性は極めて低い。さらに、得られたいずれの組換え体は、レンチウイルス複製に必要な機能的LTR及びtat配列を欠いているであろう。
各プラスミド(IL13(EQ)BBZ-T2A-CD19t_epHIV7;pCgp;pCMV-G;及びpCMV-Rev2)について、十分な量のプラスミドDNAを産生するために発酵槽に接種するために使用されるシードバンクが生成される。プラスミドDNAは、レンチウイルスベクターの産生に使用する前に、同一性、無菌性及びエンドトキシンについて試験される。
Tリンパ球は、白血球フェレーシス(leukopheresis)によって患者から得られる。適切な同種異系又は自己T細胞サブセット、例えばセントラルメモリーT細胞(TCM)は、CARを発現するように遺伝的に改変され、次いで、抗癌治療を達成するために、臨床上許容されるいずれ手段により前記患者に戻るように投与される。
実施例4に記載されている2つの前臨床的適格性評価試験製品は、以下に記載するように、前臨床研究において用いられた。図6A~Cは、表面導入遺伝子及びT細胞マーカー発現のフローサイトメトリー分析の結果を示す。IL-13(EQ)BBγ/CD19t+ TCM HD006.5及びHD187.1を、CD8+ CAR+及びCD4+(すなわちCD8陰性)CAR+細胞を検出するため、抗IL-13-PE及び抗CD8-FITCで同時染色した(図6A)。又はCD4+ CD19t+及びCD8+(すなわち、CD4陰性)CAR+細胞を検出するため、抗CD19-PE及び抗CD4-FITCで同時染色した(図6B)。IL13(EQ)BBγ/CD19t+ TCM HD006.5及びHD187.1を、蛍光色素結合抗CD3、TCR、CD4、CD8、CD62L及びCD28(グレーのヒストグラム)又はアイソタイプ対照(controls)(黒いヒストグラム)で染色した。(図6C)。図6A~Cのそれぞれにおいて、示されたパーセンテージは、アイソタイプ上に染色された生存リンパ球(DAPI陰性)に基づく。
以下は、侵襲性原発性PBT系に対する抗腫瘍活性について、送達経路、静脈内(i.v.)又は頭蓋内(i.c.)、を比較する調査である。パイロット研究(データは示されていない)において、意外にも、i.v.投与されたIL13(EQ)BBζ+ TCMは、小さい(5日目の)PBT030-2 EGFP:ffLuc腫瘍の治療のためにPBSと比較して治療上の有益性をもたらさなかったことが観察された。これは、i.c.投与CAR+ T細胞で観察された頑健な治療効果とは対照的である。PBT030-2腫瘍は5日目に末梢から治療用T細胞を動員するには小さすぎる可能性があると推測し、大きな19日目のPBT030-2 EGFP:ffLuc腫瘍に対するi.v.送達とi.c.送達との比較を行った。これらの研究のために、PBT030-2移植マウスを、IL13(EQ)BBZ+ TCMの又は模擬TCM(CARなし)の、2回のi.v.注入(5×106 CAR+ TCM;19日目及び26日目)又は4回のi.c.注入(1×106 CAR+ TCM;19日目、22日目、26日目及び29日目)のいずれかで処理した。ここでも、i.v.投与されたCAR+ T細胞についてのXenogenイメージング又はKaplan-Meier生存分析によって、治療上の利益はモニタリングされない(図7Aおよび7B)。対照的に、強力な抗腫瘍活性が、i.c.投与されたIL13(EQ)BBζ+ TCMについて観察された(図7A~B)。次に、T細胞注射の7日後のマウスコホートからの脳を採取し、IHCによってCD3+ヒトT細胞について評価した。驚くべきことに、モックTCM又はIL13(EQ)BBζ TCMのいずれかでi.v.処置したマウスでは、腫瘍において、又はヒトT細胞が典型的に存在する、他のマウス脳領域において(すなわち軟髄膜)、検出可能なCD3+ヒトT細胞はなかった(図7C)。腫瘍指向性の欠損を示唆している。これは、i.c.治療マウスで検出された有意な数のT細胞とは対照的である(図7D)。

