Description
本願は、米国特許法第119条(e)項の下で、2002年1月11日に出願された米国特許第60/347,741号に対する優先権を主張する。
本発明は、血液製剤に関し、より詳細には、酸素に対して高い親和性を有する改変されたヘモグロビンを含有する組成物およびこのような組成物を作製するための方法に関する。
(循環系およびヘモグロビンの性質)
血液は、組織に栄養分を送達し、排出のために組織からの廃棄物を除去するための手段である。血液は、赤血球(red blood cell)(RBCまたは赤血球(erythrocyte))、白血球(WBC)、および血小板が懸濁している血漿からなる。赤血球は、血液中で細胞の約99%を構成し、それらの主な機能は、酸素を組織に輸送し、組織からの二酸化炭素を除去することである。
病院および他の設定における血液製剤の需要に起因して、代用血液および血漿増量薬の開発における広範な研究が指向されている。代用血液は、酸素を組織に運搬および供給する能力を有する血液製剤である。代用血液は、多数の用途を有する。これらの用途としては、外科手順の間およびその後の急性出血により損失される血液の交換、ならびに外傷性損傷後の蘇生手順のための用途が挙げられる。血漿増量薬は、脈管系に投与されるが代表的に酸素を運搬する能力を有さない、代用血液である。血漿増量薬は、例えば、火傷により損失される血漿を交換して、容量欠乏性ショック(volume deficiency shock)を処置し、血液希釈をもたらすため(例えば、正常血液量を維持するためおよび血液粘度を低下させるため)に使用され得る。本質的には、代用血液は、これらの目的または保存血液を容易に患者に投与する任意の目的のために使用され得る(例えば、Bonsonらに対する米国特許第4,001,401号およびMorrisに対する米国特許第4,061,736号を参照のこと)。
従って、代用血液(しばしば、「酸素運搬血漿増量薬」とも称される)の製造に対する試みにより、限界効率を有するかまたはその製造が退屈であるか、高価であるかもしくはその両方である製剤が、かなり製造されてきた。頻繁に、このような製剤を製造するコストはかなり高いので、特に、最も高い需要が存在している市場(例えば、新たに独立した第3世界の経済)において、これらの製剤の広範な使用は事実上除外されている。
本発明の方法および組成物は、種々の設定(緊急治療室、手術室、軍事衝突、癌病院、および獣医診療室を含む)において有用である。本発明の低い毒性および高い安定性によって、記載される代用血液の有効性を損なうことなく、室温での保存が可能になる。本発明はまた、血液型交差適合試験およびその関連する研究室試験に対する必要性を回避する。これによって、患者の処置における介入がより早期でかつより安全になる。従って、本発明の低い毒性、長期の安定性および普遍的な適用性の組み合わせは、血液との、特定の有用な代用品を提供する。
本発明は、高い酸素親和性を有するHBOCを含む代用血液に関する。特定の適用において、これらの組成物は、ネイティブへモブロビンに近似する酸素親和性を有する代用血液より、より効率的に、組織へ酸素を送達することができる。
後述の開示に記載される本発明の理解を深めるために、多くの用語が下記で定義されている。

Hb−(S−Y−R−CH2−[O−CH2−CH2]n−O−CH3)m
ここで、Hbは、四量体のヘモグロビンであり、Sは、表面チオール基であり、Yは、HbとMal−PEGとの間のスクシンイミド共有結合であり、Rは、アルキル、アミド、カルバミン酸塩またはフェニル基(生材料の供給源および化学合成の方法に依存して)であり、[O−CH2−CH2]nは、PEGポリマーの骨格を形成するオキシエチレン単位であり、ここで、nは、ポリマーの長さ(例えば、分子量=5000)を定義し、そしてO−CH3は、末端メトキシ基である。PHPおよびPOEは、2つの異なるPEG改変ヘモグロビンである。
用語「酸素送達能力」または単に用語「酸素能力」は、酸素を送達するための血液物質の能力をいうが、それが酸素を送達する効率と必ずしも相関しない。酸素送達能力は、一般的に、ヘモグロビ濃度から算出される。なぜなら、1グラムのヘモグロビンが1.34mlの酸素と結合することが知られているからである。このように、ヘモグロビン濃度(g/dl)に1.34を掛けると、酸素受容能力(ml/dl)が算出される。ヘモグロビン濃度は、任意公知の方法(例えば、β−ヘモグロビンフォトメーター(HemoCue,Inc.,Angelholm,Sweden))によって測定され得る。同様に、酸素受容能力は、例えば、燃料電池装置(例えば、Lex−O2−Con;Lexington Instruments)を用いて、ヘモグロビンまたは血液のサンプルから放出される酸素の量によって測定される。
