【発明の詳細な説明】
【０００１】
【産業上の利用分野】本発明は手術器具に関し、特に、
トロカール（trocar）又はカニューレ（cannula）を使
用するのに適当な内視鏡鋏等の器具に関する。
【０００２】
【従来の技術】内視鏡手術の一部門は、胸腔といった内
部腔にアクセスするために被験者の皮膚の切開部を通じ
て挿入されるカニューレの使用に関連する腹腔鏡手術で
ある。そのようなカニューレの例としては、ハッソン
（Hasson）による米国特許番号No.5,002,557にて開示さ
れている。手術は、腹腔鏡器具によりなされ、それは一
般に、鋏ハンドルと、一対のピボット動作する顎をその
終端で形成する細長いシャフトとを含む。ハンドルは、
把握時及び解除時に顎をピボット動作させるためにシャ
フトを通って延びるロッドを往復運動させる鋏即ちプラ
イヤグリップを含む。顎及びシャフトは、手術がなされ
る体腔にカニューレを通じて挿入されるサイズである。
同様な装置は、胸腔鏡及び関節鏡に利用される。

【発明が解決しようとする課題】どの手術器具でもそう
であるが、器具が切開用の鋭利な顎を有している場合、
各手術でできるだけ鋭利なエッジの鋭利さを維持するこ
とが望ましい。更に、手術が終了する毎に容易かつ十分
に洗浄できるように、器具が構成されることが好まし
い。しかし、一体物の不都合がある；即ち、器具ではハ
ンドル、シャフト及び切開具は互いに永久固定されてい
て、器具の洗浄及び研磨は時間を要し、コストがかか
る。
【０００４】従って、洗浄及び研磨の時間及び経費を最
小に抑えるような腹腔鏡器具を提供する試みがなされて
いる。例えば、ある腹腔鏡器具は、各手術が完了すると
器具全体を使い捨て可能とする目的で、シャフトロッド
及び顎を除き、実質的に器具全体をプラスチックで構成
されている腹腔鏡器具が生産されている。これに代わっ
て、例えば、フォーク （Falk）他による米国特許 No.
4,569,131により開示されている器具では、ハンドル
は、シャフト及び顎からは、着脱可能であり、従って使
用完了する毎に一体のシャフト、顎及びロッドは、廃棄
されるか、またはハンドルから取り外して洗浄し、研磨
される。このタイプの構造に伴う不具合は、使い捨ての
部分、それが器具全体であろうと、シャフト及び切開具
のみであろうと、特別な廃棄処理を必要とする歓迎され
ない多量の医用廃棄物をもたらすことである。更に、お
もにプラスチック製の使い捨て器具は、薄くぺらぺらし
ていて取扱いがやっかいである。
【０００５】従って、器具の刃面の反復研磨を不要と
し、洗浄を容易に実行でき、相互汚染の可能性を低減す
る腹腔鏡器具が必要とされている。
【０００６】
【課題を解決するための手段、発明の作用及び効果】本
発明は、カニューレを通じて挿入される細長いシャフト
を有すハンドル部と複数の先端具とを含む内視鏡器具に
関し、各先端具はシャフトの端に取り付けられている。
好適な実施例では、各先端具は、手術中に被験者の組織
に実際に接触するピボット顎を含む。先端具は、コスト
低減しかつ使い捨て可能であるプラスチックを含むのが
好ましい。本発明の効果は、器具全体の使い捨て部分
は、相対的に小さく、従って何回かの手術で使用される
器具のコストを最小限に押さえ、器具の使い捨て部分を
含む医用廃棄物の量を最小限に抑えることが可能であ
る。
【０００７】好適な実施例では、ハンドル部は、ピボッ
ト鋏ハンドルと、シャフトを通じて延びる作動ロッドと
を含む。ロッドは、クレビスの終端を有し、クレビス
は、スタブシャフトの半球形の先端を受け、スタブシャ
フトは、先端具の内部にて往復動作可能に支持されてい
る。スタブシャフトは、リンクによって一対のピボット
顎に連結されていて、スタブシャフトの往復動作は、顎
を鋏の動作で先端具に対して旋回させる。
【０００８】先端具は、グラスファイバー強化プラスチ
ックで構成され、ハンドルシャフトの開口へねじ込まれ
る支持部材を含む。先端具のピボット顎は、種々の仕事
を実行するように形成される； 例えば、顎は、鋏、掴
み具、生検具または切開具の形に構成される。
【０００９】本発明の他の効果は、手術中に最も汚染さ