図22Aは、図20及び図21に示される2つのHER2特異的CAR構築物の概略図である。HER2(EQ)28ζにおいて、scFvは、CD28膜貫通ドメイン、細胞内CD28共刺激ドメイン及び細胞溶解性CD3ζドメインを含有する改変IgG4 Fcリンカー(二重変異体、L235E;N297Q)によって膜に繋がれている。T2Aスキップ配列は、細胞トラッキングのために使用される切断されたCD19(CD19t)タンパク質からCARを分離する。HER2(EQ)BBζは、共刺激ドメインがCD28ではなく4-1BBであり、且つ、膜貫通ドメインがCD28膜貫通ドメインではなくCD8膜貫通ドメインであることを除いて同様である。ヒトセントラルメモリー(TCM)細胞を、HER2(EQ)28ζ又はHER2(EQ)BBζのいずれかを発現するレンチウイルスベクターでトランスフェクトした。図22Bは、ヒトTCM表面表現型の代表的なFACSデータを示す。図22Cは、HER2(EQ)28ζ又はHER2(EQ)BBζのいずれかを発現するレンチウイルスベクターでトランスフェクトしたTCMにおけるCD19及びプロテインL発現についてのアッセイの結果を示す。これらの結果からわかるように、CD19発現によって評価したトランスフェクション効率は、両方のCARについて同様であった。しかし、タンパク質L発現は、HER2(EQ)BBζのほうがHER2(EQ)BBζより低かったが、HER2(EQ)BBζCARはHER2(EQ)BBζより安定性が低いことが示唆された。細胞増殖の分析(図22D)は、CARはどちらもT細胞増殖を妨害しないことを示す。
HER2陰性系統(LCLリンパ腫、MDA-MB-468、U87グリオーマ)、低HER2発現系統(MDA-MB-361,231BR)及び高HER2発現系統(SKBR3、BT474、BBM1)を包含する様々な乳癌細胞株が、HER2(EQ)28ζ及びHER2(EQ)BBζの特徴付けに使用された。図23Aは、これらの系統の各々のHER2発現レベルを示す。MDA-MB-361腫瘍細胞(低HER2発現)又はBBM1腫瘍細胞(高HER2発現)のいずれかと5時間の同時培養後に、フローサイトメトリー(ゲートオンした(gated on)CAR+ T細胞)を、Mock(非形質導入)、HER2(EQ)28ζ又はHER2(EQ)BBζCAR T細胞におけるCD107a脱顆粒及びIFNγ産生を特徴付けるために用いた。この分析の結果を図23Bに示す。他の乳癌細胞株についても同様の研究を行い、その結果を図23Cに要約する。組換えHER2タンパク質又は腫瘍標的による24時間培養後のHER2-CAR T細胞によるIFNγ産生の産生をELISAによって測定し、この分析の結果を図23Dに示す。
Mock(非形質導入)、HER2(EQ)28ζ又はHER2(EQ)BBζCAR T細胞を腫瘍標的と72時間共培養した後の腫瘍細胞死滅を評価するのに、フローサイトメトリーを用いた。この分析の結果を図24Aに示す。HER2陰性MDA-MB-468又はHER2陽性BBM1細胞との72時間の同時培養後、総CAR T細胞におけるPD-1及びLAG-3誘導を測定した。この分析の結果を図24Bに示す。HER2陰性(LCLリンパ腫、MDA-MB-468、U87グリオーマ)、低HER2発現(MDA-MB-361,231BR)又は高HER2発現(SKBR3、BT474、BBM1)である腫瘍標的との72時間の共培養後のCD8+ CAR T細胞におけるPD-1誘導を測定した。