CDRグラフティングプロトコルは典型的には、3つの意思決定点を含む:(1)ドナー抗体、すなわちグラフティングの標的の特異性を決定する領域の規定、(2)FRドナーとして利用されるヒト配列の供給源の特定、及び(3)特異性を規定する領域以外の残基の選択、すなわちヒト化抗体の親和性を回復させる又は改善するための復帰突然変異の標的であるアミノ酸位置の決定。
抗原と複合体を形成している非ヒト抗体の実験的構造によって、抗原と接触している残基、及びひいてはその特異性の決定に関与する残基の詳細なマップがもたらされる。構造情報を、アラニン走査突然変異誘発及び/又はコンビナトリアル突然変異誘発によって補って、結合エネルギー又は機能的パラトープに最も寄与する残基を特定することができる。機能的パラトープは接触している残基のサブセットであるため、機能的パラトープのみをグラフティングすることによって、ヒト化生成物における非ヒト残基の数が低減すると考えられる。しかしながら、抗原−抗体複合体及び/又は機能的パラトープの実験的構造がヒト化プロトコルの開始時に利用可能であることは極めて稀である。所与の抗体の特異性に関与する残基の正確な規定がない場合、特異性を規定する領域としてCDRが利用されることが多い。グラフティングのための標的としてCDR及びHVループの組合せを使用することも可能である。ヒトFR上にグラフティングされる残基の数を低減するために、SDRグラフティング、すなわち特異性決定残基(SDR)のグラフティングが説明されている。
典型的なCDRグラフティングプロトコルにおける第2工程は、ヒトFRドナーを特定することである。初期の作業は、非ヒト抗体に対するその相同性に関わらず、既知の構造のヒト抗体のFRを利用するものであった。このアプローチは、「固定FR法」として知られている。後期の作業は、非ヒト抗体に対して最高の相同性を有するヒト配列を使用するものであった。このアプローチは、「ベスト・フィット」と称されている。「ベスト・フィット」戦略はより高い親和性を有する抗体をもたらす傾向があるが、ヒト化のためのFRを選ぶ際には、低免疫原性及び生産収率等の他のパラメータも考慮に入れなければならない。したがって、「ベスト・フィット」及び「固定FR」の組合せも考え得る。例えば、VL部分を固定FR法によってヒト化することができ、VH部分をベスト・フィット法によってヒト化することができ、又はその逆も考えられる。

リサーフェシングは、CDRグラフティングに類似しており、最初の2つの意思決定点を共有する。CDRグラフティングとは対照的に、リサーフェシングは、非ヒト抗体の露出しない残基を保持する。非ヒト抗体における表面残基のみがヒト残基へと変化する。
CDRグラフティングは非ヒト配列とヒト配列との間のFRの比較によるものであるが、超ヒト化はCDRの比較に基づくものであるため、FRの相同性は無関係である。このアプローチは、機能的ヒト生殖系列遺伝子レパートリーとの非ヒト配列の比較を含む。次いで、マウスの配列と同じ又は密接に関連したカノニカル構造をコードするこれらの遺伝子を選択する。次に、非ヒト抗体とカノニカル構造を共有する遺伝子内において、CDR内で最高の相同性を有するものをFRドナーとして選ぶ。最後に、これらのFR上に非ヒトCDRをグラフティングする。
このアプローチは、ヒトストリング含有量(HSC)と称される、抗体に関する尺度「人間性」に基づくものである。簡潔に述べると、このアプローチは、マウスの配列をヒト生殖系列遺伝子のレパートリーと比較する。差異をHSCとしてスコア化する。全体的な同一性尺度を使用するのではなくそのHSCを最大化することによって標的配列をヒト化して、複数の多様なヒト化変異体を作製する。
FRライブラリアプローチでは、残基変異体の集合物をFR中の特定の位置で導入した後、ライブラリのパニングを行い、グラフティングしたCDRを最も良好に支持するFRを選択する。したがって、このアプローチはCDRグラフティングに類似するが、FR中に数個の復帰突然変異を創出する代わりに、典型的には100個を超える突然変異体のコンビナトリアルライブラリを構築する。
