发明背景
NaPi2b (SLC34A2、NaPiIIb、Npt2)为一种多次跨膜、钠依赖性磷酸盐转运蛋白(Xu等人 Genomics 62:281-284 (1999)),通常在哺乳动物小肠的刷状缘膜上表达并参与跨细胞无机磷酸盐(Pi)的吸收,助于维持磷酸盐的体内稳态。已在肝脏中,在乳腺、唾液腺和支气管的上皮细胞的顶端表面处,以及在肺、睾丸、甲状腺、小肠和子宫中检测到蛋白水平下的NaPi2b表达。NaPi2b中的突变与肺泡和睾丸微结石症的临床综合征相关。NaPi2b在非鳞状非小细胞肺癌(NSCLC)、非粘液性卵巢癌和乳头状甲状腺癌中高度表达。NaPi2b阳性组织免疫反应性存在于61%的NSCLC和92%的卵巢癌样本中。
卵巢癌为最常见的妇科恶性肿瘤之一和女性中第五大最常见的癌症死亡原因。高死亡率部分地是由于经常在晚期时诊断出卵巢癌,并且死亡率为发病率的约65%。卵巢上皮性肿瘤占所有卵巢肿瘤的58%和超过卵巢恶性肿瘤的90%。减瘤手术和基于铂的联合化疗(包括紫杉烷类)为当前的治疗模式;然而,大多数患有复发性上皮性卵巢癌的患者最终均死于该疾病。在卵巢癌中需要新型的治疗模式,包括靶向疗法,比如用单克隆抗体的免疫疗法或基于癌症疫苗的方法。

发明概述

详述
本公开提供通过给予与SLC34A2的细胞外区域特异性地结合的NaPi2b靶向的聚合物抗体-药物缀合物来治疗表达NaPi2b的癌症的方法。具体地讲,本发明提供XMT-1536在治疗表达NaPi2b的癌症中的给药方案。XMT-1536包含约10-15个分子的经聚(1-羟甲基亚乙基羟甲基-缩甲醛) (PHF)支架与NaPi2b单克隆抗体(XMT-1535)的半胱氨酸部分缀合的澳瑞他汀F-羟丙基酰胺(AF HPA)。
在剂量递增研究中,患有表达NaPi2b的卵巢癌、NSCLC、乳头状甲状腺癌、子宫内膜癌、乳头状肾细胞癌或唾液腺导管癌的患者每3周静脉内给予XMT-1536。根据RECIST 1.1版,用至少12 mg/m2 XMT-1536治疗的患者的疾病控制率为67%。XMT-1536在给予的最大剂量(40 mg/m2)下耐受良好。因此,本发明的特征在于通过给予受试者(即人)约10-45 mg/m2之间的输注剂量来治疗表达NaPi2b的肿瘤的方法。肿瘤包括卵巢癌、非小细胞肺癌(NSCLC)、乳头状甲状腺癌、子宫内膜癌、胆管癌、乳头状肾细胞癌、肾透明细胞癌、乳腺癌、肾癌、宫颈癌或唾液腺导管癌。肿瘤为原发性肿瘤或转移性肿瘤。

NaPi2b抗体

剂量和给予
以足以发挥治疗有用作用的量给予本文提供的含有NaPi2b靶向的聚合物抗体-药物缀合物的癌症疗法。一般地,以不会导致所治疗患者的不良副作用或使观察到的副作用最小化或减少的量给予活性剂。表达NaPi2b的癌症包括例如卵巢癌、非小细胞肺癌(NSCLC)、乳头状甲状腺癌、子宫内膜癌、胆管癌、乳头状肾细胞癌、肾透明细胞癌、乳腺癌、肾癌、宫颈癌和唾液腺导管癌。
确定活性剂的精确量处于本领域技术人员水平的范围内,活性剂包括要给予受试者的NaPi2b靶向的聚合物抗体-药物缀合物。例如,这种试剂和用于治疗癌症和实体瘤的用途为本领域众所周知的。因此,可基于在给定的给予途径下该试剂的标准给药方案来选择这种试剂的剂量。