れ、最も洗浄が困難な部分が器具の一部であることであ
る； 即ち、顎及び顎を作動させるリンク機構は、先端
具とともに廃棄される。好適な実施例では、洗浄ポート
がハンドル部を通じて延び、手術完了の度に適当な洗浄
液でシャフトの洗浄の便宜を図っている。本発明による
腹腔鏡器具を病院の手術室手順に直接利用すると、多数
の一体器具を配置する必要がある従来の手順に比し、相
対的に多数の先端具を有す一方、相対的に小数のハンド
ル部が必要となるのみである。更に、各外科医は、先端
具の一組を提供され、その各々が異なった機能を果た
し、各々が外科医の特定のニーズと好みに応じて先端具
の配列から選択される。
【００１０】本発明の目的は、器具の顎刃の繰り返し研
磨の必要のない内視鏡器具システム；最も洗浄が困難な
リンク部が使い捨て可能で、従って繰り返し研磨を必要
としない器具システム；各手術完了時にシャフト部が容
易に洗浄される器具システム；刃面及びリンク機構が使
い捨て可能で、従って廃棄器具の量が最小となる器具シ
ステム；支持部分が比較的安価なプラスチック材で構成
されている、使い捨て可能な刃部分を有す器具システ
ム；特定の開業医あるいは特定の応用に特別注文に対応
可能な複数の取付先端具を有する器具システム；及び比
較的製造及び使用容易な器具システムを提供することに
ある。ここで、出願人は内視鏡器具システムを、物の発
明としてとらえている。
【００１１】本発明の他の目的及び効果は、以下の説
明、添付図面及び請求の範囲から明白になろう。

【実施例】図１に於いて、本発明による内視鏡器具シス
テム全体は１０で示され、全体が１２で示されるハンド
ル部と全体が１４で示される先端具とを含む。ハンドル
部１２は、ユーザーが掴む鋏部１６を含み、かつ前部及
び後部鋏ハンドル１７、１８を有す。鋏ハンドル１７及
び１８はフィンガーループ２０、２２を含む。
【００１３】鋏ハンドル１７は、円筒ハウジング２４に
固定され、鋏ハンドル１８と連結されかつネジ２８によ
って支持されるクレビス２６を含む。鋏ハンドル１７、
１８は、ネジ２８を軸として互いに相対的に旋回可能で
あり、ハンドル１８の上端３０は、ハウジング２４に対
して往復運動をする。
【００１４】図２に示されるように、ハウジング２４
は、回転カップリング３６の内部スリーブ３４を受ける
内部空洞３２を含む。内部スリーブ３４は、フランジ３
８を含み、そのフランジ３８の周辺部では複数のオリフ
ィス４０が互いに隔てられて設けられている。カップリ
ング３６は、カップリング本体４２を含み、カップリン
グ本体４２は、複数の円筒空洞４４を含み、各円筒空洞
４４は、伸張スプリング４６がはめられ、伸張スプリン
グ４６は、対応するオリフィス４０の一つに対してボー
ル４８を抑圧する。ボール４８は、オリフィスの直径よ
り大きい直径とされているので、オリフィスは、単に、
ボール４８に対して受座を提供するにすぎない。カップ
リング本体３６の内側端５０は、スプリットリング５４
がはめ合わされる環状溝５２を含む。スプリットリング
５４は、フランジ３８に対してカップリング本体４２を
保持する。本体４２は、空洞の細長いシャフト５６（図
１参照）と一体である。好適な実施例のシャフト５６
は、手術用ステンレス鋼で構成されるが、フレキシブル
シャフトを使用することも本発明の範囲に含まれる。
【００１５】本体４２の外面には、刻みが付けられユー
ザが掴み易いようになっている。カップリング３６によ
り、シャフト５６は、ハンドル部１２のハウジング２４
に対して回転可能とされていて、ボール４８とオリフィ
ス４０との噛み合わせは戻り止めを形成している。
【００１６】ハウジング部２４は、着脱可能なナット５
８により蓋がされ、ハウジングの内部３２と連通してい
る側管５７を含む。側管５７は、スリーブ３４の開口６
０を通じて延び、カップリング３６の内部６２及びシャ
フト５６と連通している。
【００１７】ハンドル部１２は、ロッド６４を含み、ロ
ッド６４は、シャフト５６及びハウジング２４のそれぞ
れの内部６２及び３２を通じて延びている。このロッド
は、位置決め目的でスリーブ３４の内部あるいは内壁と
はめあう拡大円筒部６６と、球端７２で終端となるよう