このアプローチは、特定の抗原に特異的な所与の非ヒト抗体のVHドメイン又はVLドメインを、ヒトのVH及びVLのライブラリと組み合わせることを含む。その後、対象となる抗原に対して特異的なヒトVドメインを選択する。例えば、最初に非ヒトVHをヒト軽鎖のライブラリと組み合わせることによって、非ヒト抗体をヒト化することができる。次いで、ファージディスプレイによって標的抗原に対してライブラリを選択し、選択したVLをヒトVH鎖のライブラリ中にクローニングし、標的抗原に対して選択する。非ヒトVLをヒト重鎖のライブラリと組み合わせることから開始することも可能である。次いで、ファージディスプレイによって標的抗原に対してライブラリを選択し、選択したVHをヒトVL鎖のライブラリ中にクローニングし、標的抗原に対して選択する。結果として、非ヒト抗体と同様の親和性を有する完全なヒト抗体を単離することができる。エピトープドリフトの発生を回避するために、親抗体と同じエピトープを認識するクローンの選択を可能とするいわゆる阻害ELISAを実行することが可能である。代替的には、CDR保持法を適用して、エピトープドリフトを回避することができる。CDR保持法では、1つ又は複数の非ヒトCDRが保持され、好ましくは、重鎖CDR3は抗原結合部位の中心にあるため重鎖CDR3が保持される。
FRシャッフリングアプローチでは、FR全体を非ヒトCDRと組み合わせる。FRシャッフリングを使用して、Dall’Acqua及び共同研究者らは、マウス抗体をヒト化した。マウス抗体の6つの全てのCDRをヒト生殖系列遺伝子のFRを全て含有するライブラリ中にクローニングした(Dall’Acqua et al., 2005)。ライブラリを、最初にVLを、その後VHをヒト化する2工程選択プロセスにおいて結合に関してスクリーニングした。その後の研究において、1工程FRシャッフリングプロセスを使用することに成功した(Damschroder et al., 2007)。全ての既知のヒト生殖系列軽鎖(κ)フレームワークをコードするオリゴヌクレオチド配列が、Dall’Acqua et al., 2005(付録A)に開示されている。全ての既知のヒト生殖系列重鎖フレームワークをコードするオリゴヌクレオチド配列が、Dall’Acqua et al., 2005(付録B)に開示されている。
ヒューマニアリングによって、ヒト生殖系列遺伝子抗体に対して91%〜96%相同である抗体の単離が可能となる。この方法は、本質的な最小特異性決定基(MSD)の実験的な特定、並びにヒトFRのライブラリ中への非ヒト断片の配列の置き換え及び結合の評価に基づくものである。この方法は、非ヒトVH鎖及びVL鎖のCDR3の領域から開始され、VH及びVLの両方のCDR1及びCDR2を含む非ヒト抗体の他の領域をヒトFRへと進行的に置き換える。

本明細書中で使用する場合、「保存的置換」は、上で示される6つの標準的なアミノ酸群の同じ群内に列挙される別のアミノ酸による、或るアミノ酸の交換と定義される。例えば、GluによるAspの交換によって、そのように変更されたポリペプチドにおける1つの負電荷は保持される。加えて、グリシン及びプロリンを、α−ヘリックスを破壊するその能力に基づき互いに置換することができる。上の6つの群内における幾つかの好ましい保存的置換は、以下の下位群内における交換である:(i)Ala、Val、Leu及びIle、(ii)Ser及びThr、(ii)Asn及びGln、(iv)Lys及びArg、並びに(v)Tyr及びPhe。既知の遺伝コード、並びに組換えDNA技法及び合成DNA技法を踏まえて、熟練した科学者は、保存的アミノ酸変異体をコードするDNAを容易に構築することができる。
全身性炎症反応症候群(SIRS):以下の所見のうち2つ以上の存在によって定義される:
体温<36℃(97°F)、又は>38℃(100°F)(低体温又は発熱)。
心拍>毎分100回(頻脈)。
呼吸数>毎分20回、又は血液ガスにおいてPa CO2が32mmHg(4.3kPa)未満(頻呼吸、又は過剰換気による低炭酸ガス症)。
