Description
[1]ナノ粒子を形成することが可能なポリマーおよび抗真菌剤を含む、真菌感染症を処置するための局所組成物。
[2]上記ポリマーが、直鎖状および/もしくは分岐状もしくは環状ポリモノグアニド/ポリグアニジン、ポリビグアニド、それらの類似体または誘導体を含む、上記[1]に記載の局所組成物。
[3]上記ポリマーが、ポリヘキサメチレンビグアニドを含む、上記[1]または[2]に記載の局所組成物。
[4]上記ナノ粒子が、上記抗真菌剤と共に、および/または上記抗真菌剤の存在下で形成される、上記[1]から[3]のいずれか一項に記載の局所組成物。
[5]上記抗真菌剤が、上記抗真菌剤の治療的に有効な全身用量より少ない投与量で上記組成物中に存在する、上記[1]から[4]のいずれか一項に記載の局所組成物。
[6]1つまたは複数の以下の成分:緩衝液、賦形剤、結合剤、油、水、乳化剤、グリセリン、抗酸化剤、防腐剤および香料をさらに含む、上記[1]から[5]のいずれか一項に記載の局所組成物。

本発明の第一の態様に従って、ナノ粒子を形成することが可能なポリマーおよび抗真菌剤を含む、真菌感染症を処置するための局所組成物が提供される。
「n」は、ポリマー中の繰り返し単位の数を指し、nは、2から1000、例えば2または5から、10、20、30、40、50、60、70、80、90、100、150、200、250、300、350、400、450、500、600、700、800または900に変化し得る;
ビグアニド
または
グアニジン
ナイスタチン、テルビナフィン、ケトコナゾールおよびベルベリンを、Sigma−Aldrich、UKから粉末として入手した。アムホテリシンBを、Sigmaから脱イオン水中の溶液として入手した。ジメチルスルホキシド(DMSO)もSigmaからであった。ナイスタチン(5mg/ml)、テルビナフィン(15mg/ml)、ケトコナゾール(5mg/ml)およびアムホテリシンB(0.2mg/ml)のストック溶液をDMSO中で調製した。ベルベリンを水中に溶解して、10mg/mlのストック溶液を作った。PHMBのストック(5mg/ml)も水中で作った。ストック溶液はすべて異なる500μlのアリコートとし、−20℃で保持し、光から保護した。
出芽酵母(Saccharomyces cerevisiae)の菌株2種およびカンジダ・アルビカンス(Candida albicans)の臨床分離株1種を研究した。出芽酵母(S cerevisiae)NOD24は、Royal Veterinary Collegeから、および出芽酵母(S cerevisiae)EBY100はAllinson Bread(登録商標)、UKから入手した。カンジダ・アルビカンス(C albicans)の臨床分離株もRoyal Veterinary Collegeから入手した。
RPMI1640培地を粉末として入手して、蒸留水に溶解し、標準的な手順に従って0.165Mのモルホリノプロパンスルホン酸(MOPS)を用いて緩衝した。1Mの水酸化ナトリウム溶液を使用してpHを7に調整した。サブローグルコース寒天(SGA)を粉末として入手した。培地および化学物質はすべて、Sigma−Aldrich、UKからのものであった。
酵母の接種材料は、SGA中で24時間増殖させた培養物から5つのコロニーを採取し、RPMI1640をMOPSと混合することにより調製した。混合物の光学密度は分光光度法で、3×107コロニー形成単位の酵母/mlに相当する1に調整した。この懸濁液を、MOPSと共にRPMI1640中にさらに希釈して、最終接種材料のサイズを0.5×104CFU/mlとした。
チェッカーボード滴定試験は、CLSI推奨の標準的な手順ごとに滅菌96ウェルプレートで実行して、薬物単体およびPHMBと組み合わせた抗真菌薬効果を試験した。MOPSを用いて緩衝したRPMI1640培地を、試験培地として使用した。DMSO中でストックを作った場合、薬物の希釈液は試験培地中で調製され、水中でストックを作った場合、薬物のさらなる希釈液は水そのものの中で調製された。試験した濃度は:ナイスタチン0.000039mg/mlから0.02mg/ml、アムホテリシンB0.032mg/mlから0.0000625mg/ml、ベルベリン0.01mg/mlから0.00002mg/ml、ケトコナゾール0.032mg/mlから0.0000625mg/ml、およびテルビナフィン0.16mg/mlから0.00031mg/mlである。増殖対照および滅菌対照はすべてのプレートに存在していた。すべてのプレートを3回調製した。