この分析の結果を図24Cに示す。これらの研究は、HER2(EQ)BBζが、HER2(EQ)28ζを起こすよりも低いPD-1誘導を生じることを示唆している。エフェクター:腫瘍(E:T)比が0.25:1~2:1の範囲の腫瘍細胞死滅を、HER2(EQ)28ζ又はHER2(EQ)BBζCAR T細胞について測定した。この分析の結果は、図24Dに示されている。図24Dは、HER2(EQ)28ζ及びHER2(EQ)BBζの両方がin vitroでの腫瘍細胞死滅に有効であることを示す。MDA-MB-468又はBBM1細胞との72時間の同時培養後のHER2-CAR T細胞のCFSE増殖を、フローサイトメトリーによって測定した。この分析の結果は、図24Eに示されている。図24Eは、HER2(EQ)BBζCAR T細胞がHER2(EQ)28ζCAR T細胞よりも増殖することを示す。
腫瘍内に送達されたHER2 CAR T細胞の活性を、患者由来の乳房から脳への転移モデルで評価した。図25A~25Cは、腫瘍のH&E染色である。Mock(非形質導入)又はHER2(EQ)BBζCAR T細胞を用いて腫瘍に直接注射してマウスを処置した。図25D~25Fは、腫瘍の光学イメージングの結果を示し、図25G~25Iは、腫瘍注射後3日目、8日目又は14日目のいずれかで局所的に処置したマウスのKaplan-Meier生存曲線である。これらの研究は、HER2(EQ)BBζCAR T細胞が、腫瘍に直接注射された場合、in vivoで強力な抗腫瘍効果を有することを示している。
2つの異なる頭蓋内(ic)送達経路、即ち腫瘍内(ict)及び脳室内(icv)を、IL13(EQ)BBZ-T2A-CD19t_epHIV7レンチウイルスベクターで形質導入され、且つ、グリア芽腫(GBM)の臨床的処置のために提案されたようにin vitroで増殖された、TCMに富んだ細胞株から生成されたCAR T細胞のin vivo安全性、輸送(trafficking)及び有効性のための神経膠芽腫のネズミモデルで評価した。ict又はicvのいずれかで投与されたこれらの細胞のin vivo安全性および機能的効力を、ホタルルシフェラーゼ(ffLuc)レポーター遺伝子を発現するように操作された、IL13Rα2+原発性低継代(low-passage)GBMの腫瘍様塊(tumor sphere)系統PBT030-2を用いて、免疫不全NSGマウスにおいて調べた。
この実施例は、再発性IL13Rα2+ GBMを有する患者における自己IL13BBζ Tcmの毎週の頭蓋内注入の安全性、実現可能性及び生物活性を評価する臨床試験の初期知見を記載する。以下にさらに詳細に説明するように、登録患者は自己PBMCを採取するために白血球アフェレーシス(leukapheresis)を受け、且つ、IL13BBζ+ Tcmの製造と同時に、リザーバ/カテーテル装置を配置して腫瘍生検又は切除を行う。ベースラインのMR及びPETイメージング及び手術からの回復に続いて、患者は、IL13BBζ+ Tcmの3週間の頭蓋内注入、続いて毒性及び疾患評価のための1週間の休憩からなる4週間の治療レジメンで治療される。現在までの、3名の切除患者のこの第1の低用量コホートについての結果は、IL13BBζ Tcmの外科的切除後の局所的送達は安全であり、且つ、観察された治療に起因するグレード3以上の毒性がなく耐容性が高いことを示唆し、且つ、重要なことに、CAR T細胞投与後の抗腫瘍活性の早期証拠を示す。サンプルが入手可能であった全ての患者について、CAR T細胞は、処置後少なくとも7日間、フローサイトメトリーによって腫瘍嚢胞液または脳脊髄液(CSF)中に検出された。