白血球数<4000細胞/mm3、若しくは>12000細胞/mm3(<4×109細胞/L、若しくは>12×109細胞/L)、又は10%を超える桿状球(band forms)(未熟な白血球)。(白血球減少症、白血球増加症又は多桿状球血液症(bandemia))。
敗血症:確認された感染プロセスに応答したSIRSと定義される。感染は、疑わしいものであっても、若しくは(培養、染色又はポリメラーゼ連鎖反応(PCR)によって)証明済みのものであってもよく、又は感染に特徴的な臨床症候群であってもよい。感染の具体的な証拠は、通常は無菌状態の液(例えば尿又は脳脊髄液(CSF))中のWBC、内蔵の穿孔の証拠(腹部のx線又はCTスキャンによる自由空気流、急性腹膜炎の徴候)、肺炎と一致する異常な胸部x線(CXR)(局所不透明化を伴う)、又は点状出血、紫斑病若しくは電撃性紫斑病を含む。
重度の敗血症:臓器機能不全、低灌流又は低血圧を伴う敗血症と定義される。
敗血症ショック:十分な輸液蘇生にも関わらず難治性動脈低血圧又は低灌流異常を伴う敗血症と定義される。全身性低灌流の徴候は、終末器官機能不全、又は4mmol/dLを超える血清乳酸であり得る。他の徴候は、乏尿症及び精神状態の変化を含む。患者は、積極的な(aggressive)輸液蘇生(典型的には6リットル又は40ml/kgのクリスタロイドを超える)後に敗血症に加えて低血圧を有する場合、敗血症ショックを有すると定義される。
本発明をここでさらに記載する。以下の節では本発明の種々の態様をより詳細に規定する。そのようにして規定した各態様は、反対の内容が明示されない限り、任意の他の態様(単数又は複数)と組み合わせることができる。特に、好ましい又は有利であるとして示した任意の特徴を、好ましい又は有利であるとして示した任意の他の特徴(単数又は複数)と組み合わせることができる。
抗体INab308の軽鎖CDR2配列(配列番号12)は抗体INab708の軽鎖CDR2配列(配列番号13)と同一であるため、配列番号13を含む組は配列番号12を含む組と重複すると考えられる。したがって、この表は4組の軽鎖CDR配列のみを列挙している。

1.1 組換えC5a及びC5a突然変異体の調製:
ヒトC5aをコードするDNA配列を、末梢血白血球から単離されたRNAを使用する逆転写酵素ポリメラーゼ連鎖反応(RT−PCR)によって取得した。C5a突然変異体を、突然変異部位にGCT(アラニン)を導入することによって、PCR法を使用して作製した。引き続き、C5aのDNAを、pET−32a(Novagen、Gibbstown, NJ)とライゲーションし、ライゲーション混合物を使用して、JM109コンピテント細胞を形質転換した。発現プラスミドを、標準的な塩化カルシウム法を使用して、BL21中に形質転換した。ルリア・ベルターニ・ブロス(LB)プレートからシングルコロニーをピックアップし、アンピシリンを含むLB培地中に接種して、37℃で終夜インキュベートした。培養物を2LのLB培地に移し、指数増殖期中期(A600≒0.6)まで37℃でインキュベートした後、イソプロピル−β−D−チオガラクトピラノシド(IPTG)を0.1mMの最終濃度まで添加した。細胞を、30℃で終夜継続して成長させ、培養物を7000rpm、4℃で15分間遠沈することによって回収した。リン酸緩衝生理食塩水(PBS;10mM PB、150mM NaCl[pH7.4])で1回洗浄した後、細菌ペレットを、PBS中に再懸濁し、氷上で超音波処理した。12000rpm、4℃で15分間の遠心分離の後、可溶性画分を不溶性ペレットから分離した。ヒト組換えC5aを精製するために、細胞溶解物の上清を、PBSで予め平衡化したニッケルキレート化親和性カラムにロードした。引き続き、カラムを、それぞれPBS中の50mMイミダゾール及び200mMイミダゾールで洗浄した。最後に、結合したタンパク質を500mMイミダゾールによって溶出し、PBSに対して終夜透析し、ドデシル硫酸ナトリウム−ポリアクリルアミドゲル電気泳動(SDS−PAGE)によって分析した。
