Abstract
Description
本発明は、免疫不全疾患を処置するための方法、特に、いぼ、低ガンマグロブリン血症、免疫不全、骨髄性細胞貯留(myelokathexis)(WHIM)症候群すなわち「WHIMS」を処置するための方法に関する。WHIMSは、皮膚および性器いぼおよび再発性感染をもたらす好中球減少症およびリンパ球減少症によって特徴付けられる疾患である。
この出願は、2015年12月22日に出願された米国仮特許出願連続番号USSN 62/271,087および2016年12月1日に出願されたUSSN 62/428,964(これら各々の全体は、参考として本明細書に援用される)に対する優先権の利益を主張する。
一般的に、臨床症状は、白血球と抗体が低レベルであることによる再発性細菌感染を伴って、幼児期に最初に現れる[NORD 2015年]。通常の感染としては、中耳炎、蜂巣炎、膿痂疹、膿瘍、細菌性肺炎、副鼻腔炎、および歯周炎が挙げられる。罹患した個体は、特に、手、足、顔、および体幹を冒す広範囲に及ぶいぼを引き起こす可能性があり、不応性であることが多いヒトパピローマウイルス(HPV)に感染しやすい[NORD
2015年]。粘膜および生殖器のいぼも発症する場合があり、これらのいぼは、子宮頸癌への進行の危険性の増大を伴う[NORD 2015年]。最新の処置として、G−CSFおよび静脈内免疫グロブリンが挙げられるが、これらは、非特異的で、費用がかかり、投与するのが困難で、かつ部分的に有効であるにすぎない[Kawai 2009年]。

X4P−001は、CXCR4アンタゴニストであり、分子式C21H27N5;分子量349.48amu;外観 白色から淡黄色の固体;溶解度:X4P−001は、pH範囲が3.0から8.0の範囲で溶けやすく(>100mg/mL)、pH9.0でやや溶けにくく(10.7mg/mL)かつpH10.0で難溶性(2.0mg/mL)である。X4P−001は、水に僅かにしか溶解せず、融解点が108.9℃である。

in vitro薬理学
X4P−001(正式には、AMD11070と示される)のin−vitro薬理学を広範に研究し、その結果を報告した[Mosi 2012年]。以下に提示されるのは、Mosi 2012年の文献刊行物からの関連情報である。SDF−1αのアイソフォームを以下に記載した実験に使用した。
異種競合結合アッセイにおいて、X4P−001は、[125I]−SDF−1αのCCRF−CEM細胞(自然にCXCR4を発現するTリンパ芽球性細胞株[Crump 1997年])への結合を阻害することが示された。このアッセイの結果を以下の図2に示す。データは、単一部位結合モデルにフィッティングされ、12.5±1.3nMのIC50を得た。
CXCR4は、Gタンパク質共役受容体である[Baggiolini 1998年、Zlotnik 2000年]。したがって、GTPの非加水分解性類似体、例えば、蛍光標識したユーロピウム−GTP(Eu−GTP)または放射性標識した[35S]−GTPγSを使用して、受容体の活性化を測定することができる。図3および図4で示された結果は、X4P−001がCXCR4活性化を阻害し、Eu−GTP結合および[35S]−GTPγSアッセイにおいて、それぞれ39.8±2.5nMおよび19.0±4.1nMのIC50値を有したことを示した。
X4P−001のCXCR4に対する特異性を実証するために、ケモカイン受容体のパネルに対するカルシウムシグナル伝達アッセイ、ならびにBLT1、ロイコトリエンB4に対する受容体(LTB4)、およびCXCR7に対するリガンド結合アッセイにおいて試験した。LTB4は強力な化学誘引物質であり、その受容体はGタンパク質共役受容体である。表2の結果は、CCR1、CCR2b、CCR4、CCR5、CCR7、CXCR3、およびLTB4に対するX4P−001のIC50が、全ての場合に、>50mMであったことを示す。X4P−001は、このアッセイで試験される最大濃度である、10mMの濃度で、SDF−1αのCXCR7への結合を阻害しなかった。一緒に、これらのデータは、X4P−001がCXCR4の選択的阻害剤であることを示す。
