本出願は、2016年11月21日に提出された米国仮出願第62/424,979号明細書、及び2017年6月8日に提出された米国仮出願第62/517,065号明細書に対する優先権を主張する。なお、これら文献の各々の全内容は、参照により本明細書に組み込むものとする。
エキセンディン(9-39)は、膵臓細胞上に存在するGLP-1受容体を選択的に遮断し、これにより、インスリン分泌のGLP-1媒介性増大を阻止するグルカゴン様ペプチド-1(GLP-1)アンタゴニストである。通常生理食塩水(0.9%塩化ナトリウム、本明細書では、「0.9%通常生理食塩水」とも呼ばれる)中で製剤化されたエキセンディン(9-39)は、高インスリン性低血糖症の治療のための動物及びヒト臨床試験で投与されている。しかし、通常生理食塩水中で再構成されたエキセンディン(9-39)の皮下注射を含む最初の臨床試験では、注入液の濃度漸増と共に逆用量線形性が実証され、これは、濃度依存性ペプチド凝集及び沈殿を示唆し、濃度が高いほど薬物動態曝露及び臨床活性が低くなる。参照により本明細書に組み込まれる国際公開第2016/191395号パンフレットの実施例3を参照されたい。さらに、本明細書の実施例1及び2に記載するように、通常生理食塩水中のエキセンディン(9-39)は、特定の保存条件、又は特定のエキセンディン(9-39)の濃度といった特定の条件下で、エキセンディン(9-39)の曝露低下を招く凝集を呈示することが明らかにされた。

一部の実施形態では、本明細書に記載する方法に従って治療しようとする対象は、高インスリン性低血糖症(HH)を有する対象である。特定の実施形態では、高インスリン性低血糖症を有する対象は、以前、肥満手術(例えば、Roux-en-Y胃バイパス術)及び/又は関連代謝処置を受けたことがある。特定の実施形態では、対象は、以前、肥満手術(例えば、Roux-en-Y胃バイパス術)及び/又は関連代謝処置を受けたことがあり、高インスリン性低血糖症を発症するリスクを有する。一部の実施形態では、高インスリン性低血糖症を有する対象は、以前、胃切除術又は食道切除術などの上部消化管処置を受けたことがある。
一部の実施形態では、本明細書に記載のエキセンディン(9-39)を含む緩衝液体製剤は、皮下投与(例えば、皮下注射)により投与される。注射の部位は、限定されないが、大腿、腹部、上腕領域、又は上方臀部領域が挙げられる。
一部の実施形態では、緩衝液体エキセンディン(9-39)製剤は、例えば、複数の単回使用プレフィルシリンジ(例えば、10、20、30、40、50、又は60本のプレフィルシリンジ)を含むキット内に、単回使用プレフィルシリンジとして製剤化される。一部の実施形態では、単回使用プレフィルシリンジは、約5~75mgのエキセンディン(9-39)、等張化剤、及び5.0~6.0の範囲のpHを有する緩衝液を含むエキセンディン(9-39)液体医薬製剤を含む。
一部の実施形態では、約5~約6の範囲のpHを有する生理学的に許容される緩衝液中にエキセンディン(9-39)又はその薬学的に許容される塩及び等張化剤を含む治療有効量の液体医薬製剤を含む組成物が、高インスリン性低血糖症の治療又は予防のために、それが必要な対象に投与される。
一部の実施形態では、約5~約6の範囲のpHを有する生理学的に許容される緩衝液中のエキセンディン(9-39)又はその薬学的に許容される塩及び等張化剤を含む緩衝液体製剤は、0.9%通常生理食塩水中で製剤化された同じ用量のエキセンディン(9-39)又はその薬学的に許容される塩を含む組成物と比較して、エキセンディン(9-39)の吸収プロフィールの向上をもたらす。一部の実施形態では、緩衝液体製剤は、0.9%通常生理食塩水中で製剤化された同じ用量のエキセンディン(9-39)を投与された対象の同じ時間にわたる又は同じ時間のエキセンディン(9-39)の血漿濃度と比較して、対象への投与後約1~6時間にわたって(例えば、約1時間後、約2時間後、約3時間後、又は約4時間後、1~4時間にわたって)エキセンディン(9-39)のより高い血漿濃度をもたらす。
以下の実施例は、例示のために記載するのであり、請求される発明を限定するためではない。
この実施例は、エキセンディン(9-39)の緩衝製剤の特性と比較して、0.9%の通常生理食塩水中で再構成した凍結乾燥エキセンディン(9-39)酢酸塩の物理的及び化学的特性を記載する。
0.9%通常生理食塩水中で再構成することにより、様々な濃度(5、10.24、15.3、25.22、及び44.99mg/ml)の凍結乾燥エキセンディン(9-39)酢酸塩を調製して、5℃で保存した。再構成したエキセンディン(9-39)製剤各々のpHを第0日に記録し、最大14日間にわたってモニターした。再構成したエキセンディン(9-39)製剤の全ての濃度によって、初期pH4.4~4.5が得られた。再構成したエキセンディン(9-39)製剤各々の視覚検査も第0日に記録し、5℃に維持しながら、最大14日間にわたってモニターした。結果を以下の表1に示す。14日間にわたって5℃に維持したサンプルのいずれにおいても、再構成エキセンディン(9-39)製剤の凝集、ゲル化又は沈殿は観察されなかった。
