取り外し可能なガイドワイヤーチューブを有する拡張可能なプロステーシスデバイスを下記のように製作することができる。
1) 外径3.1cm、長さ15cm、及びグラフト壁厚約0.005インチの自己拡張型主要本体ステントグラフトを用意することができる。グラフト材料は、ePTFEとFEPとから成り、また押し出され延伸膨張された薄肉チューブから形成されることが可能である。このチューブには続いてePTFEフィルムを巻き付ける。直径約0.0165インチのニチノールワイヤーを螺旋状に巻くことにより、起伏のある正弦波パターンを有するステントを形成することができる。このように形成された熱処理済ステントをベースグラフト上に置くことができる。ステント及びベースグラフト上に、ePTFEとFEPとから成る付加的なフィルム層を巻き付けることにより、グラフトに対してステントを選択的に付着することができる。
本発明の実施形態としては、以下の実施形態を挙げることができる。

主要本体ガイドワイヤーを主要血管中に前進させる工程、
側枝ガイドワイヤーを側枝血管中に前進させる工程、
第一開口端部と、第二開口端部と、該第一開口端部から該第二開口端部まで延在する壁と、該第一開口端部から該第二開口端部まで延在するルーメンと、該壁中の少なくとも一つのサイド開口部とを有する拡張可能な主要本体デバイス及び、第一端部と、第二端部と、ルーメンとを有するガイドワイヤーチューブであって、該ガイドワイヤーチューブが、少なくとも該主要本体デバイスの該サイド開口部から、該主要本体デバイスの該ルーメンを通って、該第二開口端部に対して近位である地点まで延在するガイドワイヤーチューブ、を提供する工程、
第一カテーテルの遠位部に該拡張可能な主要本体デバイスを装填する工程であって、該第一カテーテルが、該第一カテーテルの近位端部部から遠位端部部まで延在するルーメンを有する工程、
該主要本体ガイドワイヤーに該第一カテーテルの該ルーメン及び該拡張可能な主要本体デバイスの該ルーメンを逆装填する工程、
該側枝ガイドワイヤーに該ガイドワイヤーチューブを逆装填する工程、
該第一カテーテル及び該主要本体デバイスを該主要血管中に前進させ、該側枝血管と該サイド開口部を並列にする工程、
該主要本体デバイスを拡張する工程、並びに、
該第一カテーテルを取り除く工程、
を含む方法。
(付記22)遠位端部部と、近位端部部と、遠位部と、近位部と、該遠位端部部から該近位端部部まで延在するルーメンとを有する第二カテーテル及び、該遠位部に装填される拡張可能な側枝デバイスを提供する工程、
該第二カテーテル及び該拡張可能な側枝デバイスを、前進させて前記拡張した主要本体デバイスの中を通過させてサイド開口部の外側に出して、そして少なくとも一部分的に前記側枝血管中に入れる工程、
該側枝デバイスを拡張する工程、並びに、
該第二カテーテルを取り除く工程、
を更に含む、付記21に記載の方法。
Claims (17)
- 第一開口端部と、第二開口端部と、該第一開口端部から該第二開口端部まで延在する壁と、該第一開口端部から該第二開口端部まで延在するルーメンと、該壁中の少なくとも一つのサイド開口部とを有する拡張可能な主要本体デバイス及び、
第一端部と、第二端部と、ルーメンとを有するガイドワイヤーチューブを含み、該ガイドワイヤーチューブが、少なくとも該主要本体デバイスの該サイド開口部から、該主要本体デバイスの該ルーメンを通って、該第二開口端部に対して近位である地点まで延在し、該主要本体デバイスが圧縮状態である場合に、該ガイドワイヤーチューブが該主要本体デバイスから取り外し可能であり、前記ガイドワイヤーチューブの前記第二端部が閉じられている、
拡張可能なプロステーシス。 - 前記主要本体デバイスがステントを含む、請求項1に記載の拡張可能なプロステーシス。
- 前記ステントが自己拡張型ステントである、請求項2に記載の拡張可能なプロステーシス。
- 前記拡張可能な主要本体デバイスがバルーン方式で拡張可能である、請求項1に記載の拡張可能なプロステーシス。
- 前記主要本体デバイスがステントグラフトを含む、請求項1に記載の拡張可能なプロステーシス。
- 前記グラフトが、ePTFE、ナイロン、ポリエステル、ポリエチレン、ポリプロピレン、ポリテトラフルオロエチレン、ポリウレタン及び弾性有機ケイ素ポリマーからなる群から選ばれる材料を含む、請求項5に記載の拡張可能なプロステーシス。
- 前記ステントグラフトが、前記第一開口端部の第一ステントと、前記第二開口端部の第二ステントとを含む、請求項5に記載の拡張可能なプロステーシス。
- 前記グラフト材料が前記第一開口端部から前記第二開口端部まで延在する、請求項7に記載の拡張可能なプロステーシス。
- 前記ガイドワイヤーチューブの端部が前記チューブの残部に対して拡張して前記拡張可能なプロステーシスと連結して用いられるイントロデューサーシースのルーメン中に、前記ガイドワイヤーチューブが挿入されることを防止する、請求項1に記載の拡張可能なプロステーシス。
- 前記ガイドワイヤーチューブが、PTFE、ナイロン及びポリエーテルブロックアミドからなる群から選ばれる材料を含む、請求項1に記載の拡張可能なプロステーシス。

- 前記内部側枝支持体が前記主要本体デバイスの前記サイド開口部から前記第二開口端部に向かって延在する、請求項13に記載の拡張可能なプロステーシス。
- 前記拡張可能なプロステーシスが、前記拡張可能なプロステーシスを包装するシース材料を更に含む、請求項1に記載の拡張可能なプロステーシス。
- 前記シース材料が前記シース材料内に少なくとも一つの開口部を有する、請求項15に記載の拡張可能なプロステーシス。
