Description
硫酸ビンクリスチンリポソーム注射(VSLI)の安定性はN−デスホルミルビンクリスチン(NFV)に対するその分解に反映される。VSLIは、3バイアルキット製剤からなる場合、ビンクリスチンの電位分解のため24時間以内に投与することが必要とされ得る。VSLIの調製はまた、5%以下の遊離ビンクリスチンを常に達成する。遊離ビンクリスチンは外部pH7.4の緩衝環境(buffer environment)中に存在するであろう。しかしながら、ビンクリスチンはその塩形態(pKa5.0および7.4)において最も安定し、pH7.4では平衡がpH4.0のリポソーム内部と比較して塩形態に対してあまり好ましくないと考えられている。これは、遊離ビンクリスチンがNFVの形成、およびそのVSLIの総合安定性に対する影響にどの程度寄与したかを試験するために有用であり得る。

キット成分由来の3つの別々のVSLI被包を行い(各31mL)、プールした。装填後の試料を分析のために除去し、残存する試料の量を4つに分けた(各およそ21.75mL)。これらの試料を、Spectrapor No.1(幅40mm、6〜8kDa MWCO)の透析膜バッグに設置し、リン酸緩衝生理食塩水(PBS)またはリン酸緩衝スクロース液(pH7.4)のいずれかを用いて、光から保護しながら室温で24時間にわたり4体積交換することで、20体積超過に対して透析した。調製したPBSは、20mMのリン酸ナトリウム、130mMの塩化ナトリウムを含有し、pHは7.4であった。リン酸緩衝スクロースは、10%(w/v)スクロース、20mMのリン酸ナトリウムを含有し、pHは7.4であった。透析後、同一の緩衝液由来の試料を一緒にプールし、無菌状態下において使い捨て注射器フィルター(Pall Acrodiscs、孔径0.2μm、Supor膜)を用いて滅菌ろ過し、そして個別の滅菌した管(4.2mL)に安定性研究の各時点(2〜8℃で、0、2、4、8、12週間;室温で2、4、8、12週間、および40℃で2週間)で分注した。
予備小規模評価
初期検査研究を実施して、低pH条件下におけるMarqiboの透析が実際に発生したかどうかを確認した。計8mLのVSLI生成物を上述のMarqiboキットバイアルから調製し、装填後の物質を、Spectrapor No.1(幅20mm、6〜8kDa MWCO)透析膜バッグ中にて20体積超過に対して透析するため、pH4、5、または6のいずれかであるSPI溶液を用いて光から保護しながら2〜8℃で24時間にわたり4体積交換することで、2mLの分注量に分けた。透析後の試料を遊離ビンクリスチン含量(表1)について分析した。これにより、低pHの外部緩衝液による透析が可能であることを示す。
VSLI生成物の50mL溶液を上述のMarqiboキットバイアルから調製した。装填後の生成物量を2つに分け、その各々半分をSpectrapor No.1(幅40mm、6〜8kDa MWCO)透析膜バッグ中へ設置し、20体積超過に対してpH4または5のいずれかであるリン酸緩衝スクロース液を用いて、光から保護しながら2〜8℃で72時間にわたり4体積交換することで透析した。透析後の試料を収集し、滅菌ろ過し、個別の滅菌した管(3.5mL)に安定性研究の各時点(2〜8℃で、0、2、4、8、12週間、および室温で2、4週間)で分注した。
各安定性時点にて、試料を以下について分析した:pH(Beckman Phi 360 pH meter)、重量モル浸透圧濃度(Wescor Inc., Vapro 5520 vapor pressure osmometer)、粒径、総量および遊離ビンクリスチン、ならびに薬剤関連不純物。