用語「酸素親和性」は、酸素キャリア(例えば、ヘモグロビン)が分子酸素と結合するアビディティーをいう。この特徴は、酸素分圧(X軸)と、ヘモグロビン分子の酸素飽和の程度(Y軸)に関連する酸素平衡曲線によって定義される。この曲線の位置は、酸素キャリアが、酸素で半分飽和され、そして酸素親和性に逆相関するP50値、酸素分圧によって示される。ここでP50が低ければ、酸素親和性は高くなる。全血(および全血組成物(例えば、赤血球およびヘモグロビン))の酸素親和性は、当業者の公知の種々の方法によって測定され得る(例えば、Winslowら、J.Biol.Chem.252(7):231−37(1977))。酸素親和性はまた、市販のHEMOXTMTM Analyzer(TCS Scientific Corporation,New Hope,Pennsylvania)を用いて決定され得る(例えば、VandegriffおよびShrager「Methods in Enzymology」(Everseら、編)232:460(1994)を参照のこと。)
用語「高張」は、血液(約25mg〜27mg未満)よりコロイド浸透圧を有するコロイド溶液を意味する。コロイド浸透圧は、任意の適切な技術(例えば、Wescor機器)によって測定され得る。
用語「ポリエチレングリコール」は、一般化学式H(OCH2CH2)nOH(ここで、nは、4以上である)の液体または固体ポリマーをいう。任意のPEG処方物(置換されているものまたは置換されていないもの)が使用され得る。
本発明の組成物および方法の首尾良い使用は、酸素送達および消費の基礎的な機構の理解を必要としないが、これらの推定機構のいくつかに関する基本的な知見は、以下の議論を理解することを補助し得る。一般的に、毛細管は、組織への酸素の主要な運搬体であると推定されてきた。しかし、休止している組織に関して、現在の発見は、細動脈の酸素放出と毛細管の酸素放出との間にほぼ均等分配(equipartition)が存在することを示す。すなわち、動脈系におけるヘモグロビンは、細動脈網におけるその酸素含有量の約1/3および毛細管における1/3を送達し、残りは静脈系を通じて微小循環を出ると考えられる。
高い酸素親和性を有するHBOCを開発するための原理は、一部、赤血球注入の代替として無細胞ヘモグロビンを用いた過去の研究に基づく。これらの溶液の生理学的効果のいくつかは、理解が不完全なままである。これらのうち、おそらく最大の論議は、動物および人間において高血圧として現れ得る血管収縮を引き起こす傾向である(Amberson,W.,「Clinical experience with hemoglobin−saline solutions」,Sicence 106:117−117(1947))(Keipert,P.,A.Gonzales,C.Gomez,V.Macdonald,J.Hess,およびR.Winslow,「Acute changes in sysmetic blood pressure and urine output of conscious rats following exchange transfusion with diaspirin−crosslinked hemoglobin solution」,Transfusion 33:701−708(1993))。α鎖間でビス−ジボロモサリチル−フマレートを用いて架橋されたヒトヘモグロビン(ααHb)は、モデル代用赤血球として米軍により開発されたが、肺および全身血管抵抗の重篤な増加が実証された後、米軍によって中止された(Hess,J.,V.Macdonald,A.Murray,V.Coppes,およびC.Gomez,「Pulmonary and systemic hypertention after hemoglobin administration」,Blood 78:356A(1991))。この製品の市販版もまた、第III相臨床試験が実現しなかった後に中止された(Winslow,R.M.「αα−Crosslinked hemoglobin:Was failure predicted by preclinical testing?」,Vox sang 79:1−20(2000))。
好ましい実施形態において、酸素運搬体(すなわち、酸素運搬成分)は、ヘモグロビンベースの酸素運搬体(すなわち、HBOC)である。ヘモグロビンは、ネイティブ(未改変)であり;続いて、化学反応(例えば、分子内架橋または分子間架橋、重合、または化学官能基(例えば、ポリアルキレンオキシド、または他の付加物)の付加)によって改変され得るか;またはヘモグロビンは、組換え操作され得る。ヒトα−グロビン遺伝子およびβ−グロビン遺伝子は、クローン化され、配列決定されている。Liebhaberら、P.N.A.S.77:7054−7058(1980);Marottaら、J.Biol.Chem.353:5040−5053(1977)(β−グロビンcDNA)。さらに、多くの組換え産生され改変されたヘモグロビンは、ここで、部位特異的変異誘発を使用して生成されたが、これらの「変異型」ヘモグロビン変種は、望ましくなく高い酸素親和性を有するとして報告された。例えば、Nagaiら、P.N.A.S.,82:7252−7255(1985)を参照のこと。
