Description
心房細動は、心室とは独立した心房作動の不規則かつランダムな収縮である。この結果生じる迅速で無秩序な心臓鼓動により、血管系には血液の不規則な乱流が形成され、左心房と一緒に規則的に収縮しない左心耳が形成される。従って、血液は、心耳内で淀みおよびプールとなって、心耳内に血餅(クロット)を形成する。血餅が左心室に流入すると、血餅は大脳血管系に流入して、塞栓発作および障害を引起こし、死を招くこともある。
従って、血餅の心室内への移動および頭側への循環を防止すべく心耳を閉じる最小侵襲性アプローチが有益であることが認識されている。しかしながら、これらの器具は幾つかの基準を満たす必要がある。
好ましくは、ワイヤは形状記憶材料からなり、デリバリ中はほぼ真直な位置に保持されかつ収容部材内ではコイル形状を形成して収容部材を拡大する。
好ましい実施形態では、保持脚は、デリバリのためのほぼ真直な位置と左心耳の壁と係合する第二位置との間で移行できる形状記憶材料からなる。一実施形態では、少なくとも1つの保持脚は異なる方向に延びている2つの脚からなり、第二位置において2つの脚は左心耳の壁と係合する。
他の実施形態では、収容部材は、心耳内の収容部材を保持すべく、心耳の壁と係合する粗面化された外面を有している。この粗面化表面は、少なくとも1つの保持脚に加えて、または少なくとも1つの保持脚の代わりに設けることができる。
収容部材および該収容部材を拡大させるべく収容部材内に移動できるワイヤを備えた閉塞器具と、
デリバリチューブ、収容部材を左心耳内に前進させる第一プッシャ、およびワイヤをデリバリチューブから収容部材内に前進させる第二プッシャを備えたデリバリシステムとを有している。
好ましい実施形態では、保持部材は、収容部材を固定すべく組織と係合するための、収容部材から延びている。好ましくは、収容部材およびワイヤは、デリバリチューブ内ではデリバリのために縮小位置にあり、デリバリチューブからデリバリ(分配)されると、大きい寸法に拡大され、ワイヤは、収容部材内ではコイル形態に拡大されて、収容部材を拡大する。
一実施形態では、多くの個別ワイヤが収容部材内に前進される。このような実施形態では、第一ワイヤが収容部材内に送出された後に、第二ワイヤが、第二プッシャ(すなわち新しいプッシャが装填されたプッシャ)により収容部材内に移動され、収容部材を更に拡大させる。
左心耳にデリバリを行うための収容部材および保持部材を用意する段階と、
縮小プロファイル位置にある収容部材を左心耳にデリバリする段階と、
左心耳の開口を閉塞すべく、ワイヤを現場で収容部材内に前進させて収容部材を拡大させる段階とを有し、保持部材は心耳内の収容部材を固定することを特徴とする。
本発明の方法は、好ましくは、保持部材を解放する段階を更に有し、保持部材は収容部材の遠位側に配置され、保持部材を解放する前記段階は、収容部材の拡大前に生じる。好ましくは、保持部材を解放する段階は、保持部材が形状記憶形態に移行することを可能にする。ワイヤを収容部材内に前進させる段階は、ワイヤを収容部材内で形状記憶形態に移行させることを可能にする。
本発明の方法は更に、収容部材のねじをデリバリ器具から緩めることにより、収容部材をデリバリ器具の一部から取外す段階を有している。
ここで図面を詳細に参照すると、幾つかの図面を通して、同じまたは同類のコンポーネンツは同じ参照番号で示されている。本発明は、左心耳(「LAA」)を閉じる閉塞器具および閉塞器具を配置するデリバリシステムを提供する。最初に、デリバリ形態および定置形態を示すそれぞれ図2および図6を参照すると、閉塞器具は、クリップコンポーネント12およびバッグコンポーネント20を有している。バッグ20は心耳内で拡大可能であり、かつ心耳の内壁と係合するクリップ12により保持される。閉塞器具はまた、図7に示すようなコイル形状を有するワイヤコンポーネント30を有している。バッグ20は、ワイヤ30の収容部材を形成している。すなわち、ワイヤ30は、詳細に後述するように、現場でバッグ20内に前進されてバッグ20を拡大し、開口を通って心耳内に入ることを防止する。これにより、拡大されたバッグ20がLAA開口を塞ぎ、血餅が心耳から移動することを防止する。
図11には、バッグの他の実施形態が示されている。バッグ90は、心耳の壁Wと係合してバッグを固定するように設計された一連のうろこ92のように粗面化加工された表面を有している。バッグ90は、クリップ脚94、96(クリップ14、16と同じものが好ましい)と一緒に使用されるものが示されている。しかしながら、粗面化表面は、クリップ脚を設ける必要がない充分な保持力が得られるように構成することも考えられる。
第一段階では、プッシャ60が、例えばカテーテル50の近位端に設けたハンドル(図示せず)または他の機構により、遠位側(図3の矢印Dの方向)に前進される。プッシャ60の遠位端62が支持体22を介してバッグ20の近位端に取付けられかつクリップ脚14、16が脚20の遠位端に取付けられているため、プッシャ60を遠位側に前進させると、脚14、16がカテーテル50から前進される。脚14、16が露出されると体温により暖められて、図3に示すような形状記憶された定置位置に戻り、心耳の壁Wと係合する。脚14、16がこれらの完全記憶位置に戻る度合いは、心耳のサイズおよび組織の厚さにより定まる。