モノクローナル抗体を、ハイブリドーマ法を使用して作製した。免疫化及びMAbの産生を、標準的なプロトコルを使用して実施した。5匹の4週齢の雌性BALB/cマウスを、動物1匹当たり、完全フロイントアジュバント中の精製した組換えC5a 100μgを用いて皮下的に免疫化した。動物を、不完全フロイントアジュバント中の抗原100μgを使用して4週間隔で2回追加免疫した。最後の追加免疫の3日後、マウスを屠殺し、その脾細胞をNS−1と5:1の比率で融合させ、200μLの細胞を5つの96ウェルプレート上の各ウェルにプレーティングした。ハイブリッド細胞を、20% ウシ胎児血清並びに5×10−3M ヒポキサンチン、2×10−5M アミノプテリン及び8×10−4M チミジン(HAT)を添加したダルベッコ変法イーグル培地(DMEM)において選択した。
大量のMabを作製するために、5×106のハイブリドーマ細胞をマウスの腹腔中に注射した。14日後、腹水を抜き取り、1500rpm、4℃で5分間遠心分離した。上清を収集し、PBS中で予め平衡化したプロテインA−セファロース4Bのカラムに適用した。結合したMabをクエン酸(pH4.0)で溶出し、PBSに対して終夜透析した。精製したタンパク質を、SDS−PAGEによって分析した。
脱顆粒による酵素放出の誘導は、C5aの重要な生物学的特質である。本発明者らの研究では、健康なボランティアから得た新鮮なヒト全血を使用して、リゾチーム放出に対するC5aの効果を評価した。全血細胞から放出されるリゾチームレベルを、EnzChek(商標)リゾチームアッセイキット(Invitrogen、CA, USA)によって分析した。抗C5a抗体の遮断活性を研究するために、rhC5a(100nM)を、種々の濃度の抗体と混合した。その後、全血細胞を直ぐに添加して、C5aとの抗体のプレインキュベーションを回避した。インキュベーション後、50μlの試料上清を50μlの希釈基質溶液に添加した。プレートを、暗所で37℃で30分間インキュベートし、その後、Perkin Elmer(Massachusetts, USA)の1420マルチラベルカウンターによって読み取った。蛍光強度を、490nmでの励起及び525nmでの発光によって測定し、ゼロの標準値(ブランク)を全ての試料から減算した。抗体の遮断活性を、rhC5aによって誘導されるリゾチーム放出の蛍光強度からrhC5a非依存性のリゾチーム放出(緩衝剤対照)の蛍光強度を減算した後で、算出した。遮断活性を、以下の式を用いて算出した:遮断活性=C5a蛍光−(C5a+Ab)蛍光/C5a蛍光−緩衝剤対照蛍光。
結合Elisaを実施して、ヒトC5a及びC5a突然変異体に対するINab308及びINab708の結合活性を決定した。C5a突然変異体は、cDNAレベルからの突然変異部位へのGCTの導入によって対応するアミノ酸をアラニンと置き換えることによって作製される。これらのC5a突然変異体は、24位、29位、30位、31位、32位、35位、36位、37位、30位/37位(二重突然変異)、40位、53位、64位、65位、66位、68位、64位/68位、66位/68位、69位、70位における部位突然変異、及びC−del(C5aのC末端から12個のアミノ酸を欠失させた)を含む。
簡潔に述べると、ヒツジ赤血球(sRBC)を、新鮮なヒツジ全血から遠心分離によって調製した後、抗sRBCで感作させた。健康なボランティア由来の血漿試料を段階希釈し、感作させたsRBCとともにインキュベートした。インキュベーションの30分後、溶解していない細胞を遠沈させ、上清についてプレートリーダーによって542nmで読み取った。C5の活性化に対する抗C5a抗体の効果を決定するために、等体積の抗C5a抗体と血漿とを、感作sRBCの添加の前に1時間プレインキュベートした。
臨床的敗血症に近い設定における抗C5a抗体の有効性を評価するために、健康なボランティア由来の全血250μlに抗C5a抗体を加え、引き続き、緩衝生理食塩水で希釈した大腸菌(濃度1×107/ml)250μlを添加した。37℃、4時間のインキュベーションの後、上清を、遠沈させ、IL−8に関するELISA分析のために収集した。上清中のIL−8レベルを、IL−8 ELISAキット(BioLegend、USA)によって分析した。
