Description
本願は、同時係属中の米国特許出願第09/421,635号(これは、1999年10月19日に出願され、「Hand−Held Instruments that Access Interior Body Regions」の表題であり、その内容は、本明細書中で参考として援用されている)の一部継続出願である。
(発明の分野)
本発明は、一般に、携帯型用具および器具に関し、また、身体の内部領域にアクセスするために組織を通ってこれらの器具を展開する手順に関する。
多くの異なる型および様式の携帯型外科用器具があり、これらは、医師が、内部身体領域へのアクセスを獲得するのに使用する。これらの器具は、押圧力、捻り力、および両方の組合せを加えることにより、組織を貫入することに向けられている。
本発明は、ハンドル設計を備えた外科用器具を提供し、これにより、カニューレおよび外套針の両方を内部身体領域に最初に配置でき、また、このカニューレを所定位置に残しつつ、この外套針を後に引き出すことができる。本発明により、外科的処置中に数個の器具を使用する必要かなくなり、内部アクセスプロトコルが簡単になる。同時に、この外科用器具のハンドルにより、高い機械的な長所と共に、医師が選定器具に対して加えた捻れ負荷および縦方向負荷の両方を確実に伝達できるようになる。

(6)前記複合ハンドルが、使用中、前記複合器具に、縦方向力および回転力の両方を伝達するように適合されている、項目(1)に記載の用具。
(7)前記複合ハンドルが、使用中、打撃力を受容するように適合されている、項目(1)に記載の用具。
(8)前記複合ハンドルが、打撃力を加えた場合、変形に抵抗できる材料から構成されている、項目(1)に記載の用具。
(9)前記第二機能器具が、カニューレであり、そして前記第一機能器具が、該カニューレを通過できるサイズにした外套針である、項目(1)に記載の用具。
(10)前記外套針が、前記カニューレよりも長い、項目(9)に記載の用具。
(11)項目(1)に記載の前記用具を利用する、骨にアクセスする方法。
本明細書は、組織を貫入する新しい器具を記述する。本明細書はまた、組織を貫入する新しい器具と膨張可能体を併用して骨を治療するシステムおよび方法を記述する。
図1は、組織を貫入する複合器具10を示す。複合器具10は、第一機能器具20および第二機能器具40、および複合ハンドル12を含み、この複合ハンドルは、第一ハンドル22および第二ハンドル42を含む。複合ハンドル12は、医師が複合器具10を操縦するのを助けるが、医師はまた、望ましくは、使用中、第一機能器具20を別個に操縦するために第一ハンドル22を使用するか、または第二機能器具40を別個に操縦するために第二ハンドル42を使用する。
図1〜4を参照すると、第一機能器具20は、組織を貫入する外套針器具として機能する。外套針30は、近位末端32および遠位末端34を有する。遠位末端34は、貫入面35を呈するように、テーパが付けられている。使用中、貫入面35は、医師が第一ハンドル22または複合ハンドル12で加える押圧力および/または捻れ力に応答して、軟組織および/または骨を貫入するように向けられている。
第二器具40は、カニューレ器具またはガイド鞘として機能し、カニューレ50を含む。第二器具40のカニューレ50は、望ましくは、第一器具20の外套針30よりも直径が多少大きいが、外套針30ほど長くない。図1および2で最もよく示されているように、第二器具40は、内部管腔44を含み、これは、その遠位末端54から、この器具を通って、その近位末端52へと伸長する。内部管腔44は、外套針30を受容するサイズにされている。内部管腔44のサイズは、望ましくは、後にさらに詳細に記述するように、第二器具40が第一器具20に対して滑るかおよび/または回転できるサイズであり、また逆も同様である。
図11で示すように、任意の第三機能器具70は、ドリルビットとして機能する。ドリルビット器具70は、遠位末端72および近位末端74を有し、典型的には、外套針30よりも僅かに長く、一般に、外套針30と同じ物理的寸法を有する。外套針30と同様に、ドリルビット器具70は、使用中、第二器具40の内部管腔44内で、滑り運動および回転運動に合うように向けられている。
第一ハンドル22および第二ハンドル42は、望ましくは、連動して複合ハンドル12(これは、第一ハンドル22と第二ハンドル42との間の相対的な回転に抵抗する)を形成するように手に快適に適合するように設計され、また、望ましくは、これらの器具が再使用および/または再滅菌されたかどうかを示すように設計されている。
図1〜4で示すように、複合ハンドル12は、図3で示すように、通常の人の手で快適かつ確実に握れるような形状にされている。好ましくは、複合ハンドル12の輪郭は、快適な握りを与えて外科手術用ゴム手袋の摩滅をできるだけ少なくするように、丸くされている。
複合ハンドル12に打撃力、押圧力および/または捻れ力を加えたときに正しく相互作用するように、第一ハンドル22は、望ましくは、第二ハンドル42に対して回転しない。今ここで、図5、6Aおよび6Bを参照して、相対的な回転を避けるために、第一ハンドル22は、好ましくは、第二ハンドル42の整列リッジ46を受容する整列リッジレシーバ26を含む。第一ハンドル22と第二ハンドル42との間の整列機構の相互作用は、リッジとして記述され描かれているものの、アーチ形状以外の多くの形状(例えば、ブロック形状または星形形状)を含むことができる。
(1.構造上の完全性)
第一ハンドル22および第二ハンドル42に選択される材料は、望ましくは、手の操作と手の打撃から予想される力とに耐える十分な構造上の完全性を与える。第一ハンドル22および第二ハンドル42は、著しく変形することなく、打撃力、押圧力および捻れ力に耐える十分な強度を有する成形または鋳造剛性材料から作製される。
