Description
本発明は、免疫チェックポイント介入による治療に適した癌を有する被験体を同定する方法、及びこのような被験体の治療方法に関する。本発明はさらに、癌を有する被験体の予後を予測又は決定する方法に関する。
治療的抗腫瘍免疫の活性化に対するアプローチの中で最も有望なのは、免疫チェックポイントの遮断である。免疫チェックポイントは、二次的組織損傷を最小限にするために末梢組織における自己免疫寛容を維持し且つ生理学的免疫応答の持続時間及び大きさを調節するため極めて重要な、免疫系における抑制経路である。現在、腫瘍は、特定の免疫チェックポイント経路を、特に腫瘍抗原に特異的なT細胞に対する、免疫耐性の主要な機構として利用していることが明らかになっている。免疫チェックポイントの多くはリガンド受容体相互作用によって開始されるため、これらは、抗体によって容易に遮断したり、組換え型のリガンド又は受容体によってモジュレートしたりすることができる。
本発明者らは、より多数のクローナル新生抗原、及び/若しくはより高い比率のクローナル新生抗原:サブクローナル新生抗原又は低いサブクローナル新生抗原割合を有する癌患者は、免疫チェックポイント介入による治療に応答する可能性がより高いという重要且つ驚くべき決定を行った。
(i) 前記被験体に由来する1つ以上の癌細胞におけるクローナル新生抗原の数を決定するステップ;及び/又は
(ii) 前記被験体に由来する2つ以上の癌細胞におけるクローナル新生抗原:サブクローナル新生抗原の比率及び/又はサブクローナル新生抗原割合を決定するステップ;及び/又は
(iii) 前記被験体に由来する癌細胞及び/若しくは腫瘍浸潤免疫細胞における免疫チェックポイント分子の発現プロファイル、又は腫瘍タイプを決定するステップ
を含み、ここでより多数のクローナル新生抗原、及び/又はより高い比率のクローナル新生抗原:サブクローナル新生抗原、又はより低い(若しくは低い)サブクローナル新生抗原割合、及び/又は参照サンプルと比較して差次的な免疫チェックポイント分子発現は、免疫チェックポイント介入に対する応答を示す、上記方法を提供する。
(i) 前記被験体に由来する1つ以上の癌細胞におけるクローナル新生抗原の数を決定するステップ;及び/又は
(ii) 前記被験体に由来する2つ以上の癌細胞におけるクローナル新生抗原:サブクローナル新生抗原の比率及び/又はサブクローナル新生抗原割合を決定するステップ
を含み、ここでより多数のクローナル新生抗原及び/又はより高い比率のクローナル新生抗原:サブクローナル新生抗原、又はより低い(若しくは低い)サブクローナル新生抗原割合は、改善された予後を示す、上記方法を提供する。
i) 本発明の方法に従って、免疫チェックポイント介入による治療に適した癌を有する被験体を同定するステップ;及び
ii) 前記被験体を免疫チェックポイント介入により治療するステップ
を含む、上記方法を提供する。
(i) より多数のクローナル新生抗原;及び/又は
(ii) より高い比率のクローナル新生抗原:サブクローナル新生抗原、又はより低い(若しくは低い)サブクローナル新生抗原割合;及び/又は
(iii) 参照サンプルと比較して差次的な免疫チェックポイント分子発現
を有すると決定されている、上記方法を提供する。
i) 本発明の方法に従って、免疫チェックポイント介入による治療に適した癌を有する被験体を同定するステップ;及び
ii) 前記被験体を免疫チェックポイント介入により治療するステップ
を含む、上記免疫チェックポイント介入も提供する。
(i) より多数のクローナル新生抗原;及び/又は
(ii) より高い比率のクローナル新生抗原:サブクローナル新生抗原、又はより低い(若しくは低い)サブクローナル新生抗原割合;及び/又は
(iii) 参照サンプルと比較して差次的な免疫チェックポイント分子発現
を有する、上記免疫チェックポイント介入を提供する。
「新生抗原(ネオアンチゲン)」は、癌細胞中の変異の結果として生じる腫瘍特異的抗原である。従って、新生抗原は、被験体中の健常細胞によっては発現されない。