コンピュータモデリング法によって得られたC5aの3D構造において、C5aの28位〜40位のペプチド(VNNDETCEQRAAR、配列番号67)及びC5aの65位〜70位のペプチド(ISHKDM、配列番号68)を含有する空間的エピトープは、ランダムコイルとみなすことができる。この2つのペプチドを柔軟なペプチドリンカーGGGGS(配列番号69)によって連結すると、2つのペプチドによる弱い疎水的相互作用がポケット形の立体構造を確保するため、親抗原の立体構造に類似する空間的エピトープが再構築される。ペプチドNH2−28〜40−リンカー(GGGGS)−65〜70−COOHのコンピュータモデリング解析結果は、親抗原と同じ立体構造を維持している。この新たなアミノ酸24個のペプチドを合成し、キーホールリンペットヘモシアニン(KLH)と複合体形成させて、マウスを免疫化するための免疫原を形成することができ、引き続き、伝統的なハイブリドーマ技術を適用して、この新たなアミノ酸24個のペプチドに基づくELISAをスクリーニングツールとして使用してINab308及びINab708を得ることができる。
2.1 C5aの活性に関連するアミノ酸の特定
考え得る抗体エピトープを構成するC5a分子内の複数のアミノ酸を、アラニンへと突然変異させた。特に、C5a突然変異体は、24位、29位、30位、31位、32位、35位、36位、37位、30位/37位(二重突然変異)、40位、53位、64位、65位、66位、68位、64位/68位、66位/68位、69位、70位における部位突然変異、及びC−del(C5aのC末端から12個のアミノ酸を欠失させた)を含む。C5a突然変異体を、ヒト全血細胞からのリゾチーム放出を誘導するそれらの生物活性に関して試験した(図1)。
ヒトC5aに対する抗体を分泌する2000個の細胞クローンを、機能的アッセイ(酵素放出)によってスクリーニングした。2つの抗体のみが優れた遮断活性を示した。
総溶血補体価(CH50)は、古典的な補体経路の活性化を決定するための従来的方法である(1.6節を参照されたい)。
C5aに対する抗体INab308、抗体INab708及び抗体L2B23の遮断活性を、C5aによって誘導されるリゾチーム放出のアッセイによって評価した(1.4節を参照されたい)。2個のC5a分子によって誘発される生物学的効果に対する1個の抗体の遮断活性を評価するために、抗体対C5aのモル比を1:2に設定した。これらの抗体は、二価抗体である。したがって、上記の1:2というモル比を選ぶことによって、一方では抗体のパラトープと他方ではC5aとが、等モル濃度で存在する。
臨床的敗血症に近い設定における抗C5a抗体の有効性を評価するために、全血における大腸菌によって誘導されるIL−8産生を決定した。このアッセイは、大腸菌感染のモデルとみなすことができる(1.7節も参照されたい)。
好ましい本発明の抗体INab308及びINab708の重要な特性を、以下の表3に要約する。本発明において特定される立体構造エピトープのアミノ酸配列、すなわち配列番号2及び配列番号3のアミノ酸配列の両方と同時に結合しない比較用抗体8g8、MAb 137−26、Ab11876、G57、F20、L2B23及びG13が更に含まれる。比較用抗体8g8、MAb 137−26、F20、G57、L2B23及びG13は、これらの2つのアミノ酸配列のうち1つのみと結合する(8g8、MAb 137−26、F20、G57及びG13)か、又は異なるアミノ酸配列と結合する(L2B23)。Ab11876の標的エピトープは知られていない。
ヒトC5aに対するモノクローナル抗体は、古典的なハイブリドーマ技術を使用して作製した。ヒトC5に対するMabの結合部位は、アラニンスクリーニング法によって決定した。Mabの遮断活性は、ヒト全血細胞からのC5aによって誘導されるリゾチーム放出の阻害によって定量化した。

