Description
一定の状況下では、患者の血管、房(室)、経(通)路、穴(欠損部)、内(体)腔を閉塞させて、血流の通過を阻止する必要が生じよう。
小直径の移行部を器具の中間円筒部の端部に形成した凹部内に引込めることによって、血管内に器具を正確に配置する作業が、過度に危険なことではなくなった。凹部に引込めることによって、ディスクと円筒部とは、自由スペース内にあるときに接近位置に保持できることになり、また器具が機能している間には、ディスクと円筒部との離間を僅かにすることができる。
金属ストランドは、2組の事実上平行で概ね螺旋状のストランドを形成し、一方の組のストランドは、一つの「方角」(hand)を有し、つまり一方の方向に回旋し、他方の組は反対方向に回旋する。こうすることで、繊維産業で管状編組物として知られる概ね管状の織製品が形成される。既述のアムプラッツおよびコトゥラによる特許は医療器具および医療器具の製造方法を詳細に説明しているので、これ以上の詳しい説明は不要である。
本発明に合わせて医療器具を形成する場合、金属織製品の適当な寸法の一片が、例えばワイヤストランドを編んで長尺の管状編組物にされた、より大きい織製品片から切り取られる。織製品を目標寸法に切り取る際には、繊維がほつれないように注意せねばならない。
適当な寸法の金属織製品片が得られたら、織製品片を成形部材の成形面に概ね合致するように変形させる。金属織製品のストランドの相対位置は、織製品の変形により再配向され、初期状態から再配向された第2配置形状に変わる。成形部材の形状は、拘束されない場合の医療器具の事実上目標形状に織製品を変形するように選択すべきである。
熱処理後、織製品は、成形部材から外されるが、その形状は変形状態に事実上維持される。
先行技術に比して改良点は、移行部直径Hとディスク直径Aとを有し、円筒部12とディスク部14との間のこの移行部直径Hが、円筒部の直径Bよりも小さいことである。この小移行部直径により、壁部が正確に直角ではない(直角±45度)開口を有する血管壁に容易にディスク部を配向することができる。加えて、直径Hの移行部が円筒部端部の凹部内に引込められることにより、器具は、ディスク/円筒部間の張力を維持するためのばね部材に似た作用によって、配置されている部位の解剖学的な形状に順応することが可能になる。ディスクと円筒部との間隔が器具の性能に影響することはないだろう。
当業者には、血管用器具による閉塞処置を迅速化するためには、血管用器具に、適当な血栓形成剤をコーティングするか、ポリエステル繊維を充填するか、ワイヤストランド数を増して編組できることが、理解されよう。先行技術の器具では、好ましくは、編組器具内にポリエステル繊維(図6の符号303)が用いられた。この繊維は、カテーテルを介して給送する際、血管用器具と共に容易に縮小させられる。編み込まれた繊維は、凝血に付加されることで、血管用器具が閉塞部を形成する際、血管用器具内に凝血をしっかりと拘束する。
円筒部12およびディスク14の寸法と器具の長さとは、異なる寸法の血管通路、腔、洞に応じて望むように変更できる。以下の表は、選択した器具ごとの寸法範囲を単位mmで示したものである。
肺血管閉塞疾患および肺房高血圧症(pulmonary atrial hypertension)は成人期に発症する。危険なシャントを有する心房中隔欠損(ASD)患者は、理想的には5歳の折に手術するのがよく、それ以後の年齢の場合は、いつでも診断の時点に手術するのがよい。欠損の正確な解剖学的形状は、2次元心エコー法およびカラーフロー・ドップラー法が現れてから、可視化できるようになった。使用すべきASD閉塞器具の寸法は、欠損部の寸法に対応させることになろう。
当業者は、以上の説明から、成形型のキャビティがASD器具の目標形状と合致せねばならないことが分かるだろう。改良型の場合、成形型は、小直径部309を形成するような付形を必要とする。