3バイアル製剤から調製されたVSLIの外部緩衝液から遊離ビンクリスチンの除去をもたらす安定性が、表2に示される。遊離ビンクリスチンの除去はVSLI安定性を改善するようには考えられなかった。4週間以内に、NFVの量が4℃で倍加し、8週間までには全ての緩衝液およびpH変異体に対する分解率が1.3%NFV/月超となった。ビンクリスチン総量はNFV形成と同時に減少した。遊離外部ビンクリスチンを伴わないVSLIもまた、薬剤が室温でより急速に分解される周知のビンクリスチン化学分解特性に従って、2週間以内にNFV比率を倍加した。これらの分解率は、NFVに対するビンクリスチン分解のため24時間の構成の内にVSLIを投与する必要をもたらした、3バイアルMarqiboキット安定性に対して4℃で観測された1.6%NFV/月と同様である。
Marqiboは、Marqibo Kit構成成分(VSI、SCLI、およびSPI)を薬局で一緒にインキュベートすることにより構成される。硫酸ビンクリスチン(弱塩基)は、内部SCLIのpH値4.0と得られるリポソーム外部SPI緩衝液のpH値7.4とのpHの差異によって、作成される膜貫通型勾配の反応によりリポソーム内へ装填される。装填プロセスの間、ビンクリスチンの中性型だけがリポソーム膜を通過し、クエン酸塩としてSCLI内部に補足される。クエン酸緩衝液によるこの装填は95%超のビンクリスチン装填を提供する。しかしながら、一度構成されると、リポソーム内部pHが4.0(塩種としてビンクリスチンを維持するべきである)であっても24時間以内にNFVの増殖が発生するように、内部クエン酸ビンクリスチンは中程度に不安定である。上述の初期遊離薬剤透析実験から、ビンクリスチンの分解は、外部ビンクリスチン(pH7.4)環境またはリポソームからのビンクリスチン漏出いずれからでもなく、リポソーム内部に発生するものと考えられる。内部リポソームにおけるクエン酸緩衝液能力は非効率的であり、プロトンおよび溶質イオンがリポソームの内部pH4.0緩衝液を不安定化する程に膜を通過して移動することを可能にし得るということが考えられる。この可能性を試験するため、リポソーム内部のクエン酸ナトリウム緩衝液を代替の「装填電池」と置換した。一連の実験は、内部リポソーム緩衝液が硫酸アンモニウム溶液(AS)である条件で行った。250mMの硫酸アンモニウムのpHは約5.5である:しかしながら、リポソーム内部では、アンモニアとアンモニウム塩との間の解離平衡によって、中性のアンモニア分子がリポソーム膜を通過することが可能となり、後に残された水素イオンの結果として内部pHを効果的に低下させる。得られる内部pHは約4である。この平衡状態は、Marqiboのリポソーム環境内部におけるクエン酸ナトリウム−クエン酸平衡状態よりも良好な塩形態として硫酸ビンクリスチンを維持することができる。これらの実験において、外部pHは5.5〜8の間で変化し、約4.0の内部pHを維持した(上述のアンモニア/アンモニウム平衡状態の結果として)。モル濃度または緩衝容量は250〜350mMのASの間で変化し、重量モル浸透圧濃度全体に寄与する等張性の構成成分はポリオールおよびイオン性塩を用いて変化した。全ての実験において、リポソームはSCLIと本質的に同一の組成におけるスフィンゴミエリンおよびコレステロールから構成された。変化はクエン酸緩衝液の硫酸アンモニウム緩衝液との置換だけであった。
本実施例は、スフィンゴミエリンコレステロールリポソーム生成に使用される一般的な方法を説明する。
Marqiboリポソーム膜はスフィンゴミエリン(SM)およびコレステロール(CH)を重量配分比2.5:1で含有し、ここでは最終生成物VSLIが2.37mg/mLのSMを含有する。
脂質の溶解。SMおよびCH脂質原材料を、エタノール中にて溶解する。充分なエタノールを使用して、15.7%(v/v)の水和相における最終濃度を得る。上記の実施例において、3gのSMおよび1.2gのCHを秤量し、一緒に混合し、13mLで200プルーフのエタノール中にて溶解する。溶解は、エタノール脂質混合物を密封した容器中において75℃で透明なエタノール溶液が得られるまで温めることによって達成する。
