Description
骨粗しょう症 は、時間の経過とともに骨組織が細くなり骨密度が減少することにより特徴付けられる骨の疾患であり、老年者に広く蔓延している。National Osteoporosis Foundation は、2020 年までに約1,400万人のアメリカ人が骨粗しょう症を患うと推定している。さらに追加の 1,800万人が低骨密度または骨減少症に罹患するとしている。骨粗しょう症は人体が十分な量の骨を形成できないか、古い骨を再吸収しすぎるか、またはその両方により惹起される。
本願に開示される一実施例は、PTH/PTHrP 受容体アゴニストに連結したコラーゲン結合性ポリペプチドの断片を含む組成物または生物活性物質を含む。本発明者らは、Clostridium histofyticum より得られるコラーゲナーゼであるCoIHのコラーゲン結合領域(CBD)に融合した PTHの活性アゴニスト断片であるPTHの1-33残基を含む融合タンパクを構築した。本発明者らは、上記融合タンパクが、in vivoでのマウスにおける骨成長促進作用において、全身的に投与した場合においてさえ、PTH(1-34)よりも高い活性を示すことを見出した。例えば骨折箇所への局所投与では、有効性の差はさらに大きいことが期待される。
別の実施例は、哺乳動物に(b) PTH/PTHrP 受容体アンタゴニストに連結した(a)コラーゲン結合性ポリペプチド断片を投与することを含む哺乳動物の毛髪の成長を促進する方法を提供する。
別の実施例は、組成物の投与前、投与中、投与後に哺乳動物がその組織に接するインプラントの移植を受け、さらに前記組成物の投与ステップが、前記インプラント周辺の組織成長の促進に有効である、(b) PTH/PTHrP 受容体アゴニストに連結した(a)コラーゲン結合性ポリペプチド断片を投与することを含む哺乳動物の移植部位周辺の組織成長を促進する方法を提供する。
本発明の開示には、生物活性物質、およびPTH/PTHrP 受容体アゴニストまたはアンタゴニストに連結したコラーゲン結合性ポリペプチド断片を含む融合タンパクを含む組成物が含まれる。好適な実施例では、前記組成物は前記PTH/PTHrP アゴニストまたはアンタゴニストがポリペプチド断片であり、融合タンパク中で前記コラーゲン結合性ポリペプチド断片 およびPTH/PTHrP ポリペプチド断片 はともに連結している融合タンパクである。しかしPTH/PTHrP アゴニストまたはアンタゴニスト部分は、非ペプチド性アゴニストまたはアンタゴニストであってもよい。
前記融合タンパクは、脱ミネラル骨マトリックス(Demineralized Bone Matrix :DBM)と混合可能である。脱ミネラル骨マトリックスは、同種移植片の骨の酸抽出により調製され、これによりほとんどの無機組成物が失われ、成長因子類を含むコラーゲンおよびコラーゲン様タンパクは保持される。DBMは、骨移植の代替物または拡張物として用いられる。DBMは大量のコラーゲンを含むため、結合用緩衝液中で前記融合タンパクとDBMが混合されると、前記融合タンパクはDBMのコラーゲンに結合する。
実施例 1
PTH-コラーゲン結合領域融合タンパクの発現
PTH-CBD 融合タンパクを発現するプラスミドを、前記PTH-CBD をコーディングするシーケンス をpGEX-5X-l (GE Lifesciences) に挿入することによって構築した。前記生成されたプラスミドの前記シーケンス は、SEQ ID NO:3である。 SEQ ID NO: 3のヌクレオチド 258から1409 は、PTH-CBD 融合タンパクにそのC-末端で融合したグルタチオン-S-転移酵素 (GST) を含む 融合タンパク をエンコードしている。SEQ ID NO: 4 は、エンコードされたGST-PTH-CBD 融合タンパク全体である。 222-225残基は、血液凝固因子Xa プロテアーゼ認識部位であるIEGR (SEQ ID NO: 5) である。SEQ ID NO: 4の 226-383残基は、SEQ ID NO: 1 に対応し、前記PTH- CBD 融合タンパクである。血液凝固因子Xaは、前記 SEQ ID NO:4のアミノ酸残基の225 番目のArgの後の位置を開裂させ、前記PTH-CBD 融合タンパクである、SEQ ID NO: 1を遊離させる。SEQ ID NO: 1 の1-33残基は、前記PTHの N-末端33 残基である。38-158残基は、前記 Clostridium histolyticum のCoIH コラーゲナーゼ のコラーゲン結合性領域 (CBD) である。前記融合タンパクの前記CBDは、CoIH (SEQ ID NO:6)の901-1021残基に相当する。 SEQ ID NO: 1の 34-37残基はリンカー断片である。