にハウジング２４の終端開口７０を通じて突出している
後部６８とを含む。ハウジング２４は、Ｚ形溝７５に固
定されたプラスチックの終端キャップ７４を含み、Ｚ形
溝７５により、延長部６８の周囲で密封がえられる。延
長部６８は、キャップのオリフィス７６を通過し、延長
部の端７２は、ハンドル１８の上端３０に形成されるク
レビス７８内にはめ込まれる。従って、ハンドル１８の
ハンドル１６に対する旋回運動により、上端３０は、ハ
ウジング２４に対して往復運動をなし、ロッド６４は、
ハンドル部１２に対して往復運動をなす。
【００１８】図３に示されるように、シャフト５６の外
側先端８０は、ネジ切りされた内径面８２を含む。ロッ
ド６４の終端８４は、クレビス８６を含む。先端具１４
は、５０％ガラス充填のポリエーテルイミドといったガ
ラス充填プラスチックにより構成される支持部材８８を
含む。他のプラスチックは、ポリエーテルスルホン及び
ポリエーテルエーテルケトンがある。支持部材８８は、
シャフト５６の終端８０のネジ内径面８２にねじ込まれ
るような形に加工されているネジ内部終端９０を有す。
支持部材８８は、図４が最もよく示している、一対の顎
９２、９４を支持している。
【００１９】支持部材８８は、支持部材に対して及び互
いに旋回動作をする顎９２、９４を支持するリベット１
００を受ける一対のアーム９６、９８（図３参照）を含
む。顎９２、９４の各々は、耳１０２、１０４を有し、
耳１０２、１０４は、リベット１１０、１１２によりリ
ンク１０６、１０８へ旋回可能に取り付けられている。
リンク１０６、１０８は、リベット１１６によってナッ
クル１１４へ旋回可能に取り付けられている。ナックル
１１４は、支持部材８８に形成された通路１２０内に滑
り可能に支持されるスタブシャフト１１８の一部を形成
する。スタブシャフトは、近位端１２２がネジ端９０か
ら後方へ突出するように、クレビス８６内に着座するよ
うな形に形成されている半球形１２４の終端を有すよう
に、寸法が決定される。
【００２０】図５に示されるように、すでに述べた方法
でハンドル１７、１８を旋回することでシャフト５６に
対してもたらされたロッド６４の変位により、ロッドの
終端８４がスタブシャフト１１８を支持部材８８に対し
て外側に移動させる。この外側方向の変位により、リン
ク１０６、１０８は、顎９２、９４を外側に、互いに離
れるように旋回させる。逆に、ハンドル１７、１８（図
１参照）をともに掴むことにより引き起こされたロッド
６４の内向きの変位は、支持部材８８に対してスタブシ
ャフト１１８を内向きに変位させ、その結果、リンク１
０６、１０８は、顎９２、９４を引き合わせる。従っ
て、ハンドル１７に対するハンドル１８の旋回動作は、
顎９２、９４の互いの旋回動作を引き起こす。
【００２１】シャフト５６から先端具１４を取り外すに
は、まず支持部材８８を、シャフト５６のネジ端８０か
らねじって引き出す。ハンドル１７、１８のループ２
０、２２を互いに引き寄せると、ロッド端８４がシャフ
ト端８０から外側に変位し、クレビス８６が露出する。
ロッド端８４が、シャフト端８０からいったん離れる
と、スタブシャフト１１８の先端１２４は、クレビス８
６から取り外し可能となり、先端具１４全体が廃棄でき
る。ハンドル部１２からロッド６４を取り外したり、ハ
ンドル上端３０から延長部６８を切り放すことは不要と
なる。
【００２２】新しい先端具１４の取付は、上述の手順の
反対に行うことで実行できる。具体的には、新しい先端
具１４が、シャフト端８０に隣接して配置され、先端１
２４は、クレビス８６に挿入される。ハンドル１７、１
８は、若干広がり、支持部材８８は、シャフト端８０に
ねじ込まれる。好適な実施例では、支持部材８８のネジ
端９０は、ボス１２６を有し、ボス１２６は、ネジ端８
０のネジにより変形し、先端具１４の不注意なネジのは
ずれが起こらないようになっている。
【００２３】器具システム１０のハンドル部１２を洗浄
するには、先端１４をまず上述のようにして取り外す。
次にナット５８が、管５７から取り外され、洗浄液がハ
ウジング２４及びシャフト５６の内部３２、６２を通じ
て流され、いかなる屑をもシャフトから排出する。しか
