Description
本特許出願は、2005年11月17日に公開された米国特許出願公報2005/0256505を参照して本明細書に組み入れる。
本発明は、全体として医療機器、さらに詳しくは、医療装置の少なくとも一部に動きを与えるための医療用駆動システムに関する。
医師は通常、長い可撓性内視鏡を用いて患者の胃腸(GI)管の中の組織にアクセスし、その組織を視覚化する。上部GIに対しては、食道、胃、および近位十二指腸中の組織を検査し治療するために、医師は鎮静状態の患者の口の中へ胃鏡を挿入することができる。下部GIに対しては、直腸および結腸を検査するために、医師は鎮静状態の患者の肛門から結腸鏡を挿入することができる。一部の内視鏡は、ハンドピースのポートから可撓性挿入管の遠位部分へと延びる、通常直径約2.5〜3.5ミリメートルのワーキングチャンネルを有する。医師は、患者の体内の組織の診断または治療を助けるために、ワーキングチャンネルの中に医療装置を挿入することができる。医師は、一般的に、内視鏡のワーキングチャンネルを通して可撓性生検鉗子を使用し、GI管の粘膜内層から組織生検材料を採取する。
本発明の第1の実施態様の第1の表現(first expression)は、機械化された駆動システム組立体を含む医療用駆動システムのためのものである。駆動システム組立体は、可撓性カテーテル上に係合してスライド(engagingly slide)し、そのカテーテルを包囲するように構成されている。カテーテルは、患者の体の内腔の中へ挿入可能な遠位端を有する。駆動システム組立体は、医療装置の少なくとも一部と係合し、これを動かすように構成されている。
図1は、医療用駆動システムがカテーテル上にスライドされ、カテーテルを包囲し、医療用駆動システムが、医療−治療用超音波振動子組立体(medical-treatment ultrasound transducer assembly)の形の医療器具を含む医療装置の第1の実施態様を動かし、また、医療用駆動システムの上ぶた(top cover)が取り外されている、本発明の医療用駆動システムの第1の実施態様、および可撓性カテーテルの遠位端部分の斜視図である。

機械化された駆動システム組立体、
を具備する、医療用駆動システム。
(2)実施形態1に記載の医療用駆動システムにおいて、
前記駆動システム組立体は、前記カテーテルの前記遠位端に近接して配置されるように構成されている、医療用駆動システム。
(3)実施形態2に記載の医療用駆動システムにおいて、
前記カテーテルは、内視鏡挿入管である、医療用駆動システム。
(4)実施形態3に記載の医療用駆動システムにおいて、
前記駆動システム組立体は、前記医療装置の少なくとも前記一部を、前記カテーテルの前記遠位端を越えて直線的に動かすように構成されており、
前記医療装置は、雄ネジを有する、医療用駆動システム。
(5)実施形態4に記載の医療用駆動システムにおいて、
前記医療装置は、患者組織を医学的に治療するように構成されている医療器具を含む、医療用駆動システム。
前記医療装置は、医療用ガイドワイヤーを含む、医療用駆動システム。
(7)実施形態4に記載の医療用駆動システムにおいて、
前記駆動システム組立体は、被駆動外歯を有するナットギヤであって、前記雄ネジと機能的に係合するように構成されている駆動雌ネジを有する、ナットギヤを含む、医療用駆動システム。
(8)実施形態7に記載の医療用駆動システムにおいて、
前記駆動システム組立体は、前記ナットギヤの前記外歯と係合するように配置された平歯車を含む、医療用駆動システム。
(9)実施形態1に記載の医療用駆動システムにおいて、
前記駆動システム組立体は、動いている前記医療装置が所定の閾値を超える抵抗を受ける場合に、前記医療装置の前記機械化された駆動システム組立体の運動を妨げる、摩擦クラッチを含む、医療用駆動システム。
(10)医療用駆動システムにおいて、
機械化された駆動システム組立体であって、
患者の体の内腔の中へ挿入可能な遠位端を有する可撓性カテーテルに、包囲するように取り付けられ、かつ前記カテーテルから分離するように構成され、
医療装置の少なくとも一部と係合し、前記医療装置の少なくとも前記一部を動かすように構成されている、
機械化された駆動システム組立体、
を具備する、医療用駆動システム。
前記駆動システム組立体は、動いている前記医療装置が所定の閾値を超える抵抗を受ける場合に、前記医療装置の前記機械化された駆動システム組立体の運動を妨げる、摩擦クラッチを含む、医療用駆動システム。
(12)医療用駆動システムにおいて、
機械化された駆動システム組立体であって、
前記機械化された駆動システム組立体は、
内視鏡のワーキングチャンネル入口上に係合してスライドし、前記ワーキングチャンネル入口を包囲するように構成され、
医療装置の少なくとも一部と係合し、前記医療装置の少なくとも前記一部を動かすように構成されており、
前記医療装置は、前記ワーキングチャンネル入口の中へ挿入可能である、
機械化された駆動システム組立体、
を具備する、医療用駆動システム。
(13)実施形態12に記載の医療用駆動システムにおいて、
前記駆動システム組立体は、前記医療装置の少なくとも前記一部を直線的に動かすように構成されており、