给予的频率和时机以及剂量可在给予的周期中定期给予,以在期望的时间长度内保持活性剂的连续和/或长期作用。所提供的NaPi2b靶向的聚合物抗体-药物缀合物的组合物可每小时、每天、每周、每月、每年或一次性给予。给予周期的时间长度可凭经验确定,并且取决于待治疗的疾病、疾病的严重性、特定患者以及治疗医师技术水平范围内的其他考虑因素。本文提供的联合疗法的治疗时间长度可为一周、两周、一个月、几个月、一年、几年或更长时间。
例如,NaPi2b靶向的聚合物抗体-药物缀合物的给予频率为每天一次、隔天一次、每周两次、每周一次、每两周一次、每三周一次或每四周一次。在治疗过程期间,剂量可分为多个给予周期。例如,NaPi2b靶向的聚合物抗体-药物缀合物可在约1个月、2个月、3个月、4个月、5个月、6个月、1年或更长时间段内以一定频率给予。给予频率在整个周期时间段内可以相同,也可以不同。例如,示例性的剂量频率至少在给予周期的第一周为每周两次。在第一周之后,频率可继续每周两次,可增加至多于每周两次,或者可减少至不多于每周一次。基于所给予的特定剂量、所治疗的疾病或病症、疾病或病症的严重性、受试者的年龄和其他类似因素确定特定的剂量频率和给予周期处于技术人员水平的范围内。
如果疾病症状在不存在中断治疗的情况下持续存在,则可继续治疗额外的时间长度。在治疗过程中,可监测疾病和/或治疗相关的毒性或副作用的证据。
可定制NaPi2b靶向的聚合物抗体-药物缀合物的给予周期来增加中断治疗的时间段,以提供免于暴露于试剂的休息时间。中断治疗的时间长度可为预定的时间,或者可根据患者如何反应或根据所观察到的副作用凭经验确定。例如,可将治疗中断一周、两周、三周、一个月或几个月。通常,中断治疗的时间段被建立到患者的给药方案周期中。
示例性的给药方案为21天或28天的治疗周期或者给予周期。优选地,给药方案为治疗周期或者给予周期为28天。本文公开的NaPi2b靶向的聚合物抗体-药物缀合物在第1天给予,然后20天不给药,或者在第1天给予,然后27天不给药。确定给予的精确周期和给药时间表处于本领域技术人员水平的范围内。
如上所述,给予周期可持续任何期望的时间长度。因此,可重复21天给予周期或28天给予周期持续任何时间长度。例如,可重复21天给予周期或28天给予周期持续2个月、3个月、4个月、5个月、6个月、7个月、8个月、9个月、10个月、11个月、12个月、1.5年、2年、2.5年、3年或更长时间。根据特定于待治疗的患者和疾病的个人考虑,采用满足患者需要的给予周期和给药方案处于治疗医师技术水平的范围内。
NaPi2b表达的测量
在各个方面,本发明提供一种用于通过测量从患者获得的肿瘤样品中NaPi2b表达的状态来鉴定适合于NaPi2b靶向疗法的癌症患者或监测治疗方案的方法。
在一些实施方案中,NaPi2b诊断测试可用于鉴定用于以NaPi2b靶向的聚合物药物缀合物治疗的受试者。
样品源自患有癌症的受试者。从自受试者取出或获得的组织中解剖癌细胞样品。在一些实施方案中,样品为新鲜、冷冻的或档案活检样品。

在一个优选的实施方案中,通过免疫组织化学确定来自肿瘤的样品中NaPi2b的表达。在另一个实施方案中,使用US 16/136,706 (其通过参考以其全部结合至本文中)中描述的方法免疫组织化学(IHC)确定来自肿瘤的样品中NaPi2b的表达。