し、唯一の開口は、シャフト端と支持部材８８との継ぎ
目、及び通路１２０とスタブシャフト１１８との継ぎ目
であるので、先端１４とシャフト端８０との連結は、内
部６２内への汚染物の進入を最小限にするように留意し
なければならない。
【００２４】図６、７、８は、先端具１４Ａ、１４Ｂ、
１４Ｃで示される代替実施例であり、各々が特定の機能
を有し、システム１０の部分を形成する。１４Ａでは、
顎９２Ａ、９４Ａは、後方向き鋸刃１２８を含み、従っ
て先端１４Ａはわに口掴み具を形成している。図７で
は、先端具１４Ｂは、生検具を形成する形に構成されて
いる顎９２Ｂ、９４Ｂを含む。図８では、先端具１４Ｃ
は、切開具を形成する鋸刃１３０を有する顎９２Ｃ、９
４Ｃを含む。
【００２５】以上述べた器具の構成は、本発明の好適な
実施例であって、本発明はこの器具の構成に限定され
ず、この変形も本発明の範囲内とする。

Claims (24)

    (57)【特許請求の範囲】
  1. 【請求項１】内視鏡器具を掴むための手段と、前記掴み
    手段から延びているシャフトを形成する手段と、前記器
    具を作動させるための手段とを含むハンドル部と、複数
    の器具先端具とを含み、前記器具先端具の各々は、前記
    シャフトの端に取り外し可能に取り付けられ、かつ前記
    作動手段とかみ合わすための手段を含み、前記先端具の
    取り付けられた一つは、前記作動手段により動作可能で
    あるとともに、前記掴み手段は前記作動手段を駆動し、
    前記作動手段は前記掴み手段から前記先端具の前記取り
    付けられた一つに向かって前記シャフトを通じて延びる
    と同時に、前記掴み手段から前記シャフト端に取り付け
    られた前記先端具の一つに向かって延びているロッドを
    含み、前記先端具はネジのかみ合わせによって前記シャ
    フトに連通可能であり、前記先端具の各々は前記ロッド
    とかみ合わせるための手段を含むとともに、拡大終端部
    を有するリンク手段を含み、前記作動手段は前記拡大終
    端部と取り外し可能にかみ合わせるための手段を含み、
    かつ前記かみ合わせ手段は前記先端具の取り付けられた
    一つの前記拡大終端部を受けるためのクレビス手段を含
    むことを特徴とする内視鏡器具システム。
  2. 【請求項２】前記先端具は、鋏を含む請求項１記載の内
    視鏡器具システム。
  3. 【請求項３】前記シャフトは、前記掴み手段に回転可能
    に取り付けられている請求項１記載の内視鏡器具システ
    ム。
  4. 【請求項４】前記シャフトは、前記掴み手段から取り外
    し可能である請求項１記載の内視鏡器具システム。
  5. 【請求項５】前記先端具の各々は、前記シャフトの端に
    取り付け可能で、前記リンク手段を受ける支持部材と、
    前記支持部材及び前記リンク手段へ旋回可能に取り付け
    られている第一の要素と、前記支持部材に取り付けられ
    ている第二の要素とを含み、前記作動手段の動作に応答
    した前記リンク手段の動作は、前記支持部材に対しまた
    前記第二の要素に対し前記第一の要素を旋回させる請求
    項１記載の内視鏡器具システム。
  6. 【請求項６】前記リンク手段は、前記拡大終端部を有
    し、前記支持部材により滑り可能に受けられるスタブロ
    ッドと、前記スタブロッドと前記第一要素を連結する第
    一リンク要素とを含み、前記スタブロッドの滑り動作
    は、前記第一要素を旋回させるように前記リンク要素を
    通じて伝達される請求項５記載の内視鏡器具システム。
  7. 【請求項７】前記第二の要素は、前記支持部材に旋回可
    能に取り付けられ、前記リンク手段は、前記スタブロッ
    ドと前記第二要素とを連結する第二リンク要素を含み、
    前記スタブロッドの前記滑り動作は、前記第一及び第二

    ステム。
  11. 【請求項１１】前記第一及び第二要素は、それぞれ生検
    具の第一及び第二顎を含む請求項７記載の内視鏡器具シ
    ステム。
