1から2は新生児及び乳児用(体重3−20kg)

食道構成要素の追加は、麻酔をかけられている胃スタティス(gastri statis)の疑いのある患者に、集中治療室に、そしてエアウェイに特に過敏で人工呼吸器を外す際の良く知られ広く広まっている問題を起こす患者に、使用することを目的としている。そのような患者はほぼ確実に、取り外しが容易なように上部の過敏なエアウェイにバイパスをつけるための、器官開口術で終わるのである。慢性の閉塞性/拘束性気道疾患のある患者、喘息患者、そしてヘビースモーカーは、取り外しの際の問題を起こす可能性が高い患者である。
予期していた又は/及び予想していなかった困難な挿管を伴った患者の罹患率又は死亡率は、麻酔治療の良く知られた観点であり、その状況に直面したどんな麻酔医にとっても挑戦的な悪夢である。NLAの前部に予め形成された縦の隙間を伴ったNLAの改良版は、気管内チューブを伴ったNLAを通じて、ブージー、クックのエアウェイ又はファイバースコープの助けで、容易に挿管することを補助する。
NLAのそれぞれの構成要素の形状、設計や操作構造への幾つかの改良が、NLAの改良された性能又は美的外観のどちらに必要なのかが、予想される。

Claims (35)
- 人間若しくは動物に使用するための喉頭マスクエアウェイ装置(10,30,110)であって、当該エアウェイ装置は、 (i)遠位端部(13,149,249)と、近位端部(12,147,248)を持つエアウェイチューブ(11,83,63,101,111)であって、前記エアウェイチューブの遠位端部が、膨張可能ではない喉頭カフ(14,34,94,113,121,131)で囲まれており、また、前記喉頭カフの表面(32,131)が、患者の喉頭注入口に解剖学的に適合する形状に予め形成されているとともに、前記患者の前記喉頭注入口を覆って密封するように構成されている、エアウェイチューブと、 (ii)前記喉頭カフと前記エアウェイチューブの近位端部の間において前記エアウェイチューブに接して若しくはその周囲に位置する口腔安定器(20,40,50,90,100,122)であって、前記口腔安定器は、前記患者の舌の前側面又は前面と入れ子状に重なるように構成されるとともに、前記患者の対応組織に合わせて構成されることで、安定性を提供し,それにより,使用時のエアウェイチューブの回転の又は左右の動きを防ぐ、口腔安定器と、 を具備し、 前記エアウェイ装置は、前記喉頭カフの前記表面を組み込む、前側、腹側部分(136,146,246)と、後側、背面部分(135,145,245)と、を有し、 前記喉頭カフの前記表面は、Aスケールのショア硬度が40以下の物質から形成される、エアウェイ装置。
- 前記喉頭カフの前記表面は、前記エアウェイ装置の腹側部分とは異なったショア硬度の物質で形成される、請求項1に記載のエアウェイ装置。
- 前記喉頭カフの前記表面は、Aスケールのショア硬度が0から20の間の物質から形成される、請求項1又は請求項2に記載のエアウェイ装置。
- 前記喉頭カフは、Aスケールのショア硬度が40以下の物質から形成される、請求項1に記載のエアウェイ装置。
- 前記喉頭カフは、Aスケールのショア硬度が0から20の間の物質から形成される、請求項4に記載のエアウェイ装置。
- 前記喉頭カフの軟らかさは、当該喉頭カフ自体の本体内に空洞又は通路を形成することにより構成される、請求項1から請求項5のいずれか1項に記載のエアウェイ装置。
- 前記喉頭カフの前記表面と、前記口腔安定器の前側、腹側部分は、同じショア硬度の物質から形成される、請求項1から請求項6のいずれか1項に記載のエアウェイ装置。
- 前記口腔安定器は、前記喉頭カフの開放面を含む平面と同一の面にある、第一の腹側面を持ち、当該口腔安定器の第一の面は、凹型形状である、請求項1から請求項7のいずれか1項に記載のエアウェイ装置。
- 前記口腔安定器は、前記喉頭カフと前記口腔安定器が一体の構造であるように、前記喉頭カフの近位端部から前記エアウェイチューブの近位端部まで延びている、請求項1から請求項8のいずれか1項に記載のエアウェイ装置。
- 前記口腔安定器は、その幅Wが均一ではなく、前記喉頭カフと前記エアウェイチューブの近位端部の中間地点に位置する幅の広い地点がある、請求項1から請求項9のいずれか1項に記載のエアウェイ装置。
- 前記口腔安定器の前記幅の広い地点は、前記エアウェイチューブの近位端部よりも前記喉頭カフに近い、請求項10に記載のエアウェイ装置。
