Description
〔発明の分野〕
本発明は、概して手術用装置に関し、特にエンド・エフェクター用の支持部を有する手術用ステープラーおよび／またはカッターに関するものである。

〔発明の背景〕
内視鏡下および腹腔鏡下手術用器械は、従来の開腹手術用装置よりも好まれることがよくある。これは、自然な開口部（内視鏡下）または小さな切開部（腹腔鏡下）を使用すると、術後の回復時間を短縮し、合併症を減らすのに役立つからである。この結果、著しい発展があって、内視鏡下および腹腔鏡下手術用の器械が、遠位エンド・エフェクターを所望の手術部位に正確に配置するのに適した域まで達した。このような遠位エンド・エフェクターは、多くの方法で組織と係合し、診断または治療の効果（例えば、エンドカッター、異物鉗子、カッター、ステープラー、クリップアプライヤ(clip applier)、アクセスデバイス(access device)、薬物／遺伝子治療送達装置、および超音波、ＲＦ、レーザ等を用いるエネルギー装置）を達成する。

通常、内視鏡下および腹腔鏡下手術用ステープラーはステープル留め機構を有し、このステープル留め機構は、カニューレにはめて通され、エンド・エフェクターをステープル留めすべき組織に対して位置決めするように構成されている。エンド・エフェクターは、ステープルを備えたカートリッジを収容しているカートリッジアームと、カートリッジアームに軸回転するように接続されたアンビルとを有する。カートリッジアームとアンビルは、各々、別個の顎部として作用して、いっしょになってステープル留めすべき組織を閉じて保持する。カートリッジが作動すると、ステープルがカートリッジアームから打ち出され、組織を通り、アンビルに形成されていて、カートリッジアームのステープルと位置が合わせてあるステープル成形溝にあたり、これにより、組織を合わせた状態に保持するステープルを成形する。
ステープラーが一直線に配列されている複数のステープルを同時に発射するとき、アンビルが撓むことがある。具体的には、比較的長く、支持されていないアンビル長さに加えてステープルを打ち出すのに要する力により、アンビルの遠位端に大きなトルクが生じ、撓みが生じることがある。この結果、ステープルが奇形すること、特に、比較的長いエンド・エフェクターの遠位端に向けて奇形することがある。内視鏡下または腹腔鏡下の処置では、小さな開口部またはアクセスポートを利用するので、エンド・エフェクターの厚みおよび外径は必然的に制限される。アンビルに非常に硬い材料（例えば、鋼鉄またはチタン）を使用しても、ステープラーが作動したときの撓みは顕著である。
したがって、組織をステープル留めする、および／または、切断する装置および方法を改良する必要がある。
〔発明の概要〕

