Description
穿刺では、腹部から腹水を吸引する。典型的な患者は、肝疾患や門脈圧亢進症に起因する、切迫した腹水症を発症しており、そのせいで不快な呼吸困難や、臍ヘルニアの形成と破裂を引き起こすかもしれない。穿刺は、ほとんど副作用なく、素早くて効果的な救済になるという観察結果を得てきた。利尿剤の使用といった、処置の他の選択肢も利用可能だが、穿刺ほど効果的な救済とはならないかもしれない。さらに、腹水症の多くの患者は腎臓機能障害があり、腹水症を効果的に治療するのに必要な多量の利尿剤を使えない。ピントらの「切迫した腹水症を発症した非浮腫性の患者における大量の穿刺:その血管内容量に対する効果」(『肝臓学』誌、8巻2号、207〜210ページ、1988年)(”Large-Volume Paracentesis in Nonedematous Patients with Tense-Ascites: Its Effect on Intravascular Volume”, Pinto et al., Hepatology, Vol. 8, No. 2, pp. 207-210, 1988)を参照せよ。たとえば5リットルといった比較的大量の体液が、1回の穿刺処置で患者から引き出されるかもしれない。
好ましい実施態様において、装置は真空瓶を含む。できれば瓶は、この装置のための製造設備で真空にされる。瓶と排液管の間の連絡は、装置の使用場所において確立される。排液管(または、より正確には、それに接続された針かカテーテル)は、患者の体内の体液がある場所に、従来の方式または別の方式で挿入され、瓶の真空によって、患者から排液管へ、そして瓶へと体液を引き出す。
Claims (13)
- 内部が相対的に低圧になっていて、口が封印で覆われている容器と、
その容器に接続され、該容器から伸びている排液ラインと、
前記口に近接した位置にあって、前記封印を突き刺して前記低圧を前記容器内部から前記排液ラインに伝えることができ、管状の本体を含むスパイク状の管を有する突き刺し要素と、
上部が前記スパイク状の管の前記管状の本体に接続され、下部が前記容器の口を実質的に気密を保って覆っている容器の蓋と、を備え、
前記容器の蓋は、前記封印を突き刺すために、前記スパイク状の管を下向きに動かせるようにしてあり、
前記容器の蓋が、当該容器の蓋の内壁と前記封印の間の空間を定めていて、
前記容器の蓋には、
前記空間の気圧が実質的に大気圧と同じであるときの第一の形状と、
前記空間の気圧が大気圧より低いときの第二の形状とがあり、
前記封印が突き刺されたとき、前記容器内の低圧を確認できるように、前記第二の形状が視覚的に前記第一の形状と異なる、
ことを特徴とする患者から液体を取り除くための装置。 - 内部が相対的に低圧になっていて、口が封印で覆われている容器と、
その容器に接続され、該容器から伸びている排液ラインと、
前記口に近接した位置にあって、前記封印を突き刺して前記低圧を前記容器内部から前記排液ラインに伝えることができ、管状の本体を含むスパイク状の管を有する突き刺し要素と、
上部が前記スパイク状の管の前記管状の本体に接続され、下部が前記容器の口を実質的に気密を保って覆っている容器の蓋と、を備え、
前記容器の蓋は、前記封印を突き刺すために、前記スパイク状の管を下向きに動かせるようにしてあり、
前記スパイクの下向きの動きに対応するよう、前記下部を変形させるのに十分なだけ、前記容器の蓋が柔軟であることを特徴とする、患者から液体を取り除くための装置。 - 前記封印が壊れやすいことを特徴とする、請求項1に記載の装置。
- 前記封印が、金属箔とマイラー樹脂からなる群から選択された材料で作られていることを特徴とする、請求項1に記載の装置。
- 前記突き刺し要素を前記封印に向けて押すのを容易にするためのフランジを、前記突き刺し要素が含むことを特徴とする、請求項1に記載の装置。
- 前記フランジが円周状に前記突き刺し要素を取り巻いていることを特徴とする、請求項5に記載の装置。
- 前記突き刺し要素が偶発的に前記封印を突き刺してしまうのを防ぐための安全要素を、さらに備えることを特徴とする、請求項5に記載の装置。
- 前記容器内部の低圧を示すための表示器をさらに備えることを特徴とする、請求項2に記載の装置。
- バルブをさらに備え、該バルブは、少なくとも部分的に潰れ可能で、
前記突き刺し要素が前記封印を突き刺すと、内部が前記容器の内部と連絡し、
前記封印を突き刺したことで前記容器の内部の低圧がバルブ内部に伝えられたとき、該バルブは少なくとも部分的に潰れる
ことを特徴とする、請求項8に記載の装置。 - 前記容器の外側に位置する弁をさらに備え、該弁を開けることによって、前記容器の内部の低圧が前記排液ラインに伝えられるようになっている
ことを特徴とする、請求項2に記載の装置。 - 前記弁が、一方の端を前記排液ラインに取り付けてあるストップコック弁であることを特徴とする、請求項10に記載の装置。
- 前記弁が、前記排液ラインにつけたクランプであることを特徴とする、請求項10に記載の装置。
- スエージ加工で閉じられた真空導入口を、前記容器が備えていることを特徴とする、請求項2に記載の装置。