治療的観点から、CXCR4アンタゴニストがCXCR4バリアントに作用できることが重要である。CXCR4のカルボキシ末端が切断されたバリアントは、WHIM症候群に関連して報告され、ナンセンス変異により、19個のアミノ酸の切断(R334X)、および10個のアミノ酸の切断(E343X)を生じ、フレームシフト変異により、13個のアミノ酸の切断(S339fs342X)を生じる[Hernandez 2003年、Kawai 2009年]。R334XおよびE343X CXCR4バリアントをクローニングし、イヌ胸腺細胞株Cf2Thで一時的に発現させた。この細胞株は、CXCR4の発現の欠如により選択された[Wong 2008年]。野生型CXCR4は、対照研究のためにこの細胞株に同様にサブクローニングした。対照研究では、これらのカルボキシ末端が切断されたバリアントの両方が、野生型CXCR4と同様の効力でカルシウムフラックスアッセイにおいてSDF−1αに応答することができることが実証された。SDF−1αに対するEC50値は、CXCR4の野生型、R334XおよびE343Xバリアントに対して、それぞれ、13.6、11.3および15.3nMであった(図6)。SDF−1αに媒介されるカルシウムフラックスに対するX4P−001の阻害効果を2つのCXCR4バリアントについて評価した。両方のバリアントは、野生型、R334XおよびE343Xバリアントに対して、それぞれ、3.1、8.5および4.6nMのIC50値で野生型CXCR4と同様の程度まで阻害された(図7)。
天然にCXCR4を発現するCCRF−CEM細胞株を使用して[Crump 1997年]、X4P−001が、12.5±1.3nMのIC50でCXCR4へのSDF−1αリガンド結合を阻害することが示された。X4P−001はまた、それぞれ、39.8±2.5nMおよび19.0±4.1nMのIC50値での蛍光Eu−GTPまたは放射性標識[35S]−GTPγS結合アッセイのいずれかを使用する2つのアッセイで、CXCR4受容体のSDF−1αに媒介されるGタンパク質活性化の阻害、ならびに9.0±2.0nMのIC50でのSDF−1αに媒介されるカルシウムフラックスの阻害によって示されるようにCXCR4の活性化およびシグナル伝達も阻害した。X4P−001はまた、19.0±4.0nMのIC50でSDF−1αに媒介される走化性、CXCR4に媒介される生理学的応答を阻害した。さらに、X4P−001は、それぞれ、CCR1、CCR2b、CCR4、CCR5、CCR7およびCXCR3に対するリガンドである、MIP1α、MCP−1、TARC、RANTES、MIP−3β、もしくはIP10のいずれかに媒介されるカルシウムフラックス、またはCXCR7へのSDF−1α結合、もしくは走化性を媒介する代替のGタンパク質共役受容体であるBLT1へのLTB4結合に、阻害効果をほとんど有さなかったか、または全く有さなかった。これらのデータは、X4P−001が、評価される他のケモカイン受容体に対するCXCR4の選択的阻害剤であることを示す。
X4P−001の主要なin vivoの薬理学効果は、骨髄からの白血球(WBC)の動員である。ビーグル種のイヌおよびC3W/He Jマウスの骨髄からのWBCの動員を実証する3つの研究を以下にまとめる。
3頭の絶食した雄のビーグル種のイヌは、1mL/kgの容量で、用量レベルが5、15、および35mg/kg(用量レベルごとに1頭のイヌ)の経口胃管栄養法により、水溶液中のX4P−001の単回用量を受けた。血液試料(それぞれ、およそ3mL)を、頸静脈からの直接的静脈穿刺により、各動物から複数の時点で得て、抗凝固剤としてK3EDTAを含有するVacutainer(登録商標)チューブを使用して回収した。血液試料は、用量前、用量後0.25、0.5、0.75、1、1.5、2、3、4、7、12、および24時間で得た。血液試料を自動差分解析(automated differential analysis)の前に、周囲室温で保存した。