パラフィンホイルを用いてプレートを密閉して、蒸発を防ぎ、カンジダ・アルビカンス(C albicans)は37℃で、および出芽酵母(S cerevisiae)は30℃で保持した。24時間後のカンジダ・アルビカンス(C albicans)および出芽酵母(S cerevisiae)NOD24に対して、Powerwave340ユニバーサルマイクロプレート分光計(Biotek)を利用して、MICを視覚的に記録した。しかし、出芽酵母(S cerevisiae)EBY100では、増殖対照ウェルでさえも24時間後に増殖が見られなかったので、MICは48時間後に記録された。MICは、増殖が見えない最低濃度と定義した。FICIは、薬物のin vitro相互作用を分析するために使用され、以下に示されている方程式を使用して計算した。
蛍光性抗真菌薬とPHMBの相互作用を蛍光定量的に試験した。特定の濃度の各薬物を異なる濃度のPHMBと相互作用させ、蛍光強度の変化を研究した。96ウェル平底クォーツプレート中で実験を実行した。150μg/mlのベルベリン、5μg/mlのケトコナゾールおよび5μg/mlのアムホテリシンBを、1:0から1:10のw/w比でPHMBと混合した。50μg/mlのナイスタチンを、1:0から1:4のw/w比でPHMBと混合した。蛍光性ではないテルビナフィンの場合、吸光度の変化を研究した。10μg/mlのテルビナフィンを、1:0から1:5のw/w比でPHMBと混合した。水はこの濃度でPBS中に沈殿するので、水を使用したベルベリンを除いて、すべての薬物に関して、溶液はリン酸緩衝生理食塩水(PBS)中のストックから調製した。混合物をピペット操作により完全に混合し、室温で20分間にわたり保持し、次いで、蛍光性/吸光度の変化を試験した。蛍光性は、Infinite M200Pro Fluorometer(Tecan)で試験した。蛍光強度を試験するために、励起および発光波長は、それぞれ、ナイスタチンでは320nmおよび410nm、アムホテリシンBでは340nmおよび480nm、ベルベリンでは350nmおよび550nm、ケトコナゾールでは260nmおよび375nmであった。テルビナフィンの場合、ND1000分光計(Nanodrop)を使用して、吸光度の変化を研究した。テルビナフィンの吸光度ピークは272nmの位置であった。272nmの最大吸光度の変化は、PHMBの濃度に対してプロットした。
動的光散乱(DLS)を使用して、形成された複合体のサイズを評価した。これは、動いている粒子からの散乱光を測定することにより、粒子のサイズを判定できるという原理に基づく。抗真菌薬(100μg/ml)およびPHMBをPBS中に1:3のw/w比で完全に混合し、20分間室温で保持した。Zetasizer S(Malvern Instruments、UK)を使用して、粒子サイズを測定した。
ベルベリンの細胞送達は、化合物が顕微鏡法によって検出できるほど蛍光性であるため、視覚的に検出した。カンジダ・アルビカンス(C albicans)細胞を、ベルベリン単体で、ならびにベルベリンおよびPHMBの組合せでも処置した。1μg/ml、2μg/mlおよび3μg/mlのベルベリン溶液50μlを、1μg/mlおよび2.5μg/mlのPHMB50μlと、3〜4回にわたるピペット操作により滅菌96ウェルプレート中で適切に混合し、室温で20分間にわたり保持した。RPMI1640におけるカンジダ・アルビカンス(C albicans)細胞100μlをこれらのウェル、ベルベリン単体の溶液100μlおよびPBS100μlに加え、37℃で1時間にわたり保持した。各ウェルの含有物をエッペンドルフチューブに移し、5000rpmで10分間にわたり遠心分離した。上澄みの除去後、2.5μg/mlの4’,6’ジアミノ−2−フェニルインドール(DAPI)100μlを、各チューブに加えて、真菌の核を染色した。アガロースベッドでスライドを調製し、油浸レンズを使用したDM4000正立蛍光顕微鏡(Leica microsystems)下で観察した。
