Abstract

本発明は、β脱離機構による切断を受け得るリンカーを介して高分子に結合されたトポイソメラーゼI阻害剤のコンジュゲートを利用する。適切なトポイソメラーゼI阻害剤は、一般的に、カンプトテシンおよび類似体、例えば、イリノテカン(CPT-11の別名でも知られる)およびその活性代謝産物であるSN-38、ならびにトポテカン、9-アミノ-カンプトテシン、および水溶性類似体、例えばGI 147211、GI 149893などである。

本発明において有用なコンジュゲートは、一般に、1種以上の薬学的に許容される賦形

ある実施形態では、前記コンジュゲートは、対象者に投与したとき、トポイソメラーゼI阻害剤への持続的な低用量曝露をもたらし、この場合、遊離の阻害剤の濃度は、週に1回もしくは2回の投与の間、または例えば2週間ごとに1回の投与のプロトコルで、15~5nMを維持することができる。いずれの場合でも、該コンジュゲートは活性薬物への一貫した低用量曝露をもたらす。
共投与されるDDR阻害剤および/または細胞周期チェックポイント阻害剤の正体(identity)に関しては、例えば、上記の背景技術の説明において記載したように、多くが当技術分野で知られている。
細胞周期チェックポイントには、G1-S、S、およびG2/Mが含まれる。これらはいずれも
、トポイソメラーゼI阻害剤コンジュゲートとの組み合わせで、および/または良好なチェックポイント移行に必要とされる成分を標的化する追加の薬剤との組み合わせで、標的化され得る。これは、細胞周期チェックポイントの制御における内因性の欠陥を背景にすることもできる。

投与および腫瘍体積の測定:
PLX038A(1.02mM SN38;0.26mM PLX038Aコンジュゲート)の溶液をpH5の等張酢酸中に
調製し、使用前に滅菌ろ過(0.2μm)した。BMN673(52μM)の溶液を10%ジメチルアセ
トアミド/5%Solutol HS15/85%1X PBS中に調製し、使用前に滅菌ろ過(0.2μm)した。
PLX038A(1.02mM SN38;0.26mM PLX038Aコンジュゲート)の溶液をpH5の等張酢酸中に
調製し、使用前に滅菌ろ過(0.2μm)した。BMN673(52μM)の溶液を10%ジメチルアセ
トアミド/5%Solutol HS15/85%1X PBS中に調製し、使用前に滅菌ろ過(0.2μm)した。
グループ(N=4~5/グループ)の平均腫瘍サイズが100~200mm3に達したとき、グルー
プに投薬した。マウスには、ビヒクル、1回量のPLX038A(14.7mL/kg腹腔内、15μmol/kg
)、1日量のBMN673(7.72mL/kg経口、0.4μmol/kg)、または同じ用量のPLX038AとBMN673の組み合わせを投与した。該組み合わせを受け取ったグループでは、毎日のBMN673の投与をPLX038Aの投与と同日に(図3A)、またはPLX038Aを投与してから4日遅れて(図3B)開始した。腫瘍体積(キャリパー測定:0.5×(長さ×幅2))および体重を週2回測定した。ビヒクル対照腫瘍のサイズが約3000mm3に達したとき、1回量のPLX038A(15μmol/kg)と毎日のBMN673(0.4μmol/kg)の組み合わせで、投与間の遅れなしに(図3A)、マウスを治療した。

図3Aおよび3Bに示すように、MX-1腫瘍を担持するマウスに15μmol/kgのPLX038Aを0.4μmol/kgのタラゾパリブの1日の用量と組み合わせて投与すると、これらの薬剤のいずれか単独と比較して相乗効果が得られる。このことは、TLZによる毎日の投与をPLX038Aと同時に開始した場合でも、4日遅れて開始した場合でも同じであった。単一の組み合わせを対照に投与すると、腫瘍体積が即座に減少した(図3A)。
図3Cに示すように、イベントフリー生存率は、PLX038AおよびTLZの個別投与に対して前記組み合わせで相乗的に向上した。
実施例2
PLX038Aと腫瘍細胞欠陥の相乗作用
MX-1細胞はBRCA 1欠損であり、CAPAN-1細胞はBRCA 2欠損(-/-)または欠損なし(+/+)として供給される。これらの細胞株の腫瘍を担持するマウスを用いて、実施例1の一般
的なプロトコルを行った。MX-1腫瘍があるマウスの場合、投与量は、137μg/kgのイリノ
テカンまたは4、40もしくは120μg/kgのPLX038Aの単回の腹腔内注入であった。CAPAN-1異種移植片を持つマウスの場合、投与量は、137μg/kgのイリノテカンまたは15、40もしくは120μg/kgのPLX038Aの単回の腹腔内注入であった。図4A~4Cは、週2回測定された腫瘍体積に対するこれらの投与量の結果を示す。

に止まった。また、対照腫瘍が2000mm3に達したときに120μg/kgのPLX038Aを1回投与した場合の劇的な結果も示される。

図4Bと4Cの比較は、イリノテカンまたはPLX038Aによる治療の有効性に対するBRCA 2欠損症の影響を示す-PLX038Aの最高用量のみが、BRCA 2欠損細胞と非欠損細胞の両方に同等に効果的であった。他の全ての投与量レベルの有効性は、BRCA 2欠損細胞において増強された。
Claims (17)
- がんの治療が必要な対象者におけるがんの治療方法であって、該対象者は、DNA損傷応
答(DDR)に1つまたは複数の欠陥があると診断されており、該対象者に、有効量の、β脱離機構による切断を受け得るリンカーを介して高分子に結合されたトポイソメラーゼI阻
害剤のコンジュゲートを投与することを含む方法。 - 前記対象者に、前記コンジュゲートと組み合わせて、有効量のDDRの阻害剤を投与する