該結合部分が、ヒト血漿におけるCH50活性を阻害しないものであり、並びに
該結合部分は、抗体又はその抗原結合断片である、結合部分。 - 結合部分が、DETCEQR(配列番号4)に示されるアミノ酸配列の少なくとも1つのアミノ酸と結合する、請求項1に記載の結合部分。
- 結合部分が、HKDMQ(配列番号5)、又はKDM、に示されるアミノ酸配列の少なくとも1つのアミノ酸と結合する、請求項1又は2に記載の結合部分。
- 結合部分が、ヒト全血における大腸菌によって誘導されるIL−8産生を低減することが可能である、請求項1〜3のいずれか一項に記載の結合部分。
- 結合部分が抗体又はその抗原結合断片であり、前記抗体がポリクローナル抗体、モノクローナル抗体、キメラ抗体、ヒト化抗体及びヒト抗体からなる群から選択される、請求項1〜4のいずれか一に記載の結合部分。
- 結合部分が抗体の抗原結合断片であり、前記断片がFab断片、Fab’断片、F(ab’)2断片、Fd断片、Fv断片、ジスルフィド結合したFv(dsFv)、単一ドメイン抗体及び一本鎖Fv(scFv)抗体からなる群から選択される、請求項1〜4のいずれか一に記載の結合部分。
- C5aのアミノ酸配列NDETCEQRA(配列番号2)及びSHKDMQL(配列番号3)によって形成される立体構造エピトープと結合する、抗体又はその抗原結合断片であって、
(i)配列番号6に示される重鎖CDR3配列、又は
(ii)配列番号7に示される重鎖CDR3配列
を含み、
以下の配列:
(i)配列番号10に示される重鎖CDR2配列、
(ii)配列番号11に示される重鎖CDR2配列、
(iii)配列番号12に示される軽鎖CDR2配列、
(iv)配列番号13に示される軽鎖CDR2配列、
(v)配列番号14に示される重鎖CDR1配列、
(vi)配列番号15に示される重鎖CDR1配列、
(vii)配列番号16に示される軽鎖CDR1配列、又は
(viii)配列番号17に示される軽鎖CDR1配列
のうち少なくとも1つを更に含み、
該抗体又はその抗原結合断片は1nM以下のKd値を伴うC5aに対する結合定数を有し、及び
該抗体又はその抗原結合断片のパラトープとC5aとが等モル濃度で存在する場合に、該抗体又はその抗原結合断片はC5aによって誘導されるヒト全血細胞からのリゾチーム放出に対して少なくとも90%の遮断活性を示し、並びに
該抗体又はその抗原結合断片が、ヒト血漿におけるCH50活性を阻害しない、
抗体又はその抗原結合断片。 - (i)配列番号8に示される軽鎖CDR3配列、又は
(ii)配列番号9に示される軽鎖CDR3配列
を更に含む、請求項7に記載の抗体又はその断片。 - C5aのアミノ酸配列NDETCEQRA(配列番号2)及びSHKDMQL(配列番号3)によって形成される立体構造エピトープと結合する、抗体又はその抗原結合断片であって、
(i)配列番号8に示される軽鎖CDR3配列、又は
(ii)配列番号9に示される軽鎖CDR3配列
を含み、
該抗体又はその抗原結合断片は1nM以下のKd値を伴うC5aに対する結合定数を有し、及び
該抗体又はその抗原結合断片のパラトープとC5aとが等モル濃度で存在する場合に、該抗体又はその抗原結合断片はC5aによって誘導されるヒト全血細胞からのリゾチーム放出に対して少なくとも90%の遮断活性を示し、並びに

1つ又は複数の薬学的に許容可能な担体、希釈剤、賦形剤、充填剤、結合剤、滑剤、流動促進剤、崩壊剤、吸着剤及び/又は保存料
を更に含む、医薬組成物。 - 急性炎症を伴う様々な疾患、全身性炎症反応症候群(SIRS)、敗血症、重度の敗血症、敗血症ショック、虚血/再灌流関連損傷若しくは虚血性心疾患、急性肺損傷、肺炎若しくは移植患者における急性及び慢性の移植片拒絶反応、移植片対宿主反応、並びに慢性型の炎症を伴う疾患、腎糸球体疾患、糸球体腎炎、及びその他の腎不全、関節リウマチ、及び類似の自己免疫疾患、ベヒテレフ病、狼瘡型疾患、炎症性腸疾患、クローン病、腫瘍成長、又は固形臓器がんの予防及び/又は治療用の医薬組成物の調製のための、
