Description
IIb/IIIa阻害剤(例えばインターグリリン、アブシキマブ)、セラトニンアンタゴニスト、および5−HT吸収阻害剤;コレステロール低下剤;血管拡張剤;および内因性血管活性メカニズムを妨害する薬剤が挙げられる。ポリヌクレオチドのシーケンスは、p15、p16、p18、p19、p21、p27、p53、p57、Rb、nFkBおよびE2Fデコイなどの抗再狭窄剤、チミジンキナーゼ(「TK」)およびそれらの組み合わせ、ならびに細胞増殖の妨害に有用な他の薬剤としても機能する。活性物質の選択は、所望の臨床結果、および特定の患者の病状および禁忌の性質を考慮して行うことができる。薬剤を含んでいても、含んでいなくても、本明細書で開示されるステントはいずれも、生体吸収性材料から作ることができる。
Claims (29)
- 主要脈管と第1および第2枝脈管の分岐部を治療する展開システムであって、
近位端および遠位端を有する、細長い可撓性の本体と、
約5mmから約20mmの範囲内の軸方向の長さを有する前記本体の遠位端の窪みに担持された単一のステントと、
後退すると前記単一のステントが接触する縁部を与えるように構成された管状の外装内に配置され、かつ、前記単一のステントの近位マーカーを受け入れるスロットを備えたステント止め部と、
前記ステントを前記可撓性の本体上に保持するための着脱式抑制部とを備え、
前記着脱式抑制部が、少なくとも部分的に前記細長い可撓性の本体中の管腔内に延びる、軸方向に移動可能な制御要素に連結されており、
前記単一のステントの下流側の遠位端が、抑制されていない拡張形状では、このステントの上流側の近位端より直径が大きく、かつ、前記下流側の遠位端は、前記細長い可撓性の本体の遠位端において上流側の近位端よりも遠位側に配置され、
拡張したステントは、ほぼ円錐形の形状であり、近位端から遠位端まで、その全長手方向に沿ってテーパ状になる展開システム。 - さらに、前記可撓性の本体の少なくとも一部を通って軸方向に延びるガイドワイヤ管腔を備える、請求項1に記載の展開システム。
- 前記ガイドワイヤ管腔が近位アクセス口および遠位アクセス口を有し、
前記近位アクセス口が前記可撓性の本体に沿って、前記近位端から遠位方向に間隔をおいて配置される、請求項2に記載の展開システム。 - 前記ガイドワイヤ管腔が近位アクセス口および遠位アクセス口を有し、
前記近位アクセス口が前記可撓性の本体の近位端に配置される、請求項2に記載の展開システム。 - 前記軸方向に移動可能な制御要素が、前記可撓性の本体の長さ方向に沿って延びる、請求項1に記載の展開システム。
- 前記着脱式抑制部が管状の外装を備える、請求項1に記載の展開システム。
- 前記軸方向に移動可能な制御要素が引っ張りワイヤを備える、請求項1に記載の展開システム。
- 前記着脱式抑制部が溶解性の中膜を備える、請求項1に記載の展開システム。
- 前記単一のステントの拡張した直径が、遠位方向に半径方向外側にテーパ状になる、請求項1に記載の展開システム。
- 前記単一のステントが非円筒形である、請求項1に記載の展開システム。
- 前記本体の近位端上にハンドピースをさらに備える、請求項1に記載の展開システム。
- 前記ハンドピースが、前記単一のステントを展開するための制御部を備える、請求項11に記載の展開システム。
- 前記ガイドワイヤ管腔が、オーバーザワイヤー構成のガイドワイヤ管腔である、請求項2に記載の展開システム。
- 前記ガイドワイヤ管腔が、ラピッドエクスチェンジ構成のガイドワイヤ管腔である、請求項2に記載の展開システム。
- 前記単一のステントが、約10mmから約20mmの範囲内の長さを有する、請求項1に記載の展開システム。
- 前記単一のステントが、テーパ状のステントである、請求項1に記載の展開システム。
- 前記単一のステントが、より小さい近位直径から、前記分岐部に面するより大きい遠位直径まで、遠位方向にテーパ状になる、請求項16に記載の展開システム。
- さらに、前記単一のステントを解放するように形成された、近位方向に後退可能な外装を有する送出カテーテルを備える、請求項1に記載の展開システム。
- 前記単一のステントが、少なくとも約10mmの長さを有する、請求項1に記載の展開システム。
- 前記単一のステントの遠位端が、このステントの近位端の直径の少なくとも約105%である直径まで拡張する、請求項1に記載の展開システム。
- 前記単一のステントの遠位端が、このステントの近位端の直径の少なくとも約110%である直径まで拡張する、請求項1に記載の展開システム。
- 前記本体の遠位端の窪みの軸方向の長さが、約10mmから約18mmの範囲内である、請求項1に記載の展開システム。
- 前記単一のステントの上流側の近位端の直径が、約3mmから約6mmの範囲内である、請求項1に記載の展開システム。
- 前記単一のステントの上流側の近位端の直径が、約3.5mmから約5.5mmの範囲内である、請求項23に記載の展開システム。
- 前記単一のステントの下流側の遠位端の直径が、約5mmから約8mmの範囲内である、請求項1に記載の展開システム。
- 前記単一のステントの下流側の遠位端の直径が、約5.5mmから約7.5mmの範囲内である、請求項25に記載の展開システム。
- 前記着脱式抑制部が、単一のステントの区間を囲むように構成され、前記着脱式抑制部の材料に前記単一のステントが埋め込まれるのを防止する複数の拡張制限帯を備える、請求項1に記載の展開システム。
- 前記単一のステントの下流側の遠位端の直径が、抑制されていない拡張形状で、ステントの軸方向の長さの少なくとも約40%である、請求項1に記載の展開システム。
- 前記単一のステントの下流側の遠位端の直径が、抑制されていない拡張形状で、ステントの軸方向の長さの少なくとも約50%である、請求項1に記載の展開システム。