リポソームの形成に続き、小胞は均一かつ好ましい粒子直径(この場合、約100nm)を有するリポソームの集合を作成するような大きさに作成される。これは、上述のステップ1にて作成したリポソーム懸濁液を圧力下にて所定の孔径の膜を通じて押出することによって、達成される。押出は65℃で行われ、膜の孔を通じたリポソームの通過は水和反応から残存するエタノールによって促進される。この処理を行うことによって、リポソームは使用した膜の孔の直径にほぼ一致する。これらの研究では、総容積100mLを有することのできるLipex押出器(Northern Lipids)を使用し、リポソーム懸濁液は、窒素ガスを用いて100〜400psiの圧力で、孔径0.2μm(流路3ヶ所)および0.08μm(流路5ヶ所)の25mm直径ポリカーボネート膜を通過した(Whatman Nucleopore Track-Etched Membranes)。リポソーム粒径は、動的光散乱法(Brookhaven Instruments Corporation)を使用するZetaPALS分粒器によって測定する。
この段階では、外部水相(例えば、350mMの硫酸アンモニウム溶液)を、透析またはダイアフィルトレーションによって10%のスクロース液、またはSPIといったその他所望の緩衝液と交換し、同時にエタノールを除去する。このプロセスはリポソーム勾配(すなわち、リポソームン内部の硫酸アンモニウム緩衝液およびリポソーム外部の10%のスクロース緩衝液)を確立する。硫酸アンモニウムはアンモニウムと硫酸イオンとに水性媒体中で高度に解離される。荷電イオンであるためこれらはリポソーム膜を通過することができないが、しかしながらアンモニウムイオンはまた水とアンモニアとの平衡状態にもあり、アンモニアは中性ガスとしてリポソーム膜を通過することができる。アンモニア分子がリポソーム内部を離れる時、プロトンが後に残される(リポソーム膜内部のpHを約4まで低下させる)。これは、リポソーム内部が約pH4.0であり、外部が交換緩衝液(例えば、7.4)のpHであるΔpΗ勾配を確立する。この勾配を使用してビンクリスチンをリポソーム中へ装填する。ダイアフィルトレーションプロトコル(生成物量50mL超に対する)は、QuixStandダイアフィルトレーションシステム(GE Healthcare Life Sciences)に接続された、MidGeeカートリッジ(モデルUFP−300−E−3MA、300,000MWCO)を用いて、緩衝液の15体積を交換した。透析(生成物量50mL未満に対する)は、リポソーム懸濁液を6〜8000のMWCOであるSpectrum Spectrapore分子多孔質膜中へ設置すること、およびそれを20体積超過緩衝液中にて室温で懸濁させることからなり、外部緩衝液は1日の間に4回交換する(一晩に渡る1回の交換を含む)。次の外部緩衝液交換は、各ダイアフィルトレーション後調製のSM含量はStewartリン脂質コリンアッセイを用いて測定した。
薬剤装填を、VSLI処方薬対脂質比を用いて実施して、所望の総容積を達成する。VSLI装填混合物は0.16mg/mLのビンクリスチン、2.37mg/mLのSM(リポソーム調製など)のための標的であり、所望の実験変異体に使用される外部リポソーム緩衝液によって所望の総容積まで調節される。装填は、外部緩衝液、薬剤、およびリポソーム溶液(室温まで予備平衡化した)を混合し、混合物を10分間65℃で水浴中にて穏やかに混合しながらインキュベートすることによって実施する。その後、混合物を水浴から除去して室温まで冷却し、2〜8℃で保管する。
