XMT-1536は、米国特許出願第2017/0266311号に記載されるように調製した。
研究設計
本明細書に示される研究は、3または4週間毎に1回静脈内注入として投与されるXMT-1536の非盲検、多施設第1b相試験である。試験の用量漸増(DES)パートは、白金耐性卵巣がんおよび非扁平上皮非小細胞肺がん(NSCLC)を有する患者を中心に、NaPi2bを発現する可能性が高い多数の腫瘍タイプを有する患者におけるXMT-1536についての最大耐用量(MTD)または推奨される第2相用量(RP2D)を確立する。MTDは、プロトコール特異的用量制限毒性基準によって定義される許容できない毒性を引き起こさないXMT-1536の最高用量と定義される。RP2DはMTDと異なり得、RECIST 1.1に基づいてXMT-1536の忍容性、薬物動態学的および薬力学的測定、ならびに有効性を考慮し得る。第1サイクル後、患者は、薬物が十分に許容されかつ患者が研究者の意見によれば臨床的利益を受け続けるならば、疾患進行までXMT-1536を受容し続け得る。研究のDESセグメントは、加速用量設定設計を用いた。第1の3週間処置サイクルは用量制限毒性(DLT)評価期間を構成する。第1の2つの用量レベルについて、最低1人の患者を各用量レベルで処置した。この患者がグレード2またはそれ以上の治療発現有害事象を経験する場合、2人の追加の患者がこの用量レベルで追加され、研究は、その後、下記に記載されるような、3 + 3設計に従う。この患者がDLT評価期間中にグレード2またはそれ以上の治療関連事象またはDLTを経験せず、かつ、安全性審査委員会(SRC)がこれは合理的に十分に許容される用量であったと同意する場合、登録が次の用量レベルで開始する。用量レベル3から始めて、研究は標準3 + 3設計に従い、3人の患者が最初に各用量レベルで登録される。3人の患者のうちの誰も評価期間中にDLTを経験せず、かつ、SRCがこれは合理的に十分に許容される用量であったと同意する場合、3人の患者は次の用量レベルで登録される。しかし、1つのDLTの場合、3人の追加の患者が同じ用量レベルで登録される。2つまたはそれ以上のDLTを伴ういかなる用量レベルもMTDを超えていると見なされ、その後の患者はより低い用量レベルで登録される。第1サイクル後、患者は、薬物が十分に許容されかつ患者が研究者の意見によれば臨床的利益を受け続けるならば、疾患進行までXMT-1536を受容し続け得る。
研究受診が行われる。次のサイクルにおける投薬の開始前にサイクルの終わりの安全性評価からの結果が得られかつ検討されるならば、研究全体を通じて現在のサイクルの最終日は次のサイクルの開始と同日であり得る。研究者は、患者の医学的必要性に基づいて受診スケジュールを調整する。それが緊急の必要性でない限り、何らかの調節を実施する前にメディカルモニターと潜在的なスケジュール変更を議論する。研究者は、アセスメントスケジュールに記載されるようにXMT-1536の用量を開始する前に全ての患者を評価する。完全な身体検査(PE)は、頭/目/耳/鼻/喉(HEENT)、触知可能な腫瘍、神経および筋肉系、肺および心臓血管系、腹部ならびに下肢を検査することからなる。簡単な身体検査は、HEENT、肺および心臓血管系を検査することからなる。患者は、全ての受診時に新しいおよび/または悪化する毒性の発生について評価される。全ての有害事象をNational Cancer Institute (NCI) Common Terminology Criteriaバージョン(CTCAE v4.03)に従って等級付けする。
DESにおいて登録される患者の正確な数は、先を見越して知ることはできず、MTDまたはRP2Dが達成される時に依存する。前臨床動物試験に基づいて、21〜40人の患者の範囲がDESにおいて投薬されることを予想するのが理にかなっている。研究のEXPセグメントは、3疾患コホートにおいて各々最大30人の患者を処置するように計画される。中断率を考慮して、90〜105人の患者の範囲がEXPにおいて投薬されることを予想するのが理にかなっている。
研究への登録の候補である患者は、組み入れおよび除外基準が満たされていることならびに患者が参加の資格があることを保証するために、研究者によって適格性について評価される。
薬物性肝障害(DILI)を恐らく引き起こす可能性が最も高い薬物のFDAによって分類される薬物は、研究参加中に使用されるべきではない。パラセタモール(アセトアミノフェン)の使用についての例外は除外基準6に記載される。FDAリスト上の薬物が適切な医療を与えるために研究中に使用されなければならず、かつ、代替物が利用可能でない場合、メディカルモニターは状況を議論する必要がある。
患者はいつでも研究を自由に離脱することができる。
XMT-1536は、20mmアルミニウム、緑色フリップオフキャップによって覆われたバリアフィルムを有する灰色のクロロブチルゴム栓を備えた5mL円形フリントガラスチューブバイアル中の無色から黄色または褐色の液体として提供される。各々の使い捨てバイアルは、10 mg/mLの濃度およびpH 4.0〜6.0でXMT-1536抗体薬物コンジュゲート2.5 mLを含有する。
サイクル2およびそれ以上において、研究者または被指名人は、治験薬投与を続行する前に、サイクルの終わり/プレ用量臨床および検査室所見を検討すべきである。全ての肝臓酵素が、続行前に、グレード1もしくはそれ以下へ、または患者のベースラインへ、回復されているべきである。検査室データ、特に肝臓検査が、患者のベースラインと比較されるべきであり、肝機能のより完全な回復を可能にするために、または他のAE、例えば、悪心、嘔吐、食欲不振または疲労の消散を可能にするために、投薬の遅延(例えば1週間)が検討されるべきである。