本発明者らは、腫瘍内不均一性(ITH)が、ある腫瘍の異なる領域において発現される新生抗原間及びある腫瘍における異なる細胞間で変動を生じ得ることを決定した。特に、本発明者らは、ある腫瘍内で、特定の新生抗原は腫瘍の全領域及び本質的に全ての細胞において発現されるが、他方、他の新生抗原は、一部の腫瘍領域及び細胞においてのみ発現されることを決定した。
腫瘍からの生検及びサンプルの単離は、当技術分野における一般的な実務であって、任意の好適な方法に従って実施することが可能であり、このような方法は当業者に公知であろう。
本発明は、免疫チェックポイント介入による治療に適した癌を有する被験体を同定する方法であって、前記方法は、前記被験体に由来する1つ以上の癌細胞におけるクローナル新生抗原数を決定するステップを含み、ここで多数のクローナル新生抗原は、免疫チェックポイント介入に対する応答を示す、上記方法を提供する。
(i) 前記被験体に由来する1つ以上の癌細胞におけるクローナル新生抗原の数を決定するステップ;及び
(ii) 前記被験体に由来する2つ以上の癌細胞におけるクローナル新生抗原:サブクローナル新生抗原の比率及び/又はサブクローナル新生抗原割合を決定するステップ;
を含み、ここでより多数のクローナル新生抗原及びより高い比率のクローナル新生抗原:サブクローナル新生抗原、又はより低い(若しくは低い)サブクローナル新生抗原割合は、免疫チェックポイント介入に対する応答を示す、上記方法を提供する。
(i) 腫瘍から単離されたサンプルのRNA配列を決定するステップ;及び/又は
(ii) 免疫チェックポイント関連遺伝子の差次的発現を同定するための(例えばp<0.05に調整された)トランスクリプトームワイドな差次的遺伝子発現解析を実施するステップ
によって決定することができる。例えば同じ患者又は標準参照に由来する非癌細胞データを比較として使用し得る。
(i) 目的の癌を有する患者のコホートのポータルとなるCancer Genome Atlas (TCGA)データからRNA配列決定データを得るステップ;
(ii) 各患者からLevel_3遺伝子レベルデータを得るステップ;
(iii) 解析のために生リードカウントをパッケージDESeq2に入力するステップ;及び
(iv) 有意に差次的に発現された(p<0.05に調整された)免疫チェックポイント関連遺伝子を同定するためのトランスクリプトームワイドな差次的遺伝子発現解析を実施するステップ
を含む、上記方法を提供する。
(i) 前記被験体に由来する1つ以上の癌細胞におけるクローナル新生抗原の数を決定するステップ;及び
(ii) 前記被験体に由来する癌細胞及び/若しくは腫瘍浸潤免疫細胞における免疫チェックポイント分子の発現プロファイル、又は腫瘍タイプを決定するステップ
を含み、ここでより多数のクローナル新生抗原及び参照サンプルと比較して差次的な免疫チェックポイント分子発現は、免疫チェックポイント介入に対する応答を示す、上記方法を提供する。
本発明者らは、より多数のクローナル新生抗原及び/若しくはより高い比率のクローナル新生抗原:サブクローナル新生抗原又は低いサブクローナル新生抗原割合を有する癌患者は改善された予後を有するという重要且つ驚くべき決定を行った。
(i) そのコホートにおいて予測されるクローナル新生抗原のメジアン数(ここでメジアン数は参照値である)を決定するステップ;又は
(ii) そのコホートの上位四分位において存在すると予測されるクローナル新生抗原の最小数(ここで最小数は参照値である)を決定するステップ(例えば、本実施例中のTCGAデータ解析を参照)
を包含し得る。
また本発明は、被験体における癌を治療又は予防する方法であって、前記方法が、以下のステップ:
i) 本発明の方法に従って、免疫チェックポイント介入による治療に適した癌を有する被験体を同定するステップ;及び
ii) 前記被験体を免疫チェックポイント介入により治療するステップ
を含む、上記方法も提供する。
非小細胞肺癌の腫瘍内不均一性(NSCLC ITH)の文脈での新生抗原と免疫調節との臨床的関連性、及び新生抗原反応性腫瘍浸潤T細胞の特性(identity)を調べた。
患者コホートの説明
