本開示は、ミトコンドリアおよび組み合わされたミトコンドリア剤の治療的使用に関する。
ミトコンドリアは、有核真核細胞の細胞質に見いだされる二重膜結合型細胞小器官である。これらは、赤血球を除いた人体のほぼ全ての細胞に見いだされる。これらは、細胞のエネルギー代謝の原発部位であり、かつ、様々な細胞機能のためにアデノシン三リン酸(ATP)を生成する。典型的には、細胞のATP必要量の90%超が細胞自体のミトコンドリアによって供給される。

本開示は、単離したミトコンドリアを含む組成物、組み合わされたミトコンドリア剤を含む組成物、単離したミトコンドリアと組み合わされたミトコンドリア剤との両方を含む組成物、およびそのような組成物を使用する方法を提供する。

ミトコンドリアの単離
本記載の方法において使用するためのミトコンドリアを、任意の供給源から単離または提供する、例えば、培養細胞または組織から単離することができる。例示的な細胞は、とりわけ、筋肉組織細胞、心臓線維芽細胞、培養細胞、ヒーラ細胞、前立腺がん細胞、酵母、およびそれらの任意の混合物を含むが、これらに限定されない。例示的な組織は、肝組織、骨格筋、心臓、脳、および脂肪組織を含むが、これらに限定されない。ミトコンドリアを、自原性(autogenous)源、同種由来源、および/または異種由来源の細胞から単離することができる。いくつかの事例では、ミトコンドリアは、遺伝子改変を受けた細胞、例えば、mtDNAが改変されたまたは核DNAが改変された細胞から単離される。
熟練実践者は、作用物質を、幾つもの数の手法で、例えば、ミトコンドリアに付着する、ミトコンドリア膜に完全にもしくは部分的に埋め込む、ミトコンドリアに被包化する、またはミトコンドリア内に封入することによって、ミトコンドリアに連結させることができることを認識するだろう。

投与
単離したミトコンドリアおよび組み合わされたミトコンドリア剤を、静脈内に、動脈内に、腹腔内に、筋肉内に注射することによって、および/または骨髄内注入を介して、患者に投与することができる。いくつかの態様において、単離したミトコンドリアおよび組み合わされたミトコンドリア剤を、直接注射によってまたは血管注入によって送達することができる。
心臓は、正常な機能を維持するために酸素の連続供給を必要とする高度に活動的な臓器である。好気性条件下で、心臓は、そのエネルギーを主にミトコンドリアから得て、それは心筋細胞の全体積の30%を占める。虚血の発症に続いて、ミトコンドリアの構造、体積、酸素消費、およびATP合成の変更を伴って、高エネルギーリン酸レベルの急速な低下がある。
血管注入によるミトコンドリアの送達は、平均血圧または心拍数を変化させることなく冠血流を大幅に増加させることが実証されている。心拍数の増加なしに血流を増加できることは、狭心症型傷害および虚血/再灌流関連傷害におけるならびに増加した血流および酸素送達が必要とされる組織損傷領域における臨床的利用を可能にする。したがって、本明細書に記載される方法を冠動脈介入において使用して、血管中の血栓または妨害物を除去することができる。

イメージング物質をミトコンドリアに、多くの場合ミトコンドリアとイメージング物質との共インキュベーションによって付着させることができる。そのようなイメージング物質は、Thermo Fisher Scientific Inc.社のMitoTrackerおよびpHrodoフルオロフォア、18F-ローダミン6G、ならびに酸化鉄ナノ粒子を含むが、これらに限定されない。
本明細書は、薬学的物質を、例えば、患者の細胞および/または組織に送達する方法を提供する。ミトコンドリアは、アクチン依存性内在化プロセスを介して組織細胞に取り込まれ、それによって薬学的物質を細胞に直接的に送達する手法を提供する。さらに、組み合わされたミトコンドリア剤は、注射部位近くの血管の内皮を横断する可能性がより高いので、いくつかの事例では、組み合わされたミトコンドリア剤を、標的部位に血液を運ぶ血管に注射することができる。いくつかの事例では、組み合わされたミトコンドリア剤は、毛細血管の内皮を通って組織に侵入する。
