Abstract

炎症および疼痛の予防および／または治療において有用な組成物ａ）Curcuma longa、クルクミンまたはリン脂質複合クルクミンの抽出物、ｂ）Zingiber officinaleの親油性抽出物、およびｃ）Echinacea spp.の抽出物、Zanthoxylum spp.の抽出物およびAcmella oleracea（Spilanthes oleracea）の抽出物からなる群より選択される、ポリ不飽和脂肪酸のイソブチルアミドを含む植物から得られる親油性抽出物を含む組成物が開示される。

Description

本発明は、Curcuma longa、クルクミンまたはリン脂質複合クルクミンの抽出物、Zingiber officinaleの親油性抽出物、およびポリ不飽和脂肪酸のイソブチルアミドを含む植物から得られる親油性抽出物を含んでなる、炎症および疼痛の予防および／または治療において有用な組成物に関する。
末梢炎症、特に、関節の摩耗、骨関節炎、関節リウマチおよび乾癬性関節炎に関わる炎症は、中年および高齢者の身体障害の主な原因に含まれる。
前記疾患は、非常に異なる病因を有する。それらは、ある場合には、自己免疫性疾患であり、他の場合には、特に個人が太りすぎている場合に一定の応力を受ける主要な関節の機械的摩耗による疾患である。
骨関節炎（ＯＡ）は、滑膜の変化および関節軟骨および軟骨下骨の破壊によって特徴付けられる変性疾患である。この症状は、６５〜７３歳の人口の約１０％に影響を及ぼし消耗させる。
関節リウマチおよび乾癬性関節炎のための治療法は存在するが、それらは不快で消耗性であり、一方、骨関節炎のための特定の治療法は現在のところ存在しない。
第一選択薬は、依然として、しばしば患者が耐えられないような症状を示す非ステロイド性抗炎症薬（ＮＳＡＩＤ）である。
高用量アスピリンから最新世代のものまでの古典的な抗炎症薬は、必然的に、長期にわたる治療および深刻な副作用を伴い、それは特に胃のレベルであり、そして最近発見されたように、心臓および血管レベルでもそうである。
これは、ＯＡサプリメントの消費が最近指数関数的に増加した理由の1つである。これらサプリメントは、本質的には、硫酸コンドロイチン、グルコサミンまたはヒアルロン酸、ジアセレインおよび一部の油の不けん化物をベースとする製剤であり、常に、疼痛を軽減するために主要薬剤と組み合わせて使用される。全てのこれらの産物および植物起源の多くの他の産物が使用されているが、伴う問題の1つは、疼痛軽減レベルが低いことであり、患者に上記の鎮痛剤を使用させることになる。
身体障害および生活の質の損失の最も大きな原因を確実に構成する関節リウマチの場合、および骨関節炎の場合、治療効果を提供する一方、治療の認容性も向上させる新規産物が必要とされている。
薬用植物由来の親油性抽出物、例えば、Zingiber officinale、Echinacea spp.、Zanthoxylum bungeanum、piperitumまたはarmatumおよびAcmella oleraceaの抽出物は、カンナビノイド受容体ＣＢ１およびＣＢ２およびバニロイドのリガンドであると共に特にTRPV1アゴニストとして作用する不飽和脂肪酸のイソブチルアミドの存在を伴うと、局所または全身投与において、抗炎症作用および鎮痛作用を奏することが知られている。それらの抗炎症活性は、ステロイド系および非ステロイド系の抗炎症薬のそれと比較して穏やかであることが知られている。
Curcuma longaの抽出物は、その起源の国で広く使用されており、治療的観点では、消化不良、鼓腸、下痢および関節痛を治療するために使用されてきた。これらの活性の多くが試験管内または薬理学において確認されているが、臨床レベルではほとんど確認されておらず、データが矛盾している。その植物の活性成分はクルクミンであると考えられており、クルクミンは、現在、その実際の活性を確認するために数多くの生化学的、薬理学的および臨床的試験を受けており、何百もの出版物がその潜在的利益を記載している。
予備的臨床試験は、生理学的ｐＨでのその分子の不安定性が理由の一つであるクルクミンの非常に低い全身吸収性を実証している。そのバイオアベイラビリティが低いので、試験管内試験との相関が認められない（ヒトにおける血漿濃度は、１２ｇの経口投与後に二次代謝物を含めて約５０ｎｇである）。
ＷＯ２０１５／１２４６１６には、末梢疼痛のおよび表在および深部の炎症および疼痛状態の局所的および全身的治療において用いるための、リン脂質との複合体の形態のクルクミンであってよいCurcuma spp.の抽出物、および、Echinacea spp.抽出物およびZanthoxylum spp.親油性抽出物から選択される抽出物を含む組成物が記載されている。
炎症および疼痛、特に骨関節の炎症および疼痛の予防および／または治療において有用なさらなる代替物を特定する必要性が依然として存在する。