配列決定用のサンプル(L011及びL012)を、非小細胞肺癌(NSCLC)との診断を受けた患者であって、任意の形態のアジュバント療法(例えば化学療法又は放射線療法)を受ける前に根治的な外科的切除を受けた上記患者から得た。ゲノム配列決定を許可するインフォームドコンセントは得られていた。両サンプルをロンドン大学病院(University College London Hospital)(ロンドン)(UCLHRTB 10/H1306/42)から回収し、病理検査に供して組織学的サブタイプを確立した:1つの腫瘍はCK7+/TTF1+腺癌(L011)に分類され、1つの腫瘍(L012)は扁平上皮癌組織に分類された。詳細な臨床的特徴は、表S1に示される。
臨床的有効性分析を(1)に記載のとおりに実施した。手短に言えば、ペムブロリズマブに対する客観的応答を放射線科研究医による研究者評価の免疫関連応答判定基準(irRC)によって評価した。プロトコルに概略が示されるとおり、CTスキャンを9週間毎に実施した。部分的応答及び完全応答が、応答の初期同定から最小4週間後に行われた反復画像診断により確認された。応答未確認は、2回目のCTスキャンの結果に応じて、安定な疾患又は進行性の疾患と判断された。「持続的臨床利益(DCB)」は、6ヶ月間より長く持続する安定な疾患又は部分的応答(第27週、第3のプロトコルによりスケジュールされた応答評価時)として定義された。持続的利益なし(NDB)は、治療の開始から6ヶ月間以下での疾患の進行として定義された。研究治療に対して継続中の応答を示す患者について、直近の画像診断評価の日に無増悪生存期間を打ち切り(censor)とした。生存患者については、最後に連絡をとったことがわかっている日に全生存期間を打ち切りとした。各患者についての応答に関する詳細は、表S2に見出すことができる。
肺腺癌のコホート(LUAD、n=124)及び肺扁平上皮癌のコホート(LUSC、n=124)について、以下に記載される変異コール及びHLAタイプを有する腫瘍サンプルをCancer Genome Atlas (TCGA)から得た。LUAD TCGAコホート及びLUSC TCGAコホートについて、SNVデータをTumourPortal(2)から得た(http://www.tumourportal.org/tumour_types?ttype=LUAD | LUSC)。1人のLUAD患者、TCGA-05-4396は、7000個超の、主にC[C>G]Gコンテキストでコールされる低品質変異を有していたため除外された。LUSC患者、TCGA-18-3409は、LUSC腫瘍に特徴的でない強いUVシグネチャーを有していたため除外された。
L011及びL012の両方について、単一の腫瘍塊から1cm間隔で分離された4つの原発腫瘍領域及び隣接正常組織が病理学者により選択され、写真により文書化され、急速凍結された。L011における脳転移については、ヘマトキシリン及びエオシン(H&E)染色により決定された4つの腫瘍領域が、病理学者によりホルマリン固定パラフィン埋め込み(FFPE)組織ブロックの形態で選択された。全患者から手術の時に末梢血を集め、急速凍結させた。約5x5x5mmの急速凍結させた腫瘍組織及び500μlの血液を、製造者のプロトコルに従ってDNeasyキット(Qiagen)を用いるゲノムDNA抽出に使用した。FFPE組織について、マニュアルブレード顕微解剖を用いて10〜40μmの未染色スライドから組織の腫瘍リッチ領域を取り出し、この腫瘍リッチ領域からDNeasy血液及び組織キット(Qiagen)を用いてDNAを抽出し、Qubit(Invitrogen)によりDNAを定量し、アガロースゲル電気泳動によりDNAの完全性を調べた。バリデーションコホート及びディスカバリーコホートの処理に関する詳細は、(1)の補足資料中に見出すことができる。
L012
