Claims (51)
- 神経の組織部位で欠陥部に減圧をかける装置であって、当該装置が、
神経導管であって、内面と、前記神経導管を通って延在する複数の細孔とを有し、前記内面は、減圧を受けるべく、前記欠陥部と前記神経導管の内面の間の内腔空間に流体を含むように、前記欠陥部を囲むように適合された部分と、前記神経の無傷の部分を囲むように適合された別の部分とを含み、前記神経導管の細孔は前記神経導管外から体液が前記内腔空間に入らないよう十分小さい、神経導管と、
減圧を受けるための導管に連結された多孔性材料で形成され、前記欠陥部の近くの前記内腔空間内に配置されるマニホルドであって、前記多孔性材料は減圧を前記欠陥部にかけるための複数の流路を有する、マニホルドとを含むことを特徴とする装置。 - 請求項1に記載の装置において、前記欠陥部が切断された、部分的に切断された、挟まれた、または変質した神経であることを特徴とする装置。
- 請求項1に記載の装置において、前記マニホルドが、前記組織部位に対する神経の遠位側に配置されることを特徴とする装置。
- 請求項1に記載の装置において、前記マニホルドが、前記組織部位に対する神経の遠位側に減圧をかけることを特徴とする装置。
- 請求項1に記載の装置において、前記マニホルドが、生体不活性材料からなることを特徴とする装置。
- 請求項1に記載の装置において、前記マニホルドが、生体吸収性材料からなることを特徴とする装置。
- 請求項1に記載の装置において、前記マニホルドの一部が、組織の成長を実現させる足場材料を含むことを特徴とする装置。
- 請求項7に記載の装置において、前記足場材料が、発泡体またはゲル材料から形成されることを特徴とする装置。
- 請求項7に記載の装置において、前記足場材料が、フィブリンまたはコラーゲンから選択される生物学的材料であることを特徴とする装置。
- 請求項7に記載の装置において、前記足場材料が、生物活性剤を含むことを特徴とする装置。
- 請求項10に記載の装置において、前記生物活性剤が、抗生物質、抗体および成長因子のうちの少なくとも1を含むことを特徴とする装置。
- 請求項11に記載の装置において、前記生物活性剤が、成長ホルモン(GH)、骨形成タンパク質(BMP)、トランスホーミング増殖因子−α(TGF−α)、TGF−β、繊維芽細胞成長因子(FGF)、顆粒粒コロニー刺激因子(G−CFS)、顆粒粒/マクロファージコロニー刺激因子(GM−CFS)、上皮増殖因子(EGF)、血小板由来成長因子(PDGF)、インスリン様成長因子(IGF)、血管内皮成長因子(VEGF)、幹細胞増殖因子/散乱係数(HGF/SF)、インターロイキン、腫瘍壊死因子−α(TNF−α)または神経成長因子(NGF)であることを特徴とする装置。
- 請求項1に記載の装置において、前記マニホルドが、前記内腔空間を通り概ね軸方向に延在する縦軸を有する円筒形状であることを特徴とする装置。
- 請求項13に記載の装置において、前記欠陥部が切断された端部を有する切断された神経であり、前記切断された端部間に前記マニホルドが配置されていることを特徴とする装置。
- 請求項14に記載の装置において、前記マニホルドが端面を有し、さらに前記マニホルドの端面の1に流体連結している第1の端部と、前記内腔空間から延在している第2の端部とを有する少なくとも1のマニホルドロッドを含むことを特徴とする装置。
- 請求項15に記載の装置において、前記マニホルドが、1の端面からのみ延在する少なくとも1のマニホルドロッドを含むことを特徴とする装置。
- 請求項15に記載の装置において、前記少なくとも1のマニホルドロッドが、生体吸収性材料からなることを特徴とする装置。
- 請求項15に記載の装置において、前記少なくとも1のマニホルドロッドが、組織の成長を実現させる足場材料を含むことを特徴とする装置。
- 請求項15に記載の装置において、前記少なくとも1のマニホルドが、前記内腔空間と流体連通している多孔性壁を有する管状であることを特徴とする装置。
- 請求項13に記載の装置において、前記マニホルドが、端面を有し、さらに前記マニホルドの縦軸に沿って前記端面間に延在する開口を含むことを特徴とする装置。
- 請求項20に記載の装置において、前記欠陥部が、前記マニホルドの開口を通して延在する挟まれた神経であることを特徴とする装置。
- 請求項21に記載の装置において、前記マニホルドがさらに、前記マニホルドの端面の1に流体連結される第1の端部と、前記内腔空間内から延在する第2の端部とを有する少なくとも1のマニホルドロッドを含むことを特徴とする装置。