  12. 【請求項１２】前記掴み手段は、前記シャフトを洗浄す
    るためのポート手段を含む請求項１記載の内視鏡器具シ
    ステム。
  13. 【請求項１３】前記ポート手段は、前記シャフトと連通
    し、前記掴み手段へねじ込まれているスタブチューブを
    含む請求項１２記載の内視鏡器具システム。
  14. 【請求項１４】内視鏡器具を掴むための手段を含むハン
    ドル部と、前記掴み手段から延び細長いシャフトを形成
    し、前記掴み手段と反対側に解放端を有する手段と、前
    記器具を作動させる手段とを含むタイプの前記内視鏡器
    具に使用するための、使い捨て可能な先端具であって、
    一対の顎を形成する手段と、前記顎が互いに動くように
    前記顎を支持するための手段とを含み、前記支持手段
    は、前記作動手段が前記顎の手段を動かすように、前記
    シャフトに取り付けられると同時に前記シャフト端に固
    定されるように適応した内側端を含み、この内側端は前
    記シャフトにねじ込まれるような形となっているネジ部
    を有し、かつ前記顎手段は前記支持手段に滑り可能に取
    り付けられ、前記作動手段とかみ合うように適応してい
    るスタブシャフトを含み、前記シャフト端を損傷せずに
    取り外し可能としたことを特徴とする先端具。
  15. 【請求項１５】前記スタブシャフトは、前記内側端から
    突出し、前記作動手段と結合されるような形になってい
    る端部を有する請求項１４記載の先端具。
  16. 【請求項１６】前記顎手段は、第一及び第二顎を含み、
    前記顎は、前記支持手段に旋回可能に取り付けられてい
    る請求項１５記載の先端具。
  17. 【請求項１７】前記顎手段は、前記第一の顎及び第二の
    顎のそれぞれを前記スタブシャフトに連結する第一及び
    第二リンクを含み、前記作動手段に応答して前記支持手
    段に対して前記スタブシャフトの往復運動が前記顎を互
    いに旋回させる請求項１６記載の先端具。
  18. 【請求項１８】前記支持手段は、プラスチック材料で構
    成される請求項１４記載の先端具。
  19. 【請求項１９】前記プラスチック材料は、ガラス充填の
    ポリエーテルイミド、ガラス充填のポリエーテルケトン
    及びガラス充填のポリエーテルスルホンのいずれかであ
    る請求項１８記載の先端具。
  20. 【請求項２０】前記顎は、鋏を形成する請求項１６記載
    の先端具。
  21. 【請求項２１】前記顎は、掴み具を形成する請求項１６
    記載の先端具。
  22. 【請求項２２】前記顎は、生検具を形成する請求項１６
    記載の先端具。
  23. 【請求項２３】前記顎は、切開具を形成する請求項１６
    記載の先端具。
  24. 【請求項２４】内視鏡器具を掴むための手段を含むハン
    ドル部と、前記掴み手段から延び細長いシャフトを形成
    し、前記掴み手段と反対側に解放端を有する手段と、前
    記器具を作動させるロッド手段とを含むタイプの前記内
    視鏡器具において、前記ロッド手段は、前記掴み手段に
    より滑り動作可能に前記シャフト内に取り付けられ、前
    記シャフト端に近接してクレビス端を有していて、前記
    シャフト端へねじ込めるような形のネジ部内側端を有す
    る支持部材と、往復運動のため前記支持部材内にて支持
    され、前記クレビス端と取り外し可能にかみ合わされる
    ような形の前記内側端から突出している拡大端を有する
    スタブシャフトと、前記支持部材内で前記スタブシャフ
    トに旋回可能に取り付けられている第一及び第二リンク
    と、鋏動作を実行するように前記支持部材に旋回可能に
    取り付けられた第一及び第二顎とを含み、前記第一及び
    第二顎は、それぞれ前記第一及び第二リンクへ旋回可能
    に取り付けられ、前記ロッドの往復運動は、前記スタブ
    シャフトを往復動作させ、前記顎の前記鋏動作を実行す
    ることを特徴とする使い捨て可能な先端具。