实施例1:XMT-1536的安全性和耐受性评价
研究设计
本文呈现的研究为作为每三或四周一次静脉内输注给予的XMT-1536的开放标签、多中心1b期研究。研究的剂量递增(DES)部分将为患有多种可能表达NaPi2b的肿瘤类型的患者确定XMT-1536的最大耐受剂量(MTD)或2期推荐剂量(RP2D),主要针对铂类耐药性卵巢癌和非鳞状非小细胞肺癌(NSCLC)患者。MTD定义为不会引起方案特定的剂量限制性毒性标准所定义的不可接受的毒性的XMT-1536的最高剂量。RP2D可能不同于MTD,并且可能考虑到基于RECIST 1.1的XMT-1536的耐受性、药代动力学和药效学度量以及功效。在第一个周期之后,如果根据调查者的意见药物耐受良好并且患者继续获得临床益处,则患者可继续接受XMT-1536直至疾病进展为止。该研究的DES部分使用了加速滴定设计。治疗的第一个3周周期构成剂量限制性毒性(DLT)评估期。对于前两个剂量水平,每个剂量水平治疗最少1名患者。如果该患者经历2或更高级别的治疗期间出现的不良事件,则将以该剂量水平再增加2名患者,并且该研究随后将遵循3 + 3设计,如下所述。如果该患者在DLT评估期期间没有经历2或更高级别的治疗相关事件或DLT,并且安全审查委员会(SRC)同意这是合理耐受良好的剂量,则以下一个剂量水平开始入选。在剂量水平3开始,研究将遵循标准的3 + 3设计,每个剂量水平最初入选3名患者。如果3名患者在评估期期间均未经历DLT,并且SRC同意这是合理耐受良好的剂量,则3名患者将以下一个剂量水平入选。然而,如果有1个DLT,则将以相同的剂量水平再入选3名患者。具有2个或更多个DLT的任何剂量水平均将视为超过MTD,并且随后的患者将以较低剂量水平入选。在第一个周期之后,如果根据调查者的意见药物耐受良好并且患者继续获得临床益处,则患者可继续接受XMT-1536直至疾病进展为止。