〔発明の詳細な説明〕

支持部３０は、図２Ａから図２Ｄにより詳細に示されている。図示のように、支持部３０は、リンクアセンブリという形態となっており、細長いシャフト１２とアンビル２２との間に可動に連結されている。具体的には、支持部３０が近位リンク部３２、遠位リンク部３４、および中間リンク部３６を有し、近位リンク部３２、遠位リンク部３４、および中間リンク部３６は、互いに軸回転するように連結されており、かつ、細長いシャフト１２およびエンド・エフェクター２０に対して可動となっている。近位リンク部３２は、近位端３２ａを有し（図２Ｄ）、この近位端３２ａは、細長いシャフト１２の一部分に軸回転するように、かつ、スライド可能に連結されている。また、近位リンク部３２は、遠位端３２ｂを有し、この遠位端３２ｂは、遠位リンク部３４の近位端３４ａに軸回転するように連結されている。遠位リンク部３４は、近位リンク部３２の遠位端３２ｂから遠位へ延びており、また、遠位リンク部３４は、アンビル２２に軸回転するように、かつ、スライド可能に連結されている遠位端３４ｂを有している。近位リンク部３２の近位端３２ａと細長いシャフト１２ｂとの間、および、遠位リンク部３４の遠位端３４ｂとアンビル２２との間のスライドし、かつ、軸回転する連結部は、さまざまな手法を用いて形成できる。例えば、近位リンク部３２は、例えば、（図２Ｄの）スライド式プッシュ・ロッド５０のようにスライドし、かつ、近位リンク部３２の相対的な軸回転を許容する、細長いシャフト１２の一部に連結することができる。遠位リンク部３４は、ピン部材を有することもあり、このピン部材は、アンビルに形成したスロット２２ｃ内でスライドし、また、遠位リンク部３４がアンビル２２に対して軸回転できるようにする。前記したように、支持部３０には、中間リンク部３６がさらにあってもよい。図２Ｃおよび図２Ｄに示すように、中間リンク部３６は、第１の端部３６ａにおいて、近位リンク部３２の遠位端３２ｂに、および遠位リンク部３４の近位端３４ａに、軸回転するように（pivotally）連結されており、これにより、３つのリンク部３２、３４、３６の全てが、支持部３０の中央部分において、互いに軸回転するように連結されている。中間リンク部３６の第２の端部３６ｂは、固定された回転軸３６ｃにおいて、細長いシャフト１２に軸回転するように連結されていて、これにより、中間リンク部３６が細長いシャフト１２に対してスライドできないようになっている。
使用時には、リンク部３２、３４、３６の間にある連結部により、支持部３０が、アンビル２２がカートリッジアーム２４に対して軸回転できる近位の折畳み位置と、カートリッジアーム２４から１つ以上のステープルを打ち出し、アンビル２２にあてて変形させるときに、支持部３０がアンビル２２の撓みを制限するまたは防止するのに有効となる遠位の、伸長位置との間で動くことができる。近位の折畳み位置は、図２Ａと図２Ｂに示されおり、図示のように、リンク部３２、３４、３６は、エンド・エフェクター２０と実質的に軸方向に位置が合わせてあり、細長いシャフト１２、エンド・エフェクター２０、および支持部３０の最大外径が、その全長にわたって実質的に一定であり、装置１０の遠位部分を小さな経路、例えば直径が１６ｍｍ以下である経路を介して挿入することができるようになっている。具体的にいうと、後でより詳しく述べるように、例えば、ステープルのハンドルに形成した引き金または他の機構を用いて、近位リンク部３２を近位方向に完全に後退させて、遠位リンク部３４を近位方向に引き、遠位リンク部３４の遠位端３４ｂがアンビル２２に対して最も近位の位置にくるようにする。近位リンク部３２を近位側に後退させると、（図２Ａと図２Ｂに不図示の）中間リンク部３６の第１の端部３６ａもまた、近位方向に軸回転し、中間リンク部３６が、細長いシャフト１２に設けられた開口部の中に受け入れられるようになる。この位置では、アンビル２２がカートリッジアーム２４に対して自由に開閉できる。図２Ａは、間に組織を受け入れるために開位置にあるアンビル２２を図示しており、図２Ｂは、間に組織を係合させるために閉位置にあるアンビル２２を図示している。別個の閉鎖機構を利用してアンビル２２を図２Ｂに示した閉位置へ動かすこともできし、あるいは、支持部３０を遠位の伸長位置に動かしたときに、支持部３０を利用してアンビル２２をカートリッジアーム２４の方に押しつけることもできる。
支持部３０は、図２Ｃおよび図２Ｄでは、遠位の伸長位置にあるところが示されている。図示のように、近位リンク部３２は、後でより詳しく述べるように、例えば、ステープルのハンドルに形成した引き金または他の機構を用いて、遠位方向へエンド・エフェクター２０に向けて前進させる。近位リンク部３２が遠位の方へ動くことにより、中間リンク部３６の第１端部３６ａが、中間リンク部３６の第２端部３６ｂと、細長いシャフト１２との間に形成された固定回転軸３６ｃの回りに軸回転する。中間リンク部３６の第１端部３６ａが動いて細長いシャフト１２から離れ、固定回転軸３６ｃ回りに軸回転すると、遠位リンク部３４の近位端３４ａも同様に動いて細長いシャフト１２から離れる、つまり、細長いシャフト１２に対して回転し、そして遠位リンク部３４の遠位端３４ｂは、アンビル２２に形成された細長いスロット２２ｃ内を遠位方向にスライドする。遠位リンク部３４が最も遠位の位置に達すると、近位リンク部３２に加えた力はもはや遠位リンク部３４を前進させることはなく、むしろ、遠位リンク部３４がアンビル２２に加える下方の力となり、ステープルをカートリッジアーム２４からアンビル２２に対して打ち込むときのアンビル２２のカートリッジアーム２４に対する撓みを制限または防止する。
支持部３０は、アンビル２２の任意の部分に係合させることができるが、典型的な実施形態では、支持部３０をアンビル２２の実質的な中央部分と係合させるように構成することができる。このような構成により、遠位リンク部３４がアンビル２２に下方の力を加えることが保証され、このことは、オプションとして、アンビルを閉じた位置に動かすのに有効であることがある。アンビル２２の中央部への係合は、例えば、停止部材を用いて支持部３０のアンビル２２に対する遠位方向の動きを制限することで達成できる。図２Ｃおよび２Ｄに示した実施形態では、アンビル２２に形成した細長いスロット２２ｃの終点２２ｄが停止部材として機能する。これは、このスロット２２ｃがアンビル２２の最も遠位の端部に対して近位側で終端するからである。このように、終点２２ｄは、遠位リンク部３４がスロット２２ｃの最も遠位の端部を越えて遠位へスライドすることを防止し、これにより、遠位リンク部３４をアンビル２２の中央部分と係合させる。スロットの終点は、可動の停止ピンを使って調整可能にすることもできる。スロットには、停止ピンを挿入できる多数の箇所があってもよく、そして、停止ピンの位置は、遠位リンク部がこれ以上スライドできなくなる前に、遠位リンク部がアンビルの遠位端の方へスライドできる範囲を決めることができる。当業者には分かるであろうが、支持部の遠位方向への動きを制限する停止部または他の機構を作るのにさまざまな他の手法を用いることができる。
