Description
本願は、ネイティブ血管(native vessels)から血栓を除去するための回転血栓摘出ワイヤに関する。
ガイドワイヤ及び第1カテーテルを大腿動脈に導入する工程と、
脈管系を通して第1カテーテルを前進する工程と、
ガイドワイヤを取り除く工程と、
ハウジング及びこのハウジングから遠位方向に延びる第2カテーテルを提供する工程と、
導入器シースを提供する工程と、
導入器シースをハウジングに連結する工程と、
導入器シース及び第2カテーテルを通して回転血栓摘出ワイヤを挿入する工程と、
血栓摘出ワイヤをカテーテル内で前進し、脳動脈にアクセスする工程と、
次いで、モータを血栓摘出ワイヤの近位端に作動的に連結する工程と、
モータを賦勢し、血栓摘出ワイヤを回転し、脳動脈内の血栓を浸軟する工程とを含む、方法を提供する。
本開示の好ましい実施例を添付図面を参照して説明する。
図1の血栓摘出装置の全体に参照番号10が付してある。図1及び図2を参照すると、装置は、モータハウジング12と、回転血栓摘出ワイヤ30と、回転止血バルブ(RHV)40と、導入器シース60と、入れ子チューブ即ちチューブ状コネクタ80とを含む。RHV40は、使用方法と関連して以下に論じるように(例えば図10を参照されたい)導入器カテーテル100に連結できる。導入器シース60は、血栓摘出ワイヤ30をRHV40及び導入器カテーテル100を通して容易に挿入するため、RHV40に挿入できる。
本明細書中に開示した血栓摘出装置即ちアッセンブリ10は、回転血栓摘出ワイヤ30をカテーテルとは別のユニットとして提供する。即ち、血栓摘出ワイヤ30は、RHV40を通して挿入できる別のユニットとして提供される。手術箇所にアクセスするため、RHV40の遠位端52は、導入器カテーテル100の近位端に連結される。導入器シース60は、血栓摘出ワイヤをRHV40内に、及び導入器カテーテルを通して挿入するのを補助する。導入器シース60の壁は、ワイヤがRHV40に進入するとき、ワイヤ30の直線状でない遠位端を実質的に真っ直ぐな(実質的に直線状の)形状に維持する。

Claims (14)
- 血管血栓又は他の閉塞体を破壊するための回転血栓摘出ワイヤにおいて、
近位領域及びこの近位領域よりも小径の遠位領域を持ち、モータによって回転できるコアと、前記コアの前記遠位領域に連結され、そこから遠位方向に延びるケーブルと、前記ケーブルに被せて位置決めされたトルクチューブと、前記ケーブルの遠位部分に被せて位置決めされたコイルとを含み、前記ケーブルの前記遠位部分および前記コイルは非線型形状を有し、更に、第1カバー材料が前記コイルに被せて位置決めされている、血栓摘出ワイヤ。 - 請求項1に記載の血栓摘出ワイヤにおいて、更に、
前記コアの前記遠位領域を前記ケーブルの前記近位領域に連結するハイポチューブを含む、血栓摘出ワイヤ。 - 請求項1又は2に記載の血栓摘出ワイヤにおいて、更に、
前記第1カバー材料を覆う熱収縮体を含む、血栓摘出ワイヤ。 - 請求項1、2、又は3に記載の血栓摘出ワイヤにおいて、更に、
前記コアの一部に被せた、前記トルクチューブ上を前記第1カバー材料の近位領域まで延びる第2カバー材料を含む、血栓摘出ワイヤ。 - 請求項1乃至4のうちのいずれか一項に記載の血栓摘出ワイヤにおいて、
前記ケーブルの前記非線型形状遠位部分および前記非線型形状コイルは曲がりくねった形状をしている、血栓摘出ワイヤ。 - 請求項5に記載の血栓摘出ワイヤにおいて、
前記曲がりくねった形状は、第1振幅とこの第1振幅の遠位側の第2振幅とを有し、前記第2振幅は前記第1振幅よりも大きい、血栓摘出ワイヤ。 - 請求項1乃至4のうちのいずれか一項に記載の血栓摘出ワイヤにおいて、
前記ケーブルの前記非線型形状遠位部分および前記非線型形状コイルは、J字形状である、血栓摘出ワイヤ。 - 請求項1乃至7のうちのいずれか一項に記載の血栓摘出ワイヤにおいて、
前記ワイヤは、近位端がモータ駆動シャフトに連結されている、血栓摘出ワイヤ。 - 請求項1乃至8のうちのいずれか一項に記載の血栓摘出ワイヤにおいて、
前記ワイヤは、ハウジングの内腔内で移動自在であり、前記内腔と連通した吸引ポートが前記ハウジングから延びている、血栓摘出ワイヤ。 - 血管血栓又は他の閉塞体を破壊するためのアッセンブリにおいて、
内腔を持つ導入器シースと、
請求項1に記載の回転血栓摘出ワイヤであって、前記導入器シースの前記内腔内で摺動自在の回転血栓摘出ワイヤと、
遠位部分及び近位部分を持ち、前記遠位部分は前記導入器シースに連結でき、前記近位部分はモータハウジングに連結できるコネクタをと、を含み、前記ワイヤは、前記モータハウジング内に位置決めされたモータに作動的に連結できるアッセンブリ。 - 請求項10に記載のアッセンブリにおいて、更に、
第1内腔を持つハウジングを含み、前記導入器シースは前記ハウジングに連結でき、前記第1内腔を通って挿入できる、アッセンブリ。 - 請求項11に記載のアッセンブリにおいて、
前記ハウジングは、第2内腔を持つ吸引アームを含み、前記第2内腔は、前記ワイヤの回転によって除去されたパーティクルを取り除くように形成されている、アッセンブリ。 - 請求項11又は12に記載のアッセンブリにおいて、更に、
前記ハウジングから遠位方向に延びるカテーテルを含み、前記ワイヤを前記カテーテルから露出することにより、前記ワイヤの遠位部分を非線型形状にする、アッセンブリ。 - 請求項10乃至13のうちのいずれか一項に記載のアッセンブリおいて、
前記ケーブルの前記遠位部分及び前記コイルは、前記カテーテルから露出されたとき、非線型形状をとる、アッセンブリ。