在2018年8月方案修订案之后,每个周期将为4周(28天)剂量周期。前两个为期4周的治疗周期(56天)构成剂量限制性毒性(DLT)的评估期。从该修订案开始,将在新的4周周期剂量方案下以20.0 mg/m2入选最多10名患者进行评估。如果SRC通过了该剂量,则后续剂量组将遵循修改的3 + 3设计。最初将入选3名患者,并且如果出现毒性问题,SRC将审查所有相关的数据,并可决定增加更多患者以充分评估所讨论的剂量和/或降低剂量。如果剂量耐受良好,并且3名患者达到第2周期结束而没有DLT,则SRC可通过该剂量并开放下一个剂量。DLT的观察期为56天,介于第1天到第2周期结束之间,其中包括接受第3周期剂量之前的给药前评价。在SRM之前,每剂量水平入选最多10名患者,SRM要在至少每个水平给药的第六名患者完成了第2周期第28天评估之后的不迟于5天进行。对于每个适用的剂量递增SRM,把所有先前给药的患者的可用数据均作为因素纳入数据审查中。在6名或更少所治疗的患者中出现2个或更多个DLT的任何剂量水平均将被认为超过MTD,并且随后的患者将以较低剂量水平入选。在第二个周期之后,如果根据调查者的意见药物耐受良好并且患者继续获得临床益处,则患者可继续接受XMT-1536直至疾病进展为止。在DES完成之后,研究的EXP部分将开放。
所有不良事件将根据国家癌症研究所(National Cancer Institute) (NCI)通用术语标准版本(Common Terminology Criteria version)(CTCAE v4.03)进行分级。DLT的观察期为56天,介于第1天至第2周期结束之间,其中包括接受第3周期剂量之前的给药前评价。通常,不良事件级别≥3为DLT,其中对以下标准进行了修改:中性粒细胞减少症血液学毒性、胃肠道毒性、肝毒性和电解质失衡。将第3周期的开始延迟多于2周的XMT-1536相关的毒性、住院以治疗输注相关的反应以及任何导致修改在第3周期中要给予的剂量的毒性也为DLT。将进行血液采样以确定XMT-1536、其释放产物澳瑞他汀F-HPA和选择代谢产物的血浆PK参数。将进行抗药物抗体(ADA)和中和抗体(nAb)的测试。在第2周期结束时,将使用实体瘤反应评估标准(Response Evaluation Criteria in Solid Tumors) (RECIST)版本1.1由调查者评价肿瘤反应,并且之后每2个周期进行一次。
研究访问
将进行研究访问。在整个研究中,当前周期的最后一天可与下一周期的开始为同一天,前提是获得周期结束安全性评价的结果并在下一个周期开始给药之前进行审查。调查者可基于患者的医学需要在访问时间表中安排住宿。除非其是紧急需要,否则在进行任何调整之前与医学监查员讨论可能的时间表变更。调查者将在开始如评价时间表中所述的XMT-1536剂量之前对每位患者进行评估。全面身体检查(PE)由以下组成:检查头/耳/眼/鼻/喉(HEENT)、可触及的肿瘤、神经和肌肉系统、肺部和心血管系统、腹部和下肢。简单身体检查由检查以下组成:HEENT、肺部和心血管系统。将在每次访问时评估患者的发生情况或者新的和/或恶化的毒性。所有不良事件将根据国家癌症研究所(National CancerInstitute) (NCI)通用术语标准版本(CTCAE v4.03)进行分级。
受试者的人数
无法前瞻性地获知DES中要入选的患者的确切人数,并且这取决于达到MTD或RP2D的时间。基于临床前动物研究,可合理地预期将在DES中给药的范围为21-40名患者。该研究的EXP部分计划在3个疾病队列中分别治疗最多30名患者。给定退出率,可合理地预期将有90-105名范围的患者在EXP中给药。
资格性
调查者评估作为参加研究的候选者的患者的资格性,以确保纳入和排除标准得到满足,并且患者具有参与资格。

与肝毒性相关的药物的使用
在研究参与期间,不应使用由FDA分类的最有可能引起药物诱导性肝损伤(DILI)的那些药物中的药物。排除标准6中描述了对乙酰氨基酚(醋氨酚)的使用例外。如果在研究期间必须使用FDA清单上的药物来提供适当的医学护理,并且对其没有备选可用,则医学监查员需要讨论这种情况。
受试者退出标准
患者可自由地随时退出研究。
出于安全性监测目的,如果可能的话,希望在早期终止研究参与的患者将被要求在给药后至少30天继续进行一般医学随访。进入临终关怀将优先于该要求。将遵循对治疗结束访问所述的程序。
在所有情况下,退出原因均必须记录于eCRF中。如果最初未知原因,则应跟踪患者以确定原因是否为不良事件。如果是的话,此事件将被记录为研究终止的原因。
剂量和给予
XMT-1536作为在5 mL圆形燧石玻璃管形瓶中的无色至黄色或棕色液体提供,管形瓶带有灰色氯丁基橡胶塞,橡胶塞带有由20 mm铝绿色钳口盖覆盖的阻隔膜。每个一次用小瓶含有2.5 mL XMT-1536抗体药物-缀合物,浓度为10 mg/mL和pH为4.0-6.0。
小瓶必须在-20℃ (±5℃)下储存于安全的温度控制的冰箱中。使用机构的标准药房程序对XMT-1536小瓶进行盘点和控制,以控制研究物质。指派的CRA将在定期的现场访问期间审查储存条件和这些条件的保持。
在准备患者剂量之前,每个XMT-1536单个小瓶均允许在室温下解冻最多60分钟。使用之前检查解冻的小瓶:液体应澄清为黄色至棕色,基本上没有可见的颗粒。不能摇动小瓶或将其置于阳光直射下。一旦准备好用于输注的剂量,就可在给予之前于室温下,在典型的室内照明条件下将其保留最多4个小时。
XMT-1536将根据体表面积(BSA)给予。经BSA调整的剂量按照每个机构的标准做法计算得出。如果可能,将使用Mosteller公式。基于首次剂量的14天内收集的身高和体重来计算起始剂量(可在首次剂量的当天收集)。基于后续重量测量的剂量调整根据每个机构的标准做法进行。如果不存在标准做法,则在第2周期及之后每两个周期给药之前获得另外的重量测量并经BSA确认或更改。
每个剂量以100 mL,0.9%NS PVC输液袋制备。剂量制备根据研究的现场程序进行。有关详细说明,请参阅药房手册。剂量制备将记录于每位患者的研究参与记录中,并遵循所有机构的做法,包括质量保证检查。
在研究的剂量递增部分中,计划的起始剂量为3 mg/m2的XMT-1536 ADC。剂量递增方案如以下表5所示。截至2018年8月,SRC已通过了DL 5 (30 mg/m2)。然而,基于新出现的安全性数据,SRC决定将DLT观察期延长至两个4周周期(56天),修订纳入和排除标准,并以DL 4 (20 mg/m2)增加另外的患者,意图如果发现该剂量为可以耐受的,则根据方案指南重新开始剂量递增。SRC可以决定:(1) 基于表明药物耐受性的数据,将剂量递增至下一个计划的水平或小于计划的增加的更高剂量水平。(2) 将最多3名另外的患者增加至当前剂量水平以进行另外的评估。(3) 递减至更低剂量水平以进行进一步评估;可基于患者安全性数据选择先前规定的剂量水平或新的更低剂量水平。(4) 已达到MTD,并且不会发生进一步的剂量递增。在研究扩展阶段开启之前,将有至少6名患者以MTD或RP2D进行治疗。根据给定剂量水平的耐受性以及在审查任何或所有剂量水平的所有相关患者数据之后,SRC可决定以比表5中所示的更小增幅递增。