本発明は、
ａ）Curcuma longa、クルクミンまたはリン脂質複合クルクミンの抽出物、
ｂ）Zingiber officinaleの親油性抽出物、および
ｃ）Echinacea spp.の抽出物、Zanthoxylum spp.の抽出物およびAcmella oleracea（Spilanthes oleracea）の抽出物からなる群より選択される、ポリ不飽和脂肪酸のイソブチルアミドを含む植物から得られる親油性抽出物
を含む組成物に関する。
本発明はまた、炎症および疼痛の予防および／または治療における該組成物の使用に関する。
驚くべきことに、Curcuma longa、クルクミン又はリン脂質複合クルクミンの抽出物、Zingiber officinaleの親油性抽出物、およびポリ不飽和脂肪酸のイソブチルアミドを含有する植物から得られる抽出物を含む組成物が、炎症および疼痛、特に骨関節の炎症および疼痛の予防および／または治療において有効であることが発見された。
本発明は、
ａ）Curcuma longa、クルクミンまたはリン脂質複合クルクミンの抽出物、
ｂ）Zingiber officinaleの親油性抽出物、および
ｃ）Echinacea spp.の抽出物、Zanthoxylum spp.の抽出物およびAcmella oleracea（Spilanthes oleracea）の抽出物からなる群より選択される、ポリ不飽和脂肪酸のイソブチルアミドを含む植物から得られる親油性抽出物
を含む組成物に関する。
好ましい態様によれば、Echinacea spp.の抽出物は、Echinacea angustifoliaまたはpurpureaから得ることができ、Zanthoxylum spp.の抽出物は、Zanthoxylum bungeanum、Zanthoxylum piperitumまたはZanthoxylum armatumから得ることができ、好ましくはZanthoxylum bungeanumの抽出物である。
Curruma longaの抽出物は、好ましくは塊茎から得られ、より好ましくはアルコール抽出物である。
Zingiber officinaleの親油性抽出物は、好ましくは、根および根茎から得られる。この抽出物は、また、ジンゲロールとショウガオールの含有量が高いことが好ましく、ジンゲロールとショウガオールの含有量が２０〜３０％ｗ／ｗの範囲、好ましくは２５％ｗ／ｗであるZingiber officinaleの抽出物が特に好ましい。
Echinacea spp.、Zanthoxylum spp.またはAcmella oleracea（Spilanthes oleracea）の抽出物は、一般的に抽出に用いられる植物の果実または一部から非プロトン性溶媒で抽出することによって得ることができる。
Echinacea spp.、Zanthoxylum spp.、Zingiber officinaleおよびAcmella oleraceaの親油性抽出物は、根または根茎から、アルコール、ケトンまたは脂肪族エーテルで、または好ましくは、ＥＰ０４６４２９８Ａ１（２頁１〜５２行、および５頁４５行〜６頁７行））に従う超臨界条件下での二酸化炭素で抽出することによって得ることができる。Zanthoxylum spp.の親油性抽出物は、国際公開ＷＯ００／０２５７０Ａ１（１頁２６行〜２頁１３行、および４頁２８行〜７頁２１行）に従って調製することができる。
本発明の好ましい態様によれば、組成物は、
ａ）リン脂質複合クルクミン、
ｂ）Zingiber officinaleの親油性抽出物、および
ｃ）Echinacea spp.、Zanthoxylum spp.またはAcmella oleracea（Spilanthes oleracea）の親油性抽出物、好ましくはZanthoxylum spp.の抽出物、より好ましくはZanthoxylum bungeanumの抽出物
を含む。
経口投与のための本発明による組成物は、単位用量当たり：
ａ）リン脂質複合クルクミンの５０〜１０００ｍｇ、
ｂ）Zingiber officinaleの親油性抽出物の１０〜８０ｍｇ、および
ｃ）Zanthoxylum spp.の親油性抽出物の１〜５０ｍｇ、あるいは、Echinacea spp.またはAcmella oleracea（Spilanthes oleracea）の親油性抽出物の５〜５０ｍｇ
を含む。
特に好ましい態様によれば、経口投与のための組成物は、経口単位用量あたり、リン脂質複合クルクミン２５０ｍｇ、Zingiber officinaleの親油性抽出物５０ｍｇおよびZanthoxylum spp.の親油性抽出物１０ｍｇを含む。
さらなる態様によれば、局所投与のための本発明の組成物は、
ａ）リン脂質複合クルクミンの０．１〜０．５％ｗ／ｗ、
ｂ）Zingiber officinaleの親油性抽出物の０．５％ｗ／ｗ、および
