Claims (21)
- 体開口部に通して組織検体を取り出すシステムであって、
内部を画定するよう構成された収容袋を含み、前記収容袋は、第1の端部及び第2の端部を有し、前記収容袋は、前記第1の端部のところに設けられていて、前記内部中に通じると共に該内部と互いに連結された袋開口部を有し、前記袋開口部は、前記組織検体を受け入れるよう寸法決めされると共に形作られており、
前記収容袋の前記内部の少なくとも一部を包囲した状態で前記開口部と保護された状態のルーメンを画定する前記第2の端部との間で前記収容袋の少なくとも一部に沿って延びる保護ゾーンを含み、前記保護ゾーンは、近位端部、遠位端部及び長手方向軸線を有し、前記保護ゾーンは、当該保護ゾーンに前記長手方向軸線に沿った可変性を与えるように拡張された細長い形態と、圧縮形態とを含み、かつ、前記収容袋の穿通を阻止するよう構成されており、
前記収容袋と前記保護ゾーンは、前記収容袋の前記内部内に配置された前記組織検体のための取り出し経路を画定し、前記取り出し経路は、前記保護された状態のルーメンを通る経路及び前記袋開口部を出る経路を含む、システム。 - 前記保護ゾーンは、前記近位端部のところに設けられていて前記長手方向軸線から外方に延びる近位フランジ及び前記遠位端部のところに設けられていて前記長手方向軸線から外方に延びる遠位フランジを含む、請求項1に記載のシステム。
- 前記近位フランジは、前記体開口部の前記近位端部のところに定着すると共に前記保護ゾーンの遠位側への並進を阻止するよう構成され、前記遠位フランジは、前記体開口部の前記遠位端部のところに定着すると共に前記保護ゾーンの近位側への並進を阻止するよう構成されている、請求項2に記載のシステム。
- 前記保護ゾーンの前記長手方向軸線に垂直に取った断面は、前記近位端部のところの第1の直径、前記遠位端部のところの第2の直径、及び前記近位端部と前記遠位端部との間の第3の直径を定め、前記第1及び前記第2の直径は、休止形態では前記第3の直径よりも大きい、請求項1に記載のシステム。
- 前記保護ゾーンは、前記長手方向軸線に沿って前記近位端部から前記遠位端部までの分割部を含み、前記保護ゾーンの一部分は、前記分割部を閉じて前記保護ゾーンに調節可能な直径を与えるよう前記保護ゾーンの別の部分とオーバーラップし、このようにオーバーラップすることにより、直径方向の可変性を与える、請求項1に記載のシステム。
- 前記保護ゾーンは、前記長手方向軸線に垂直な平面で見て、前記長手方向軸線によって定められた固定中心点周りに巻いていて、前記中心点からの距離が連続して増加し又は減少する曲線を構成している、請求項1に記載のシステム。
- 前記保護ゾーンは、休止形態、縮小形態、及び拡大形態を有し、前記保護ゾーンは、前記休止形態と、前記縮小形態又は前記拡大形態との間で動くことができ、前記休止形態は、前記長手方向軸線に垂直な休止側方寸法を有し、前記縮小形態は、前記休止側方寸法よりも小さい減少側方寸法を有し、前記拡大形態は、前記休止側方寸法よりも大きな拡大側方寸法を有する、請求項1に記載のシステム。
- 前記拡張された細長い形態は、前記保護ゾーンの少なくとも一部が前記保護ゾーンの隣接するオーバーラップ部分の外面の陥凹内に嵌め込まれるように、前記保護ゾーンの内面を前記保護ゾーンの外面にオーバーラップさせることにより、圧縮状態に変えることができる、請求項1に記載のシステム。
- 前記保護ゾーンは、前記収容袋に対して耐切断性材料で作られており、かつ、前記収容袋に対して取り外し可能である、請求項1に記載のシステム。
- 前記保護ゾーンはバンドを備え、前記バンドは、第2の端部にオーバーラップする第1の端部を有し、前記バンドは前記第1の端部と第2の端部との間を、前記長手方向軸線の周りに延びる、請求項1に記載のシステム。
- 前記収容袋は、前記袋開口部の近くに配置された第1のリングと、前記第1のリングから遠位に配置された第2のリングを含み、前記第2のリングは、前記第1のリングと前記収容袋の前記第2の端部との間に配置されている、請求項1に記載のシステム。