经BSA调整的剂量按照每个机构的标准做法计算得出。如果可能,使用Mosteller公式。基于首次剂量的14天内收集的身高和体重(并且可在首次剂量的当天收集)来计算起始剂量。基于后续重量测量的剂量调整根据每个机构的标准做法进行。如果不存在标准做法,则在第2周期及之后每两个周期给药之前获得另外的重量测量并经BSA确认或更改。
常规的支持性护理药物可用于治疗与给药相关的并发的急性病症,例如过敏反应、呕吐、腹泻和发烧。不建议在任何患者的首次剂量之前进行输液反应或过敏性的预防性治疗,但如果这些反应在首次剂量之后发生,则可在第二和/或后续剂量之前给予该预防性治疗。输注之后可指示持续几天用止吐药和退热药进行治疗,并且如果患者的病史指示,可在首次剂量之前使用。在特定情况下,允许在第1周期之后进行剂量调整。
用于每位患者的初始剂量在90分钟内给予。如果没有发生输注相关的反应(IRR),则由调查者自行决定在30-90分钟内给予所有后续剂量。IRR的治疗应根据机构标准或研究指南进行。
如果根据调查者的意见,患者经历毒性但会从用XMT-1536的进一步治疗中受益,则一旦患者完成第1周期(DLT评估期)即可进行剂量调整。
继续给药的标准
在第2周期及以后,调查者或指定人员应在进行研究药物给予之前审查周期结束/给药前的临床和实验室研究结果。在进行之前,所有肝酶均应恢复至1级或更低,或至患者的基线。应将实验室数据(特别是肝脏测试)与患者的基线进行比较,并应考虑延迟给药(例如1周)以使得肝功能能够更完全恢复或使得能够消退其他AE,例如恶心、呕吐、厌食或疲劳。

任何经历Gr 4肝转氨酶或ALP的患者均将中断研究并且不经受剂量降低。至少每周监测Gr 4毒性,直至恢复至Gr≤1或基线。
无论是否达到剂量降低的阈值,肝转氨酶或ALP高于1级的任何升高必须在研究药物的下一个剂量之前恢复至≤1级或基线。
治疗的持续时间