前記PTH-CBD 融合タンパクのコラーゲンへの結合の実証
5mg の不溶性タイプ Iコラーゲン (C-9879; Sigma) を、0.22 μm 低結合性DURAPORE 膜 (Millipore、マサチュセッツ州ベッドフォード: Bedford、 MA) と共にULTRA FREEミクロ遠心器具に加え、ミクロ遠心チューブ内に入れた (カタログ番号:UFCSOGVOO-Millipore) 。 特に断らない限りすべての実験操作は室温で行われた。前記コラーゲン繊維を膨潤させるため、コラーゲン 結合緩衝液 (200 μL) (50 mM トリス塩酸緩衝液、 pH 7.5、 5 mM CaCl2) を加えた。30分間インキュベートした後、前記チューブを15,000 x g で15 分間遠心分離した。ローター内の前記チューブ の方向を変えて遠心分離を繰り返した。前記コラーゲンの沈殿物を、100 pmol の 融合タンパクを含む60 μL のコラーゲン結合緩衝液 に再懸濁し、30 分間インキュベートした。その後、前記器具を介して前記混合物を15,000 x g で15 分間遠心分離した。前記コラーゲンに結合したタンパクは、前記コラーゲンとともに前記フィルター上に保持された。コラーゲンに結合しなかったタンパクは、前記フィルターを通過して濾液に入る。前記濾液は濾液 SDS-PAGEで分析された。
PTH-CBD 融合タンパクのin vitro 生物活性
ヒトPTHlRを安定的にトランスフェクトした LLCPK細胞であるHKrK-B7細胞は、マサチューセッツ総合病院内分泌部門の Tom Gardellaより親切に提供された。前記細胞は参考文献 (7) に記載されている。 HKrK-B7 細胞を90 %密集まで24 穴プレート内で成長させた。一般的に播種の後2から3 日目で成長は達成された。前記カルチャー培地はDMEM (L-glutamine入り) + 10%ウシ胎仔血清(FBS) である。
PTH-CBD 融合タンパクのin vivo 活性
5から8 週齢の 13から18 グラムの健常メスC57BL/6J マウスを、前記Jackson Laboratory (バー・ハーバー、メイン州:Bar Harbor、 ME、 アメリカ合衆国) から購入し、Ochsner Clinic Foundation の動物施設のケージ内で標準条件下に飼育した。実験に先立ち動物に2週間の馴化期間を維持した。ベースラインの全身DEXA (dual emission x-ray absorptiometry) 測定を、以下のようにマウスへの適用に適応させたHologic QDR -1000 plus装置を用い各動物に対して2回ずつ行った。超高解像度モード (線間隔0.03950 cm および解像度0.03749 cm) を使用した。前記動物をペンタバルビツールで麻酔し、DEXA スキャンのために腹臥位に配置した。骨の無機質密度(BMD) を、動物の腰椎領域をカバーする8 x 16 画素ボックス内で決定した。 各単独画素の縦縞についてBMDを測定しピーク値を決定した。各マウスについて2回測定して、その測定値を比較することによりこの技術の妥当性を確実化した。

In Vivo でのPTH-CBD の月一回投与
週一回のPTH-CBD投与が骨無機質密度増加に対して有効であるという上記の勇気付ける結果により、次いで本発明者らはこの融合タンパクの月一回の投与での有効性を試験した。マウスは PTH-CBD (344 μg/kg/一回あたり)、 PTH (80 μg/kg/一回あたり) またはビークル単独の、それぞれ実施例 4で述べた緩衝液の溶液を月に一回腹腔内 注射された。各群は10 匹のマウス を含む。 骨の無機質密度(BMD) は2ヶ月ごとに実施例 4 で述べたDEXA により測定した。DEXA 測定と前記週一回投与の実施例で述べた切除組織の測定との相互関係から、DEXA 測定を絶対骨無機質密度に相関づけた。BMDの2ヵ月後との連続測定により、4ヶ月の処置後に、月一回のPTH-CBD投与がBMDの有意な増加を引き起こしたことが示され、増加は6ヶ月の処置中維持された (図8) (p<0.01、図8中 ** で示される) 。