前述したように、さまざまな手法を用いて支持部３０を近位位置と遠位位置との間で動かすことができる。例えば、ある実施形態では、近位リンク部を軸回転するようにプッシュ・ロッド５０（図２Ｄ）に連結することができ、このプッシュ・ロッド５０は、細長いシャフト１２を通って延びていて、細長いシャフト１２に対してスライド可能に移動して、近位リンク部３２を前進および後退させるように構成されており、これにより、支持部３０を近位位置と遠位位置との間で動かす。装置のハンドルに形成したレバー、引き金、または他の機構を、プッシュ・ロッド５０を動かすために設けることができる。当業者には分かるであろうが、支持部の移動および／またはアンビルおよびカートリッジアームの軸回転は、互いに同時に行ってもよいし、独立に行ってもよく、また、ステープルをカートリッジアーム３４から発射するのとは独立に行ってもよい。
支持部もまた、手術用ステープラーにあるアンビルが撓むのを防止するために、さまざまな他の構造を有することができる。他の実施形態では、図３Ａから図３Ｃに示すように、支持部が、第１の位置と第２の位置との間でエンド・エフェクターに相対的に可動であるスリーブという形態となっていることがある。図示のように、手術用ステープラー３００には、普通、細長いシャフト３１０があり、この細長いシャフト３１０は、エンド・エフェクターを有している。エンド・エフェクターは、細長いシャフト３１０の遠位端に形成されており、アンビル３２０と、アンビル３２０に軸回転するように連結されたカートリッジアーム３３０とを有する。スリーブ３４０が細長いシャフト３１０の外側ハウジング内にスライド可能に配置されており、近位の後退位置（図３Ａに示されている）と、遠位の伸長位置（図３Ｂに示されている）との間で可動である。スリーブ３４０は、さまざまな形状および大きさを有することができ、実質的に管状であってもよく、あるいは、図３Ａおよび３Ｂに示されているように、全体的に細長い形状をしていて、実質的に断面が半円形をしていてもよい。スリーブ３４０が図３Ａに示すように近位位置にあるとき、スリーブ３４０はアンビル３２０に対して完全に後退していて、アンビル３２０がカートリッジアーム３３０に対して軸回転することを許容する。スリーブ３４０を遠位位置に前進させると、スリーブ３４０は、アンビルを越えるようにスライドして、回転軸３５０の遠位側にある位置でアンビル３２０に接触し、これにより、アンビル３２０がカートリッジアーム３３０に対して軸回転運動をするのを妨げる、すなわち、防止する。したがって、ステープルを発射したときに、スリーブ３４０がアンビル３２０の撓みを制限または防止する。
図２Ａから図２Ｄに示した実施形態と同様に、支持スリーブ３４０の動作は、いろいろな方法で実現することができる。ある実施形態では、図３Ｃに示したように、デバイス３００のハンドル３７０はスライド式ボタン３６０を含む。このスライド式ボタン３６０は、ハンドル３７０の上に形成されており、かつ、スリーブ３４０の近位端に連結されている。このように、近位位置と遠位位置との間のスライド式ボタン３６０の動きが、スリーブ３４０を近位位置と遠位位置との間で動かすのに有効である。
当業者には分かるであろうが、スリーブはさまざまな他の構成を有することがあり、また、スリーブは後退・伸長位置の間で動く必要もない。例えば、スリーブは、スリーブがカートリッジアームの近傍にあり、アンビルとカートリッジアームの軸回転を許容する第１の位置と、スリーブがアンビルの近傍にあり、アンビルがカートリッジアームに対して軸回転することを制限または防止する第２の位置との間で回転可能であってもよい。
組織をステープル留めする代表的な方法も提供する。ある典型的な実施形態では、この方法は、手術用ステープラーのエンド・エフェクターを手術部位に導入することを含む。エンド・エフェクターは、内視鏡を用いて自然な開口部を介して挿入してもよいし、腹腔鏡を用いて、小さな切開部またはカニューレのようなアクセスポート(access port)を介して挿入することもできる。エンド・エフェクターは、好ましくは、装置の細長いシャフトの外径が最小である、または、最小に維持される後退位置に支持部がある状態で挿入される。例えば、図２Ａから図２Ｄに示した装置１０に関して言えば、支持部３０は、好ましくは、近位の折り畳み位置で導入される。図３Ａから図３Ｃに示した実施形態に関して言えば、支持スリーブ３４０は、いずれの位置でも導入することができる。これは、スリーブ３４０が装置の外径を大きくしないからである。
いったんエンド・エフェクターをステープル留めされるべき組織近傍に配置したら、アンビルとカートリッジアームを操作して組織をその間に配置する。その後、例えば、支持部を用いて、または、別個の閉鎖機構を用いて、アンビルをカートリッジアームの方へ回し、組織をアンビルとカートリッジアームの間で挟む。支持部がアンビルを閉じるために使われていない場合は、その後、支持部を前進させて、アンビルのカートリッジアームに対する運動を妨げるまたは防止することができる。オプションとして、組織を一定時間押しつぶして組織の「搾り(milking)」をさせてもよく、その後、エンド・エフェクターを作動させ、カートリッジアームからステープルを打ち出して、アンビルとカートリッジアームの間にある組織に通し、アンビルにあてることができる。ステープルはアンビルにあたって変形し、支持部はアンビルの撓みを防ぐであろう。
当業者には、上述した実施形態に基づいて本発明の他の特徴および利点が分かるであろう。したがって、本発明は、添付した特許請求の範囲により示したことを除いて、具体的に図示し、説明したことには限定されない。