- 耐切断性材料で作られた前記保護ゾーンは、前記第1のリングと前記第2のリングとの間に配置される、請求項11に記載のシステム。
- 耐切断性材料で作られた前記保護ゾーンは、前記第1のリングと前記第2のリングとの間に配置されると共に前記収容袋と一体的に形成されている、請求項11に記載のシステム。
- 体開口部に通して組織検体を取り出すシステムであって、前記体開口部は、組織辺縁を定め、前記システムは、
近位端部、遠位端部及び長手方向軸線を有する細切器を含み、前記細切器は、前記近位端部のところに位置するハウジング及び前記ハウジングから前記長手方向軸線に沿って遠位側に延びる外側シャフトを有し、前記外側シャフトは、前記ハウジングの遠位端部から前記細切器の前記遠位端部までの長さを有し、前記細切器は、前記外側シャフトと同軸の内側シャフトを有し、前記内側シャフトは、前記遠位端部のところに位置するブレードを有し、前記細切器は、前記近位端部のところに位置する近位開口部と前記遠位端部のところに位置する遠位開口部との間で前記ハウジング及び前記内側シャフトを貫通して延びる作業チャネルを有し、前記内側シャフトは、組織を前記遠位ブレードのところで切断するよう前記外側シャフトに対して回転するよう構成され、
側壁によって互いに連結された近位端部と遠位端部を有する耐切断性材料で作られている保護ゾーンを含み、前記保護ゾーンは、内面、外面及び前記近位端部から長手方向軸線に沿って前記遠位端部まで延びる細長い管の形をした中央ルーメンを有し、前記保護ゾーンは、前記組織辺縁に定着するよう構成され、
前記細切器の前記外側シャフトは、前記保護ゾーンの前記中央ルーメン中に取り出し可能に挿入可能であり、前記保護ゾーンは、当該保護ゾーンに前記長手方向軸線に沿った可変性を与えるように拡張された細長い形態と、圧縮形態とを含み、かつ、前記組織辺縁を細切中、前記ブレードから保護するよう構成されている、システム。 - 収容袋を更に含み、前記収容袋は、内部及び前記収容袋の前記内部に接近するための口を有し、前記保護ゾーンは、前記収容袋中に取り出し可能に挿入可能であり、前記保護ゾーンは、前記収容袋を細切中、前記ブレードから保護するよう構成されている、請求項14に記載のシステム。
- 前記保護ゾーンは、第2の端部にオーバーラップする第1の端部を有してオーバーラップ部分とともに螺旋形態を形成し、前記保護ゾーンの中央ルーメンの直径は、前記オーバーラップ部分を変化させることにより調整可能であり、前記保護ゾーンの長さは実質的に前記細切器の外側シャフト又は内側シャフトの長さと実質的に等しい、請求項14に記載のシステム。
- 前記細切器の外側シャフトが前記保護ゾーンの中央ルーメンに挿入されるときに、前記ブレードは前記保護ゾーンにより囲まれ、前記外側シャフトが前記保護ゾーンの前記中央ルーメンに挿入されるときに、前記ブレードは前記保護ゾーンの遠位端部をわずかに越えて延び、前記ハウジングの遠位端部は前記保護ゾーンの近位端部に接するように構成されている、請求項14に記載のシステム。
- 頂部開口部及び底部開口部を有する中央ルーメンを画定する側壁によって互いに連結された第1のリングと第2のリングを有するレトラクタをさらに備え、前記第2のリングは、前記体開口部を通って挿入するための低プロフィール形態に圧縮可能であり、前記レトラクタは、前記側壁を前記第1のリング周りに丸く巻いて前記レトラクタの長さを減少させると共に前記組織辺縁をレトラクトすることができるよう構成されている、請求項14に記載のシステム。
- 前記保護ゾーンは、前記拡張された細長い形態と、前記圧縮形態との間で移動可能である、請求項1又は14に記載のシステム。
- 前記拡張された細長い形態が、前記保護ゾーンの内面を前記保護ゾーンの外面にオーバーラップすることによって、前記保護ゾーンの少なくとも一部が保護ゾーンの隣接するオーバーラップ部分の外面の陥凹内に嵌め込まれるように、前記圧縮形態に変換可能である、請求項14に記載のシステム。
- 前記保護ゾーンが、前記保護ゾーンを前記体開口部から徐々に解き放つための引きひもを有する方向性タブまたはガイド穴をさらに備える、請求項1または請求項14に記載のシステム。