遺伝子治療は、疾患を処置する薬物としての核酸高分子の患者の細胞への治療用送達である。単離したミトコンドリアを、核酸高分子を細胞に送達するための担体として使用することができる。いくつかの事例では、核酸高分子を含む組み合わされたミトコンドリア剤を、疾患を引き起こす対象の突然変異遺伝子を置換する、突然変異遺伝子を不活化する、すなわち「ノックアウトする」、または対象に新たな遺伝子を導入するために、対象に投与することができる。例示的な核酸高分子は、二本鎖DNA、一本鎖DNA、二本鎖RNA、一本鎖RNA、または三重らせん核酸分子を含むが、これらに限定されない。ある特定の事例では、核酸高分子は、DNA、干渉RNA(siRNA)、およびマイクロRNAである。ミトコンドリアDNAの機能不全に関連するミトコンドリアミオパチーの場合、ミトコンドリアの骨格筋または筋肉への直接注入によって遺伝子治療を実施することができる。核DNAが関連するミトコンドリアミオパチーの場合、経時的な複数回注入が有益であるかまたは必要とされ得る。

白色脂肪組織または白色脂肪は、哺乳類において見いだされる2つのタイプの脂肪組織の1つである。それは、しばしば、エネルギーの貯蔵として身体によって使用され、かつ、多くの白色脂肪細胞を含む。他の種類の脂肪組織は、褐色脂肪組織である。褐色脂肪組織の機能は、食物から熱へエネルギーを移行させることである。
老化したまたは損傷を受けた皮膚および筋肉(例えば、顔面筋)は、ミトコンドリアの損傷および機能不全と関連している。単離したミトコンドリアまたは組み合わされたミトコンドリア剤を使用して、これらの損傷を受けたまたは老化した組織のミトコンドリア機能を改善し、それによって、皮膚のしわ、瘢痕を取り除くか、または弛緩した皮膚、熱傷、創傷、脂肪腫などを処置することができる。
ミトコンドリア遺伝子は、核遺伝子と同じ機序によって継承されるわけではない。これらは、典型的には、一方の親からのみ継承される。ヒトでは、ミトコンドリアは、卵子、したがって母親に由来する。ミトコンドリア提供は、母親の突然変異したミトコンドリア遺伝子が新生児に継代されないようにする特殊な形態の体外受精である。通常、未来の新生児のミトコンドリアDNAは、第三者の卵子に由来する。そのような手順の1つの顕著な問題は、それが3人の遺伝学的な親を有するヒト子孫をもたらすことである。それは、生命倫理学の分野において大きな論争を引き起こしている。
本開示は、動物細胞をインビトロで維持または培養する方法を提供する。動物細胞を、ミトコンドリア剤または組み合わされたミトコンドリア剤の存在下で培養または単に維持することができる。
組織試料または培養細胞からミトコンドリアを単離する実験を実施した。
無傷の生存可能な呼吸能力のあるミトコンドリアを単離するために以下の溶液を調製した。本方法を使用してミトコンドリアをうまく単離するために、溶液および組織試料は、ミトコンドリアの生存能力を保つために氷上で維持されるべきである。氷上で維持しても、単離したミトコンドリアは、経時的に機能活性の減少を示すだろう(Olson et al., J Biol Chem 242:325-332, 1967)。これらの溶液は、可能なら前もって調製されるべきである。

単離したミトコンドリアのアリコート(10μl)をMitoTracker Orange CMTMRos(5μmol/l;Invitrogen, Carlsbad, CA, 現在Thermo-Fisher Scientific, Cambridge, MA)で標識することによって、生存ミトコンドリア数を測定した。標識したミトコンドリアのアリコートをスライド上にスポットし、63×C-アポクロマート対物レンズ(1.2 W Korr/0.17 NA, Zeiss)を備えたスピニングディスク共焦点顕微鏡を使用して計数した。ミトコンドリアを、ミトコンドリア特異的色素MitoFluor Green(Invitrogen, Carlsbad, CA, 現在 Thermo-Fisher Scientific, Cambridge, MA)で対比染色した。未染色細胞および組織を用いて自己蛍光およびバックグラウンド蛍光の測定のために適切な波長を選択した。簡潔には、標識したミトコンドリア1μlを顕微鏡スライド上に載せて、カバーした。MetaMorphイメージング解析ソフトウェアを使用して、全標本領域を網羅する低い(×10)倍率でミトコンドリア数を測定した。