驚くべきことではないが、月一回のPTH(1-34) 投与は、骨無機質密度に影響しなかった。6ヵ月後 (図8中、矢印で示される) に PTH-CBDの投与を中止し、動物をPTH (1-34) の週一回投与群に2週間組み込んだ。2ヵ月後のBMD測定 は、 (前記ビークル 対照群での加齢により見込まれるBMDの減少にもかかわらず) PTH-CBD投与後に獲得された骨無機質密度が維持されたことを示し、および 前記 PTH (1-34) の一日一回投与群ではBMD の増加が引き起こされ、その増加は前記PTH-CBD 群に近づいたが達しなかった。
前記実施例 4のデータとともに、これらのデータは、月一回の PTH-CBD 投与が、骨無機質密度促進において少なくとも一日一回のPTH注射と同等の有効性を有することを示した。重要なことには、各注射の前記PTH-CBD薬量は、前記一日一回のPTH(1-34)の 薬量と等しいモル数であり、このように合計投与薬量としては、実際には前記 PTH(1-34)の1/30であることである。前記データは、月一回より長い間隔での投薬が有効である可能性、および治療終了後に、PTH-CBDの前記BMD への効果がPTH(1-34)の効果よりもより長期間持続することを示唆する。
3および 6ヶ月に一回のPTH-CBD in Vivo 投与
月一回のPTH-CBD投与が骨無機質密度増加に対して有効であるという上記の勇気付ける結果により、次いで本発明者らはこの融合タンパクの3ヶ月または6ヶ月に一回の投与の有効性を試験した。マウスは PTH-CBD (344 μg/kg/一回) (図の9CBD-PTH-6)、PTH (344 μg/kg/一回あたり、0ヶ月および3ヶ月目) (図の9CBD-PTH-6)、 PTH(1-34) (80 μg/kg/薬量 一日一回2週間)またはビークル単独(1回)のそれぞれ実施例 4で述べた緩衝液の溶液を腹腔内注射された。各群は11匹のマウス を含む。骨の無機質密度(BMD) は3ヶ月目とその後は毎月DEXA により測定した。DEXA 測定と前記週一回投与の実施例で述べた切除組織の測定との相互関係から、DEXA 測定を絶対骨無機質密度に相関づけた。本研究は進行中であり、データは5ヶ月目の時点まで得られている。
前記データ 実施例4 および 5のデータとともに、これらのデータは、維持的な骨無機質密度増加を促進 するのに単回投与のPTH-CBDで十分であることを示した。重要なことには、各注射の前記PTH-CBD薬量は、前記一日一回のPTH(1-34)の 薬量と等しいモル数であり、このように現時点でデータが得られている前記5ヶ月の研究期間中にわたる合計投与薬量としては、実際には前記 PTH(1-34)の1/14であることである。本研究者らはさらに7ヶ月間に渡り研究データの収集を継続する予定である。前記データは、治療終了後に、PTH-CBDの前記BMD への効果がPTH(1-34)の効果よりもより長期間持続することを示唆する。
準備的薬量 および 時間的応答の研究
大体のPTH-CBD前記最適薬量を決定するために、2 から 8,000 μg /kgの範囲の前記融合タンパクをマウスに皮下に単回投与し、4週間ごとに20週目まで前記マウスのBMD をDEXA により試験した。前記最大薬量では、BMD は4週および12週の間減少した後に増加した。このように一時的な異化作用があり、その後に同化作用が起きているように考えられる。実施例4および5で用いられた前記薬量の344 μg /kg をまたぐ、中間的な薬量である40-400 μg/kgでは、最初の8週間にわたり最大の同化作用を持つように考えられた。試験された最低の薬量である 2 μg /kg では、最初の16週間にわたり、より少ない同化作用を持つように考えられた(図11)。
毛髪成長促進へのPHT-CBD の使用
遺伝的および化学療法後の脱毛の動物モデルにおいて、PTH アゴニストおよび アンタゴニストが髪の成長を調節するという報告がある (8、9)。本発明者らは、シクロホスファミドによる化学療法により誘発された脱毛において、PTH-CBD が皮下投与後に毛髪の成長パターンを変化させることができるか試験した。
材料および方法:
健常メスC57BL/6J マウス(実施例4と同じ)を、150 mg/kgシクロホスファミド で、毎月一回3ヶ月にわたり処置した。前記化学療法剤は毛髪の減少および黒色から白色の毛髪の色変化を引き起こした。本発明者らは追加的に背中の一点を剃毛した。前記毛髪を除去された点に、本発明者らはPTH-CBD 320 mg/kgを皮下注射により投与した。本発明者らは、さらにPTH/PTHrP 受容体アンタゴニストを含むCBD 融合タンパク (SEQ ID NO:9) も注射した。この融合タンパクは、スロンビン開裂シーケンスである (Leu-Va1-Pro-Arg-Gly-Ser、 SEQ ID NO: 12) を、前記融合タンパク SEQ ID NO:1のGST および PTH(1-33) 断片の間に挿入して作成した。その結果生成されたGST-PTH- CBD 融合タンパクは、スロンビンにより、スロンビン開裂シーケンスである前記Arg および GIy 残基の間で開裂され、Gly-Ser-PTH-CBD 融合タンパク、SEQ ID NO 9を遊離させた。
前記PTH-CBD により処理された動物は、前記毛髪を取り除いた点でより急速な毛髪 の再成長を示し、さらに前記PTH-CBD 注射部位から離れた部位においてさえ、前記化学療法誘発の毛髪減少および色変化の両方を逆転させた (図12)。 PTH/PTHrP 受容体アンタゴニストを含むCBD 融合タンパク も予備実験で試験された。しかし前記アンタゴニスト融合タンパクは、注射部位に桃の産毛のような毛髪を 生やしたのみで、および前記PTH-CBD アゴニスト融合タンパクのように良好には作用しなかった (結果は示されていない)。前記アンタゴニスト融合タンパクは、ビークル対照群よりは多くの毛髪 を生やさせた(結果は示されていない) 。
PTH-CBD は、化学療法誘発の脱毛症を逆転させ、および前記効果は注射部位に制限されなかった。
免疫再構築促進へのPHT-CBD の使用
メスC57B1/6 マウスを 10 Gy の放射線で照射した(放射線源は137Cs )。 24時間後にマウスにドナーのB6.SJL マウスよりの2 x 105の骨髄単核細胞 (BMMNC) を注射した。前記BMMNC投与直前に、前記受容マウスに生理食塩水 (ビークル対照)、344 μg/kgのPTH-CBD (SEQ ID NO: 1)、または80 μg/kgの PTH(1-34)を注射した。
骨髄幹細胞可動化促進へのPTH-CBD の使用
6から8週齢のオスC57BL/6 マウスに80 μg/kg PTH (1-34) または44 μg/kg PTH-CBD (SEQ ID NO: 1) または生理食塩水 (ビークル 対照) を皮下注射により単回投与した。14日後に前記マウス より末梢血を採取し、c-KIT/Sca-1 細胞量を蛍光標識細胞分取法 (fluorescence activated cell sorting: FACS) により決定した (21)。PTH-CBD が、単回投与のPTH (1-34) よりも、c-KIT/Sca-1による二重陽性細胞を増加させることが示された。
乳ガン骨転移の予防または治療へのCBD-PTH/PTHrP 受容体アンタゴニスト融合タンパクの使用
一日一回注射で投与した場合PTH(1-34) は、数多くの動物種および骨粗しょう症の女性での骨成長を促進する。しかしながら輸液による PTH の連続投与は (すなわち副甲状腺腺腫) は骨の減少をもたらす。
実験方法:
13〜18gの 3〜5週齢のヌードマウス22匹を入手する。前記動物の初期体重を一般的健康状態とともに記録する。実験に先立ち動物に2週間の馴化期間を維持する (最終的に5-8週齢) 。 Bioluminescent/Fluorescent Imager (Xenogen Biosciences、クランバリー、ニュージャージー州: Cranbury、 NJ) 全身イメージング装置を用いて、イソフルレン麻酔後に各動物のベースラインのイメージを測定する。その後、ホタルのルシフェラーゼを発現するプラスミドを安定に組み込んだMCF-7 細胞を動物に注射により単回投与する(23、 24)。前記注射後およびその後は週一回のベースで動物の再イメージングを行い、骨転移をモニターする。
Group 1:動物11匹は、ビークル腹腔内投与を一回受ける。
Group 2:動物11匹は、344 μg/kg のPTH (7-33)-CBD (SEQ ID NO: 10) の腹腔内投与を一回受ける。