器具による血管の閉塞を迅速化するために、器具は、適当な血栓形成剤でコーティングされるか、ポリエステル繊維を添加されるか、ワイヤストランドの数を増して編組されるかすることができる。ポリエステル繊維303,303´(図4および図8Cに示す)は、凝血過程を加速するために、任意に編組物内へ添加される。この繊維は、カテーテルを介して給送する際、容易に器具と共に縮小し、ディスク、中間部、複数部分の組み合わせのいずれか内へ添加することができる。凝血に付着することで編み混ぜられるこの繊維は、器具が閉塞部を形成する時に、凝血をしっかりと器具内に拘束する。
大腿静脈から進入して、給送カテーテル29はASDを通過する。器具300´は、給送カテーテル内を前進させられ、先端側ディスク304´が給送カテーテル端部を出て拘束を解かれ、左心房内でディスク形状に復元される。
給送カテーテル29は、その後でASDを通って基端方向に引き戻され、給送手段28が静止保持され、中隔318に対し先端側ディスク304´が当て付けられる。給送カテーテル29は、次いで更に中隔318から引き離され、基端側ディスク302´が給送カテーテル29を出て拡張し、予め決められた拡張ディスク形状に弾発的に復元できる。このようにして、ASD用器具300´が配置され、それによって先端側ディスク304´が中隔318の一方の側を押圧するのに対し、基端側ディスク302´が中隔318の他方の側を押圧するようにされる。
ASD閉塞器具300´が適切に配置された場合、医師は、給送手段28を回転させ、給送手段28を閉塞器具300´の把持手段310´から外す。把持手段310´のねじは、給送手段28を回転させることで、給送手段が閉塞器具300´の把持手段310´から外れるようにしたもので、単に閉塞器具300´を回転させるだけのものではない。既述のように、別の実施例では、適切な位置決めが保証されるように、操作者には、器具の展開中、器具の保持またはばね作用の制御が可能である。
器具、例えば既述の器具10,300が、患者の体内で永久的に通路閉塞に使用される場合には、給送手段(例えば図6の例)を、器具との逆転可能の結合部を逆回転させることで簡単に解離し、カテーテルと給送手段とを患者の体内から引き出すことができる。こうすることで、医療器具は、患者の血管系内に配置されたままとなり、患者の体内で血管その他の通路を閉塞することができる。
例えば、2重ディスク構成の場合、器具の一方の端部のみが、ディスク/隣接中間円筒部間に小さい移行部直径を有するのが望ましいことが予想される。また、中間円筒部または円筒部が一方のまたは双方のディスクと非同心的でよいことも予想される。更に、円筒部は、本発明の範囲を逸脱することなく、バレル形状、凹状、凸状、テーパ状、これらの形状の組み合わせのいずれかであってよいことも予想される。同じように、円筒部の先端および基端部は、説明した逆円錐形とは異なる形状を有するようにして、しかも既述の利点が失われないようにすることもできるだろう。
Claims (10)
- 縮小可能な医療器具において、
複数の金属ストランドを含み、基端部及び先端部を有する管状金属織製品であって、前記管状金属織製品は、患者の体内通路を通じた給送用の縮小形状と、前記患者の体内の開口を実質的に閉塞する拡張形状とを有する、管状金属織製品を備えており、
前記拡張形状では、基端部に近接する第1直径の第1部分と、先端部に近接する第2直径の第2部分と、前記第1部分と前記第2部分の間を延びる第3直径の第3部分と、を有し、前記第3直径は前記第2直径よりも小さく、かつ、前記第2直径は前記第1直径よりも小さくされ、
前記第2部分は、該第2部分が患者の体内の前記開口内に配置されたときに、前記第1部分と前記第2部分の間に張力を維持するように構成された予め設定された形状を有する、医療器具。 - 前記第2部分の先端側は、テーパ面を有する、請求項1に記載の医療器具。
- 前記第3部分は、少なくとも部分的に前記第2部分内に引っ込んでいる、請求項1に記載の医療器具。
- 前記第2部分は、前記医療器具が配置されたときに、前記患者の体内の前記開口に近接する組織に係合するように構成された少なくとも1つの鉤を有する、請求項1に記載の医療器具。
- 前記第2部分は、円錐表面を有する、請求項1に記載の医療器具。
- 前記第1直径の前記第3直径に対する比を約3〜30とする、請求項1に記載の医療器具。
- 前記第1直径の前記第3直径に対する比を約20〜25とする、請求項6に記載の医療器具。