ミトコンドリアを18F-ローダミン6G、酸化鉄ナノ粒子、MitoTracker Orange CMTMRos(Invitrogen, Carlsbad, CA, 現在Thermo-Fisher Scientific, Cambridge, MA)と組み合わせる実験を実施した。

MitoTracker(登録商標)フルオロフォア(5μmol/l;Invitrogen, Carlsbad, CA, 現在Thermo-Fisher Scientific, Cambridge, MA)を、単離したミトコンドリア(1.0mL)と呼吸緩衝液中で混合した。プローブを機能的ミトコンドリアと混合したとき、これらは酸化され、次いで、ミトコンドリア上のタンパク質およびペプチド上のチオールと反応してコンジュゲートを形成する。混合物を氷上4℃にて10分間暗所でインキュベートした。混合物を、9,000rpm(10,000g)で10分間の遠心分離によって3回洗浄し、ペレットをミトコンドリア単離溶液Aに毎回再懸濁した。最後の洗浄に続いて、ペレットを呼吸緩衝液に再懸濁した。
組み合わされたミトコンドリア剤のイメージング使用を示す実験を実施した。
ニュージーランドホワイトウサギ(Millbrook Farm, Amherst, MA)を実験に使用した。実験は、ハーバード大学医学大学院(Harvard Medical School)の動物実験委員会(Institutional Animal Care and Use Committee)によって承認され、実験動物の管理および使用に関する米国国立衛生研究所(NIH: National Institutes of Health)ガイドラインに準拠した(NIH Publication No. 5377-3, 1996)。全ての調査は、動物の管理および使用(Care and Use of Animals)の米国生理学会(American Physiological Society)の基本理念に従って実施された。
左前下行枝動脈(LAD)を位置特定し、テーパー針を用いてプロリン(Prolene)糸(3-0)(Ethicon, Somerville, NJ)を動脈周囲に通し、プロリン紐の両端を小さなビニル管に通してスネアを形成した。スネアを引っ張ることによって冠動脈を閉塞させ、次いで、モスキート鉗子で管を挟むことによってこれを固定した。心筋表面の局所チアノーゼによって局所虚血を目視で確認した。スネアを取り外すことによって再灌流を達成した。
1×108の二重標識ミトコンドリアをランゲンドルフウサギの心臓のリスクのある領域に注射するかまたはその冠動脈に灌流するかのいずれかを行った。
Siemens Focus 120 MicroPETスキャナーを使用してイメージングを実施した。データを60分間取得し、単一画像へと再構成した。画像を生成する非加重型(unweighted)OSEM2Dを使用して再構成を実施した。ASIProソフトウェアパッケージ(Siemens Medical Solutions)を使用して画像解析を実施した。
画像およびT2*緩和時間を取得するために、ParaVision Version 5.1ソフトウェアで作動するBioSpec 70/30 USR 7T MRI System(Bruker)またはParaVision Version 4.0で作動するBioSpec 4.7T MRI system(Bruker)に心臓を入れた。初期位置決めスキャン後、多重スライスのFLASHシネ画像を取得した。画像を再構成し、そして、ImageJソフトウェアを使用して強度データを分析した。