WBCならびに絶対好中球およびリンパ球数に対するX4P−001の効果を図8に示す。WBCの最大増加は、用量後4〜12時間で生じた。ピークの上昇は、15および35mg/kgの用量レベルで、ベースライン値の1.8〜2.9倍の範囲であり、5mg/kgの用量レベルではいくらか低い(1.5倍)上昇が観察された。試料サイズが小さいことによって制限されるが、これらの結果は、より高い用量レベルで最大増加が達成されうることを示唆する。WBC、好中球、およびリンパ球数は、ベースラインへ戻るというエビデンスと共に、24時間において15および35mg/kgの用量レベルで上昇したままであった。他の血液学的効果は観察されなかった。
雄および雌のビーグル種のイヌにおいて、X4P−001を用いて、28日のGLP経口(カプセル剤)毒性学研究を行い、経口カプセル剤により1日2回(少なくとも7時間隔てて)28日間X4P−001を投与して、血液学効果を観察した。処置された動物のサブセットは、14日の回復期間後に評価した。表3はプロトコール設計を、表4は評価スケジュールを提示する。
X4P−001が、マウスの前駆/幹細胞を動員するかどうかを決定するために、さらなる研究を行った。全ての実験は、C3W/He Jマウスで実施した。X4P−001およびAMD3100/プレリキサホルを、以下に記載する用量で単回皮下注射により投与した。X4P−001の動員能力を、血液1mL当たりの顆粒球マクロファージ(CFU−GM)、赤血球(BFU−E)および多分化能(CFU−GEMM)前駆細胞の数によって評価した。lU/mLのrhu EPO、50ng/mLのrmu SLF、5%vol/volのポークウィードマイトジェンマウス脾臓細胞馴化培地(PWMSCM)、および0.1mMのヘミンを組み合わせて、in vitroでコロニーを形成するために、前駆細胞を刺激した。37℃、低下させた5%のCO2(5%CO2)および加湿チャンバ内でのインキュベーションの7日後にプレートをスコアリングした。
X4P−001により、単回皮下注射後のC3H/HeJマウスにおいて、前駆細胞が動員された。最初の実験では(表6に示したデータ)、マウスは、5mg/kgの用量を受け、循環血液中の前駆細胞数を様々な時点(0.25、0.5、1、2、6および24時間)で測定した。有核細胞の動員のピークは、注射のおよそ1〜2時間後に生じた。CFU−GM、BFU−EおよびCFU−GEMMのピーク増加は、対照(生理食塩水の注射)に対して、それぞれ、4.21倍(30分)、2.49〜2.54倍(30〜60分)、および2.58〜2.67倍(30〜60分)であった。
ビーグル種のイヌにおける5、15、および35mg/kgのX4P−001の単回経口用量により、総循環WBC、好中球、およびリンパ球のレベルの増加が生じた。増加は、一貫して4時間で現れ、典型的には、12時間で、時にはそれより早くピークとなった。5mg/kgにおいて、3つの細胞数の全てがベースラインの1.47倍まで増加した。15mg/kgにおいて、好中球は1.8倍まで、リンパ球は2.9倍まで、35mg/kgにおいて、好中球は2.7倍まで、リンパ球は1.9倍まで増加した。
本発明により処置される可能性のある患者には、WHIMSかMKXと診断されたことのある患者、およびそのCXCR4遺伝子に特徴的変異を示す患者が含まれる。本発明から利益を得る可能性のある他の患者には、以下のスクリーニング基準を示す個体が含まれる場合がある:
1.WHIM症候群と一致する遺伝子型が確認されるCXCR4変異を有する、かつ
2.最大14日の期間にわたって回収された少なくとも2つの独立した血液試料で、ANC≦400もしくは≦600/μL、またはALC≦650もしくは≦1000/μL、またはその両方を有する。
3.以下の所見のうちの1つを有する:
・骨髄性細胞貯留を示す骨髄穿刺液または生検