患者L012に由来する各腫瘍領域及び及び対応する生殖細胞系列について、Illumina Nexterキットを製造者のプロトコル(Illumina)に従って使用し、1〜2μgのDNAに対してエクソームキャプチャーを実施した。以前に記載されたとおり(3、4)、サンプルは、LRIの先端配列決定施設(Advanced Sequencing Facility)にあるIllumina HiSeq 2500でペアエンドマルチプレックスシーケンスを行った。キャプチャーされた各ライブラリをIlluminaプラットフォーム上にロードし、所望の平均シーケンス深度までペアエンドシーケンスを行った(平均エクソーム幅=392.75)。Illuminaパイプラインにより生成されたFastQフォーマットの生ペアエンドリード(100bp)を、bwa mem(bwa-0.7.7)(6)を使用し、GATKバンドル2.8(5)から得られた全hg19ゲノムアセンブリ(未知のコンティグを含む)に対してアラインメントした。Picardツール v1.107を使用して、同一の患者領域に由来するファイルをクリーンにし、ソートし及びマージし、重複リードを除去した(http://broadinstitute.github.io/picard)。picardツール(1.107)、GATK(2.8.1)及びFastQC(0.10.1)(http://www.bioinformatics.babraham.ac.uk/projects/fastqc/)の組み合わせを用いて、品質管理尺度を得た。
生殖細胞系列及び原発腫瘍領域のシーケンス及び解析は、(13)において以前に記載されている。転移領域のシーケンスは、BGI Techにより、(13)に記載されるプロトコルに従って実施された。転移領域の演算処理を、L012について上述される方法を用いて、複数サンプルにまたがる平均メジアン深度93.7で実施した。L012と同様に、IGVを用いて非サイレント変異を手動で再調査した。
(i) ディスカバリーコホートを示す16個のサンプル及びバリデーションコホートを示す18個のサンプル(Rizviデータ)から、生殖細胞系列と腫瘍領域の両方を示すBAMファイルを得て、picardツール(1.107) SamToFastqアラインメントを用いてFASTQフォーマットに変換し、L012について上記のように変異コーリングを実施した。
TCGAサンプルについて、以前に記載されるとおり(14)、各変異のクローン状態を、野生型アレルカウント及び変異型アレルカウント、メジャーコピー絶対数及びマイナーコピー絶対数、並びに腫瘍純度推定値を統合することによって推定した。L011及びL012について、多領域配列決定解析に基づいて各変異のクローン状態を推定した。手短に言えば、各変異は、腫瘍内の配列決定される各腫瘍領域及び全ての腫瘍領域において同定され且つ存在する場合、クローナルであると分類された。逆に、全ての腫瘍領域において普遍的に存在しない変異はサブクローナルであると分類された。
(1)の処理サンプルから得られたデータについて、VarScan2(v2.3.6)を用いて腫瘍-正常ペアに由来するエクソームSNP及びコピー数データを作成した。最小カバレッジ(21221095)及びデータ比以外はデフォルトパラメーターを用いて、Varscan2コピー数を実行した。(22300766)に記載されるとおり、サンプル毎の基準でデータ比を計算した。ASCAT v2.3(20837533)を用いてVarscanからの出力を処理し、全サンプルについて、エクソーム配列データに基づき、セグメント化されたコピー数データ並びに細胞充実度(cellularity)推定値及び倍数性推定値を与えた。以下の設定をそのデフォルト値から変更した:ACFを1に設定する閾値を0.2から0.15に調整し、ガンマ設定1でパッケージを実行した。TCGAサンプルについては、McGranahan(2015)に記載されるとおり、SNP6.0データを処理してコピー数情報を得た。
(12)に記載のように、腫瘍内の変異の領域的分布から構築された二成分存在/不存在マトリクスを用いて、系統樹を構築した。腫瘍L011について、L012及びL011の転移領域について記載される方法を用いて原発腫瘍データを再分析し、原発領域と転移領域の両方を扱う複合系統樹を可能とした。
全てのTCGA患者について、POLYSOLVER(POLYmorphic loci reSOLVER)(16)を用いて4桁のHLAタイプを決定した。患者L011及びL012は、Optitype(17)を用いて血清型が同定され且つ同時に遺伝子型が同定され、一致した結果が得られた。