Albira Software Suite version 1.530で作動するAlbira Preclinical Imaging System(Bruker)を使用してマイクロ-CTを実施した。切除した心臓を、それぞれ45kVおよび400μAのX線管電圧および電流で、1スキャン当たり600回の投射を使用してスキャンした。再構成した画像は、512×512×512ボクセルであり、等方性ボクセルサイズは125μmであった。Amideソフトウェアパッケージ(http://amide.sourceforge.net)を使用してSPIO勾配画像を解析した。VolView, version 3.4(Kitware)を使用して断面画像およびボリュームレンダリング画像を作成した。
組織化学的および顕微鏡法研究のための組織試料を、虚血発症の約30分後に収集した。
局所虚血のウサギ心臓に、再灌流の開始時にそれぞれ1×108の二重標識ミトコンドリアを注射(図2A)または灌流した(図2B)。図2Aおよび図2Bの上の列は、各心臓のボリュームレンダリングを表示し、そして、マイクロ-コンピュータ断層撮影(μCT)、ポジトロン断層撮影(PET)、および合成レンダリングが左から右に示される。金属縫合糸(μCTで明るいシグナル)は、左前下行枝(LAD)冠動脈結紮の部位を示す。図2Aおよび図2Bの中央の列は、心臓の単冠状動脈スライスであり、そして、磁気共鳴画像法(MRI)、PET、および合成画像が左から右に描写される。図2Aおよび図2Bの画像の下の列は、注射または灌流した心臓の単横断スライスであり、MRI、PET、および合成画像が左から右に示される。鉄に由来する低強度のT2*MRIシグナルの領域は、18F-ローダミン6G由来のPETシグナルと相関する。これらの図はまた、外来性ミトコンドリアの冠動脈灌流がこれらの細胞小器官の心臓全体にわたる広範な分布をもたらしたことを示す。
実施例3に記載の動物モデルで、未標識の、自己に由来する肝ミトコンドリアを使用して、組み合わされたミトコンドリア剤を虚血心臓に送達することによる心臓保護効果を判定する、さらなる実験を実施した。
トリフェニルテトラゾリウムクロリドを使用して、代謝的に活性な組織と代謝的に不活性な組織とを区別した。典型的なテトラゾリウム試験では、種々の脱水素酵素(有機化合物の酸化、ひいては細胞代謝において重要な酵素)の活性が原因で、生組織では、白色の化合物は、赤色のTPF(1,3,5-トリフェニルホルマザン)へと酵素的に還元される一方で、壊死領域ではこれらの酵素が変性または分解されるかのいずれかであるので白色のTTCとして残る。この理由から、TTCは、剖検病理学において、心筋梗塞の死後特定を援助するために採用されている。健常な生存心筋は、心臓の乳酸脱水素酵素から深い赤色に染色するが;一方、潜在的な梗塞領域はより薄くなるだろう。
再灌流の前の血管注入によるミトコンドリア移植は、筋細胞壊死を有意に減少させ、かつ、虚血後の機能を有意に増強した(図4)。心筋傷害の程度を定量するために、梗塞サイズをTTC染色で生化学的に測定した。TTC染色による梗塞サイズの絶対測定は、対照群と灌流群との間でリスク領域(すなわち、LAD閉塞によって虚血にされた領域)のサイズに有意差はなかったが(図4)、対照と比較してミトコンドリアで処置された心臓ではリスク領域の百分率として表される心筋梗塞サイズが有意に減少した(P<0.05)を明らかにした(図4)。図5は、3つの圧電超音波トランスデューサを使用した局所心筋収縮期短縮によって評価した場合の虚血領域の局所心筋機能を示す。それは、ミトコンドリアが虚血および再灌流傷害に対して心臓保護を提供することができることを裏付ける。
ミトコンドリアを細胞へ投与することがミトコンドリア機能を救出しかつ損傷を受けたミトコンドリアDNAを置換することができるかどうかを判定する実験を実施した。