・<900個/μLの絶対好中球数および/または<1,500個/μLの絶対リンパ球数を示す末梢WBC数(急性感染の徴候または症状のない状態で得られ、かつ過去7日中にG−またはGM−CSFを受けなかった場合、≧2つの独立した試料)。
・入院の50%低減
・全身的抗生物質のクール(course)を必要とする感染の50%低減
・皮膚のいぼを含む面積の50%低減
・循環好中球の持続した増加(例えば、評価の少なくとも85%で、ANC>600個/μL、ANC>800個/μL、ANC>1000個/μL、またはANC>1,200個/μL)
・循環リンパ球の持続した増加(例えば、評価の少なくとも85%で、ALC>1000個/μL、ALC>1,200個/μL、またはALC>1,500個/μL)
・予め規定されたレベルを達成することなく、以前に投与した少なくとも2種の承認されたワクチンに応答して、保護抗体についてそのレベルを達成する。
・感染により欠勤または学校を欠席した日数の50%低減
・循環好中球の持続した増加。
WHIM症候群を有する患者に対する投薬レジメン
1日に25mgまたは50mgの規定用量でX4P−001を経口投与する。患者は、投薬スケジュールと投薬時に近い食物または飲料に関する要件の両方について指示される。
患者の初期絶対好中球数(ANC)(好中球減少症)(ANC<400個/μLまたは<600個/μL)および/または絶対リンパ球数(ALC)(リンパ球減少症)(ALC<650個/μLまたは<1000個/μL)が測定される。[注記 − 後者は、他の慢性好中球減少症に特徴的ではない]。患者が400個/μL未満もしくは600個/μL未満のANCを示し、および/またはALCが650個/μL未満もしくは1000個/μL未満のままである場合、X4P−001による処置が開始される。患者は、X4P−001 25mgを1日1回、経口で、25mgを1日2回、経口で、または50mgを1日1回、経口で用いる処置を開始する。。毒性の事象において用量低減(例えば、1日おきまたは1週間に2回に、間隔を増大することによってであってもよい)もしくは投与の停止、または不適切な応答の事象において用量増加(例えば、1日に>50mg、またはより高い毎日の投薬量に)への準備がなされる。
QDの最大用量まで、25mgまたは50mg増分で、X4P−001の毎日の用量を増加させる。WHIM患者のANCおよびALCは、急性感染によってかなり影響を受けるため、このような患者が少なくとも2週間無熱性を維持するまで、単独でまたは抗生物質、G−CSFもしくはIVIG処置とともに、AUCのモニタリングは、急性感染を有する患者で、遅らせるかまたは中断するべきである。
循環白血球
全血試料を以下について解析する:
・IgG、IgA、およびIgMレベルの増加
骨髄穿刺液を、4週の処置後および20週の処置後に、スクリーニングで同意する患者から得る。穿刺液を盲検の血液病理学者によって調査し、細胞充実性および骨髄性細胞貯留について等級付けする。十分な材料が利用可能である場合、好中球アポトーシスとリンパ球亜集団のマーカーについて試料を解析する。
・細胞過多の減少
・アポトーシスWBCの画分の減少
いぼ。いぼは、病変の位置、数、およびサイズの写真および/または記録によってモニタリングする。
所望の場合、X4P−001の血漿レベルについて、血液試料の薬物動態評価を行ってもよい。血液試料をスケジュール通り回収する。試料は、MS/MS検出を用いる逆相高速液体クロマトグラフィー(RP−HPLC)を使用して、X4P−001濃度について解析する。この生物解析方法の検証範囲は、血漿中で30から3,000ng/mLである。
・1日目:用量前、用量後30、60、90分(それぞれ、±10%)および2、3、4時間(それぞれ±15分)
・5週目の来院:用量前、用量後30、60、90分(それぞれ、±10%)および2、3、4、8時間(それぞれ±15分)
・9週目および13週目の来院:用量前
・総白血球(WBC)数
・循環CD34+陽性細胞数
・評価には、PBMCの亜集団に対するフローサイトメトリーによって解析された試料が含まれうる。