ヘモグロビンは、第1鉄形態(Fe2+)から第2鉄形態(Fe3+)(すなわち、メトヘモグロビン形態)に可逆的に変化する場合、自己酸化を示すことが公知である。これが生じる場合、分子酸素は、スーパーオキシドアニオン(O2−)の形態において、オキシヘモグロビンから解離する。これはまた、ヘム−グロビン複合体の不安定化を生じ、結果としてグロビン鎖の変性を生じる。酸素ラジカル形成およびタンパク質変性の両方は、HBOCのインビボ毒性において役割を果たすと考えられる(Vandegriff,K.D.,Blood Substitutes,Physiological Basis of Efficacy,第105頁−第130頁,Winslowら編、Birkhauser,Boston,MA(1995))。
好ましい実施形態において、酸素運搬成分は、改変ヘモグロビンである。ヘモグロビンに対する好ましい改変は、「表面改変」であり、すなわち、化学的官能基のヘモグロビン分子上の露出アミノ酸側鎖への共有結合である。
Hb−(S−Y−R−CH2−CH2−[O−CH2−CH2]n−O−CH3)m 式I
ここで、Hbは、四量体のヘモグロビンを表し、Sは、表面チオール基であり、Yは、HbとMal−PEGとの間のスクシンイミド共有結合であり、Rは、アルキル、アミド、カルバメートまたはフェニル基(原材料の供給源および化学合成の方法に依存する)であり、[O−CH2−CH2]nは、PEGポリマーの骨格を形成するオキシエチレン単位であり、ここで、nは、このポリマーの長さを規定し(例えば、MW=5000)、そしてO−CH3は、末端メトキシ基である。
本発明の1つの実施形態において、代用血液はまた、晶質を含有し得る。晶質成分は、任意の晶質であり得、これは、代用血液組成物の形態において好ましくは800mOsm/lより大きい(すなわち、代用血液を「高張性」にする)オスモル濃度を達成し得る。適切な晶質およびそれらの代用血液における濃度の例としては、例えば、3%のNaCl、7%のNaCl、7.5%のNaCl、および6%デキストラン中7.5%のNaClが挙げられる。より好ましくは、この代用血液は、800mOsm/lと2400mOsm/lとの間のオスモル濃度を有する。300〜800mOsm/lの間のオスモル濃度を有し、さらにコロイド(すなわち、デキストロースほど分散性ではない分子)を含む溶液中で、組換え産生されたヘモグロビンを使用することは、以前に報告されている。例えば、米国特許第5,661,124号を参照のこと。しかし、この特許は、800より大きいオスモル濃度を有する代用血液を産生することを教示せず、そしてヘモグロビン濃度は、6〜12g/dLの間であるべきであることを示唆する。対照的に、本発明の組成物の酸素保有効率は、より低い濃度のヘモグロビン(例えば、6g/dlより高濃度、またはさらに、4g/dlより高濃度)が使用されることを可能にする。代用血液が、晶質をさらに含有し、そして高張度である場合、本発明の組成物は、他の代用血液組成物(これは、コロイド成分を含む)より優れた、血流力学的パラメータの迅速な回復のための、改善された機能を提供し得る。少量の、非常に高張性の晶質注入(例えば、1〜10mg/kg)は、制御された出血で、認容可能な血液力学的パラメータの、迅速かつ持続性の回復において、有意な利点を提供する(例えば、Przybelski,R.J.,E.K.DailyおよびM.L.Birnbaum「The pressor effect of hemoglobin−good or bad?」Winslow,R.M.K.D.Vandegriff、およびM.Intaglietta編、Advances in Blood Substitutes.Industrial Opportunities and Medical Challenges.Boston,Birkhaeuser(1997),71−85を参照のこと)。しかし、等張性の晶質溶液は、単独では、大脳の酸素輸送を十分には回復させない。D.Proughら、「Effects of Hypertonic saline versus Ringer’s solution on cerebral oxygen transport during resuscitation from hemorrhagic shock」、J.Neurosurg.64:627−32(1986)を参照のこと。
本発明の代用血液は、酸素キャリアと他の任意の賦形剤とを、適切な希釈剤で混合することによって処方される。この希釈剤中の酸素キャリアの濃度は、適用に従って変動し得、そして特に、予測される投与後の希釈に基づくが、好ましい実施形態において、増強した酸素送達および治療効果を提供する、本発明の組成物の他の特徴に起因して、通常、6g/dlより高い濃度は必要ではなく、そしてより好ましくは、0.1〜4g/dlである。
(A.臨床適用)
本発明およびその実施形態は、O2レベルの迅速な回復または増加したO2レベルもしくはO2レベルの置き換えが臨床的に示される適用において、有用であることが意図される。例えば、米国特許第6,054,427号を参照のこと。本発明の方法および組成物が用途を見出す多数の状況としては、以下が挙げられる:
外傷。白血球の急激な損失が、間質空間および細胞内空間からの流体シフトを生じ、血液の損失された体積を置き換え、同時に血液を優先度の低い器官(皮膚および腸を含む)から短絡させ得る。器官からの血液の短絡は、これらの器官におけるO2レベルを低下させ、そして時々排除し、そして進行的な組織の死を生じる。O2レベルの迅速な回復は、このような急激な血液の損失を被る患者において、組織の有意により良好な救出をおそらく生じると予想される。
(B.獣医学的適用)
本発明はまた、非ヒトにおいて使用され得る。本発明の方法および組成物は、家畜およびコンパニオンアニマル(例えば、イヌ、ネコ、ウマ、トリ、爬虫類)、ならびに水族館(aquaria)、動物園、海洋水族館(oceanaria)および他の家畜動物の施設における他の動物のような家畜で使用され得る。本発明は、損傷、溶血性貧血などに起因する血液の喪失を被った家畜および野生動物の緊急処置において有用性が見出されることが企図される。例えば、本発明の実施形態は、ウマ感染性貧血、ネコ感染性貧血、化学物質および他の物理学的因子に起因する溶血性貧血、細菌感染、第IV因子断片化、脾機能亢進症(hypersplenation)および脾腫、家禽の出血性症候群、低形成貧血、再生不良性貧血、特発性免疫溶血性状態、鉄欠乏症、同種免疫溶血性貧血、微小血管症性溶血、寄生などのような状態において有用であることが企図される。特に、本発明は、稀な種および/または外来種の動物のための血液ドナーが探し出すことが困難な領域において用途が見出される。
(支質非含有ヘモグロビンの産生)
(工程−1−期限切れの赤血球の調達)
期限切れのパック詰めの赤血球を、市販の供給源(例えば、San Diego Blood Bankまたはthe American Red Cross)から調達する。好ましくは、期限切れの材料は、回収時から45日以下で受け取られる。パック詰めのRBC(pRBC)をさらなる処理まで(1〜7日間)4±2℃で保管する。全てのユニットを、ウイルス感染についてスクリーニングし、使用の前に核酸試験に供する。
パック詰めの赤血球をクリーン施設の滅菌容器にプールする。パック詰めの赤血球の体積を記載し、そして市販の同時オキシメータまたは他の当業者に認識される方法を使用してヘモグロビン濃度を測定する。
白血球枯渇(すなわち、白血球の除去)を、膜ろ過を用いて実施する。このプロセスの効率をモニタリングするために、開始および最終の白血球数を数える。
赤血球を、6倍容量の0.9%塩化ナトリウムで洗浄する。このプロセスは4±2℃で実施する。細胞洗浄物を、アルブミンについての分光光度的分析によって、血漿成分の除去を検証するために分析する。
洗浄した赤血球を、6倍容量の水を用いて攪拌しながら少なくとも4時間または一晩、4±2℃で溶解する。溶解物を、冷却した状態で処理してヘモグロビンを精製する。これは、溶解物を0.16μm膜に通す処理によって達成する。精製したヘモグロビンは、加熱滅菌容器(sterile depyrogenetaed vessel)に回収する。この処理における全工程を4±2℃で実施する。
ウイルス除去を、4±2℃にて、限外濾過によって実施する。
溶解物および限外濾過から精製したヘモグロビンを、10kD膜を用いて、Ringer’s lactate(RL)またはリン酸緩衝化生理食塩水(PBS、pH7.4)中に交換する。次いで、このヘモグロビンを、同じ膜を用いて1.1〜1.5mM(四量体)の最終濃度まで濃縮する。10〜12容量のRLまたはPBSを、溶媒交換に使用する。この処理を4±2℃で実施する。RL中で調製した溶液のpHを、7.0〜7.6に調整する。
PBSまたはRinger’s lactate(RL)中のヘモグロビンを、0.45または0.2μmの使い捨てフィルタカプセルに通して滅菌濾過し、化学改変反応を実施するまで4±2℃で保管する。
(支質非含有ヘモグロビンの改変)
(工程−1−チオール化)
チオール化を、ヘモグロビンについて10倍モル濃度の過剰なイミノチオラン(iminothiolane)を使用して、4±2℃にて4時間、連続攪拌しながら実施する。
・RL(pH7.0−7.5)またはPBS(pH7.4)中、1 mMヘモグロビン(四量体)
・RL(pH7.0−7.5)またはPBS(pH7.4)中、10 mMイミノチオラン
1:10 SFH:イミノチオランの比率および反応時間を、PEG化チオール基の数を最大にし、かつ異成分の生産を最小にするように最適化した。
チオール化したヘモグロビンを、開始の四量体ヘモグロビン濃度に基づいて20倍モル濃度の過剰なMal−PEG(アルキルリンカーまたはフェニルリンカーを有する)を用いて、PEG化する。ヘモグロビンを、まず、ヘモグロビンを酸化するための雰囲気で平衡化させる。この反応物を、連続攪拌しながら4±2℃で2時間置く。
・RLまたはPBS(pH7.4)中、1 mMチオール化ヘモグロビン
・RLまたはPBS(pH7.4)中、20 mM Mal−PEG。