ヒーラp0細胞は、発酵によりエネルギーを生成できるが、mtDNAによってコードされる電子伝達系タンパク質の枯渇が原因で酸素消費能を欠如している。本実験は、ヒーラp0細胞のミトコンドリア機能を回復させるように設計される。
ATP含量は、ミトコンドリアとの24、48、72時間ならびに1および2週間の共インキュベーション後にヒーラp0細胞において有意に増加した。増強された細胞内ATP含量は、ミトコンドリアの内在化後のヒーラp0細胞の酸素消費率の有意な増加に対応した。
有効量の単離したミトコンドリアを、体外膜型酸素供給(ECMO)または体外生命維持装置(ECLS)下の臨床状態にある2人の患者に投与して、そのような処置の治療効果を評価した。症例毎に、約100マイクロリットルの約8〜10の別個の注射(呼吸緩衝液中1×107のミトコンドリアを含有する)を、左室のリスク領域(前部および後部)に送達した。これらの手順の全ては、治験審査委員会(Institutional Review Board)によって審議および承認されている。症例毎に、これらの手順と関連するリスクが予想される利益によって正当化されることができると決定されている。
重大な併存症を有する9日齢の男児患者を、冠不全の外科合併症が原因で体外膜型酸素供給(ECMO)下に置いた。ECMO9日目に、患者は、冠動脈修復のための外科処置を受けた。外科修復後、患者を、ECMO補助下で心臓集中治療室(Cardiac Intensive Care Unit)(CICU)に戻した。
第二の患者は、VACTERL症候群(椎骨欠損、肛門閉鎖、心臓欠陥、気管食道瘻、腎臓異常、および四肢の異常)の病歴、単心室循環の段階的緩和のために提示された複雑な心臓手術歴を有する肺動脈弁狭窄(PS)および心室中隔欠損(VSD)を伴う三尖弁閉鎖症1Bを有する2歳の女児であった。術中経過は、低い酸素供給および通気ならびに低下した右肺動脈(RPA)血流のための多数の心肺バイパス法(CPB)の実行によって複雑になった。術後経食道心エコー図(TEE)は、正常と中程度に落ち込んだ状態の間で変動する心室機能を明らかにした。彼女は、安定になり、開胸および人工呼吸器を付けた状態で心臓集中治療室(CICU)に移された。
心臓細胞におけるミトコンドリア融合をイメージングして融合機序を実証する実験を実施した。3D超解像顕微鏡法を使用して、ヒトiPS由来心筋細胞および初代ヒト心臓線維芽細胞におけるミトコンドリアの内在化を研究した。
本開示の例は、血管系を通って心臓にミトコンドリアを送達することができることを実証した。冠動脈を通るミトコンドリアの血管内送達は、注射の10〜30分後に心臓全体にわたるミトコンドリアの送達および取り込みをもたらす。これは、心臓自体へのミトコンドリアの直接注射(ミトコンドリアが注射された領域にミトコンドリアが留まる)とは対照的である。したがって、ミトコンドリアの血管内送達は、心臓全体のミトコンドリアの分布を可能にし、かつ、心臓全体へエネルギーを提供および回復させる迅速な方法を提供するが、いくつかの懸念がある。心臓が傷害を受けると、血管系は変化する。これらの変化は、注射されたミトコンドリアの送達または取り込みに影響を及ぼし得る。ミトコンドリアは、心臓細胞まで移動しようとせず、その変化した状態が原因で血管系に足止めされ、血管系を詰まらせ得る可能性がある。これは、心臓への血流を止めることによって心臓に損傷を与え、死に至ることもある。これらの懸念に取り組むために、ミトコンドリアが心臓の血流を変化させないことを実証する実験を実施した。
また、臨床的に関連するブタモデルにおいて実験的に誘導した可逆的心筋虚血後の、心筋損傷を制限しかつ心筋機能を増強する血管注入によるミトコンドリアの送達の有効性を実証する実験を実施した。
いずれの濃度の単回または複数回の自己由来または同種由来ミトコンドリア注射に対してもB細胞またはT細胞の免疫応答がないことを実証する実験を実施した。
最適なローダミン6Gの濃度、インキュベーション時間、および単離したミトコンドリアがローダミン6Gを取り込む温度を決定する実験を実施した。