前記医療装置は、雄ネジを有する、医療用駆動システム。
(14)実施形態13に記載の医療用駆動システムにおいて、
前記医療装置は、患者組織を医学的に治療するように構成されている医療器具を含む、医療用駆動システム。
(15)実施形態13に記載の医療用駆動システムにおいて、
前記医療装置は、医療用ガイドワイヤーを含む、医療用駆動システム。
前記駆動システム組立体は、被駆動外歯を有するナットギヤであって、前記雄ネジと機能的に係合するように構成されている駆動雌ネジを有する、ナットギヤを含む、医療用駆動システム。
(17)実施形態16に記載の医療用駆動システムにおいて、
前記駆動システム組立体は、前記ナットギヤの前記外歯と係合するように配置された平歯車も含む、医療用駆動システム。
(18)医療用駆動システムにおいて、
平歯車、およびナットギヤを含む、機械化された駆動システム組立体、
を具備し、
前記平歯車は、駆動シャフトにより回転自在に駆動されるように構成され、
前記ナットギヤは、前記平歯車により係合される外歯を含み、かつ、医療装置の外部表面特徴部と係合して前記医療装置の少なくとも一部を動かすように構成されている内部特徴部を含む、医療用駆動システム。
(19)実施形態18に記載の医療用駆動システムにおいて、
前記医療装置は、患者組織を医学的に治療するように構成されている医療器具を含む、医療用駆動システム。
(20)医療用駆動システムにおいて、
内視鏡の可撓性挿入管のワーキングチャンネルの中へ挿入可能であり、かつ前記ワーキングチャンネルから引き抜くことが可能である、機械化された駆動システム組立体であって、回転可能な可撓性駆動シャフト、および前記駆動シャフトに固定して取り付けられたナットギヤを含む、機械化された駆動システム組立体、
を具備し、
前記ナットギヤは、雌ネジを有し、
前記駆動シャフトは、少なくとも1つの空洞部分を有し、
医療装置が前記空洞部分に配置されると、前記雌ネジは、前記医療装置の雄ネジと機能的に係合し、これにより、前記ナットギヤが前記駆動シャフトにより回転された時に、配置された前記医療装置の少なくとも一部を、前記ナットギヤを通して、前記挿入管の遠位端を越えて延びるように動かすよう、前記雌ネジが構成されている、医療用駆動システム。
前記駆動システム組立体は、前記駆動シャフトを包囲するシースを含み、
前記駆動シャフトは、前記シースに対して回転可能であり、
前記シースは、半径方向外向きの複数の突起部を含む、医療用駆動システム。
(22)実施形態20に記載の医療用駆動システムにおいて、
前記医療装置は、患者組織を医学的に治療するように構成されている医療器具を含む、医療用駆動システム。
(23)実施形態20に記載の医療用駆動システムにおいて、
前記医療装置は、医療用ガイドワイヤーを含む、医療用駆動システム。
(24)医療用駆動システムにおいて、
回転可能な可撓性駆動シャフト、および前記駆動シャフトに固定して取り付けられたナットギヤを含む、機械化された駆動システム組立体、
を具備し、
前記ナットギヤは、雌ネジを有し、
前記駆動シャフトは、少なくとも1つの空洞部分を有し、
医療装置が前記空洞部分に配置されると、前記雌ネジは、前記医療装置の雄ネジと機能的に係合し、これにより、前記ナットギヤが前記駆動シャフトにより回転された時に、配置された前記医療装置の少なくとも一部を、前記ナットギヤを通して動かすように、前記雌ネジが構成されている、医療用駆動システム。
(25)実施形態24に記載の医療用駆動システムにおいて、
前記駆動システム組立体は、前記駆動シャフトを包囲するシースを含み、
前記駆動シャフトは、前記シースに対して回転可能である、医療用駆動システム。
前記医療装置は、患者組織を医学的に治療するように構成されている医療器具を含む、医療用駆動システム。
Claims (9)
- 医療用駆動システムにおいて、
機械化された駆動システム組立体であって、
患者の体の内腔の中へ挿入可能な遠位端を有する可撓性カテーテル上に係合してスライドし、前記カテーテルを包囲するように構成されており、
医療装置の少なくとも一部と係合し、前記医療装置の少なくとも前記一部を動かすように構成されている、
機械化された駆動システム組立体、
を具備し、
前記駆動システム組立体は、スリーブ部分であって、前記医療用駆動システムが前記カテーテル上に係合してスライドする時に、前記カテーテルの一部に係合して包囲する内面と、これと反対側の外面であって、前記外面上で前記駆動システム組立体が前記医療装置と係合することができ、前記医療装置を動かすことができる、前記外面とを有する、スリーブ部分を具備する、
医療用駆動システム。 - 請求項1に記載の医療用駆動システムにおいて、
前記駆動システム組立体は、前記カテーテルの前記遠位端に近接して配置されるように構成されている、医療用駆動システム。 - 請求項2に記載の医療用駆動システムにおいて、
前記カテーテルは、内視鏡挿入管である、医療用駆動システム。 - 請求項3に記載の医療用駆動システムにおいて、
前記駆動システム組立体は、前記医療装置の少なくとも前記一部を、前記カテーテルの前記遠位端を越えて直線的に動かすように構成されており、