PHMBと組み合わせた場合の、カンジダ・アルビカンス(C albicans)中へのベルベリンの細胞送達を、フローサイトメトリーを使用して定量的に試験した。96ウェルプレート中で、1μg/ml〜5μg/mlのベルベリン単体、ならびにこれらの濃縮物と1.25、2.5、5μg/mlのPHMBの組合せで酵母細胞を処置した。手順は、顕微鏡法のものとまったく同じであった。遠心分離後、100μlの上澄みを除去し、400μlのPBSを加え、FACS Canto(商標)IIフローサイトメーター(BD Biosciences)にFITC(フルオレセインイソチオシアネート)フィルタを使用してベルベリンの送達について試験した。FACS Divaソフトウェアを使用してデータを取得し、Flowjo5.6.5ソフトウェアを使用して分析した。
ウィルコクソンの符号順位検定を使用して、PHMBと組み合わせた場合の抗真菌薬のMICの変化を統計的に分析した。0.05未満のp値は有意とみなした。SPPSSバージョン2.0ソフトウェアを使用して、統計解析を行った。
チェッカーボードアッセイを用いて、PHMBおよび薬物が共に作用する場合に何らかの低下が生じるか否かを知るために抗真菌薬のMICの変化を評価することにより、選択した抗真菌薬およびそれとPHMBの組合せに対する真菌の感受性を測定した。すべての薬物は、PHMBと組み合わせた場合のMICにおいて有意な低下を示した(p値=0.043)。抗真菌薬のMICおよびFICI値は、以下の表2に要約されている。
PHMBおよび抗真菌薬の間における相互作用を、それらの蛍光強度の変化を研究することにより試験した。この結果は、図4で描写されている。すべての化合物の蛍光性で低下が生じた。ケトコナゾールおよびPHMBの間の比が1:6では、蛍光性はほぼ50%に下落した。ナイスタチンの蛍光性は、わずかな低下しか示さなかった。研究中に、ベルベリンはPBS中に沈殿することを見出したので、水を溶媒として使用した。ベルベリンの蛍光性の下落は生じたが、次いで、わずかな上昇が始まったものの、ベルベリン単体を用いた試料を超えることはなかった。アムホテリシンBを用いた観察も同様であった。試料の蛍光性におけるこの下落は、蛍光クエンチと呼ばれる。テルビナフィンの場合、吸光度はわずかな上昇が見出された。これは色素増強効果と呼ばれる。蛍光クエンチおよび色素増強効果は、PHMBが抗真菌薬と相互作用することを示す。
PHMBおよび抗真菌薬の間で形成された複合体のサイズは、DLSを使用して測定した。ケトコナゾールおよびベルベリンの場合(図5)、ナノ粒子を観察した。しかし、ケトコナゾールの場合、いくつかのより大きいサイズの粒子も生じた。ベルベリンの場合(図5(a))、粒子の64.5%がおよそ22.2±1.5nmであった。しかし、ケトコナゾール(図5(b))では、サイズが10nm未満のものは、粒子の10%未満にすぎなかった。1を超える多分散性指標は、より広い粒子サイズ分布を意味する。他の薬物の場合、50から≦1000nmの粒子を観察した。この結果は、PHMBがすべての抗真菌薬と粒子を形成し、テルビナフィン、ケトコナゾールおよびベルベリンとナノ粒子を形成することを示す。
蛍光顕微鏡検査法を使用して、カンジダ・アルビカンス(C albicans)中へのベルベリンの細胞送達を試験した。ベルベリンをPHMBと組み合わせた場合に、図6に描写されているベルベリン単体と比較して、細胞の蛍光性にかなりの上昇が生じた。蛍光性の上昇は、同一の濃度のベルベリンに対して、PHMBの濃度が上昇した場合にも観察された。図6は、1例のベルベリン濃度しか表さないが、研究したすべての濃度で同様の結果が観察された。PHMBを加えると、ベルベリン単体と比較してベルベリンの細胞への進入が増加することが結果から指し示される。これは、PHMBがベルベリンの真菌への送達を向上できると定性的に証明している。
カンジダ・アルビカンス(C albicans)へのベルベリン送達の向上を、フローサイトメトリーを使用して定量した。ベルベリンの発光ピークがFITCの吸収スペクトルに近づくようにFITCフィルタを使用した。図7に描写されているように、PHMBと組み合わせた場合に、ベルベリンに陽性の細胞の数に明らかな増加が生じた。観察は、顕微鏡法と同様であった、すなわち、PHMB濃度の上昇に伴って、ベルベリンに陽性の細胞は増加した。より高い濃度のベルベリン単体を用いても、わずかな増加が観察された。これらの結果は、PHMBが、ベルベリンのカンジダ・アルビカンス(C albicans)への細胞送達を増加させることを指し示す。
PHMBは低毒性で、臨床的に安全な消毒薬であり、実験は、抗真菌薬とこのカチオン性ポリマーを組み合わせることにより、抗真菌薬の送達の向上を示す。この薬物を、PHMBと組み合わせた場合の、カンジダ・アルビカンス(C albicans)および出芽酵母(S cerevisiae)に対する薬物の抗真菌薬活性の変化に関して試験した。蛍光性/吸光度の研究および動的光散乱を使用して、ポリマーおよび薬物の間における相互作用を試験した。フローサイトメトリーおよび蛍光顕微鏡検査法を使用して、ベルベリンの細胞送達を研究した。