様々なイオン強度(10mM~30mM)及び異なるpH(pH3.5~6.0)の2つのイオン緩衝液(酢酸緩衝液又はクエン酸緩衝液)の一方で、エキセンディン(9-39)酢酸塩を製剤化した。各サンプルを5℃又は50℃で最大5日間保存した。製剤を凝集について視覚的に検査し、純度及び効力を強カチオン交換(SCX)-HPLCにより試験した。
この実施例は、約5~6の範囲のpHを有する緩衝液中で製剤化されたエキセンディン(9-39)が、通常生理食塩水中で再構成したエキセンディン(9-39)と比較して、向上した薬物動態特性を示すことを示す。この実施例のために、7.2mgの用量(ヒトの場合、30mgの用量と同等)のエキセンディン(9-39)酢酸塩を10mM酢酸ナトリウム及び45mg/mlのマンニトール中で製剤化した。製剤は、5.5のpHを有した。3つの濃度:15mg/ml、30mg/ml、又は45mg/mlのうちの1つで皮下注射により製剤をイヌ(群当たり3匹の雄イヌ)に投与した。エキセンディン(9-39)の血漿濃度は、投与後、0~24時間の期間にわたってモニターした(図5B)。
要約
肥満手術後の低血糖症(PBH)は、徴候的な食後低血糖症の一般的エピソードによって発現される肥満手術の稀だが、重篤な合併症であり、そのための承認された薬物療法はない。インスリン分泌の調節異常を伴うインクレチンホルモン、グルカゴン様ペプチド-1(GLP-1)の食事誘導型分泌の増大の主要な役割が立証され、これによりGLP-1受容体拮抗は魅力的な標的治療手法となった。GLP-1受容体アンタゴニストのエキセンディン(9-39)の単回静脈内(IV)注入又は皮下(SC)注射の使用を評価する試験は、単回用量のエキセンディン(9-39)が、経口グルコース負荷試験(OGTT)中に、PBH患者における食後低血糖症を予防し、β細胞機能を正常化すると共に、神経低血糖症状を軽減することができることを実証した。
PBHは、徴候的な食後低血糖症の一般的エピソードによって発現される肥満手術の稀だが、重篤な合併症であり、そのための承認された薬物療法はない。インスリン分泌の調節異常を伴うインクレチンホルモン、グルカゴン様ペプチド-1(GLP-1)の食事誘導型分泌の増大の主要な役割が立証され、これによりGLP-1受容体拮抗は魅力的且つ標的化された治療手法となった。GLP-1受容体アンタゴニストEx-9-39のIV注入(Salehi et al.,Gastroenterology,2014,146:669-680;Craig et al.,Diabetologia,2017,60:531-540)又はSC注射(国際公開第2016/191395号パンフレットを参照)を評価する試験は、単回用量のエキセンディン(9-39)が、OGTT中に、PBH患者における食後低血糖症を予防し、β細胞機能を正常化すると共に、神経低血糖症状を軽減できることを実証した。本試験は、緩衝液体製剤中で製剤化された皮下投与エキセンディン(9-39)のヒトにおける最初の評価となることを示す。本発明者らは、PBH参加者に最大3日間BIDで投与した、皮下投与エキセンディン(9-39)の2つの製剤(食塩水中で再構成した凍結乾燥エキセンディン(9-39)、又は緩衝液体製剤中のエキセンディン(9-39))の複数漸増用量の効果、耐容性、及び動態プロフィールを評価することを目的とするこの研究から得られた中間結果(参加者20人のうちの19人)を示す。
試験設計及び手順:
この第2相MAS試験を2つのパート、パートA及びBで実施した。パートAでは、14人の参加者が、ベースラインOGTTに続いて、2.5~32mgの範囲のエキセンディン(9-39)の再構成凍結乾燥製剤(「Lyo」)の最大3日間のBID投与を受け、また投与最終日に反復OGTTを受けた。パートBでは、5人の参加者が、ベースラインOGTTに続いて、10mM酢酸ナトリウム及び45mg/mlのマンニトールを含み、pH5.5を有する緩衝液中で製剤化した30mgBIDのエキセンディン(9-39)(「Liq」)を受け、処置の第3日の反復OGTTを受けた(図6を参照)。両方のパートで、代謝、臨床、及び薬物動態応答を評価し、耐容性及び安全性をモニターした。用量レベル及び頻度の決定は、PK、PD、及び安全性データの中間評価に基づいて行った。低血糖症の症状は、エジンバラ低血糖症状尺度(Edinburgh Hypoglycemia Symptom Scale)(EHSS)(4、5)を使用して、各OGTT中に評価した。血漿グルコース≦50mg/dLで、IVデキストロースを用いた治験責任医師レスキューによりOGTTを停止した。