同定された非サイレント変異を使用して、変異アミノ酸を可能な各位置に示した9〜11アミノ酸長のペプチドの包括的リストを生成した。全ての変異ペプチド及びその対応する野生型ペプチドの、患者の生殖細胞系列HLAアレルに対する結合親和性を、netMHCpan-2.8(18、19)を用いて予測した。候補新生抗原は、<500nmの予測結合強度を有する新生抗原として同定された。
TCGAデータポータルを通じてTCGA患者の臨床データにアクセスし、
https://tcgadata.nci.nih.gov/tcgafiles/ftp_auth/distro_ftpusers/anonymous/tumour/CANCER.TYPE/bCR/biotab/clin/からダウンロードした。生存率パッケージを用いて、Rにおいて生存率分析を実施した。
TCGAデータポータルからRNA配列決定データをダウンロードした。各LUAD患者について、全ての入手可能な「Level_3」遺伝子レベルデータを得た。生リードカウントを、分析のためのRパッケージDESeq2への入力として使用した。トランスクリプトームワイドな差次的遺伝子発現解析を実施して、有意に差次的に発現された(p<0.05に調整された)免疫関連遺伝子(表S1にリストされる)を同定した。これらの遺伝子は、尺度1-r2を用いてそれらの共発現についてクラスタ化された。
腫瘍を手術室から直接病理部に送り、そこでサンプルを領域に分割した。その後サンプルを滅菌条件下で刻み、続いて37℃で30分間にわたり酵素消化した(リベラーゼTLリサーチグレード(Roche)及びDNAse I(Roche)を添加したRPMI-1640(Sigma))後、gentleMACS(Miltenyi Biotech)を用いて機械的に解離させた。結果として得られた単一細胞懸濁液を、Ficoll-paque(GE Healthcare)勾配を通過させることによって白血球について富化させた。生細胞を計数し、-80℃にて10%ジメチルスルホキシドを添加したヒトAB血清(Sigma)中で凍結させた後、液体窒素に移した。
TILを、急速増殖プロトコル(REP)を用いて、10%ヒトAB血清(Sigma)、可溶性抗CD3(OKT3、BioXCell)、6000IU/mL組換えヒトrhIL-2(PeproTech)、及び3人の同種異系の健常ドナーからプールした2x107放射線照射PBMC(30Gy)を加えたEX-VIVO培地(Lonza)を含むT25フラスコ中で増殖させた。rhIL-2を3000IU/mLで含む新鮮培地を必要に応じて3日毎に添加した。2週間の増殖後、TILを計数し、フローサイトメトリーにより表現型決定し、ヒトAB血清(Sigma)中で-80℃で凍結させた後、関連のアッセイにおいて使用するか、又は液体窒素中で長期保存した。
予測ネオエピトープを保持するMHC多量体を、自家生成した(デンマーク工科大学SRH研究室)。Pepscan Presto (オランダ)において合成ペプチドを購入した。L011のHLA発現と一致するHLA分子(HLA-A1101、A2402、及びB3501)並びにL012のHLA発現と一致するHLA分子(HLA-A1101、A2402、及びB0702)をUV感受性ペプチドでリフォールディングさせ、UV曝露後に目的のペプチドに交換した(20-23)。手短に言えば、UV感受性ペプチドをロードしたHLA複合体を、4℃にて1時間、384ウェルプレート中で候補新生抗原ペプチドの存在下に、366nm UV光(CAMAG)に曝した。ペプチド-MHC多量体を、合計9つの異なる蛍光ストレプトアビジン(SA)コンジュゲート:PE、APC、PE-Cy7、PE-CF594、ブリリアントバイオレット(BV)421、BV510、BV605、BV650、ブリリアントウルトラバイオレット(BUV)395(BioLegend)を用いて生成した。各抗原応答性T細胞のコンビナトリアルコーディングを可能とするために各ペプチド特異性について2つの異なるストレプトアビジン-コンジュゲートを用いてMHC多量体が生成され、それにより36個までの異なるペプチドに対する反応性についての並行した解析が可能になった(24、25)。