実験を実施して、テルビナフィンおよびPHMBを用いて形成されるナノ粒子を形成した。
10倍の、30:10(PHMB:テルビナフィン、μg/ml)のナノ粒子製剤を、300μlで作った。288μlのPBSを1.5ml滅菌チューブに加えた。1mg/mlのPHMB9μlを加え、5回にわたる上下のピペット操作により混合した。次いで、1mg/mlのテルビナフィン3μlを徐々に加え、5回にわたる上下のピペット操作により混合した。次いで、PHMBの存在下で、室温で1時間にわたりインキュベーションを続け、ナノ粒子を形成した。
経皮パッチは、皮膚を通して直ちに吸収できる、小分子の脂溶性薬物の投与に長らく使用されている。この非侵襲的な送達経路は、多くの薬物のバイオアベイラビリティを劇的に低下させることもある、消化管および肝臓の門脈系の両方を迂回させることにより、薬物を全身循環に直接吸収させることができるため、経口送達に合わない多くの薬物の投与に有利である。経皮的送達は、患者の不快感、針への不安、投与する者に対する不慮の針刺し損傷の危険性、および鋭利な物の廃棄を取り巻く問題を大幅に低減することにより、皮下注射に関連する多くの課題も克服する。
本発明の組成物は、エアロゾル製剤にも製剤できる。この作業例では、PHMBナノ粒子をカスポファンギン(リポペプチド抗真菌薬)と形成し、乾燥させる。乾燥させたナノ粒子を、次いで噴霧剤に加える。噴霧剤は、エアロゾルクラウドを生成するための力を提供し、カスポファンギンのナノ粒子の活性成分を懸濁または溶解させることになる媒体にもなる。
・−100から+30℃の範囲の沸点を有し、
・おおよそ1.2から1.5gcm−3の密度(おおよそ懸濁または溶解させる薬物の密度)を有し、
・40から80psigの蒸気圧を有し、
・患者に非毒性であり、
・不燃性であり、
・一般的な添加剤を溶解させることが可能でなければならない。活性成分は完全に可溶性、または完全に不溶性であるべきである。

Claims (11)
- ポリマーおよび抗真菌剤から形成されたナノ粒子を含む、真菌爪感染症または真菌皮膚感染症を処置するための局所組成物であって、前記ポリマーが、直鎖状および/もしくは分岐状もしくは環状ポリモノグアニド/ポリグアニジン、またはポリビグアニドを含み、前記抗真菌剤がテルビナフィンを含む、局所組成物。
- 前記ポリマーが、ポリヘキサメチレンビグアニド(PHMB)を含む、請求項1に記載の局所組成物。
- 前記抗真菌剤が、前記抗真菌剤の治療的に有効な全身用量より少ない投与量で前記組成物中に存在する、請求項1または2に記載の局所組成物。
- 1つまたは複数の以下の成分:緩衝液、賦形剤、結合剤、油、水、乳化剤、グリセリン、抗酸化剤、防腐剤および香料をさらに含む、請求項1から3のいずれか一項に記載の局所組成物であって、尿素をさらに含んでもよい、局所組成物。
- 前記真菌皮膚感染症が、水虫または皮膚糸状菌感染症および/もしくは酵母菌感染症を含む、請求項1から4のいずれか一項に記載の局所組成物。
- 前記抗真菌剤が、以下の群:ナイスタチン、ケトコナゾール、アムホテリシンB、イトラコナゾールまたはベルベリンから選択される1つまたは複数のさらなる薬剤を含む、請求項1から5のいずれか一項に記載の局所組成物。
- クリーム剤、軟膏剤、スプレー剤または散剤の形態である、請求項1から6のいずれか一項に記載の局所組成物。
- マイクロニードルアレイにより投与される、請求項1から7のいずれか一項に記載の局所組成物であって、前記マイクロニードルアレイが、接着パッチに組み込まれていてもよい、局所組成物。
- ナノ粒子を形成するために適切な条件下で、直鎖状および/もしくは分岐状もしくは環状ポリモノグアニド/ポリグアニジン、またはポリビグアニドを含むポリマーと、テルビナフィンを含む抗真菌剤とを混合するステップを含む、真菌爪感染症または真菌皮膚感染症を処置するための局所組成物の製造方法。
- 請求項1から8のいずれか一項に記載の組成物を製造するために使用される、請求項9に記載の製造方法。
- マイクロニードルアレイとの組合せで使用される請求項1から8のいずれか一項に記載の局所組成物あって、前記マイクロニードルアレイが、接着パッチに組み込まれていてもよい、局所組成物。
