Description
〔実施例1:末梢血単核球の培養物の上清による、創傷治癒の大幅な強化〕
非治癒性の皮膚潰瘍は、ほとんどの一般的な治療法に対して耐性を有することが多い。従来の研究では、末梢血単核球(peripheral blood mononuclear cell; PBMC)を塩基性線維芽細胞増殖因子とともに適用することは、糖尿病性壊疽に対する有用な治療法であると考えられることが示された。本実施例では、PBMC(放射線の照射を受けたPBMCか、放射線の照射を受けていないPBMCかのいずれか)の培養物の上清が、マウスモデルにおいて創傷治癒の強化を誘発するために十分であるかどうかを調べた。さらに、これらの上清の、ヒトの一次線維芽細胞(fibroblast; FB)、ケラチノサイト(keratinocyte; KC)、および内皮細胞(endothelial cell; EC)に対する効果を分析した。
活性化された末梢血単核球(peripheral blood mononuclear cell; PBMC)およびその上清(supernatant; SN)は、創傷の再生において有益であると考えられている(Holzinger C et al. Eur J Vasc Surg. 1994 May; 8(3): 351-6.)。非活性化PBMCおよび非活性化PBMCに由来するSNには、実験的な急性心筋梗塞(acute myocardical infarct; AMI)および創傷を形成したモデルにおいて薬効があることが、実施例1および実施例2において示された。PBMCの非活性化は実験的に検証されなければならないので、PBMCの培養が、T細胞活性化マーカー(CD69、CD25)の強化または炎症性サイトカインの分泌(単球の活性化=TNFα、T細胞活性化=INFγ)の強化につながるかどうかを調べた。対照実験では、培養されたT細胞が、CD3 mAb刺激または植物性血球凝集素(Phytohemagglutinin; PHA)によってトリガーされる。
静脈血を健常人からEDTA管に収集した。フィコール・ハイパック密度グレード分離(density grade separation)を実施した後に、PBMCを収集し、生存細胞と放射線の照射を受けたアポトーシス細胞(IA−PBMC)とに分けた。アポトーシス細胞を得るために、PBMCを60Gy(セシウム137)に曝露した。フローサイメトリー法による分析を実施するために、500,000個のPBMCを200μlの無血清培地中で培養した。細胞に対してPHA(7μg/ml)もしくはCD3−mAb(10μg/ml)で刺激を与えるか、または刺激を全く与えなかった。24時間のインキュベーションの後に、細胞を洗浄し、CD3、CD69、およびCD25について染色(R&D System社)し、FC500(Coulter社)で表面活性化マーカーについて評価した。ELISAアッセイを実施するために、PBMCを、PHA刺激またはCD3刺激を与えるものと与えないものとに分けて、2.5×106個/mlの密度で一晩培養した。24時間後に、上清を回収し、−20℃で凍結させた。TNF−α(R&D社)用およびINF−γ(Bender社)用の市販のELISAキットを購入した。手短に説明すると、MaxiSorpプレートをTNF−αおよびINF−γに対する抗体でコーティングし、一晩保存した。24時間後に、プレートを洗浄し、試料を各ウェルに2個ずつ添加した。インキュベーションならびに検出用抗体およびStrep−tavidin−HRPの添加の後、TMB基質を各ウェルに添加した。発色後、スルヒック酸(sulphic acid)を添加することによって、酵素反応を停止させた。光学濃度値を、Wallac Victor3プレート読み取り装置で読み取った。
FACS分析
CD3で刺激されたT細胞およびPHAで刺激されたT細胞は、24時間のインキュベーション後に、活性化マーカーCD69およびCD25の上方制御を示した。無刺激の細胞およびアポトーシス細胞は、ほんのわずかな量のCD69およびCD25を発現させるだけであった(図6a(代表的な試料、図6b、ヒストグラム、n=4))。統計的有意性は星印で表す(**p<0.001、*p<0.05)。
無刺激のPBMCに由来する上清においてはTNF−αもINF−γもいずれも検出されなかったが、PHAまたはCD3で刺激を与えたPBMCから得られた上清は、ELISA分析が示唆するように、これらのサイトカインが高い値であることを示した(**p<0.001、*p<0.05、n=8)。この結果は、無刺激のPBMCとは炎症性サイトカインの分泌パターンが異なることを明確に示している。