適格参加者は、年齢18~65歳の男性又は女性で、少なくとも12ヵ月前にRoux-en Y胃バイパス(RYGB)手術を受けており、ウィップルの三徴の病歴を有し、低血糖症(≦55mg/dL)の際に不適切に上昇したインスリン濃度(>3μU/mL)及び患者の報告により最低限毎月1つの症状エピソードを有する。14人の試験参加者の特徴を以下の表5に記載する。
代謝及び臨床応答:
パートA:再構成したエキセンディン(9-39)の凍結乾燥製剤を用いた処置により、全ての用量レベルで低血糖症の存在及び程度を低減した。用量≧18mgを受けた参加者は、IVデキストロースレスキューを必要としなかった。再構成凍結乾燥エキセンディン(9-39)の場合、グルコース最下点、インスリンピーク、及び症状スコアにおいて徐々に増加する改善と共に、用量-応答関係が観察された(図8A~8C及び以下の表6)。3日にわたり平均して約30mgの再構成凍結乾燥エキセンディン(9-39)BIDを受けた上位2つの用量コホートは、グルコース最下点の平均37%増加、ピークインスリン濃度の50%低下、並びに全体的低血糖症状スコアの50%低下、及び神経低血糖症状の50%低下を示した(図7A及び7C並びに以下の表6)。全ての用量が、軽度の頭痛又は吐き気が報告されただけで、良好な耐容性を示し、薬剤関連有害事象(DRAE)は観察されなかった。
パートA:再構成凍結乾燥エキセンディン(9-39)の用量の増加によって、Cmax及び12時間AUC濃度によって示されるように、エキセンディン(9-39)曝露が徐々に増加した(図9A~9B及び表6)。
難治性PBH患者において、皮下投与エキセンディン(9-39)の反復投与は、OGTT誘発中に下記の結果をもたらした:(1)食後高インスリン性低血糖症の用量依存的改善及び関連症状の実質的な低減;(2)用量≧18mgで、レスキュー療法を必要としない神経低血糖症の予防;並びに(3)薬剤関連の有害事象又は耐容性に対する懸念がないこと。エキセンディン(9-39)緩衝液体製剤、すぐに使える製剤は、徴候的な高インスリン性低血糖症に対して少なくとも同等の防御をもたらし、より長い作用持続時間と共に、より高い薬物動態暴露を賦与し得る。
単一及び複数漸増用量のエキセンディン(9-39)の皮下投与緩衝液体製剤の安全性、耐容性、薬物動態及び薬力学プロフィールを調べる第1相試験を実施した。この試験のために、10mM酢酸ナトリウムと45mg/mlマンニトールを含み、pH5.5を有する液体緩衝剤中30mg/mlの濃度で、エキセンディン(9-39)を製剤化した。健常なボランティアによるこの単一施設試験では、32人の対象に、7.5mg~45mgの範囲の単一漸増用量のエキセンディン(9-39)(24人のボランティア)又はプラセボ(8人のボランティア)のいずれかを皮下投与し(パートA);16人の対象が、3つの漸増用量のうちの1つ(連続した3日間60、75若しくは90mgのエキセンディン(9-39)を1日1回投与)を受けた。
さらに、本発明は次の態様を包含する。

- 前記液体医薬製剤が、ヒト対象に投与されるとき、0.9%通常生理食塩水中で製剤化された同じ用量のエキセンディン(9-39)又はその薬学的に許容される塩を含む組成物よりも高いエキセンディン(9-39)の12時間AUCを呈示する、請求項19に記載の液体医薬製剤。
- 前記液体医薬製剤が、ヒト対象に投与されるとき、0.9%通常生理食塩水中で製剤化された同じ用量のエキセンディン(9-39)又はその薬学的に許容される塩を含む組成物よりも高いエキセンディン(9-39)のトラフ血漿濃度を呈示する、請求項19に記載の液体医薬製剤。
- 対象の高インスリン性低血糖症を治療又は予防する方法であって、請求項1~22のいずれか1項に記載の液体医薬製剤を前記対象に投与するステップを含む方法。
- 前記対象が、上部消化管処置を以前受けたことがある、請求項23に記載の方法。
- 前記対象が、肥満処置を以前受けたことがある、請求項24に記載の方法。
- 前記方法が、エキセンディン(9-39)を含む液体医薬製剤を、約45mg~約75mgの用量で1日1回(QD)前記対象に皮下投与するステップを含む、請求項23~25のいずれか1項に記載の方法。
- 前記方法が、エキセンディン(9-39)を含む液体医薬製剤を、約60mgQDの用量で前記対象に皮下投与するステップを含む、請求項26に記載の方法。
- 前記方法が、エキセンディン(9-39)を含む液体医薬製剤を、約30mgの用量で1日2回(BID)前記対象に皮下投与するステップを含む、請求項23~25のいずれか1項に記載の方法。
