Description
1950年代初期の外科的移植術の導入以来、脈管内におけるステント(鋳型)の使用は、血管(脈管)外科の分野において、最も重要な変化の1つを示している。
図17は、エミトロン・コーポレーション・ディメド・II(登録商標)イメージング・システムを用いて、本願発明に係る装置が配置(展開)されている静脈システム部分に関する、ブタの被検材料#5020について撮られたインヴィヴォでの写真イメージである。ステントとバルブ装置202は、配置(展開)用カテーテル内で、その圧縮された形態において示されている。装置202は、約2cmの長さがあり、十分に拡張せしめられたときの直径は約15mmである。同様のテストにおいて強膜を用いることに成功したが、この例においては、SISを有するバルブ材料が用いられている。図18は、安定したプラット・フォームを確立するべく配置(展開)された第1のステージ205と、配置(展開)のプロセスにおける(その中にバルブ材料を有する)第2のステージ208とを有する装置202を示している。図19は、静脈サイトの内腔を収納すると共にバルブ材料を所定位置に配置した、十分に拡張した装置202を示している。図20は、心臓収縮に際して血液が装置202を通して流動している間の装置202の更なる図である。ここでは、イメージ(画像)システム測定ゲージ213は、適切な配置(展開)を保証するために、検証(実証)モード(verification mode)で示されている。バルブの機能性の検証(実証)は、また、図21に示されている。その図面において、静脈部分は、血液が(イメージの方向性に基づいて部分的に隠れている)弁洞領域220,221に溜まる状態の弛緩期において示されている。図21は、本願発明の幾つかの教示内容に係る装置202の反逆行特質を明確に示している。

ステント・バルブの組み合わせの実行可能性は、ラボラトリで、ブタのモデルを使って研究された。変形(修正)を加えた自己拡張(型)ステントは、有効性と、血栓形成性(thrombogenicity)と、新たな補てつ(人工器官、人工補装具、プロテーゼ)の組織適合と、を評価するために、生体適合性材料と組み合われた。その材料は、球体状に構成されると共に、2弁構成(bi-valve design)として隣接するリーフレットに形成された。リーフレットは、7.0−ナイロン遮断縫合をもって、ステントに固定された。流体力学的テスト及び気圧テストが、可変式脈動流れを有する透明な管状装置内で行われた。バルブの保全性の確認時に、パイロット動物の研究(pilot animal study)が実施された。一般的な麻酔の下で、4頭の4ヶ月になるブタの末端側IVCにおいて、頸部から、バルベステント(登録商標)というトレードネームを有する補てつが埋め込まれた。動物は、塞栓(そくせん)症のリスクを減じるために、ワルファリン坑凝血剤により維持された。

- 脈管(10)の内腔内埋め込み用に構成された自己拡張型置換バルブアセンブリであって、
複数の柔軟な部材(89;146)を有し、
各柔軟な部材は、脈管(10)の流体がバルブアセンブリを通して一定方向(17)に許容されるようにすると共にバルブアセンブリ内の脈管(10)の流体の逆流を防止するように、少なくとも1つの他の柔軟な部材(89;146)と調和して協働するようにしたアセンブリにして、
上記柔軟な部材(89;146)は、柔軟であり且つ弾性を有する複数のストラットであって脈管(10)の内腔に使用されるように構成されたストラット(93;174)によって支持され、
該ストラット(93;174)は、所定の高さと、上流側端部と、下流側端部とを備える円筒をなし、
上記柔軟な部材(89;146)は、夫々、上流側端部と下流側端部の少なくとも一方を含む複数のストラット(93;174)に上記円筒沿いの複数の高さ(91)で接続され、
上記柔軟な部材(89;146)は、上記ストラット(93;174)がなす円筒内に含まれる、アセンブリ。 - 上記柔軟な部材(89;146)の1つの少なくとも一部は、強膜若しくは小腸の副粘膜材料を含む請求項1記載のアセンブリ。