- 請求項22に記載の装置において、前記マニホルドが、前記1端面からのみ延在する少なくとも1のマニホルドロッドを含むことを特徴とする装置。
- 請求項22に記載の装置において、前記少なくとも1のマニホルドロッドが、生体吸収性材料からなることを特徴とする装置。
- 請求項22に記載の装置において、前記少なくとも1のマニホルドロッドが、組織の成長を実現する足場材料を含むことを特徴とする装置。
- 請求項1に記載の装置において、前記神経導管の壁が、開口を形成する前記神経導管の長さに沿ったスライスを含み、これにより前記神経導管が、前記組織部位周辺に移植可能であり、1以上の閉塞要素で密閉可能であることを特徴とする装置。
- 請求項1に記載の装置において、前記神経導管が、生体不活性材料からなることを特徴とする装置。
- 請求項1に記載の装置において、前記神経導管が、生体吸収性材料からなることを特徴とする装置。
- 請求項28に記載の装置において、前記神経導管が、コラーゲンからなることを特徴とする装置。
- 請求項1に記載の装置において、前記細孔が、5μm乃至50μmの内径を有することを特徴とする装置。

前記減圧を供給する圧力源と、
神経導管であって、内面と、前記神経導管を通って延在する複数の細孔とを有し、前記内面は、減圧を受けるべく、前記欠陥部と前記神経導管の内面の間の内腔空間に流体を含むように、前記欠陥部を囲むように適合された部分と、前記神経の無傷の部分を囲むように適合された別の部分とを含み、前記神経導管の細孔は前記神経導管外から体液が前記内腔空間に入らないよう十分小さい、神経導管と、
減圧を受けるための導管に連結された多孔性材料で形成され、前記欠陥部の近くの前記内腔空間内に配置されるマニホルドであって、前記多孔性材料は減圧を前記欠陥部にかけるための複数の流路を有する、マニホルドとを含むシステム。 - 請求項31に記載のシステムがさらに、前記内腔空間に流体を供給するために前記マニホルドと流体連通している流体源を含むことを特徴とするシステム。
- 請求項31に記載のシステムにおいて、前記欠陥部が、切断された、部分的に切断された、挟まれた、または変質した神経であることを特徴とするシステム。
- 請求項31に記載のシステムにおいて、前記マニホルドが、前記組織部位に対する神経の遠位側に配置されることを特徴とするシステム。
- 請求項31に記載のシステムにおいて、前記マニホルドが、減圧を前記組織部位に対する前記神経の遠位側に提供することを特徴とするシステム。
- 請求項31に記載のシステムにおいて、前記マニホルドが、生体不活性材料からなることを特徴とするシステム。
- 請求項31に記載のシステムにおいて、前記マニホルドが、生体吸収性材料からなることを特徴とするシステム。
- 請求項31に記載のシステムにおいて、前記マニホルドの一部が、組織の成長を実現する足場材料を含むことを特徴とするシステム。
- 請求項38に記載のシステムにおいて、前記足場材料が、発泡体またはゲル材料から形成されることを特徴とするシステム。
- 請求項38に記載のシステムにおいて、前記足場材料が、フィブリンまたはコラーゲンから選択される生物学的材料であることを特徴とするシステム。
- 請求項31に記載のシステムにおいて、前記マニホルドが、前記欠陥部と流体連通している経路を形成する内腔面を有する概ね管状の形状であることを特徴とするシステム。
- 請求項41に記載のシステムにおいて、前記欠陥部が、前記マニホルドの経路を通り延在する挟まれた神経であることを特徴とするシステム。
- 請求項31に記載のシステムにおいて、前記欠陥部が切断された神経であり、前記マニホルドが、前記切断された神経の各端部と流体連通しており前記マニホルドを通り概ね軸方向に延在する複数の経路を有するほぼ円筒の形状であることを特徴とするシステム。
- 請求項38に記載のシステムにおいて、前記足場材料が、生物活性剤であることを特徴とするシステム。
- 請求項44に記載のシステムにおいて、前記生物活性剤が、抗生物質、抗体および成長因子の少なくとも1であることを特徴とするシステム。
- 請求項45に記載のシステムにおいて、前記生物活性剤が、成長ホルモン(GH)、骨形成タンパク質(BMP)、トランスホーミング増殖因子−α(TGF−α)、TGF−β、繊維芽細胞成長因子(FGF)、顆粒粒コロニー刺激因子(G−CFS)、顆粒粒/マクロファージコロニー刺激因子(GM−CFS)、上皮増殖因子(EGF)、血小板由来成長因子(PDGF)、インスリン様成長因子(IGF)、血管内皮成長因子(VEGF)、幹細胞増殖因子/散乱係数(HGF/SF)、インターロイキン、腫瘍壊死因子−α(TNF−α)または神経成長因子(NGF)であることを特徴とするシステム。