以上のデータは、「無刺激PBMC」が、刺激PBMC(PHAおよびCD3 mAb)とは異なる別の表現型(活性化マーカー、サイトカインの分泌)であることを示唆している。
本実施例の目的は、特異的(CD3)、非特異的(レクチン、PHA)、および同種のT細胞の誘発(混合リンパ球反応、MLR)を2日(CD3、PHA)および5日(MLR)間の刺激アッセイにおいて利用する免疫アッセイに比べると、PBMCは増殖活性を有しないことを証明することである。
PBMCを、フィコール密度勾配遠心法によって若い健常人から分離し、0.2%のゲンタマイシン硫酸塩(Sigma Chemical Co.社、アメリカ)および1%のL−グルタミン(Sigma Chemical Co.社、アメリカ)を含有するRPMI(Gibco社、アメリカ)中で、200μLにつき細胞が1×105個の割合で再懸濁した。レスポンダー細胞に、CD3に対するMoAb(10μg/ml、BD社、NJ、アメリカ)によって、または、PHA(7μL/ml、Sigma Chemical Co.社、アメリカ)によって、刺激を与えるか、あるいは、放射線の照射を受けた同種のPBMCを1:1の比(MLRの場合)で用いることによって刺激を与えた。プレートを48時間または5日間インキュベーションし、その後、3[H]チミジン(3.7×104Bq/ウェル;Amersham Pharmacia Biotech社、スウェーデン)を用いて18時間パルスを与えた。細胞を回収し、3[H]チミジンの取り込み量を液体シンチレーション計数器において測定した。
刺激PBMCは、3[H]チミジン取り込み法によって測定した増殖速度が、RPMI中で無刺激で培養される生存PBMCに比べて高いことを示した(図8)。この効果は、増殖が抗原提示細胞(MLR)によってトリガーされるアッセイ同様、T細胞に特異的な刺激(PHA、CD3)を付加することによっても観察された。
この一連の実験は、培養液中で最長5日間まで保持される生存PBMCが増殖しない一方で、異なる方法で刺激を与えられたPBMCは顕著な増殖応答を示したことを示唆している。以上のデータから、無刺激でPBMCを培養すると増殖応答を引き起こさないと結論できる。
血管新生と炎症とはインビボにおいて強く関連しているので、これらのPBMCのセクレトームがT細胞に対して抗増殖効果をも示し、その結果、炎症性の免疫応答に干渉するかどうかを調べた。
フィコール密度勾配遠心法によって若い健常人から分離したPBMC(2.5×106個/ml)を、0.2%のゲンタマイシン硫酸塩(Sigma Chemical Co.社、アメリカ)および1%のL−グルタミン(Sigma Chemical Co.社、アメリカ)を含有するRPMI(Gibco社、CA、アメリカ)中で24時間インキュベーションすることによって、セクレトームを得た。上清を細胞分画から分離し、−80℃で保存した。増殖アッセイを実施するために、分離後に、同種のPBMCを200μLのRPMIにつき細胞が1×105個の割合で再懸濁した。レスポンダー細胞に、CD3に対するMoAb(10μg/ml、BD社、アメリカ)によって、または、PHA(7μL/ml、Sigma Chemical Co.社、アメリカ)によって、刺激を与えた。上清の異なる希釈物を添加した。プレートを48時間インキュベーションし、その後、3[H]−チミジン(3.7×104Bq/ウェル;Amersham Pharmacia Biotech社、スウェーデン)を用いて18時間パルスを与えた。細胞を回収し、3[H]−チミジンの取り込み量を液体シンチレーション計数器において測定した。
同種のPBMCのセクレトームが、3[H]−チミジン取り込み法によって測定する増殖速度は、正のコントロールに比べて有意に低減することを示した(図9)。この効果は用量に依存し、PHA刺激時とともに、抗CD3刺激時にも観察された。
この一連の実験は、生存PBMCから得られるセクレトームを24時間培養液中に保持すると、有意な抗増殖効果をインビトロで示すことを示唆している。以上のデータは、PBMCに由来する上清または凍結乾燥された状態にある上清は、低酸素症によって誘発される炎症またはその他の高炎症性疾患(例えば自己免疫疾患、炎症性皮膚疾患など)に関連するヒトの疾患を治療するための、潜在的な治療用処方物として作用する可能性があることを示唆している。
