Description
図4A、図4B、および図5は、非特効性の第2の刺激22を生成する非侵襲性の第2の刺激ユニット12の実施形態を示している。図4Aおよび図4Bに示した実施形態においては、第2の刺激ユニットは、いわゆる「時計型条件付け装置(conditioning clock)」として設計されており、患者は快適に着用することができる。図4Aは、時計型条件付け装置12の正面図を示しており、図4Bは、時計型条件付け装置12の背面図を示している。時計型条件付け装置12は、中央部40と、バンド41と、締結部42と、対応する穴43とからなる。これに代えて、マジックテープ(登録商標)式の締結部または任意の他のタイプの締結部を使用することもできる。中央部40は、非特効性の聴覚刺激22(例えばメロディや心地よい着信音)を生成するためのスピーカ44と、まぶしすぎず心地よい非特効性の視覚刺激22(例えば、風の中で揺れる花や、明るい暖色の抽象的な光のパターン)を生成するためのディスプレイ45とを含んでいる。さらには、時計型条件付け装置12は、非特効性の触覚刺激22もしくは振動刺激22またはその両方を生成する1つまたは複数の振動子46を備えることができる。これらの振動子は、触覚刺激22を生成するため、例えば1Hz未満の周波数で動作させることができる。特に、この場合、圧力刺激が良好に印加されるように(すなわち振動子46の運動の主方向は皮膚の方に向いている必要がある)、振動子46の可動部を整列させることができる。さらに、触覚刺激22は、皮膚に対してゆっくり動く圧力アクチュエータまたは圧力要素によって生成することもでき、これらを例えば時計型条件付け装置に組み込むことができる。振動子46によって振動刺激22を生成する場合、10〜160Hzの範囲内またはそれを超える振動周波数を使用することができる。この場合、振動子46の可動部は、皮膚の表面に実質的に平行な方向に動くことができる。皮膚の表面に垂直な動きも同様に可能である。触覚刺激22と振動刺激22とを同時に生成するように、振動子46を動作させることもできる。
以下では、第1の視覚刺激を生成する非侵襲性の第1の刺激ユニット11の実施形態について説明する。このような刺激ユニットは、特許文献1にも記載されている。特許文献1の開示内容は、その全体が参照によって本明細書に組み込まれている。
以下では、第1の聴覚刺激を生成するための非侵襲性の第1の刺激ユニット11の実施形態について説明する。このような刺激ユニットは、特許文献2にも記載されている。特許文献2の開示内容は、その全体が参照によって本明細書に組み込まれている。
以下では、第1の触覚刺激21、第1の振動刺激21、第1の温度刺激21のうちの1つまたは複数を生成する非侵襲性の第1の刺激ユニット11の実施形態について説明する。このような刺激ユニットは、特許文献3にも記載されている。特許文献3の開示内容は、その全体が参照によって本明細書に組み込まれている。
1.刺激周期Tstimを単位とし、すべてのモジュールにおいて同一パターンで行われる、刺激順序のランダム化。すなわち、各刺激周期長Tstimの開始時において、刺激要素が刺激を生成する順序が決定され(例:刺激要素#4、刺激要素#2、刺激要素#3、刺激要素#1の順)、この順序がすべてのモジュールに適用される。
2.いくつかの連続する刺激周期Tstimのブロックを単位とし、すべてのモジュールにおいて同一パターンで行われる、刺激順序のランダム化。すなわち、図48に示したいくつかの連続する刺激周期Tstimのブロックの開始時(または刺激の中断の後)において、刺激要素が刺激を生成する順序が決定され(例:刺激要素#4、刺激要素#2、刺激要素#3、刺激要素#1の順)、次の中断までの刺激周期のブロックにおいて、すべてのモジュールにこの順序が適用される。
3.全モジュールにわたり同一パターンで変化させるのではなく、全モジュールのうちの一部のモジュールのみにおいて同一パターンで行われる、刺激順序のランダム化。すなわち、上の項目1または項目2によるランダム化が、特定のモジュール(例:モジュール#2)においてのみ行われ、残りのモジュールは図47に示したような挙動をとる。
4.全モジュールにわたり同一パターンで変化させるのではなく、全モジュールのうちの複数のモジュールのみにおいて同一パターンで行われる、刺激順序のランダム化。すなわち、上の項目1または項目2によるランダム化が、2つ以上のモジュール(例:モジュール#2およびモジュール#4)のみにおいて行われ、残りのモジュールは図47に示したような挙動をとる。
5.異なるモジュールにおいて互いに無関係に行われる、刺激順序のランダム化。すなわち、刺激要素が刺激を生成する順序は、モジュールごとに、刺激周期Tstimを単位として、または2つの中断の間のいくつかの連続する刺激周期Tstimのブロックを単位として、他のモジュールには無関係に決定される。
Claims (11)
- 装置(100)であって、
− 第1の刺激(21)を生成する第1の刺激ユニット(11)であって、前記第1の刺激(21)が、患者に印加されたとき、前記患者の脳もしくは脊髄またはその両方におけるニューロンの病的に同期した活動を抑制する、前記第1の刺激ユニット(11)と、
− 第2の視覚刺激(22)、第2の聴覚刺激(22)、第2の触覚刺激(22)、第2の振動刺激(22)、第2の温度刺激(22)のうちの1つまたは複数を生成する第2の刺激ユニット(12)と、
− 前記第1の刺激ユニット(11)および前記第2の刺激ユニット(12)を制御する制御ユニット(10)と、
を備えており、
− 前記第1の刺激ユニット(11)および前記第2の刺激ユニット(12)が非侵襲性の刺激ユニットであり、
− 前記第1の刺激(21)および前記第2の刺激(22)の生成が、第1の動作モードまたは第2の動作モードにおいて選択的に行われ、
− 前記制御ユニット(10)が、前記第1の刺激ユニット(11)および前記第2の刺激ユニット(12)を次のように制御する、すなわち、前記第1の動作モードにおいては、前記第2の刺激(22)の少なくとも60%の生成が、前記第1の刺激(21)の生成に時間的に連結されており、前記第2の動作モードにおいては、前記第2の刺激(22)の少なくとも60%の生成が、前記第1の刺激(21)の生成なしに行われる、ように制御する、
装置(100)。 - 前記第1の刺激(21)が、視覚刺激、聴覚刺激、触覚刺激、振動刺激、温度刺激のうちの1つまたは複数である、
請求項1に記載の装置(100)。 - 前記第2の刺激(22)が、単独で印加されたとき、脳もしくは脊髄またはその両方におけるニューロンの病的に同期した活動の抑制をもたらさない、
請求項1または請求項2に記載の装置(100)。 - 前記第1の刺激ユニット(11)が透過メガネを備えており、前記透過メガネが、複数の光源、音響発生器、直流モータ、発振コイル、圧電変換器、電気活性高分子、加熱素子、冷却素子、赤外光源のうちの1つまたは複数を有する、
請求項1から請求項3のいずれかに記載の装置(100)。 - ニューロンの病的に同期した活動を再現する測定信号(25)を記録する測定ユニット(15)、
