Abstract

Obturateur diaphysaire destiné à permettre le scellement d'une prothèse fémorale (2) dans le canal médullaire d'un fémur par injection sous pression d'un liant au travers de la prothèse (2) . Il est de dimension transversale permettant son introduction au plus juste dans le canal médullaire du fémur, et est solidarisable à l'extrémité distale (20) de la prothèse fémorale (2) . Il comporte d'une part un rebord périphérique proximal (16) débordant l'extrémité distale (20) et constituant le fond de la cavité à remplir par le liant (4) , et d'autre part au moins un trou (15) communiquant avec le canal d'injection (21) du liant pratiqué dans la prothèse (2) , et débouchant au niveau du rebord périphérique proximal (16) .

Description

La présente invention a pour objet un obturateur diaphysaire destiné à permettre le scellement d'une prothèse fémorale dans le canal médullaire d'un fémur par injection sous pression d'un liant au travers de ladite prothèse.
On connaît par le document FR-A-2 676 100 un procédé de scellement d'une prothèse fémorale, laquelle comporte à cet effet un canal axial destiné à l'injection sous pression d'un liant de manière que celui-ci, en débouchant à l'extrémité distale de la prothèse, remonte extérieurement le long de celle-ci en remplissant complètement le canal médullaire et en évacuant les fragments osseux, l'air et le sang.
Une telle prothèse nécessite la mise en oeuvre d'éléments complémentaires tels que d'une part au moins un élément de centrage de la tige fémorale, et d'autre part un obturateur diaphysaire constituant la limite inférieure de remplissage du canal médullaire.
L'inconvénient de la prothèse proposée dans le document précité réside dans le fait que l'obturateur diaphysaire est placé à une certaine distance de l'extrémité distale de la prothèse, en sorte qu'avant de remonter extérieurement le long de cette dernère, le liant doit au préalable combler l'espace séparant ladite extrémité distale dudit obturateur diaphysa7re avec comme principale conséquence l'éventuel emprisonnement d'air et de sang, ce qui est particulièrement préJuduciable à la tenue dans le temps du scellement.
On connaît également par le document DE-A-39 37 786 des obturateurs dnaphysaires solidarisés à l'extrémité distale de la prothèse. Ce document propose notamment un obturateur diaphysaire se présentant sous la forme d'une feuille en matière plastique fermement rapportée à l'extrémité distale de la prothèse à l'aide d'un disque et d'une vis, enveloppant ladite extrémité, et qui sous la pression du liant se déploie, à la manière d'un parapluie, et épouse la paroi du canal médullaire, afin de constituer la limite inférieure de remplissage.
Cet obturateur diaphysaire présente toutefois des inconvénients, d'une part la nécessité d'employer au moins un élément de centrage du fait de la relative souplesse de la feuille plastique qui ne peut permettre à elle seule le blocage en position centrale de l'extrémité distale, et d'autre part, du fait du positionnement des trous de sortie du liant à une certaine distance de l'extrémité distale pour permettre le déploiement de la feuille plastique, la possibilité d'emprisonnement d'air et de sang en dessous desdits trous de sortie, entre la base de la feuille plastique et l'extrémité distale de la prothèse.
On connaît également par le document EP-A-0 434 604 un obturateur diaphysaire solidarisé à l'extrémité distale de la prothèse, qui fait également office d'élément centrage, mais qui présente le même inconvénient que celui du document précité, en ce sens que les trous de sortie du liant sont situés à une certaine distance de l'extrémité distale, en sorte que cela peut également favoriser l'emprisonnement d'air et de sang.
La présente invention a pour but de remédier à ces différents inconvénients en proposant un obturateur diaphysaire permettant de réaliser d'une part le centrage de la tige fémorale et d'autre part un remplissage complet du canal médullaire sans risques d'emprisonnement d'air et/ou de sang.
L'obturateur diaphysaire objet de la présente invention se caractérise essentiellement en ce que d'une part il est de dimension transversale permettant son introduction au plus juste dans le canal médullaire du fémur, d'autre part il est solidarisable à l'extrémité distale de la prothèse fémorale, et d'autre part encore, il comporte un rebord périphérique proximal débordant ladite extrémité distale et constituant le fond de la cavité à remplir par le liant; et en ce qu'il comporte au moins un trou communiquant avec le canal d'injection du liant pratiqué dans ladite prothèse, et débouchant au niveau dudit rebord périphérique proximal.