領域特異的肺癌サンプル及び隣接正常肺組織から単離された、in vitro増殖させたCD8+Tリンパ球に対しMHC多量体解析を実施した。290個及び355個の候補変異ペプチド(<500nmの予測HLA結合親和性を有し、同じミスセンス変異に由来する複数の可能なペプチドバリエーションを含む)を合成し、それを使用して、増殖させたL011及びL012 TILをそれぞれスクリーニングした。増殖CD8+Tリンパ球の染色のため、サンプルを解凍し、DNAseで10分間処理し、洗浄し、37℃にて15分間MHC多量体パネルで染色した。その後、細胞を、633nm又は635nm励起用のLIVE/DEAD(登録商標)固定可能近赤外線死細胞染色キット(Invitrogen、Life Technologies)、CD8-PerCP(Invitrogenha Life Technologies)、及びCD4、CD14、CD16、CD19(全てBD Pharmingenより)及びCD40(AbD Serotec)の一群に対するFITC結合抗体を用いて、4℃にてさらに20分間染色した。データ取得は、LSR IIフローサイトメーター(Becton Dickinson)で、FACSDiva 6ソフトウェアを用いて実施した。陽性応答の定義のためのカットオフ値は、全CD8+細胞の0.005%以上及び10以上のイベントであった。
腫瘍サンプルを解凍し、洗浄し、暗所で37℃で10〜15分間、カスタムメイドのMHC多量体でまず染色した。その後、細胞を濡れた氷上に移し、表面抗体の一群(CD8-V500、SK1クローン(BD Biosciences)、PD-1-BV605、EH12.2H7クローン(Biolegend)、CD3-BV785、OKT3クローン(Biolegend)、LAG-3-PE、3DS223Hクローン(eBioscience))を製造者の推奨希釈で用いて、暗所で30分間染色した。細胞を、細胞内固定及びeBioscience製の透過化バッファーセットを使用して20分間透過化した。細胞内染色パネルは氷上、暗所で30分間適用され、製造者の推奨希釈で使用される以下の抗体で構成されていた:グランザイムB-V450、GB11クローン(BD Biosciences)、FoxP3-PerCP-Cy5.5、PCH101クローン(eBioscience)、Ki67-FITC、クローンB56(BD Biosciences)及びCTLA-4-APC、L3D10クローン(Biolegend)。データ取得は、BD FACSAria IIIフローサイトメーター(BD Biosciences)で実施され、Flowjoバージョン10.0.8(Tree Star Inc.)で解析された。
患者L011及びL012由来のサンプル並びに反応性ヒト扁桃腺を、緩衝ホルマリン中で固定し、従来の組織学的プロトコルに従ってパラフィン包埋した。パラフィンブロックから2〜5マイクロメートルの組織片を切り出し、電気的に荷電したスライドに移し、免疫組織化学に供した。一次使用抗体の詳細は以下の表に列挙されている。最適染色条件(すなわち、抗体希釈及びインキュベーション時間、抗原回収プロトコル、好適な色原体)を確立するため、各抗体を試験し、自動化プラットフォームBenchMark Ultra(Ventana/Roche)及びBond-III Autostainer(Leica Microsystems)を他の文献(26、27)に記載されるプロトコルに従って用いる従来の単一免疫組織化学によってヒト反応性扁桃腺の切片に対し最適化した。
大きな腫瘍新生抗原負荷は、T細胞による腫瘍認識を増加させ、免疫回避の可能性を低下させ得る(12)。腫瘍新生抗原の臨床的関連性(7)の裏付けとして、高い新生抗原ロード(コホートにおいて予測される新生抗原の数の上位四分位として定義される)が、対応する臨床データを有するLUADサンプル(n=117)において、残りの四分位の腫瘍と比較した場合により長い全生存時間に関連していたことが見出された(図1B、対数順位p=0.011;図5A)。