统计分析

在EXP给药开始之前,至少由SRC对来自DES的数据进行充分汇总和审查。基于对DES数据的审查,可在开始EXP部分之前创建并提交方案修订案以供批准。统计分析将由Novella Clinical使用SAS 9.3或更高版本进行。
连续变量,包括基线特征,将通过报告观察次数、平均值、标准偏差、中位数、最小值和最大值进行汇总。分类/离散变量将使用频率表进行汇总,该表格显示类别内患者的人数和百分比。事件发生时间数据将使用Kaplan-Meier方法进行汇总。
除非另外指明,否则概括统计将仅针对观察到的数据进行报告。缺失数据将不会估算。如果缺失基线值,则不会计算与基线相比较的变化。基线定义为在第1周期第1天首次给予研究药物之前的最后可用观察结果。将在SAP中规定缺失数据的处理以及用于报告终点的方法。
结果
截至2019年2月,已有20名患者在6个剂量水平(3 mg/m2-40 mg/m2)、21天周期内给药,和有16名患者在2个剂量水平(20 mg/m2-30 mg/m2)、28天周期内给药。20名患者中有13名完成了21天的DLT评估期。16名患者中有16名完成了28天的评估期。以40 mg/m2、21天周期给药的患者中发生了一个归因于研究药物的DLT,和以30 mg/m2、28天周期给药的患者中发生了一个归因于研究药物的DLT。发生了两个与研究药物相关的SAE:DL 4、2 mg/m2、21天周期中的充血性心力衰竭和DL 5、30 mg/m2、21天周期中的发热。治疗相关的不良事件(TRAE)为1级或2级;迄今最常见的TRAE为贫血、关节痛、疲劳、呕吐、厌食、AST升高、腹泻、呼吸困难(不相关或不太可能相关)、低白蛋白血症(不相关或不太可能相关)、缺氧(不相关)、头痛、恶心、蛋白尿和呕吐。10名患者进行了治疗后的再分期扫描,其中稳定疾病(SD)为9名患者的最佳应答。
截至2019年8月,已对51名患者进行了9个剂量水平的给药(21天方案中3 mg/m2-40 mg/m2 (n=20),和28天方案中20 mg/m2-36 mg/m2 (n=31))。以30 mg/m2、28天周期给药的一名患者中发生了一个DLT,和以36 mg/m2、28天周期给药的一名患者中发生了一个DLT。两者均被认为与XMT-1536相关。

表10概述了在≥10%的患者的TRAE。报告了13名患者(33%)的严重不良事件(SAE)。两个可能的治疗相关SAE为DL5 (30 mg/m2)的2级发热和DL4 (20 mg/m2)的3级心力衰竭充血。11名患者中发生了17个与治疗不相关或不太可能相关的SAE:肠/小肠梗阻(3)、疾病进展(2)、缺氧(2)、胸腔积液(2)、腹痛、急性失血性贫血、葡萄球菌性蜂窝织炎、脑血管意外、出血性贫血、恶性腹水、心包积液、硬膜下血肿和阴道出血。在DL 5A (30 mg/m2)和DL 6(40 mg/m2)下报告有两种剂量限制性毒性。在DL 5A,3级AST在第1周期第8天升高,并在7天内恢复至2级和在13天内恢复至1级。AST升高伴有1级碱性磷酸酶升高,但未报告ALT或胆红素升高。作为DL6,3级AST在第1周期第8天升高,并在21天内恢复至1级。AST升高伴有2级ALT升高(在8天内消退至1级)和1级碱性磷酸酶升高但未报告胆红素升高。

21.权利要求20 的方法,其中所述卵巢癌为上皮性卵巢癌、输卵管癌或原发性腹膜癌。
22.权利要求21 的方法,其中所述上皮性卵巢癌被亚型化为高级别浆液性卵巢癌、低级别卵巢癌或透明细胞卵巢癌。