ｃ）Zanthoxylum spp.の親油性抽出物の０．１〜１％ｗ／ｗ、好ましくは０．５％ｗ／ｗ
を含む。
特に好ましい態様によれば、局所投与のための組成物は、リン脂質複合クルクミン０．０５％ｗ／ｗ、Zanthoxylum spp.の親油性抽出物０．５％ｗ／ｗおよびZingiber officinaleの親油性抽出物０．５％ｗ／ｗを含む。
本発明は、炎症および疼痛の予防および／または治療における使用のための、
ａ）Curcuma longa、クルクミンまたはリン脂質複合クルクミンの抽出物、
ｂ）Zingiber officinaleの親油性抽出物、および
ｃ）Echinacea spp.の抽出物、Zanthoxylum spp.の抽出物およびAcmella oleracea（Spilanthes oleracea）の抽出物からなる群より選択される、ポリ不飽和脂肪酸のイソブチルアミドを含む植物から得られる親油性抽出物
を含む組成物に関する。
本発明による組成物は、糖尿病性神経障害および異なる起源の筋肉痛のようなあらゆる種類の末梢の疼痛、および異なる起源の皮膚炎のような炎症状態の予防および／または治療において有用であると判明している。
この組成物は、骨関節の炎症および疼痛、例えば、末梢性および全身性の疼痛、特に小および大関節における症状の治療において特に有効であると判明している。
本発明による組成物は、抽出物が別々に使用された場合に見られるよりも強力な鎮痛および抗炎症活性を示す、すなわち、相乗効果を示すものである。
本発明による組成物の成分は、個々に摂取した場合、穏やかな有効性を示したが、適切に組み合わせると、忍容性および有効性の点で全く異なるプロファイルを生じ、相乗効果を示した。
本発明の組成物を含む製剤は、例えば、「Remington's Pharmaceutical Handbook」, Mack Publishing Co., N.Y., USAに記載されているような慣用の技術によって得ることができる。特に、本発明による組成物は、賦形剤およびカプセルマトリックスとの相互作用を避けるために特別の注意を払わなければならない植物成分のために使用される従来の製剤技術によって製剤化することができる。
本発明による組成物は、局所的または経口的に投与することができる。
経口製剤の例は、錠剤、糖衣錠、軟質および硬質ゼラチンカプセル、およびセルロースカプセルである。
本発明による組成物は、好ましくは、軟質カプセルまたはエマルションの形態で製剤化することができる。
本発明による組成物は、関節の炎症および疼痛の予防および／または治療において有用な他の生成物と組み合わせて投与することができる。
以下の実施例が、本発明をさらに説明する。
以下を含有する軟質ゼラチンカプセルを調製した。
リン脂質複合クルクミン： ２００．００ｍｇ
Zingiber officinaleの親油性抽出物（ジンゲロール２５％）：５０．００ｍｇ
Zanthoxylum bungeanumの抽出物： １５．００ｍｇ
イソブチルアミド中２５％に標準化されたＣＯ2で抽出
アマニ油： 適量〜６００ｍｇ
以下を含有する軟質ゼラチンカプセルを調製した。
リン脂質複合クルクミン： ３００．００ｍｇ
Zanthoxylum piperitumの抽出物： １５．００ｍｇ
イソブチルアミド中２５％に標準化されたＣＯ2で抽出
Zingiber officinaleの抽出物（ジンゲロール25％）： ５０．００ｍｇ
アマニ油： 適量〜７００ｍｇ
実験動物（ラット）におけるインビボ活性の試験：ラットにおけるテイル-フリック試験
リン脂質複合クルクミン、Zingiber officinale抽出物およびZanthoxylum bungeanum抽出物を含む本発明による組成物の実施例１による鎮痛活性を、ラットにおいて別々に投与された個々の成分の鎮痛活性と、および本発明の実施例１に記載の組成物中と同じ量でリン脂質複合クルクミンおよびZanthoxylum bungeanum抽出物を含むＷＯ２０１５／１２４６１６による組成物の鎮痛活性と比較した。
この結果は、以下の表1に示すように、本発明による組成物の３つの成分間の明確な相乗効果を示した。
鎮痛活性をラットのテイル-フリック試験で評価した。処理前に、３つの基本的な測定を動物に施し、含まれる取り扱いおよび装置に適していることを確認した。使用されたパラメータは、放射熱１５Ｖとカットオフ１５秒（動物への不可逆的な害を防ぐため）であり、テイル-フリックの評価を行った。
動物を実施例１の組成物１００ｍｇ／Ｋｇで処理した。
同じ量で実施例１に記載の組成物中に存在する３つの活性成分のうちの１つを各々が含んでいる３つの別々の製剤において、同じ実験モデルを用いて組成物の３つの個々の成分を評価した。
０．１ｍｌの油を用いて成分（担体）を溶解し、対照動物を処理した。
鎮痛効果を投与の１５および３０分後に測定した。