- 免疫チェックポイント介入による治療に適した癌を有する被験体を同定する方法であって、前記方法が、以下のステップ:

ここでより多数のクローナル新生抗原、及び/又はより高い比率のクローナル新生抗原:サブクローナル新生抗原、又はより低い(若しくは低い)サブクローナル新生抗原割合、及び/又は参照サンプルと比較して差次的な免疫チェックポイント分子発現は、免疫チェックポイント介入に対する応答を示す、前記方法。 - 免疫チェックポイント分子の発現プロファイルの決定が、免疫チェックポイント関連遺伝子の差次的発現を同定するためのトランスクリプトームワイドな差次的遺伝子発現解析によって実施される、請求項1(iii)に記載の方法。

ii) 前記被験体を免疫チェックポイント介入により治療するステップ
を含む、前記方法。 - 免疫チェックポイント介入により癌を有する被験体を治療するステップを含む、被験体における癌を治療又は予防する方法であって、ここで該被験体は、以下:
(i) より多数のクローナル新生抗原;及び/又は
(ii) より高い比率のクローナル新生抗原:サブクローナル新生抗原、又はより低い(若しくは低い)サブクローナル新生抗原割合;及び/又は
(iii) 参照サンプルと比較して差次的な免疫チェックポイント分子発現
を有すると決定されている、前記方法。 - 被験体における癌の治療又は予防方法において使用するための免疫チェックポイント介入であって、その方法が、以下のステップ:
i) 請求項1又は2に記載の方法に従って、免疫チェックポイント介入による治療に適した癌を有する被験体を同定するステップ;及び
ii) 前記被験体を免疫チェックポイント介入により治療するステップ
を含む、前記免疫チェックポイント介入。 - 被験体における癌の治療又は予防において使用するための免疫チェックポイント介入であって、該被験体が、以下:
(i) より多数のクローナル新生抗原;及び/又は
(ii) より高い比率のクローナル新生抗原:サブクローナル新生抗原、又はより低い(若しくは低い)サブクローナル新生抗原割合;及び/又は
(iii) 参照サンプルと比較して差次的な免疫チェックポイント分子発現
を有する、前記免疫チェックポイント介入。 - 被験体における癌の治療又は予防において使用するための免疫チェックポイント介入の使用であって、該被験体が、以下:
(i) より多数のクローナル新生抗原;及び/又は
(ii) より高い比率のクローナル新生抗原:サブクローナル新生抗原、又はより低い(若しくは低い)サブクローナル新生抗原割合;及び/又は
(iii) 参照サンプルと比較して差次的な免疫チェックポイント分子発現
を有する、前記使用。 - 免疫チェックポイント介入が、CTLA4、PD-1、PD-L1、Lag-3、Tim-3、TIGIT又はBTLAと相互作用する、請求項1〜8のいずれか1項に記載の方法、免疫チェックポイント介入、又は使用。
- 免疫チェックポイント介入が、ペムブロリズマブ、ニボルマブ、アテゾリズマブ又はイピリムマブである、請求項9に記載の方法、免疫チェックポイント介入、又は使用。
- 前記癌が、膀胱癌、胃癌、食道癌、乳癌、結腸直腸癌、子宮頸癌、卵巣癌、子宮内膜癌、腎臓癌(腎細胞)、肺癌(小細胞、非小細胞及び中皮腫)、脳癌(神経膠腫、星状細胞腫、神経膠芽腫)、黒色腫、リンパ腫、小腸癌(十二指腸及び空腸)、白血病、膵臓癌、肝胆道腫瘍、生殖細胞癌、前立腺癌、頭部及び頸部癌、甲状腺癌及び肉腫から選択される、請求項1〜10のいずれか1項に記載の方法、免疫チェックポイント介入、又は使用。
- 癌が肺癌又は黒色腫である、請求項11に記載の方法、免疫チェックポイント介入、又は使用。
- 癌が非小細胞肺癌(NSCLC)である、請求項12に記載の方法、免疫チェックポイント介入、又は使用。
- 前記被験体が、哺乳動物、好ましくはヒト、ネコ、イヌ、ウマ、ロバ、ヒツジ、ブタ、ヤギ、ウシ、マウス、ラット、ウサギ又はモルモットである、請求項1〜13のいずれか1項に記載の方法、免疫チェックポイント介入、又は使用。
- 前記被験体がヒトである、請求項14に記載の方法、免疫チェックポイント介入、又は使用。