PEG化−Hbを、70kD膜を通して処理し、過剰な未反応の試薬またはヘモグロビンを除去する。20−容量の濾過を、未反応の試薬の除去を確実にするために実施し、サイズ排除クロマトグラフィーによってモニタリング(540nmおよび280nm)する。このタンパク質濃度を、4g/dlに希釈する。pHは、1N NaOHを用いて7.3〜0.3に調整する。
最終MalPEG−Hb生成物を、0.2μm無菌ディスポーザブルカプセルを用いて無菌濾過し、4±2℃で加熱滅菌容器(sterile depyrogenated vessel)に回収した。
PEG化したHbを4g/dl RLに希釈し、pHを7.4±0.2に調整した。
最終代用血液組成物を無菌濾過(0.2μm)し、無菌ガラスバイアルに重量で分取し、層流フード内で捲縮シールを備える無菌ゴム栓で封じ、そして、使用まで−80℃で保存した。
(生理化学分析の方法論)
MalPEG−Hb代用血液の同質性および分子サイズは、液体クロマトグラフィー(LC)により特徴付けられる。分析用LCを使用して、PEG化ヘモグロビンの同質性および未反応のMal−PEGの除去程度を評価する。540nmでの吸光度を使用して、ヘモグロビンを評価し、ピーク位置により未反応のヘモグロビンからPEG化ヘモグロビンを分離する。280nmでの吸光度を使用して、遊離Mal−PEGからPEG化ヘモグロビンを分離する。この遊離Mal−PEGは、MalPEGの環状構造に起因して紫外(UV)スペクトルで吸収する。

表面改変について、式Iの数「m」は、ヘモグロビン表面に結合するPEGポリマーの数を規定するパラメータである。
この数を決定するために、ジチオピリジン比色定量アッセイ(Ampulski,R.,V.AyersおよびS.Morell.Determination of the reactive sulfhydryl groups in heme proteins with 4,4’−dipyridinesdisulde.Biocheim.Biophys.Acta 163−169,1969)を使用して、チオール化の前後、次いで、Hbの再PEG化の後に、Hb四量体の表面の利用可能なチオール基の数を測定する。ヒトヘモグロビンは、2つの内因性の反応性チオール基をβ93Cys残基に含み、この反応性チオール基は、ジチオピリジン反応によって確認される。1:10 SFH:イミンチオラン(iminothiolane)の比でのSFHのチオール化の後、ジチオピリジン反応による反応性チオール基の数は2チオールから6チオールに増加する。PEG化反応の後、反応性チオール基の数は、1.3に減少する。このことは、MalPEG−Hbに4〜5のPEG化部位が存在することを示す。
最終MalPEG−Hb生成物の分析のためにFPLCを実施する。未改変のSFHと比較したMalPEG−Hbについての代表的なクロマトグラムを図1に示す。SFHに対する保持時間は約57分である。MalPEG−Hbに対する保持時間は約44分である。
血液および未改変のヒトヘモグロビン(SFH)と比較したMalPEG−Hbの物理的性質を以下の表2に示す。
以前に記載されるように、ヘモグロビン−酸素平衡結合曲線を測定した(Vandegriff,K.D.,R.K.Rohlfs,M.D.Magde,およびR.M.Winslow.Hemoglobinoxygen equilibrium cures measured during enzymatic oxygen consumption.Anal.Biochem.256:107−116,1998)。MalPEG−Hbは、高い酸素親和性(P50=5mmHg)および低い協同性(n50=1.0〜1.4)を示す。図2は、支質非含有ヘモグロビン(SFH)溶液とMalPEG−Hb溶液を比較する代表的な曲線を示す。
MalPEG−Hbのこの溶液特性は、ポリエチレングリコール鎖と溶媒の水分子との間の強い相互作用に起因する。このことは、以下の2つの理由から代用血液のための重要な特性であると考えられる:1)より高い粘度が溶媒を介して拡散するPEG−Hb分子とガス状のリガンド分子の両方の拡散定数を減少させ、そして2)より高い粘度が内皮壁に向って流れる溶液のずり応力を増加させ、血管狭窄に対抗するために血管拡張剤の放出を誘発する。表2に示すように、MalPEG−Hb溶液の粘度は2.5cPsである。
未改変の分子内および分子間架橋ヘモグロビンまたはPEG−表面−結合体化ヘモグロビンを含有するヘモグロビン溶液のCOPを測定して、その高分子溶液特性を決定した(Vandegriff,K.D.,R.J.RohlfsおよびR.M.Wislow.Colloid osmotic effects of hemoglobin−based oxygen carriers.Winslow,R.M.,K.D.VandegriffおよびM.Intaglia編、Advances in Blood Substitutes Industrial Opportunities and Medical Challenges.Boston,Birkhauser,207−232頁(1997))。四量体ヘモグロビンは、理想溶液に近い挙動を示すが、PEGと結合体化されたヘモグロビンは、有意により高い膠質浸透活性を有し、非理想的な溶液を示す(Vandegriff,K.D.,M.Mcarthy,R.J.RohlsおよびR.M.Winslow.Colloid osmotic properties of modified hemoglobins:chemically cross−linked versus polyethylene glycol surface−conjugated.Biophys.Chem.69:23−30(1997))。表2に示す様に、MalPEG−Hb溶液のCOPは50である。