実施例1では、急性心筋梗塞(acute myocardical infarct; AMI)の動物モデルにおける、PBMCセクレトームの抗炎症効果が示された。本実施例では、AMIを誘発後のPBMCセクレトームの適用が、免疫応答を大きく下方制御することによって心筋の炎症性損傷を阻害することを示す。
〔PBMCセクレトームの生成〕
健常人から得られるPBMCを、フィコール密度勾配遠心法によって分離した。細胞を、ウルトラカルチャー(Ultra Culture)の培地(Lonza社、Basel、スイス)中で1×106個/ml(sup liv)の濃度で再懸濁した。アポトーシス性PBMCからセクレトームを生成するために、60Gy(sup APA)を照射することによって、アポトーシスを誘発した。細胞を、加湿雰囲気(5% CO2、37℃、相対湿度95%)中で24時間インキュベーションした。上清を除去し、50mMの酢酸アンモニウムに対して3.5kDaのカットオフ(Spectrum laboratories、オランダ)で4℃で一晩透析した。その後、上清を無菌濾過し、凍結乾燥した。凍結乾燥したセクレトームを−80℃で保存し、実験を行うたびに新たに再懸濁した。セクレトームを無作為抽出して、そのpH値を計測した。
MACSビーズシステム(Miltenyi社、Bergisch Gladbach、ドイツ)を使用してCD4+T細胞以外の細胞を枯渇させることによって、CD4+細胞を分離した。細胞は新鮮なものを準備し、ただちに各実験に使用した。
アポトーシスを、市販のアネキシンV/PIキット(BD社、New Jersey、アメリカ)を用いてフローサイトメトリーによって検出した。アポトーシスをアネキシン陽性染色法によって規定し、後期アポトーシスをPIの陽性度によって規定した。
PBMCまたは精製済みCD4+細胞を、96個の丸底ウェルを有するプレートにおいて、0.2%のゲンタマイシン硫酸塩(Sigma Chemical Co.社、St. Louis、MO、アメリカ)、0.5%のβ−メルカプトエタノール(Sigma Chemical Co.社、St Louis、MO、アメリカ)、および1%のGlutaMAX−I(インビトロゲン社、Carlsbad、CA、アメリカ)を補充したウルトラカルチャー(Ultra Culture)中で、1×105個/ウェルの濃度にまで希釈した。PHA(7μg/ml、Sigma Chemical Co.社、アメリカ)、CD3(10μg/ml、BD社、New Jersey、アメリカ)、IL−2(10U/ml、BD社、アメリカ)を用いて、または放射線の照射を受けた(60Gy)同種のPBMCを1:1の比で用いて、MLRを得るために、細胞に刺激を与えた。細胞を、複数の濃度のPBMCセクレトーム、IL−10、またはTGF−βを用いて48時間または5日間(MLR)インキュベーションした。その後、3[H]チミジン(3.7×104Bq/ウェル;Amersham Pharmacia Biotech社、Uppsala、スウェーデン)を用いて、細胞に18時間パルスを与えた。細胞を回収し、3[H]チミジンの取り込み量を液体シンチレーション計数器において測定した。
精製済みCD4+細胞を、抗CD3(10μg/ml)を用いて刺激し、複数の濃度のPBMCセクレトームでコインキュベーションした。細胞を、標準的なフローサイメトリー法の染色プロトコールにしたがって、CD69およびCD25について染色し、フローサイトメーターFC500(Beckman Coulter社、Fullerton、CA、アメリカ)で分析した。
予備試実験において、生細胞(sup liv)から得られるPBMCの上清の抗増殖性特性について試験を行った。抗CD3刺激実験およびPHA刺激実験では、増殖速度が、セクレトーム(n=10)を添加することによって大幅に低下した。
これらの実験によって、PBMCのセレクトームがインビトロで免疫抑制特性を有することが初めて示された。上清は、a)抗CD3刺激実験、PHA刺激実験、およびMLR刺激実験において増殖速度を低下させ、b)T細胞の誘発時にCD4+細胞のアポトーシスを誘発し活性化を阻害する効力を有することが示された。
Claims (24)
- 炎症性の皮膚性状を治療するための局所用薬剤であって、
末梢血単核球(PBMC)を、PBMC増殖性物質およびPBMC活性化物質を含有しない細胞培養培地または血清中で少なくとも1時間培養することによって得られる細胞培養培地または血清の上清を包含し、かつ、末梢血単核球(PBMC)を包含しない薬剤。 - 上記性状が、虚血に関連する性状であることを特徴とする、請求項1に記載の薬剤。