前記実験期間中、動物は、一週間に一回体重を測定し、なにか疾患兆候があるかを検出するために検査する。全身イメージングは、前記転移病巣の数、および各病巣からの蛍光の発光強度を決定するために解析される。
PTH (7-33)-CBDを注射されたマウスでは、対照のビークル投与マウスに比べて、より少ない骨転移病巣の発症、および、より遅い骨転移病巣の成長が期待される。
CBD-PTH/PTHrP受容体アンタゴニスト融合タンパク の腎性骨形成異常症の予防または治療への使用
腎性骨形成異常症は、腎臓が適正水準の血中カルシウム および リンを維持できない場合に起こる骨の疾患である。この疾患は腎臓病患者では共通の問題であり、および 90 %の透析患者に影響する。腎性骨形成異常症 は、慢性腎臓病患者の骨折の主原因である。本研究では、本発明者らはPTH-CBD アンタゴニスト・ペプチドが腎性骨形成異常症を治療または予防する能力を有することを示す。モデル動物は、腎性骨形成異常症を誘発するために高リン食を与えられたメスのマウスである。

前記実験期間中、一週間に一回動物の体重を測定し、なにか疾患兆候があるかを検出するために検査する。骨の無機質密度測定は、各時点でANOVAにより解析される。各血液サンプルのアルカリ性ホスファターゼおよびカルシウム値を測定し、および各時点でANOVA により解析する。
PTH(7-33)-CBDを注射される前記動物の全ての測定で、対照のビークル投与マウス群に比較して、骨の無機質密度の増加または減少の遅延の応答が生じることが期待される。PTH(7-33)-CBD注射マウス では、対照のビークル投与マウス群に比較して、さらに骨梁の成長または骨梁の損失の遅延が示されることが期待される。

Claims (9)
- PTH/PTHrP受容体アゴニストに連結したコラーゲン結合性ポリペプチド断片を含む、骨成長を促進するための組成物であって、前記コラーゲン結合性ポリペプチド断片がクロストリジウム(Clostridium)由来のバクテリア由来コラーゲン結合性ポリペプチド断片であり、
前記コラーゲン結合性ポリペプチド断片が、
配列番号1の残基38-158を含むか、または、
配列番号1の残基38-158と少なくとも90%同一であり、
前記PTH/PTHrP受容体アゴニストが、
配列番号1の残基1-33を含むか、
配列番号1の残基1-14を含むか、または、
配列番号7の残基1-34を含む、
ことを特徴とする組成物。 - PTHと等モル投与量での週一回の8週間にわたる対象者への投与後の骨無機質密度の増加により測定した際に、前記組成物がPTH(1-34)よりも少なくとも50%大きい活性を有することを特徴とする、請求項1に記載の組成物。
- PTH/PTHrP受容体アゴニストに連結したコラーゲン結合性ポリペプチド断片を含む、骨成長を促進するための組成物であって、
前記コラーゲン結合性ポリペプチド断片が、
配列番号1の残基38-158を含むか、または、
配列番号1の残基38-158と少なくとも90%同一であり、
前記PTH/PTHrP受容体アゴニストが、
配列番号1の残基1-33を含むか、
配列番号1の残基1-14を含むか、または、
配列番号7の残基1-34を含み、
PTHと等モル投与量での週一回の8週間にわたる対象者への投与後の骨無機質密度の増加により測定した際に、前記組成物がPTH(1-34)よりも少なくとも50%大きい活性を有することを特徴とする組成物。 - 前記コラーゲン結合性ポリペプチド断片および前記PTH/PTHrP受容体アゴニストが、互いに化学的に架橋結合されるか、または融合タンパク質中のポリペプチドの部分である、請求項1から3のいずれか一項に記載の組成物。
- 前記コラーゲン結合性ポリペプチド断片が、
配列番号1の残基38-158を含む、
請求項1から4のいずれか一項に記載の組成物。 - 前記PTH/PTHrP受容体アゴニストが、
配列番号1の残基1-33を含む、
請求項1から5のいずれか一項に記載の組成物。 - 前記PTH/PTHrP受容体アゴニストが、ポリペプチド断片であり、
前記コラーゲン結合性ポリペプチド断片のN-末端が直接的に、またはリンカーを介して、PTH/PTHrP受容体アゴニストのC-末端に連結されている、
請求項1から6のいずれか一項に記載の組成物。 - 骨成長の促進を必要とする疾患を治療する薬剤を調製するための、請求項1から7のいずれか一項に記載の組成物の使用。
- 哺乳動物において、骨成長の促進に有効な薬剤を調製するための、請求項1から7のいずれか一項に記載の組成物の使用。