幹細胞の統合および血管新生は、幹細胞の治療可能性の主な障害である。養子ミトコンドリア移入が内皮細胞生着を増強することを示す実験を実施した。

Angiogenesis, 16(4), 735-744に記載されているとおり、治験審査委員会が承認したプロトコルに従って臨床的に適応がある手順の間に得られた正常な処分された皮下白色脂肪組織からヒトMSCを単離した。MSC培地:10% MSC適格FBS(Hyclone)、1×グルタミン-ペニシリン-ストレプトマイシン(GPS; Invitrogen)を補充したMSCGM(Lonza)を使用して未コートプレートでヒトMSCを培養した。全ての実験を4〜6継代のMSCで行った。
培養物からECFC(9×106の細胞)を採取して、Mitochondria Isolation Kit for Cultured Cells(Thermo Scientific)の試薬A 800μLに再懸濁した。Dounce Homogenization(VWR)を使用して細胞を氷上で30秒間溶解した。細胞溶解物を試薬C 800μLと混合し、次いで、4℃にて700gで10分間遠心分離した。上清を新しい管に移し、 4℃にて12,000gで15分間遠心分離した。遠心分離後、ペレットを試薬C 500μLに再懸濁し、再び4℃にて12,000gで15分間遠心分離した。最終ペレットは、移入可能な状態にある単離したミトコンドリアを含有する。
単離したミトコンドリアをECFC培地1mLに再懸濁し、培養下のECFCに直接加えた。9×106のECFCから単離したミトコンドリアを使用して、3×106の細胞に移入した(ドナー対レシピエント比 3:1)。ミトコンドリア移入の5時間後、培地を新しいECFC培地に一新し、次いで、レシピエントECFCをインビボ移植実験にすぐさま使用した。レシピエントECFCは、ECFC-Mitoと称され、そして、外来性ミトコンドリアを受けなかった対照のECFCは、ECFC対照と称される。
動物実験は、ボストン小児病院(Children's Hospital Boston)の動物実験委員会(Institutional Animal Care and Use Committee)によって承認されたプロトコル下、AAALACによって承認されている施設で実行された。コラーゲン-フィブリン-ラミニンゲル200μLに再懸濁したヒトECFC(2×105の細胞;ミトコンドリア移入ありまたはなし)およびMSC(3×105の細胞)を、6週齢の雄の無胸腺nu/nuマウスの背中に皮下注射した(Massachusetts General Hospital, Boston, MA)。マウスを安楽死させ、7日後にインプラントを採取した。
インプラントを7日後に採取した。外植片を10%緩衝ホルマリン中で一晩固定し、パラフィン包埋し、そして、切開した(7μm厚)。ヘマトキシリンおよびエオジン(H&E)染色した切片を、ImageJ 1.47vソフトウェア(米国国立衛生研究所)を使用して血管構造の存在について調べた。ヘマトキシリンおよびエオジン(H&E)染色した切片を、赤血球を含有する血管の存在について調べた。免疫染色のために、切片を脱パラフィンし、抗原回復をtris-EDTA緩衝液(10mM Tris-Base、2mM EDTA、0.05% Tween-20、pH 9.0)で行った。次いで、切片を5〜10%ブロッキング血清中で30分間ブロッキングし、マウス抗ヒトCD31一次抗体(1:50; abcam)と室温で1時間インキュベートした。セイヨウワサビペルオキシダーゼコンジュゲートマウス二次抗体(1:200; Vector Laboratories)および3,3’-ジアミノベンジジン(DAB)を使用して、hCD31の検出、続いて、ヘマトキシリン対比染色およびパーマウント(Permount)封入を行った。ローダミンコンジュゲートUEA-1(1:200)を使用して蛍光染色、続いてDAPI対比染色(Vector Laboratories)を実施した。