PEG−表面結合体化ヘモグロビンを含むヘモグロビン溶液の安定性を、自己酸化の速度を試験することによって、決定した。室温において、MalPEG−Hbの自己酸化は、図5に示されるように、10時間で、約5%MetHbから5.5%MetHbに増加した。MalPEG−Hbの自己酸化速度は、0.05%/時間であった。
(改変ヘモグロビンと異なるP50との比較)
ポリオキシエチレン(POE)への結合体化によって改変されたヘモグロビンを使用する無細胞ヘモグロビンの効力における酸素親和性の役割は、代用血液としてのこのような物質の効力を研究する際に特に興味が持たれている。この改変は、最初に、Iwashitaおよび共同研究者(Ajisaka,K.and Y.Iwashita,「Modification of human hemoglobin with polyethylene glycol:A new candidate for blood substitute.BBRC 97:1076−1081(1980))(Iwasaki,K.,K.Ajisaka,and Y.Iwashita,「Modification of human hemoglobin with polyoxyethylene glycol:A new candidate for blood substitutes」,Biochem Biophys Res Comm 97:1076−1981(1980))に記載され、高血圧効果を保持し、そして敗血症ショックの処置に有用であることが見出された。この産物の調製の一部として、ヘモグロビンは、ピリドキサール−5−ホスフェート(PLP)と反応して、そのP50を、ヒト血液についての値近くまで上昇させる。このように、POE改変ヘモグロビンの2つの溶液(1つは、PLPでの前改変を有し、そして1つは、PLPでの前改変を有さない)を調製することが可能であった。これらの溶液は、それらのP50を除いて、全ての方法で同一であり、そして重篤な(血液の60%)出血の間、ラットにおける生理学的機能を支持するそれらの能力について試験した。
(代用血液)
改変ヘモグロビン溶液(「PHP」を形成するためのPLP改変を有するかまたは有さない)を、実施例1において上記されるように、調製した。
雄性Sprague−Dawleyラットを、この研究のために使用した。収縮期血圧および拡張期血圧を、この研究の間にモニターした;最大圧力および最小圧力それぞれ、および平均動脈圧(MAP)は、拡張期血圧+1/3(収縮期血圧−拡張期血圧)であった。dP/dtを、各圧力サイクルについての最大の正の傾きから計算した。心拍数、収縮期血圧、拡張期血圧、平均動脈圧、脈圧およびdP/dtの平均値を、各分のデータについて平均化した。
動脈pH、PCO2、およびPO2を、100μlのヘパリン処理血液サンプルを使用して、血液ガス分析器において測定した。乳酸を、Lactate Analyzerを使用して、動脈血において測定した。全CO2、標準ビカーボネート(HCO3 −)、および塩基過剰(BE)を、以前に記載されたアルゴリズム(Winslow,R.,「A model for red cell O2 uptake」,.Int J Clin Monit Comput 2:81−93(1985))を使用して、PCO2、pHおよびヘモグロビン濃度から計算した。全ヘモグロビンおよび血漿ヘモグロビンを、市販の装置を使用して、各々測定した。ヘマトクリットを、微小遠心分離によって、動脈血の約50μlサンプルを使用して測定した。
交換輸血を、約0.5ml/分の速度で、推定血液容積の50%に等しい溶液の合計容量まで実施した。血液容量は、65ml/kgであると推定された。血液が、試験物質が注入される速度と正確に同じ速度で除去されるように、蠕動ポンプを操作した。試験溶液を、注入の前に、水浴中に37℃まで温め、そして注入の間、温め続けた。
本発明者らが使用した出血プロトコルは、Hannon and Wade(Hannon,J.,C.Wade,C.Bossone,M.Hunt,R.Coppes,and J.Loveday,「Blood gas and acid−base status of conscious pigs subjected to fixed−volume hemorrhage and resuscitated with hypertonic sali dextran」,Circ Shock 32:19−29(1990))のモデルに基づいた。出血を、大腿動脈から0.5ml/分の速度で、動脈血をポンプで抜き出すことによって、交換輸血の完了の約3分後に始めて、60分の終わりまで、血液容量の60%を除去した。血液サンプル(0.3ml)を、出血および血液ガス分析のために10分毎に採取した。