（１） 手術用ステープラーにおいて、
少なくとも１つのステープルを保持するためのカートリッジアーム、および、前記カートリッジアームに軸回転するように連結されており、前記少なくとも１つのステープルを変形させるのに有効であるアンビル、を有するエンド・エフェクターと、
前記エンド・エフェクターに連結されている支持部であって、前記アンビルおよび前記カートリッジアームが互いに対して軸回転できる近位位置と、前記支持部が前記エンド・エフェクターの少なくとも中央部分と係合して、少なくとも１つのステープルが前記カートリッジアームから打ち出され、前記アンビルにあてて変形されるときに、前記カートリッジアームに対する前記アンビルの撓みを防止する遠位位置との間で可動である、支持部と、
を備える、手術用ステープラー。
（１） 手術用ステープラーにおいて、
少なくとも１つのステープルを保持するためのカートリッジアーム、および、前記カートリッジアームに軸回転するように連結されており、前記少なくとも１つのステープルを変形させるのに有効であるアンビル、を有するエンド・エフェクターと、
前記エンド・エフェクターに連結されている支持部であって、前記アンビルおよび前記カートリッジアームが互いに対して軸回転できる近位位置と、前記支持部が前記エンド・エフェクターの少なくとも中央部分と係合して、少なくとも１つのステープルが前記カートリッジアームから打ち出され、前記アンビルにあてて変形されるときに、前記カートリッジアームに対する前記アンビルの撓みを防止する遠位位置との間で可動である、支持部と、
を備える、手術用ステープラー。
（２） 実施態様１記載の手術用ステープラーにおいて、
前記支持部は、前記近位位置にあるときには、前記エンド・エフェクターと軸方向に位置が揃っており、前記遠位位置にあるときには、前記エンド・エフェクターに対して実質的に横方向に広がっている、手術用ステープラー。
（３） 実施態様１記載の手術用ステープラーにおいて、
前記支持部は、前記アンビルにスライド可能に連結されたリンク機構を含む、手術用ステープラー。
（４） 実施態様３記載の手術用ステープラーにおいて、
前記リンク機構は、
前記エンド・エフェクターから近位方向に延びている細長いシャフトに軸回転するように連結されている近位リンク部と、
一方の端部において前記近位リンク部に軸回転するように連結されており、反対側の端部で前記アンビルに軸回転するように連結されている遠位リンク部と、
を含む、
手術用ステープラー。
前記支持部は、前記近位位置にあるときには、前記エンド・エフェクターと軸方向に位置が揃っており、前記遠位位置にあるときには、前記エンド・エフェクターに対して実質的に横方向に広がっている、手術用ステープラー。
（３） 実施態様１記載の手術用ステープラーにおいて、
前記支持部は、前記アンビルにスライド可能に連結されたリンク機構を含む、手術用ステープラー。