いったん患者がサイクル1を完了すると、患者が毒性を経験するが、研究者の意見によれば、XMT-1536を用いるさらなる処置から恩恵を受ける場合は、用量調節が行われ得る。減少用量での処置は、観察された毒性が毒性開始後21日間または28日間以内にGr≦1またはベースラインへ消散する場合にのみ、およびメディカルモニターとの議論後に研究者の自由裁量で、開始し得る(表6)。
グレード≧3の肝トランスアミナーゼまたはALPレベルを経験するDESおよびEXPの両方における患者は、診療室へ戻り、最初のグレード≧3値の48〜72時間以内に再検査され、そしてレベルがグレード≦1へまたはその患者のベースラインへ戻るまで、少なくとも毎週モニタリングされる。患者が戻るのが困難であるような研究施設からかなりの距離に居住する場合、地方での採血を有するようにアレンジを行うことができ、結果および局所正常範囲がモニタリングのために研究者へ戻される。
処置は、下記のもののうちの1つが生じない限り、無期限に継続する:疾患進行、処置のさらなる投与を妨げる併発疾患、許容できない有害事象または妊娠、患者のプロトコール不順守、患者が研究参加同意を撤回すること、および/または、患者の状態の全体的なもしくは特定の変化が研究者の判断で患者をさらなる処置について許容不可とすること。
用量漸増(DES)研究における主目的は、3または4週間毎に1回静脈内投与されるXMT-1536の最大耐用量(MTD)および推奨される第2相用量(RP2D)を決定し、XMT-1536の安全性および忍容性を評価することである。
用量漸増(DES)研究における副次的目的は、XMT-1536の予備的抗新生物活性を評価することである。
研究の両方のセグメントにおいて、コンピュータ断層撮影(CT)スキャンおよびRECISTバージョン1.1基準での評価を、サイクル2の終わりにおよびその後の偶数サイクル毎の終わりに行う。患者は、いかなるサイクルにおいても、有害事象、併用薬の使用、および注入反応の発生について継続的に評価される。XMT-1536および選択される放出産物/代謝産物の薬物動態プロフィールならびに抗薬物抗体の発生が評価される。RECISTバージョン1.1が、腫瘍反応を測定するために使用される。客観的奏効率(ORR - 完全奏効[CR]および部分奏効[PR]の率)ならびに病勢コントロール率(DCR - CR、PR、および安定している疾患[SD]の率)が、各コホートについて測定される。奏効期間、無進行生存(PFS)および全生存(OS)が推定される。
XMT-1536の活性成分、その放出産物および選択される代謝産物のPKプロフィールが、標準PKソフトウェア(例えば、Phoenix WinNonlin)を使用してノンコンパートメント解析によって各患者について決定される。患者当たりのPKパラメータは、最高観察濃度の時間(tmax)、最高濃度(Cmax)、および最終測定可能濃度についての濃度曲線下面積(AUC0-last)を含む。最終排出相が同定されると、追加のパラメータ、例えば、排出半減期、クリアランス、および分布体積が決定される。
統計解析計画書(SAP)が作成され、3つのセクションを含有する:(1)DESからの、(2)EXPからの、ならびに(3)DESおよびEXPからの組み合わせデータのデータ解析。このSAPは、臨床データベースに記録されたデータならびに検査室データおよび他のデータの解析に取り組む。各セクションは、そのセクションにおける登録が完了する前に、かつ、特定の研究セグメント、即ち、DESまたはEXPについてのデータベースをロックする前に、仕上げられる。DESおよびEXPの終了後の選択された表、リストまたは図(TLFと集合的に呼ばれる)が、検討目的のために作成され、SAP中に示され、しかし解析報告書はこれらの中間報告書のために作成されない。1つの最終解析報告書が作られ、最終臨床研究に含まれる。最終TLFおよび臨床研究報告書は、両方のフェーズからのデータがロックされた後に作成される。そのセクションにおける登録が完了した後にセクションについてのSAPへ行われたいかなる変更も、研究者で共有される。計画された解析からのいかなる逸脱も最終研究報告書に記載される。
2019年2月の時点で、20人の患者が6用量レベル(3 mg/m2〜40 mg/m2)、21日サイクルにわたって、16人の患者が2用量レベル(20 mg/m2〜30 mg/m2)、28日サイクルにわたって投薬された。20人の患者のうち13人が21日DLT評価期間を完了した。16人の患者のうち16人が28日評価期間を完了した。40 mg/m2、21日サイクルで投薬された患者において生じた1つのDLTが治験薬に起因し、30 mg/m2、28日サイクルで投薬された患者において生じた1つのDLTが治験薬に起因した。治験薬に関連した2つのSAEが生じた:DL 4、2 mg/m2、21日サイクルにおいてうっ血性心不全、およびDL 5、30 mg/m2、21日サイクルにおいて発熱。治療関連有害事象(TRAE)はグレード1または2であり;現在までの最も一般的なTRAEは、貧血、関節痛、疲労、嘔吐、食欲不振、AST上昇、下痢、呼吸困難(関連しないかまたは関連する可能性が低い)、低アルブミン血症(hypoalbuminema)(関連しないかまたは関連する可能性が低い)、低酸素症(関連しない)、頭痛、悪心、蛋白尿および嘔吐であった。10人の患者が処置後再病期分類スキャンを有し、9人の患者について最良効果として安定している疾患(SD)であった。
XMT-1536は、抗腫瘍活性の早期サインを伴って30 mg/m2または36 mg/m2または43 mg/m2用量レベルまで十分に許容された。MTDもRP2Dも同定されなかった。NaPi2bを発現する卵巣がん、NSCLC、甲状腺乳頭がん、子宮内膜がん、乳頭状腎細胞がん、および唾液腺管がんを有する患者において、用量漸増は継続している。