Selon une caractéristique additionnelle de l'obturateur diaphysaire selon l'invention, il comporte deux parties, une partie extrême d'introduction dans le canal médullaire, de forme sensiblement hémisphérique, c'est-àdire qu'elle comprend une face distale arrondie et une face proximale sensiblement plane constituant le rebord périphérique, et, faisant saillie de la région médiane de ladite face proximale, une partie tubulaire d'accouplement à l'extrémité distale de la prothèse fémorale qui comporte à cet effet un tenon dans lequel se prolonge le canal d'injection du liant.
Selon une autre caractéristique additionnelle de l'obturateur diaphysaire selon l'invention, la face proximale de la partie extrême est légèrement concave.
Selon une autre caractéristique additionnelle de l'obturateur diaphysaire selon l'invention, il est solidarisable par collage, vissage ou encliquetage à l'extrémité distale de la prothèse fémorale.
Selon une autre caractéristique additionnelle de l'obturateur diaphysaire selon l'invention, il est réalisé en une matière plastique du type polyéthylène ou en tout autre matériau biocompatible.
Selon un premier mode de réalisation de l'obturateur diaphysaire selon l'invention, les trous de sortie du liant sont pratiqués dans la base de la partie tubulaire d'accouplement, et ils affleurent la face proximale de la partie extrême.
Selon un second mode de réalisation de l'obturateur diaphysaire selon l'invention les trous de sortie du liant sont pratiqués dans la partie extrême et débouchent dans le rebord périphérique proximal.
Les avantages et les caractéristiques de la présente invention ressortiront plus clairement de la description qui suit et qui se rapporte au dessin annexé, lequel en représente un mode de réalisation non limitatif.
Dans le dessin annexé
- la figure 1 représente une vue en perspective d'un obturateur diaphysaire selon l'invention et l'extrémité distale d'une prothèse fémorale.
- la figure 2 représente une vue en coupe longitudinale d'un fémur dans le canal médullaire duquel est scellée une prothèse fémorale munie du méme obturateur diaphysai re.
Si on se réfère à la figure 1 on peut voir un obturateur diaphysaire 1 selon l'invention et l'extrémité distale 20 d'une prothèse fémorale 2 qui comporte un canal axial 21 permettant l'injection d'un liant.
L'obturateur diaphysaire 1 comporte deux parties, une partie extrême 10 de forme approximativement hémisphérique, c'est-à-dire qu'elle comprend une face distale arrondie 11 et une face proximale plane 12, et une partie 13 d'accouplement à l'extrémité distale 20 de la prothèse fémorale 2.
La partie 13 d'accouplement est de forme tubulaire, elle fait saillie axialement de la région médiane de la face proximale plane 12 de la partie extrême 10, et elle comporte une cavité cylindrique 14 destinée à recevoir un tenon 22 faisant saillie axialement de l'extrémité distale 20 de la prothèse fémorale 2, et dans lequel se prolonge le canal 21, la longueur du tenon 22 étant inférieure à celle de la partie 13 d'accouplement.
La partie 13 d'accouplement comporte à sa base des ouvertures 15 qui affleurent la face proximale plane 12, débouchent sur un rebord périphérique 16 qui subsiste autour de la partie 13 d'accouplement, et qui communiquent avec la cavité 14.
Dans un autre mode de réalisation, non représenté, des trous sont pratiqués dans la partie extrême 10, et débouchent dans la face proximale plane 12.
Si on se réfère maintenant à la figure 2 on peut voir que l'obturateur diaphysaire 1 est solidarisée à l'extrémité distale 20 de la prothèse fémorale 2, laquelle est introduite dans le canal médullaire 30 d'un fémur 3.
Le diamètre de la partie extrême 10 de l'obturateur diaphysaire correspond à celui du canal médullaire 30, en sorte que d'une part le rebord périphérique 16 constitue a limite inférieure de remplissage du canal médullaire 30 au moyen d'un liant 4, et d'autre part la tige fémorale 23 est centrée.
On notera qu'afin d'accroître l'étanchéité de la liaison entre la matière osseuse et l'obturateur diaphysaire 1, celui-ci peut comporter au moins une lèvre périphérique relativement souple, bordant la partie extrême 10.
Le liant 4 injecté sous pression dans le canal 21, non visible sur la figure, sort par les ouvertures 15 et remonte le long de la tige fémorale 23, sans création de poches d'air et/ou de sang, puisqu'il sort au niveau le plus bas.