特に興味深い1人の患者は、再発性多巣性GBM(脊椎の1つの転移部位及び広範な軟膜疾患を包含する)を呈した。この患者は、最初にプロトコルごとに、後頭側頭-後頭部領域における最も大きな再発性腫瘍病巣(focus)の切除腔内への、IL13BBζ Tcmの6回の局所注入をによって、治療された。促進するように(Encouragingly)、このCAR T細胞注射部位は、7週間以上にわたり疾患再発の証拠なしで安定したままであったが、前記CAR T細胞注射部位から離れた他の病巣は進行し続けた。次いでこの患者を、IL13BBζ Tcmの5週間の(five weekly)脳室内注入を用いて、他の治療的介入なしに特例使用プロトコルで処置した。最終的な脳室内CAR T細胞注入の1週間後、全ての頭蓋内腫瘍および脊髄腫瘍は退縮し、大部分が75体積%を超えて減少し、この患者はCAR T細胞治療の開始後4ヶ月で臨床的に安定したままであった。

50歳の男性は、最初に大発作を呈した後、右頭葉に低悪性度脳腫瘍と診断された。4カ月間のモニタリング後、この右側頭腫瘍はMRIによる増強を示した。前記患者は腫瘍切除を受け、WHOグレードIV神経膠腫(GBM)の診断を確定した。次いで前記患者は、同時のテモゾロミド(毎日140mg)と合わせて59.4コバルトGy相当量の総線量まで、標準治療アジュバント陽子線照射を、続いて、Novocure装置(NovoTTF-100A)を併用してテモゾロミド4サイクルを3ヶ月間受けた。原発腫瘍切除術後6ヵ月後、PET及びMRI画像は疾患の進行の証拠を示した。
特例使用プロトコルを包含するこれらの研究は、施設の審査委員会によって承認された。患者は書面によるインフォームドコンセントを提供した。適格性は、現在再発しているIL13Rα2+グレードIV神経膠腫の組織学的に確認された診断、カルノフスキーのパフォーマンスステータス(KPS)>60を有する年齢>18歳、適切な心肺機能、及び生存期待>4週間、を包含した。患者は、参加前少なくとも12週間に初期放射線治療を完了していなければならず、いずれの他の活動性悪性腫瘍、感染症又は間接性疾患を有してはならない。又は、他の治験薬を受けていなければならず、又は、T細胞療法中に2mg TID(3x/day)より多いデキサメタゾンを必要とする。
4-1BB共刺激性、IL13Rα2標的化CAR、IL13BBζをコードするレンチウイルスベクターは本明細書に詳述されている。簡潔には、コドン最適化されたCAR配列は、IL13Rα1、Fcレセプター仲介認識を妨げる2つの突然変異(L235E;N297Q)を含有するヒトIgG4 Fcスペーサー、ヒトCD4膜貫通ドメイン、ヒト共刺激4-1BB細胞質シグナル伝達ドメイン、及びヒトCD3ζ細胞質シグナル伝達への潜在的結合を減少させるため、単一部位(E13Y)で突然変異した膜係留ヒトIL-13リガンドを含有する。次いで、T2Aリボソームスキップ配列は、このIL13BBζCAR配列を、不活性非免疫原性細胞表面マーカーである短縮型ヒトCD19配列(CD19t)から分離する。
脳及び脊椎のポストガドリニウムT1加重MRI配列を、Siemens Viro 3 Teslaスキャナー上で獲得した。Multihance(登録商標)の投与後に得られたアキシャルT1 のMPR加重画像上で病変を測定した。GE Discovery DST HP60 PET-CTスキャナー(70cmアキシャル視野、スライス厚3.75mm)を用いて、18-F-フルオロデオキシグルコース(18-F-FDG)による画像化を行った。最大標準取込値(SUV)は、Vital Images Vitreaバージョン6.7.2ソフトウェアを利用して得た。造影増強腫瘍病巣の領域を、最大腫瘍面積(mm2)の測定のための放射線科医によって概説した。及び腫瘍容積(cm3)を計算した。