インプラントの中央部由来のH&E染色した切片における赤血球充填血管の平均数(血管/mm2)として微小血管密度を報告した。各切片の領域全体を分析した。各実験条件について報告された値は、4つの個々のインプラントから得られた平均±平均の標準誤差(SEM)に対応する。
Axio Observer Z1倒立顕微鏡(Carl Zeiss)およびAxioVision Rel. 4.8ソフトウェアを使用して画像を撮影した。ApoTome.2 Optical sectioning system(Carl Zeiss)および40×/1.4油対物レンズで蛍光画像を撮影した。40×/1.4対物油レンズを使用してAxioCam MRc5カメラで非蛍光画像を撮影した。
データを平均±平均の標準誤差(SEM)として表した。対応のないスチューデントのt検定を使用して平均を比較した。GraphPad Prism v.5ソフトウェア(GraphPad Software Inc)を使用して全ての統計分析を実施した。P<0.05を統計学的に有意とした。
ドナーECFCからミトコンドリアを単離し、レシピエントECFCにドナー対レシピエント比3:1で移入した。単離したミトコンドリアをレシピエントECFCによって5時間吸収させた。次いで、外来性ミトコンドリアを受けたレシピエントECFC(ECFC-Mito)をマウスに移植して、それらの血管新生能を評価した(図26)。
肺動脈を通じて血管注入によって送達されたミトコンドリアが肺内に位置することを示す実験を実施した。
ミトコンドリアをマウスの総頸動脈に注射することによってミトコンドリアを視神経に送達できることを示す実験を実施した。PETスキャン画像は、注射されたミトコンドリアが視神経に位置することを示す(図32)。
心筋の虚血-再灌流傷害を患っている小児患者に対する現在の処置は、強心および機械循環補助を含む。体外膜型酸素供給(ECMO)補助後の心筋機能の回復は、ECMOからの離脱に40%失敗したことに反映されるとおり一貫性はない。ミトコンドリアの損傷および機能不全は、虚血-再灌流傷害を伴うそのような患者の心筋機能不全に大きく寄与する。非虚血骨格筋から採取された健常な自原性ミトコンドリアが傷害された心筋へ移植される、「ミトコンドリア自己移植」と呼ばれる、損傷を受けたミトコンドリアを修復および補完する新規戦略が開発されている。本開示における例は、移植されたミトコンドリアが、ミトコンドリア機能および生存能力を回復させ、かつ、高エネルギー合成、トランスクリプトームおよびプロテオーム変化、ならびにDNA修復を含む内部および細胞外機序によって虚血後の心筋機能を改善することを実証した。
心臓外科手術後の虚血-再灌流関連心筋機能不全のために開胸下ECMO(central ECMO)補助を必要した小児患者は、ミトコンドリア自己移植を受ける資格があった。患者が外科手術介入およびECMO補助によって改善しなかった心臓手術後の心筋虚血性イベントを経験した場合に、その患者を含めた。患者が末梢血管(頸部または大腿部)を通じたECMOカニューレ挿入を受けた場合に(心筋注射への手段がこのアプローチに不適格であるので)、その患者を除外した。
ミトコンドリア自己移植を受けた患者の特徴および結果(死亡率および全体心臓機能および局所低収縮区分)を表1に記載する。表1ついての心臓セグメンテーションスキームを図33に示す。いずれの患者も心外膜注射に関する不整脈も出血も経験しなかった。5人の対象のうち4人は、心室機能の改善を実証し、ECMO補助から離脱させることに成功した。

- 単離したミトコンドリアまたは組み合わされたミトコンドリア剤が、ミトコンドリア単離プロセスが始まった時点から30分、40分、50分、または60分以内に対象に投与される、請求項69記載の方法。
- 前記組成物が、心臓手術の前、その間、またはその後に冠動脈に注射される、請求項69記載の方法。
- 前記組成物が、血液を臓器に運ぶ血管に該組成物を注射することによって対象に投与される、請求項69記載の方法。
