Description
第2の目的は、処置操作が簡便で、かつ処置時間が短い胃食道逆流症の治療ができる治療システムを提供することにある。
第3の目的は、内視鏡の先端から突出した固定器具により、胃食道接合部を容易に把持、引き下げすることを可能にする胃食道逆流症の治療ができる治療システムを提供することにある。
第4の目的は、固定手段と穿刺針を独立させることにより、様々な大きさの隆起を形成可能な胃食道逆流症の治療ができる治療システムを提供することにある。
第5の目的は、体腔内を傷つけることなく安全に隆起を形成可能な胃食道逆流症の治療ができる治療システムを提供することにある。
また、内視鏡と固定手段とは独立して操作可能な第1の内視鏡及び穿刺装置を設けることにより、噴門部の引き下げ量、針の穿入点の位置などを術者の意志でコントロールできるため、患者の症状に応じて様々な大きさの弁を形成することが可能となる。
穿刺装置は、2つの穿刺針が予め一体化された装置となっているため、穿刺装置を取付けた内視鏡の先端を一度食道壁の穿刺位置にアプローチすると、直ちに2本の穿刺針を穿刺可能である。この点からも、処置操作が簡便となり、処置時間を大幅に短縮する。
穿刺装置は内シースを穿刺部位に突き当ててから、針の穿刺ができるため、穿刺部位の位置決めが容易に行える。そして、その結果として確実に胃食道接合部から胃内に向けた穿刺が行えるため、必要な弁がそれほど大きくなくてもよい場合には、固定手段による生体組織の把持・固定をしなくても弁形成ができ、さらに処置操作が簡便になり、処置時間を大幅に短縮する。
穿刺装置は2つの穿刺針が予め一定の間隔で平行に配置されているために、2本の針の穿入点及び穿出点も所望の一定間隔となる。その結果、縫合糸により短縮される組織の範囲が極端に小さくなることなく所望の大きさとなるため、狙いとおりの大きさの弁を確実に形成可能である。
さらに、先端円筒部17には側孔18a,18bが互いに略180度の角度で対向する位置にあるように設けられている。この側孔18a,18bが後述する縫合糸の結び目の押し込む機能を有しており、穿刺装置1自体が結び目の押し込め具を兼ねている。側孔18a,18bの内径は縫合糸が挿通可能な大きさであり、φ1mm程度が望ましい。
図21〜図23に示すように、穿刺装置101の穿刺針4a,4bを穿刺針102a,102bに変えたものである。穿刺針102は針本体34a,34bとその手元側に接続された針グリップ104と針本体43及び針クリップ104内に進退自在に挿入された縫合糸103とから構成されている。針グリップ104には、針連結部材6を嵌め込むための縮径部43が設けられている。また、第2の内視鏡57の外周面に固着されたアウターチャンネル51の内腔には糸把持鉗子105が進退自在に挿入される。
穿刺針120の糸把持鉗子121は、操作パイプ40の先端に把持部122が設けられている。その際、把持部122は、把持部122を形成するループの中心軸123が操作パイプ40の長手軸方向に対して傾いた角度に位置するように予め曲げられた状態で操作パイプ40に取り付けられている。
穿刺装置140は第1の実施形態の穿刺装置1の穿刺針4a,4bのうちの一方を第2の実施形態の穿刺針102に、もう一方を穿刺針141に変えたものである。穿刺針102の構成は第2の実施形態と同じである。穿刺針141内から突没可能な糸把持鉗子145は、操作パイプ40の先端に把持部142が設けられている。その際、把持部142は、把持部142を形成するループ144の中心軸143が操作パイプ40の長手軸方向に対して傾いた角度に位置するように予め曲げられた状態で操作パイプ40に取り付けられている。
穿刺装置150は穿刺装置1の針連結部材6を針連結部材151に変えたものである。針連結部材151は、先端部152と中間部153と手元部154でクランク状に形成されており、先端部152と手元部154は前後に間隔をあけて位置している。先端部152と手元部154の間隔は、少なくても穿刺する組織の厚さ以上となっており、5mm程度以上であることが望ましい。
図36(a)に示すように、固定用把持鉗子160は、組織を把持する先端部161に一対の鉗子162a,162bが設けられている。鉗子162a,162bには側孔163a,163bが設けられており、その先端開口部164a,164bは鉗子162a,162bの先端近傍に位置している。側孔163a,163bは縫合糸が挿通可能な大きさであり、φ1mm程度以上であることが望ましい。また、固定用把持鉗子160は、図36(b)に示すような構成であっても良い。図36(b)は固定用把持鉗子160の鉗子162a,162bが開いた状態を先端側から見た図である。鉗子162a,162bの先端近傍に側孔165a,165bが設けられており、更にその側孔165a,165bから鉗子162a,162bの側面に向けてスリット166a,166bが延びている。側孔165a,165b及びスリット166a,166bの幅は縫合糸が挿通可能な大きさであり、1mm程度以上であることが望ましい。

Claims (1)
- 内視鏡と組み合わせて使用されて体腔内の組織を縫合あるいは結紮するために用いられる治療システムにおいて、
経口的に体腔内に挿入される第1及び第2の内視鏡と、
各々穿刺針を有する2つの穿刺部材を一体化し、前記2つの穿刺針が平行かつ一定の間隔を存した状態で配置され、両方の穿刺針には夫々縫合糸が別々に挿通される、前記第1の内視鏡に装着可能な組織穿刺装置と、
前記第2の内視鏡の先端から突き出して体腔内の穿刺部位を固定する組織固定器具と、
前記第2の内視鏡により経口的に体腔内に挿通され、体腔内の穿刺部位に穿刺した両方の穿刺針の先端から夫々突き出された2つの縫合糸の先端の両方をループ内に取り込み、前記両方の縫合糸の先端を把持可能なループ部を有し、前記ループ部で両方の縫合糸を把持し、前記両方の縫合糸を前記第2の内視鏡で体外に引き出し可能な糸把持具と、
を具備したことを特徴とする治療システム。