前記遠位リンク部は、前記アンビルと前記カートリッジアームとの間に形成された軸回転連結部に対して遠位の位置で前記アンビルに連結されている、手術用ステープラー。

前記支持部は、前記アンビルの少なくとも一部にわたってスライド可能に配置できるスリーブを含む、手術用ステープラー。
（７） 実施態様１記載の手術用ステープラーにおいて、
前記エンド・エフェクターに連結されており、前記アンビルを前記カートリッジアームの方へ押しつけるように構成されている作動部、

（１０） 実施態様９記載の手術用ステープラーにおいて、
前記支持部は、前記アンビルが前記回転軸回りに動くことができる近位位置と、前記支持部が前記回転軸回りに前記アンビルが動くことを防止する遠位位置との間を動くことができる、手術用ステープラー。
複数のステープルを保持するように構成されているカートリッジアームと、
近位端および遠位端を有するアンビルであって、前記近位端が前記カートリッジアームに回転軸において連結されている、アンビルと、
前記アンビルに可動に連結されている支持部であって、前記回転軸より遠位の位置で前記アンビルに接触して、複数のステープルが前記カートリッジアームから打ち出され、前記アンビルにあてて変形されるときに、前記アンビルの撓みを減らすように構成されている、支持部と、

（１０） 実施態様９記載の手術用ステープラーにおいて、
前記支持部は、前記アンビルが前記回転軸回りに動くことができる近位位置と、前記支持部が前記回転軸回りに前記アンビルが動くことを防止する遠位位置との間を動くことができる、手術用ステープラー。
（１１） 実施態様９記載の手術用ステープラーにおいて、
前記支持部は、互いに軸回転するように連結されている少なくとも２つのリンク部を有し、前記リンク部の少なくとも１つは、前記アンビルにスライド可能に連結されている、手術用ステープラー。
（１２） 実施態様９記載の手術用ステープラーにおいて、
前記支持部は、
前記アンビルにスライド可能に連結された遠位リンク部と、
一方の端部において前記遠位リンク部に軸回転するように連結されており、反対側の端部において、前記カートリッジアームから近位方向に延びている細長いシャフトに軸回転するように連結されている近位リンク部と、
一方の端部において前記細長いシャフト部に軸回転するように連結されており、反対側の端部において前記遠位リンク部および前記近位リンク部に軸回転するように連結されている中間リンク部と、

前記カートリッジアーム、前記アンビル、および前記支持部は、前記支持部が近位位置にあるときに、最大外径が１６ｍｍ未満である、手術用ステープラー。
（２１） 組織をステープル留めする方法において、
手術用ステープラーのカートリッジとアンビルとの間に組織を配置する段階と、
支持部を前記アンビルに対して前進させて、前記アンビルの前記カートリッジに対する動きを妨げる段階と、

手術用ステープラーのカートリッジとアンビルとの間に組織を配置する段階と、
支持部を前記アンビルに対して前進させて、前記アンビルの前記カートリッジに対する動きを妨げる段階と、
少なくとも１つのステープルを前記カートリッジから打ち出し、前記アンビルと前記カートリッジとの間に配置された組織に通し、前記アンビルにあてて前記ステープルを変形させることで前記組織をステープル留めする段階と、
を含む、方法。

- 手術用ステープラーにおいて、
少なくとも１つのステープルを保持するためのカートリッジアーム、及び、前記カートリッジアームに軸回転するように連結されており、前記少なくとも１つのステープルを変形させるのに有効であるアンビル、を有するエンド・エフェクターと、
遠位端部を有し、前記エンド・エフェクターに連結されている支持部であって、前記遠位端部が、
前記アンビル及び前記カートリッジアームが最開位置と最閉位置との間を軸回転できるようにする最近位位置と、
前記支持部の前記遠位端部が前記エンド・エフェクターの中央部分と係合して、少なくとも１つのステープルが前記カートリッジアームから打ち出され、前記アンビルにあてて変形されるときに、前記アンビルの撓みを防止する最遠位位置と、
の間で移動可能である、支持部と、
を備え、
前記支持部は、前記近位位置にあるときには、前記エンド・エフェクターと軸方向に位置が揃っており、前記遠位位置にあるときには、前記エンド・エフェクターの軸方向に直交する向きに広がっている、手術用ステープラー。 - 請求項１記載の手術用ステープラーにおいて、