ヒトにおける鎮痛活性の評価
恒常的疼痛のある膝関節疾患に罹患している４０人の患者を無作為化し、実施例１に従って１日２錠、朝に１錠および夕方に１錠、またはプラセボ（担体のみからなる）で処理した。
有効性は、スコア０〜１０の国際的アナログ疼痛スケールに基づいて採点し、１０点は最大疼痛を示し、０点は疼痛の消失を示す。効果は、錠剤投与の２日目の朝、処理の６０および１２０分後に評価した。
結果を以下の表２に示す。

表３は、カルノフスキー指数（J. Clin. Oncology 1984; 2：187-193）に基づいて募集された患者における骨関節炎の全体的効果に対する、本発明の組成物による３ヶ月までの治療後に得られた結果を示す。
３ｋｍ／ｈおよび傾斜１０％に設定したトレッドミルにおいて、疼痛無く、および異なる程度の疼痛で移動した距離を測定することにより、評価を行った。膝の骨関節炎に罹患している８０人の患者を２つの群に分けた。ランダム化後、１つの群はプラセボで、他の群は実施例１に記載の組成物で処理した。処理中、疼痛はＷＯＭＡＣで毎週評価し、炎症パラメータを示す体液パラメータは毎月評価したところ、それらが改善されていることが分かった。