- 上記性状が、創傷、組織の虚血、慢性の創傷、糖尿病性の創傷、皮膚潰瘍、皮膚の熱傷、および形成手術における皮膚の皮弁からなる群より選択されることを特徴とする、請求項1または2に記載の薬剤。
- 上記細胞培養培地が、RPMI、DMEM、X−vivo(商標)、およびウルトラカルチャー(商標)からなる群より選択されることを特徴とする、請求項1〜3のいずれか一項に記載の薬剤。
- 上記PBMCが、ストレスを誘発する条件の下で培養されることを特徴とする、請求項1〜4のいずれか一項に記載の薬剤。
- 上記ストレスを誘発する条件には、低酸素症、オゾン、熱、放射線、化学物質、浸透圧、pHの変化、またはこれらの組み合わせが含まれることを特徴とする、請求項5に記載の薬剤。
- 上記PBMCが、培養中に少なくとも10Gyの放射線、オゾン、高温、またはUV線によってストレスを与えられることを特徴とする、請求項1〜6のいずれか一項に記載の薬剤。
- 上記PBMCが、培養中に少なくとも20Gyの放射線、オゾン、高温、またはUV線によってストレスを与えられることを特徴とする、請求項7に記載の薬剤。
- 上記PBMCが、培養中に少なくとも40Gyの放射線、オゾン、高温、またはUV線によってストレスを与えられることを特徴とする、請求項7または8に記載の薬剤。
- 上記薬剤が、ゲル、軟膏、皮膚パッチ、薬学的に許容可能なマトリクス、ペースト、クリーム、粉末、リニメント剤、またはローションとして提供されることを特徴とする、請求項1〜9のいずれか一項に記載の薬剤。
- 上記ゲルが、ハイドロゲルであることを特徴とする、請求項10に記載の薬剤。
- 上記薬学的に許容可能なマトリクスが、コラーゲン/エラスチンのマトリクスの上であることを特徴とする、請求項10に記載の薬剤。
- 上記上清が凍結乾燥されることを特徴とする、請求項1〜12のいずれか一項に記載の薬剤。
- 上記PBMCが、上記細胞培養培地または血清中で少なくとも4時間培養されることを特徴とする、請求項1〜13のいずれか一項に記載の薬剤。
- 上記PBMCが、上記細胞培養培地または血清中で少なくとも6時間培養されることを特徴とする、請求項14に記載の薬剤。
- 上記PBMCが、上記細胞培養培地または血清中で少なくとも12時間培養されることを特徴とする、請求項14または15に記載の薬剤。
- 末梢血単核球(PBMC)を、PBMC増殖性物質およびPBMC活性化物質を含有しない細胞培養培地または血清中で少なくとも1時間培養することによって得られる細胞培養培地または血清の上清を包含し、かつ、末梢血単核球(PBMC)を包含しない、炎症性の皮膚性状を治療するための局所用薬剤を、炎症性の皮膚性状を治療するための薬物の製造の用途に用いる使用方法。
- 上記性状が、虚血に関連する性状であることを特徴とする、請求項17に記載の使用方法。
- 炎症性の皮膚性状を治療するための、末梢血単核球(PBMC)を包含しない局所用薬剤を調製する方法であって、
a)末梢血単核球(PBMC)を準備するステップと、
b)ステップa)のPBMCを、PBMC増殖性物質およびPBMC活性化物質を含有しない細胞培養培地または血清中で少なくとも1時間培養するステップと、
c)ステップb)の上清を単離するステップと、
d)ステップc)の上清を用いて上記薬剤を調製するステップとを含む、方法。 - ステップb)の前またはステップb)の途中で、上記PBMCがストレスを誘発する条件に曝され、
このストレスを誘発する条件には、低酸素症、オゾン、熱、放射線、化学物質、浸透圧、pHの変化、またはこれらの組み合わせが含まれることを特徴とする、請求項19に記載の方法。 - ステップb)の前またはステップb)の途中で、上記PBMCが、少なくとも10Gyの放射線、オゾン、高温、またはUV線に曝露されることを特徴とする、請求項19または20に記載の方法。
- ステップb)の前またはステップb)の途中で、上記PBMCが、少なくとも20Gyの放射線、オゾン、高温、またはUV線に曝露されることを特徴とする、請求項21に記載の方法。
- ステップb)の前またはステップb)の途中で、上記PBMCが、少なくとも40Gyの放射線、オゾン、高温、またはUV線に曝露されることを特徴とする、請求項21または22に記載の方法。
- 請求項19、21、22または23に記載の方法によって得られる薬剤。