たった1回の使用を奨励し再使用および/または再滅菌を阻止するために、新しい手動用具と再使用および/または再滅菌した手動用具との間で区別することが好ましい。
以下は、骨を治療することに関連して、図9〜15で示すように、カテーテル部品130、診断および治療要素132、注射器136およびタンピング器具142と併用して、複合器具10、器具20、40および70の使用を記述している。これは、これらの品目がこの目的に有利に使用できるからである。依然として、複合器具10は、骨の治療での使用には限定されず、診断機能または治療機能を果たすために組織と接触する目的の器具にも限定されないことが理解できるはずである。第一ハンドル22および第二ハンドル42と連動した複合ハンドル12の構造は、種々の他の携帯型器具と共同で使用できる。
このシステムの1用途は、椎体を治療することにある。図7Aで示すように、脊柱80は、多数の独特形状の骨を含み、これらは、椎骨82、仙骨84および尾骨86と呼ばれる。脊柱80を構成する椎骨82の数は、動物の種に依存している。ヒトでは、(図7Aで示すように)、24個の椎骨82があり、これらは、7個の頚椎88、12個の胸椎90および5個の腰椎92から構成されている。
典型的な処置では、患者は、手術台に寝て、その間、医師は、患者の背中にある軟組織(図9で指定したS)に、複合器具10を導入する。患者は、医師の好みに依って、手術台に俯せになるか、またはどちらか横向きになるか、斜めになり得る。さらに、この処置は、開放前方処置または内視鏡前方処置によって実行できる。
放射線モニタリングまたはCTモニタリングの下で、医師は、図9で示すように、軟組織Sを通って、下に、標的椎骨82へと、複合器具10を前進させる。医師は、典型的には、標的領域に、局所麻酔(例えば、リドカイン)を投与する。ある場合には、医師は、他の麻酔形態(例えば、全身麻酔)を選び得る。
図12で示すように、医師は、今ここで、カテーテル部品130を取得できる。医師は、ハンドルボア48およびカニューレ50を通って、椎体96の内部容量100へと、診断および治療要素132(これは、カテーテル部品130で運ばれる)を前進できる。
望ましくは、材料138は、椎体のさらなる破損を防止するために、椎体内で十分な支持を与える。椎体が負荷に耐える能力は、それにより、向上する。この材料はまた、椎体の治癒を促進し得る。
図19および20は、組織を貫入する複合器具210の代替実施形態を示し、これは、先に記述した複合器具10の特徴の多くを共有している。図22で示すように、複合器具210は、分解すると、先に記述した複合器具10と同様に、第一機能器具220および第二機能器具240を含む。複合ハンドル212は、組み立てたとき、(図19および20で示すように)、2個の器具220および240を接合する。複合ハンドル212は、(また、図22で示すように)、第一ハンドル222(これは、第一器具220に付属している)および第二ハンドル242(これは、第二器具240に付属している)を含む。複合器具10の複合ハンドル12と同様に、器具210の複合ハンドル212は、医師が複合器具210を操作するのを助けるが、医師はまた、望ましくは、使用中にて、第一器具220を単独で操作するために第一ハンドル222を使用か、または第二器具240を単独で操作するために第二ハンドル242を使用できる。
図22を参照すると、第一器具220は、組織を貫入する外套針器具230として機能する。外套針230は、近位末端232および遠位末端234を有する。遠位末端234は、第一ハンドル222(または複合ハンドル212)で医師が加える押圧力および/または捻れ力に応答して、軟組織および/または骨を貫入するように向けられている。もし望ましいなら、遠位末端234は、実質的に鈍い先端および/またはカニューレ挿入先端で終わることができ、あるいは、当該技術分野で公知であるように、組織を通って切断する鋭い先端で終わることができる。
さらに、図22を主に参照すると、第二器具240は、カニューレ器具またはガイド鞘として機能し、そしてカニューレ250を含む。第二器具240のカニューレ250は、望ましくは、第一器具220の外套針230よりも直径が多少大きいが、外套針230ほど長くない。図21で最もよく示されているように、第二器具240は、内部管腔244を含み、これは、その遠位末端254から、この器具を通って、その近位末端252へと伸長する。内部管腔244は、(図21で示すように)、外套針230を受容するサイズにされている。内部管腔244のサイズは、望ましくは、後に記述するように、2本のハンドル222および242が共にロックされていないなら、第二器具240が第一器具220に対して滑るかおよび/または回転できるサイズであり、逆もまた同様である。
第一ハンドル222および第二ハンドル242は、望ましくは、連動して複合ハンドル212(これは、第一ハンドル222と第二ハンドル242との間の相対的な回転に抵抗する)を形成するように手に快適に適合するように設計されている。
複合ハンドル12と同様に、複合ハンドル212は、図21で一般に示すように、通常人の手で快適かつ確実に握れるような形状にされている。好ましくは、複合ハンドル212の輪郭は、快適な握りを与えて外科手術用ゴム手袋の摩滅をできるだけ少なくするように、丸くされている。図3のハンドル12について示したものと同じ様式で、第一ハンドル222は、望ましくは、2個のフィンガーレシーバ238を備えており、これらは、医師の人差し指と小指とを受容するように向けられている。
複合ハンドル212に打撃力、押圧力および/または捻れ力を加えるときに正しく相互作用するように、第一ハンドル222は、望ましくは、2個のハンドル222および242を共に固定したとき、第二ハンドル242に対して回転しない。相対的な回転を避けるために、第一ハンドル222は、好ましくは、受容チャンネル226を含み、その中に、第二ハンドル242のショルダー246が前進され入れ子になる(図23および24を参照)。