- 対象の血管における閉塞を除去する方法であって、単離したミトコンドリアまたは組み合わされたミトコンドリア剤を含む組成物を該血管に注射する工程を含む、前記方法。
- 対象が末梢血管疾患を有する、請求項74記載の方法。
- 前記組成物が、ミトコンドリア単離プロセスが始まった時点から約30分、40分、50分、または60分以内に血管に注射される、請求項74記載の方法。
- 細胞または組織を対象に移植する方法であって、以下を含む方法:
該細胞または該組織と、単離したミトコンドリアまたは単離したミトコンドリア剤を含む有効量の組成物とを接触させる工程;および
該細胞または該組織を該対象に移植する工程。 - 細胞が幹細胞である、請求項77記載の方法。
- 組織が骨髄である、請求項77記載の方法。
- 細胞または組織におけるミトコンドリア機能を改善する方法であって、該細胞または該組織と、単離したミトコンドリアまたは組み合わされたミトコンドリア剤を含む有効量の組成物とを接触させ、それによって該細胞または該組織におけるミトコンドリア機能を改善する工程を含む、前記方法。
- 細胞が移植細胞である、請求項80記載の方法。
- 細胞が幹細胞である、請求項80記載の方法。
- 組織が移植組織である、請求項80記載の方法。

- 対象における創傷を処置する方法であって、単離したミトコンドリアまたは組み合わされたミトコンドリア剤を含む組成物を、該創傷を処置するのに十分な量で該対象の創傷領域に投与する工程を含む、前記方法。
- 創傷が、開放創、または熱傷創である、請求項85記載の方法。
- 前記組成物が、創傷組織に該組成物を注射することによって投与される、請求項85記載の方法。
- 代謝障害を有する対象を処置する方法であって、単離したミトコンドリアまたは組み合わされたミトコンドリア剤を含む組成物を、該代謝障害を処置するのに十分な量で該対象の白色脂肪組織に投与する工程を含む、前記方法。
- 代謝障害が、肥満またはII型糖尿病である、請求項88記載の方法。
- 前記組成物が、白色脂肪組織に該組成物を注射することによって投与される、請求項88記載の方法。
- 対象の白色脂肪組織におけるミトコンドリア機能を増加させる方法であって、単離したミトコンドリアまたは組み合わされたミトコンドリア剤を含む組成物を、ミトコンドリア機能を増加させるのに十分な量で該白色脂肪組織に投与する工程を含む、前記方法。
- 前記組成物が、白色脂肪組織に該組成物を注射することによって投与される、請求項91記載の方法。
- 対象における脂肪沈着を減少させる方法であって、単離したミトコンドリアまたは組み合わされたミトコンドリア剤を含む組成物を、該対象における脂肪沈着を減少させるのに十分な量で該脂肪沈着部に投与する工程を含む、前記方法。
- 脂肪組織が白色脂肪組織である、請求項93記載の方法。
- 脂肪問題が、対象の顎下または腹部に位置している、請求項93記載の方法。
- 対象における皮膚のしわまたは瘢痕を処置する方法であって、単離したミトコンドリアまたは組み合わされたミトコンドリア剤を含む組成物を、皮膚のしわまたは瘢痕を処置するのに十分な量で該皮膚のしわまたは瘢痕領域に投与する工程を含む、前記方法。
- 前記組成物が、28、29、30、31、32、33、または34ゲージの皮下注射針によって投与される、請求項96記載の方法。
- 対象の皮膚におけるミトコンドリア機能を改善する方法であって、単離したミトコンドリアまたは組み合わされたミトコンドリア剤を含む組成物を、該対象の該皮膚におけるミトコンドリア機能を改善するのに十分な量で該対象に投与する工程を含む、前記方法。

- 組み合わされたミトコンドリア剤が、共有結合によってミトコンドリアに連結された薬学的物質を含む、請求項101または102記載の組成物。
- 組み合わされたミトコンドリア剤が、ミトコンドリアに埋め込まれた薬学的物質を含む、請求項101または102記載の組成物。
- 組み合わされたミトコンドリア剤が、抗体または抗原結合断片を含む、請求項101または102記載の組成物。