次に、プッシャ70が、図6の矢印Eで示すように遠位側に前進されると、プッシャ70の遠位端72がワイヤ30の近位端31と係合して、ワイヤ30を、心耳A内に位置するバッグ20内に押し込まれる。ワイヤ30がカテーテル50を出て心耳A内のバッグ20内に入ると、体温により暖められて、その形状記憶されたコイル形態に移行する。このコイル形態が、バッグ20を拡大させる。図6は、コイルの拡大により拡大された形態にあるバッグ20を示しているが、この段階ではプッシャ70が未だ、ワイヤ30を完全に定置させるまで完全に前進されていないため、バッグ20も未だ完全に拡大されていない。図5および図7は、ワイヤ30を、カテーテル50から最初に出た位置から移動させて、その完全コイル形態に拡大させるところを示すものである。明瞭化のため、バッグ20は示されていない。同じく、アナトミー(解剖学的構造)も明瞭化のため示されていない。図9および図10は、閉塞器具の完全定置位置を示し、バッグ20の一部は明瞭化のため除去されている。
図示の好ましい実施形態では、プッシャ70が放射線不透過性マーカ74(図4)を備えており、該放射線不透過性マーカ74は、カテーテル50の放射線不透過性マーカー56と整合できることに留意されたい。これにより、両マーカ74、56が整合したときにプッシャ70がその移動を完了しかつワイヤコイル30が挿入されたことが使用者に表示され、またイメージングのための比較的大きい一様領域が形成される。これは、図6の位置より遠位側の位置へのプッシャ70の完全前進時に生じる。バッグ20に隣接する脚14、16の基部にも、イメージングのための放射線不透過性マーカ19を設けておくのが好ましい。
心耳のサイズおよびバッグをどれほど拡大すなわち充満するかに基いて、ワイヤ30の長さすなわち拡大を異ならせることができる。従って、特別注文の閉塞器具を作ることができる。心耳Aのサイズは測定でき、これにより、次に、バッグの長さおよび拡大直径を調節できる。これが、例えば図12から図15に示されている。
以上、本発明の閉塞器具を心臓の左心耳を閉じるものについて説明したが、本発明の閉塞器具は、例えば血管、フィステルの尿管等の他の導管に具現することもできることは理解されよう。
上記説明は多くの特定事項を含んでいるが、これらの特定事項は開示の範囲を限定するものではなく、本発明の好ましい実施形態の単なる例示である。当業者ならば、特許請求の範囲の記載により定められた開示の範囲および精神内に含まれる他の多くの変更形態を考え得るであろう。

Claims (11)
- 左心耳を閉じるシステムであって、該左心耳を閉じるシステムは、
第一の材料からなる封入された収容バッグと、前記第一の材料と異なる第二の材料からなり、左心耳において前記収容バッグを縮小形状から拡大させるように前記収納バッグ内に移動可能なワイヤと、左心耳内に収容バッグを固定する少なくとも1つの保持脚と、を有する閉鎖器具と、
前記収容バッグの近位側に前記ワイヤを位置決めして前記収納バッグを収容するデリバリチューブと、前記デリバリチューブ内に位置決めされ、前記収納バッグを左心耳内に前進させる第一プッシャと、を有し、前記第一プッシャは、前記収納バッグを前進させるために近位位置から遠位位置に移動可能であり、前記収容バッグが前記デリバリチューブ内で縮小位置にあるときに前記デリバリチューブ内で前記第一プッシャの近位側に位置決めされ、前記ワイヤを前記デリバリチューブから前記収納バッグ内に前進させるために前記第一プッシャ内で前進位置に移動可能な第二プッシャを更に有するデリバリシステムと、を備えることを特徴とする左心耳を閉じるシステム。 - 前記ワイヤは形状記憶材料からなることを特徴とする請求項1記載の左心耳を閉じるシステム。
- 前記ワイヤは、デリバリ中はほぼ真直な位置に保持されかつ前記収容バッグ内ではコイル形状を形成することを特徴とする請求項1または2記載の左心耳を閉じるシステム。
- 前記閉鎖器具は、前記収容バッグを拡大すべく、現場で収容バッグ内に移動される多くの別個のワイヤを更に有することを特徴とする請求項1乃至3のいずれか1項記載の左心耳を閉じるシステム。
- 前記収容バッグは、デリバリシステムの第一プッシャに着脱可能に連結されることを特徴とする請求項1乃至4のいずれか1項記載の左心耳を閉じるシステム。
- 前記収容バッグは、第一プッシャに着脱可能に連結するためのねじ山を有していることを特徴とする請求項5記載の左心耳を閉じるシステム。
- 前記少なくとも1つの保持脚は、デリバリのためのほぼ真直な位置と左心耳の壁と係合する第二位置との間で移行できる形状記憶材料からなることを特徴とする請求項1乃至6のいずれか1項記載の左心耳を閉じるシステム。
- 前記少なくとも1つの保持脚は異なる方向に延びている2つの脚からなり、第二位置において2つの脚は左心耳の壁と係合することを特徴とする請求項7記載の左心耳を閉じるシステム。
- 前記収容バッグは、左心耳内の収容バッグを保持すべく、左心耳の壁と係合する粗面化された外面を有していることを特徴とする請求項1乃至8のいずれか1項記載の左心耳を閉じるシステム。
- 前記第二プッシャは第一放射線不透過性マーカを備え、デリバリチューブは第二放射線不透過性マーカを備え、第一放射線不透過性マーカおよび第二放射線不透過性マーカは、実質的に整合して、第二プッシャの前進の完了を使用者に表示することを特徴とする請求項1乃至9のいずれか1項記載の左心耳を閉じるシステム。
- 前記ワイヤが送出された後にデリバリチューブ内に配置できる第二ワイヤを更に有し、第二ワイヤは、収容バッグを更に拡大させるべく収容バッグ内に移動できることを特徴とする請求項1乃至10のいずれか1項記載の左心耳を閉じるシステム。