濃縮TCM(36×106)をex vivoで刺激し、レンチウィルスで形質導入し、そして17日間で638×106個の全細胞を得るため増殖した。最終的なT細胞の生成物(CD3+及びTCR+)は、CD4(74%)及びCD8(16%)T細胞サブセットからなり、両方の細胞表面タンパク質についての遺伝子改変共染色と共にIL13BBζとCD19tを発現した(図39A)。CAR T細胞生成物は、CD45RO(97%)、CD62L(57%)、CCR7(28%)、CD28(97%)及びCD27(59%)を発現する、セントラルメモリーT細胞の表現型を示した(図39A)。この製品はまた、TIM-3(65%)及びLAG-3(49%)を包含するが、PD-1及びKLRG1の有意なレベルを包含しない、いくつかの疲弊(exhaustion)マーカーを示した(図39A)。
患者は、2つの異なる頭蓋内送達経路、即ち腫瘍切除後の腔内(ICT)送達(サイクル1~6)及び脳脊髄液(CSF)への脳室内(ICV)送達(サイクル7~11)である、を介してリザーバ/カテーテル装置を介して投与されたIL13BBζ TCMの毎週の注入によって治療された。10×106 CAR+ T細胞の最大細胞用量での11回の頭蓋内注入は、観察された治療に対する可能性又はそれ以上の帰属を伴う、グレード3以上の毒性(NCI共通毒性基準)なしで良好に耐容された。CAR T細胞注入後に認められる軽度の事象には、以下が含まれる。
治療の時点で、患者の腫瘍は、不良な予後特徴を有する非常に攻撃的な再発性GBMの特徴を示した。これには、標準治療後の治療から6ヶ月以内の一次診断からの再発の証拠、脊髄病変及び広範囲の軟部腫瘍疾患を包含する、多病巣性腫瘍病変の提示(図38)、脱分化型GBMの組織学的特徴、及び60%を超える細胞がKi67陽性に染色された高い腫瘍増殖率(図37B)が含まれた。切除された腫瘍組織のFFPEに対するIHCによって評価されたIL13Rα2の腫瘍発現は、それぞれ100及び80のHスコアを有する原発腫瘍と再発腫瘍との間で類似していた(図37)。再発腫瘍の腫瘍内IL13Rα2発現は不均一であった。細胞の10%が高い染色強度(2-3+)を示し、60%が低発現(1+)を示し、細胞の30%がバックグラウンド(0+)よりも染色されなかった。潜在的な関心事の中で、最も高いレベルのIL13Rα2発現は、他のGBM腫瘍について言及された発現パターンである、偽柵構造(pseudopalisading)壊死の腫瘍領域においてしばしば観察された(図37A)。

IL3BBζ TCMのICV注入(サイクル6~11)後に観察された抗腫瘍応答に関連する免疫学的変化を解明するために、CSFを細胞浸潤、CAR+ T細胞持続性、及びサイトカインレベルについて評価した。各ICV注入の直後に(すなわち、サイクル6~11の第1-2日目)、CSFのmL当たりの細胞数は、プレ注入レベル(各サイクルの0日目)と比較して7.0±3.6倍増加し、且つ、プレICV(C7D0)レベルと比較して153±128倍増加した(図42)。CSF中の総細胞数は、典型的には、7日間の治療サイクルにわたって減少した。C9D2について評価したところ、細胞浸潤物は、CD14+ CD11b+ HLA-DR+成熟骨髄集団と同様に、内因性及びCAR発現性の両方の、CD3+ T細胞の大部分を包含した(図42A)。まれなCD19+ B細胞及びCD11b+ CD15+顆粒球のみが検出された(図42A)。フローサイトメトリーデータと一致して、C11D1についてのCSF細胞病理学は、反応性リンパ球、単球及びマクロファージの存在を報告した。