生存分析のために、出血の開始の後、最小で120分間、観察した。データを10分間隔に分類し、そして各間隔について、累積的な生存割合およびその標準誤差を計算した。
(溶液特性)
使用した溶液を以下の表3に記載する。全ヘモグロビン濃度、粘度およびコロイド浸透圧(COP)は、十分に一致する。PHPのP50(19.7トル)は、POEのP50(12.2トル)よりも高い。協同性の程度(Hillのパラメーター、n)は、2つの溶液について等しかった。
全ての実験を、表4に示す。多くの動物は、POE(11)よりPHP(18)を受け入れ、そしてPHP動物の平均重量は、POE群の平均重量より有意に大きかった(p<0.001)。しかし、重量に関するこの差異は、出血の程度を計算し、65ml/kgの全血液容量を推定する際に明らかになった。従って、交換輸血および出血の結果量は、同様に異なった。それにもかかわらず、死滅するまでの平均時間は、POE動物(116分)と比較してPHP動物について、有意により短かった(93分)。この差異は、統計学的に有意であった(P<0.02)。動物が、出血の開始後120分の観察期間、生存する場合、動物を、Kaplan−Meier生存分析の目的で「検閲された」とみなした(図5)。
ベースライン、ET後および出血後(60分)測定を、表5に示す。平均ヘマトクリットは、PHP動物と比較してPOEにおいてわずかにより高かったが、交換輸血後、この値は、2つの群に関して一致した。出血期間の最後に、POE動物の平均ヘマトクリットは、PHP動物より再びわずかに高かった。同様の小さな差異を、全ヘモグロビン値に関して見出し、POE動物が、全てのサンプリング位置でわずかにより高いことがわかった。血漿ヘモグロビンは、2つの群に関して異ならなかったが、交換期間後、POE動物に関して有意により高かった。
交換輸血の間の平均動脈圧力を、図6に示す。ベースライン平均動脈圧力は、2つの群について区別できない。しかし、PEG−ヘモグロビンの輸血に対する血圧応答は、2つの群の間で有意に異なる。MAPに関する最初の上昇は、POE動物と比較してPHPにおいてより大きく、そしてそれは、輸血期間の持続のために保たれる。対照的に、POE動物のMAPは、輸血の最後までにベースラインに戻る。
本研究において、本発明者らは、重症(血液量の60%)出血からラットを保護するためのそれらの能力に関して2つに匹敵する改変ヘモグロビン溶液(「代用血液」)を研究した。この能力を試験するため、動物に最初、2つの試験溶液の1つを有する50%(血液量の)交換輸血を与えた。溶液自体は、それらの酸素親和性に関してのみ異なり、そしてFPLCパターン、膨張圧力、粘度および濃度に関して非常に密接に匹敵した。生理学的結果に対する特定の変数の効果を示すことを試みる他の研究は、調和と同様に溶液を比較することができなかった。例えば、Sakai、H.、Hara、H.、Tsai、A.G.、Tsuchida、E.、Johnson、P.C.、およびIntaglietta、M.、「Changes in resistance vessels during hemorrhagic shock and resuscitation in conscious hamster model」Am.J.Physiol 276(45)、H563−H571.(1999)、Sakai、H.、Hara、M.、Yuasa、A.Tsai、S.Takeoka、E.Tsuchida、およびM.Intaglietta、「Molecular dimensions of Hb−based O2 carriers determine constriction of resistance arteries and hypertension」、Am.J.Physiol 279:H908−H915、(2000)を参照のこと。これらの実験は、このような親密に調和した溶液が、有意な差異である、一つの変数P50だけと比較され得るという最初の例示を示す。
この研究の目的は、第I相臨床試験のためのサンプルの貯蔵のシミュレーションの間のMalPEG−Hbの安定性および取り扱い条件を決定することであった。取り扱いの三段階の間の安定性を評価した。段階Iは、製造施設での凍結保存から臨床現場に輸送される間の温度条件への移動を示した(凍結貯蔵研究)。段階IIは、+4℃で24時間にわたるそのMalPEG−Hbの融解およびその後の4℃で5日間にわたる貯蔵を示した(冷蔵研究)。段階IIIは、+4℃まで24時間でのそのMalPEG−Hbの融解およびその後の患者に投与する前に数日間室温でのMalPEG−Hbの貯蔵を示した(室温研究)。
安定性を、そのMalPEG−Hb試験材料の酸化の速度によって規定した。同時酸素測定(co−oximetry)(IL Co−oximetry 682)を使用して、サンプル中のメトヘモグロビンの割合を測定した。プロトコルに従って、各時点で、二連で測定を行った。