ｘｘは、倫理的な理由で試験をやめたまたは他の薬剤で治療された患者を示す。
経口および局所製剤の組み合わせは、臨床の分野で、特に膝関節障害を患っている患者に有効であると判明した。局所製剤は、末梢疼痛の治療、および表在および深部の炎症および疼痛状態の治療において特に有用であると判明した。この組み合わせは、可溶化されている油中の皮膚に直接適用することができる、または投与に適したクリームまたは軟膏に組み込むことができる。処理は、１日に１〜３回行うことができ、有痛性疾患を患う身体の部分に局所用製剤の用量０．５〜５ｇを適用する。

Claims (11)

  1. ａ）Curcuma longa、クルクミンまたはリン脂質複合クルクミンの抽出物、
    ｂ）Zingiber officinaleの親油性抽出物、および
    ｃ）Echinacea spp.の抽出物、Zanthoxylum spp.の抽出物およびAcmella oleracea（Spilanthes oleracea）の抽出物からなる群より選択される、ポリ不飽和脂肪酸のイソブチルアミドを含む植物から得られる親油性抽出物
    を含む組成物。
  2. ａ）リン脂質複合クルクミン、
    ｂ）Zingiber officinaleの親油性抽出物、および
    ｃ）Echinacea spp.、Zanthoxylum spp.またはAcmella oleracea（Spilanthes oleracea）の親油性抽出物、好ましくはZanthoxylum spp.の抽出物、より好ましくはZanthoxylum bungeanumの抽出物
    を含む、請求項１に記載の組成物。
  3. 経口投与のための単位用量当たり
    ａ）リン脂質複合クルクミンの５０〜１０００ｍｇ、
    ｂ）Zingiber officinaleの親油性抽出物の１０〜８０ｍｇ、および
    ｃ）Zanthoxylum spp.の親油性抽出物の１〜５０ｍｇ、あるいは、Echinacea spp.またはAcmella oleracea（Spilanthes oleracea）の親油性抽出物の５〜５０ｍｇ
    を含む、請求項１または２に記載の組成物。
  4. 経口投与のための単位用量当たり
    ａ）リン脂質複合クルクミン２５０ｍｇ、
    ｂ）Zingiber officinaleの親油性抽出物５０ｍｇ、および
    ｃ）Zanthoxylum spp.の親油性抽出物の１０ｍｇ
    を含む、請求項３に記載の組成物。
  5. ａ）リン脂質複合クルクミンの０．１〜０．５％ｗ／ｗ、
    ｂ）Zingiber officinaleの親油性抽出物の０．５％ｗ／ｗ、および
    ｃ）Zanthoxylum spp.の親油性抽出物の０．１〜１％ｗ／ｗ、好ましくは０．５％ｗ／ｗ
    を含む、局所投与のための請求項１または２に記載の組成物。
  6. ａ）リン脂質複合クルクミン０．０５％ｗ／ｗ、
    ｂ）Zingiber officinaleの親油性抽出物０．５％ｗ／ｗ、および
    ｃ）Zanthoxylum spp.の親油性抽出物の０．５％ｗ／ｗ
    を含む、請求項５に記載の組成物。
  7. Echinacea spp.の抽出物がEchinacea angustifoliaまたはpurpureaから得られる、請求項１〜３のいずれか１項に記載の組成物。
  8. Zanthoxylum spp.の抽出物がZanthoxylum bungeanum、Zanthoxylum piperitumまたはZanthoxylum armatumから得られる、請求項１〜６のいずれか１項に記載の組成物。
  9. Zingiber officinaleのジンゲオールとショウガオールの含量が２０〜３０％ｗ／ｗ、好ましくは２５％ｗ／ｗである、請求項１〜８のいずれか１項に記載の組成物。
  10. 炎症および疼痛の予防および／または治療における使用のための、請求項１〜９のいずれか１項に記載の組成物。
  11. 炎症および疼痛が骨関節のものである、請求項１０に記載の使用のための組成物。