Claims (8)

REVENDICATIONS
1) Obturateur diaphysaire destiné à permettre le scellement d'une prothèse fémorale (2) dans le canal médullaire (30) d'un fémur (3) par injection sous pression d'un liant (4) au travers de ladite prothèse caractérisé en ce que d'une part il est de dimension transversale permettant son introduction au plus juste dans le canal médullaire (30) du fémur (3), d'autre part il est solidarisable à l'extrémité distale (20) de la prothèse fémorale (2), et d'autre part encore, il comporte un rebord périphérique proximal (16) débordant ladite extrémité distale (20) et constituant le fond de la cavité à remplir par le liant (4); et en ce qu'il comporte au moins un trou (15) communiquant avec le canal d'injection (21) du liant (4) pratiqué dans ladite prothèse (2), et débouchant au niveau dudit rebord périphérique proximal (16).
2) Obturateur diaphysaire selon la revendication 1 caractérisé en ce qu'il comporte deux parties (10, 13), une partie extrême (10) d'introduction dans le canal médullaire (30), de forme sensiblement hémisphérique, c'està-dire qu'elle comprend une face distale (11) arrondie et une face proximale (12) sensiblement plane constituant le rebord périphérique (16), et, faisant saillie de la région médiane de ladite face proximale (12), une partie tubulaire (13) d'accouplement à l'extrémité distale (20) de la prothèse fémorale (2) qui comporte à cet effet un tenon (22) dans lequel se prolonge le canal d'injection (21) du liant (4).
3) Obturateur diaphysaire selon la revendication 1 ou la revendication 2 caractérisé en ce que la face proximale (12) de sa partie extrême (10) est légèrement concave.
4) Obturateur diaphysaire selon l'une quelconque des revendications précédentes caractérisé en ce qu'il est solidarisable par collage, vissage ou encliquetage à l'extrémité distale (20) de a prothèse fémorale (2).
5) Obturateur diaphysaire selon l'une quelconque des revendications précédentes caractérisé en ce qu'il est réalisé en une matière plastique du type polyéthylène ou en tout autre matériau biocompatible.
6) Obturateur diaphysaire selon l'une quelconque des revendications précédentes caractérisé en ce que les trous (15) de sortie du liant (4) sont pratiqués dans la base de la partie tubulaire d'accouplement (13) et ils affleurent la face proximale (12) de la partie extrême (10).
7) Obturateur diaphysaire selon l'une quelconque des revendications de 1 à 5 caractérisé en ce que les trous (15) de sortie du liant (4) sont pratiqués dans la partie extrême (10) et débouchent dans le rebord périphérique proximal (16).
8) Obturateur diaphysaire selon l'une quelconque des revendications précédentes caractérisé en ce que la partie extrême (10) comporte au moins une lèvre périphérique relativement souple, bordant la partie extrême 10.