MalPEG−Hbは、図8に示されるように、−20℃での6日間の貯蔵の間に%メトヘモグロビンにおいて変化を示さなかった。同様に、MalPEG−Hbは、図9に示されるように、+4℃での5日間の貯蔵の間に、%メトヘモグロビンにおいて変化を示さなかった。室温での貯蔵の間に、MalPEG−Hbは、図10に示されるように、10時間にわたってメトヘモグロビンにおいて1%未満の増大を示した。
Claims (15)
- マレイミジル−PEGと結合体化された酸素化ヘモグロビンを含む、外傷、急激な血液の損失、虚血、造血、癌、敗血症性ショック、慢性貧血、心臓麻痺または低酸素症の処置のために組織に酸素を送達するために有用な医薬の調製のためのポリアルキレンオキシド改変ヘモグロビン製品であって、アルキレンまたはフェニレンからなるリンカーが該マレイミジル部分と該PEG部分とを結合させ、該PEGは、式−(OCH 2 CH 2 ) n OCH 3 を有し、ここで、nは4以上であり、該酸素化ヘモグロビンの結合体は、pH7.4および37℃で測定したときに15トルより小さなP50を有し、該マレイミジル部分は、該ヘモグロビン上の表面チオール基に結合され、該ヘモグロビンは、該ヘモグロビンが大気と平衡化されることにより酸素化されている、ポリアルキレンオキシド改変ヘモグロビン製品。
- 代用血液として使用するための組成物であって、水性希釈液中に請求項1に記載のポリアルキレンオキシド改変ヘモグロビン製品を含む、組成物。
- 請求項2に記載の組成物であって、ヘモグロビンの濃度は、0.1〜4.0g/dlの間である、組成物。
- 請求項2に記載の組成物であって、前記粘度は、2.5+/−1.0cPsである、組成物。
- 請求項1に記載のポリアルキレンオキシド改変ヘモグロビン製品であって、前記酸素化ヘモグロビンは、10トルより小さなP50を有する、ポリアルキレンオキシド改変ヘモグロビン製品。
- 請求項1に記載のポリアルキレンオキシド改変ヘモグロビン製品であって、前記酸素化ヘモグロビンは、6トルより小さなP50を有する、ポリアルキレンオキシド改変ヘモグロビン製品。
- 請求項1に記載のポリアルキレンオキシド改変ヘモグロビン製品であって、前記酸素化ヘモグロビンは、6+/−2トルのP50を有する、ポリアルキレンオキシド改変ヘモグロビン製品。
- 請求項1に記載のポリアルキレンオキシド改変ヘモグロビン製品であって、前記表面チオール基のうちの少なくともいくつかは、結合の前に前記ヘモグロビンに化学的に付加される、ポリアルキレンオキシド改変ヘモグロビン製品。
- 請求項1に記載のポリアルキレンオキシド改変ヘモグロビン製品であって、該ポリアルキレンオキシド改変ヘモグロビン製品は、式:Hb−(S−Y−R−CH2−CH2[OCH2CH2]nO−CH3)mを有し、
ここでHbは、四量体ヘモグロビンであり;
Sは、表面チオール基であり;
Yは、ヘモグロビンとPEGとの間の共有結合であり;
Rは、リンカーであり;
mは、2より大きい、
代用血液製品。 - 請求項9に記載のポリアルキレンオキシド改変ヘモグロビン製品であって、mは4〜5である、ポリアルキレンオキシド改変ヘモグロビン製品。
- 請求項1に記載のポリアルキレンオキシド改変ヘモグロビン製品であって、前記ヘモグロビンは、メトヘモグロビンをさらに含む、ポリアルキレンオキシド改変ヘモグロビン製品。
- 請求項11に記載のポリアルキレンオキシド改変ヘモグロビン製品であって、%メトヘモグロビンは、10%未満である、ポリアルキレンオキシド改変ヘモグロビン製品。
- 請求項1に記載のポリアルキレンオキシド改変ヘモグロビン製品を作製する方法であって、該方法は、以下の工程:
a)ヘモグロビンの溶液を調製する工程;
b)該ヘモグロビンをチオール化して、表面チオール基を付加する工程;
c)該ヘモグロビンを酸素化して、酸素化ヘモグロビンを形成する工程;
d)前記マレイミジル部分を、該ヘモグロビン上の該表面チオール基に共有結合して、マレイミジル改変酸素化ヘモグロビンを形成する工程、
を包含する、方法。 - 請求項13に記載の方法であって、工程d)は、ネイティブのチオール基および付加されたチオール基の両方に対する結合を生じる、方法。
- ポリエチレングリコール(PEG)−ヘモグロビン結合体および水性希釈剤を含む、外傷、急激な血液の損失、虚血、造血、癌、敗血症性ショック、慢性貧血、心臓麻痺または低酸素症の処置のために組織に酸素を送達するために有用な医薬の調製のためのヘモグロビン組成物であって、該PEG−ヘモグロビン結合体のPEG部分は、アルキレンまたはフェニレンからなるリンカーにより、該ヘモグロビン上の表面チオール基と反応するマレイミジル部分と結合され、該組成物は、4.2±0.2g/dlのヘモグロビン濃度、10%未満のメトヘモグロビン、2.5±1.0cPsの粘度、50±20mmHgのコロイド浸透圧、pH7.4および37℃の温度で測定される場合の6±2トルのP 50 、ph7.4±0.4、0.5EU/ml未満のエンドトキシン、ならびに1.2±0.5のヒル係数(P 50 で)を有する、組成物。