- 前記口腔安定器の最も幅の広い地点での幅Wの、同じ地点における前記口腔安定器の高さHに対する比率は、2.7±10%である、請求項1から請求項11のいずれか1項に記載のエアウェイ装置。
- 前記口腔安定器は、前記エアウェイチューブと一体をなす一部として形成される、請求項1から請求項12のいずれか1項に記載のエアウェイ装置。
- 前記口腔安定器、前記エアウェイチューブ、及び前記喉頭カフ領域は一体となったユニットとして形成される、請求項1から請求項13のいずれか1項に記載のエアウェイ装置。
- 前記口腔安定器は、膨張可能ではない、請求項1から請求項14のいずれか1項に記載のエアウェイ装置。
- 前記エアウェイ装置の後側すなわち背面部分と、前記エアウェイ装置の前側すなわち腹側部分は、異なったショア硬度の物質で形成される、請求項1から請求項15のいずれか1項に記載のエアウェイ装置。
- 前記エアウェイ装置の前記後側すなわち背面部分は、Aスケールのショア硬度が60未満の物質から形成される、請求項16に記載のエアウェイ装置。
- 前記エアウェイ装置の前記後側すなわち背面部分は、Aスケールのショア硬度が25から45の間の物質から形成される、請求項17に記載のエアウェイ装置。
- 前記エアウェイ装置は、更に、前記喉頭カフの縁から、前記エアウェイ装置の近位端部まで延びている胃管通路(70)を組み込む、請求項1から請求項18のいずれか1項に記載のエアウェイ装置。
- 前記胃管通路は、前記エアウェイ装置の本体の内部に収容される、請求項19に記載のエアウェイ装置。
- 前記胃管通路は、前記エアウェイチューブと平行して延びている、請求項19又は請求項20に記載のエアウェイ装置。
- 前記胃管通路は、前記エアウェイ装置の近位端部の開口から、前記喉頭カフの遠位先端の開口まで通っている、請求項19から請求項21のいずれか1項に記載のエアウェイ装置。
- 前記喉頭カフは、空気で予め膨らませられるか、若しくは適当な流体を予め充填させている、請求項1から請求項22のいずれか1項に記載のエアウェイ装置。
- 前記喉頭カフの前記表面は、患者の喉頭注入口に解剖学的に適合するように構成されており、患者の喉頭注入口の梨状陥凹や披裂喉頭蓋ひだで密封されるよう設計された突起を組み込む、請求項1から請求項23のいずれか1項に記載のエアウェイ装置。
- 前記喉頭カフの前記表面は、前記患者の喉頭注入口に解剖学的に適合するように構成されており、蓋谷、喉頭蓋、披裂喉頭蓋ひだ、梨状陥凹そして甲状及び輪状軟骨の前面の周りで密封されるように設計された突起を組み込む、請求項24に記載のエアウェイ装置。
- 前記喉頭カフの遠位先端は、凹型形状である、請求項1から請求項25のいずれか1項に記載のエアウェイ装置。
- 前記喉頭カフは、更に、喉頭蓋が喉頭注入口の方へ折り畳まれるのを防ぐような大きさと形状である、前記喉頭カフの近位端部に位置する縁(130)を有している、請求項1から請求項26のいずれか1項に記載のエアウェイ装置。
- 前記縁は、前記喉頭カフから延びる葉状構造をしている、請求項27に記載のエアウェイ装置。
- 前記喉頭カフは、更に、一つ若しくはそれ以上の羽のようなフランジ(140,141,240)を有する、請求項1から請求項28のいずれか1項に記載のエアウェイ装置。
- エアウェイ装置の製造方法であって、(a)前記エアウェイ装置の第一の部分と第二の部分の鋳型を形成し、(b)コネクター周囲に前記第一の部分を前記第二の部分へ結合する、という工程を含む、請求項1から請求項29のいずれか1項に記載のエアウェイ装置を製造するための製造方法。
- 前記第一の部分は、喉頭カフの表面を組み込むエアウェイ装置の前側、腹側部分であり、前記第二の部分は、後側、背面部分である、請求項30に記載のエアウェイ装置の製造方法。
- 請求項1から請求項31のいずれか1項に記載のエアウェイ装置を製造するための製造方法であって、当該エアウェイ装置を、一体成形のプラスチック成形物として形成する、エアウェイ装置の製造方法。
- 前記エアウェイ装置を、材料を別々に注入して構成材の関連部分を形成する、2ショット射出成形により形成する、請求項32に記載のエアウェイ装置の製造方法。
- 前記2ショット射出成形におけるそれぞれの注入に際して、異なるショア硬度の材料を使用する、請求項33に記載のエアウェイ装置の製造方法。
- 単一射出方法を使用して、同じ硬度の材料により全体を形成し、腹側又は背側の区域の全ての若しくは一部を、異なったショア硬度の材料で被覆することで、前記エアウェイ装置を形成する、請求項33に記載のエアウェイ装置の製造方法。