薬剤装填バルクリポソーム懸濁液は、従来法のリポソーム滅菌技術(保存に好適なバイアル中へのろ過など)によって滅菌することができる。無菌/滅菌技術は全体を通して使用され、操作はNunair Class IIのA型/B3型Biological Safety Cabinet中で実施される。室温でのバルク生成物は、滅菌した25mm直径の0.2μm孔径Acrodisc(登録商標)シリンジフィルター、ルアーフィッティングを有する滅菌した10mLシリンジを用いるSupor(登録商標)膜(Pall Corp.)を通じてろ過する。バルクは10mLの定量で滅菌した受け取り容器へとろ過する。
二価イオンおよびポリオールによる安定性
一連の実験は、VSLI含有二価イオンまたはポリオールが被包ビンクリスチンを増強するかどうかを試験するために行われた。

前記連続した水相および前記安定化水溶液は、少なくとも2pH単位のpH差を有する、
即時使用可能なビンクリスチン組成物。
前記第2水性緩衝液がアンモニウム塩を含む、付記1に記載の組成物。
前記第1水性緩衝液がリン酸緩衝液を含む、付記1に記載の組成物。
前記リポソーム相がスフィンゴミエリンコレステロールリポソームを含む、付記1に記載の組成物。
前記第2水性緩衝液が硫酸アンモニウムを含む、付記2に記載の組成物。
前記ビンクリスチンが硫酸ビンクリスチンを含む、付記1に記載の組成物。
前記安定化水溶液のpHが、約3〜約5である、付記1に記載の組成物。
前記連続した水相のpHが、約5〜約8.8である、付記1に記載の組成物。
前記連続した水相のpHが、約7〜約8.8である、付記1に記載の組成物。
前記連続した水相のpHが、約7.5〜約8.8である、付記1に記載の組成物。
前記安定化水溶液のpHが、約3〜約5である、付記10に記載の組成物。
前記ビンクリスチンが硫酸塩形態にある、付記1に記載の組成物。
前記リポソームが加水分解抵抗性である、付記1に記載の組成物。
薬剤が、前記連続した水相中よりも前記安定化水溶液中においてより安定である、付記1に記載の組成物。
前記連続した水相と前記安定化水溶液との割合が、前記2つの相の混合がpH約6〜約8.8の結合水相を結果として生じるような割合である、付記1に記載の組成物。
前記アンモニウム塩が、約150mM〜約350mMの濃度で前記第2水性緩衝液中に存在する、付記1に記載の組成物。
前記アンモニウム塩が硫酸アンモニウムである、付記16に記載の組成物。
哺乳類においてがんを治療する方法であって、付記1に記載の組成物の治療量を、それを必要とする前記哺乳類へ投与することを含む、方法。
前記がんが、リンパ腫、白血病、または骨髄腫である、付記18に記載の方法。
前記ビンクリスチンが、約1.5mg/m2〜約2.5mg/m2の用量で投与される、付記18に記載の方法。
哺乳類におけるがんの再発を治療する方法であって、前記哺乳類へ付記1に記載の組成物を投与することを含む、方法。
前記がんの再発が、リンパ腫、白血病、または骨髄腫である、付記21に記載の方法。
前記哺乳類が、以前に少なくとも1回の多剤併用レジメン(multi-agent combination regime)を受けたことがある、付記21に記載の方法。
リポソームで被包された前記ビンクリスチンが、少なくとも1つのさらなる化学療法薬と同時投与される、付記21に記載の方法。

前記第2水性緩衝液は、前記リポソーム相の外に輸送することができ、前記安定化水溶液中に正電荷を持つ溶質またはヒドロニウムイオンを残す、少なくとも1つの溶質を有する塩を含み、前記正電荷を持つ溶質またはヒドロニウムイオンは前記ビンクリスチンを安定化し、且つ、
前記連続した水相および前記安定化水溶液は、少なくとも2pH単位のpH差を有する、
方法。

Claims (41)
- 即時使用可能なビンクリスチン組成物であって、
第1水性緩衝液を含む連続した水相、
前記第1水性緩衝液内にて分散したリポソーム相、および、
前記リポソーム相内にてカーゴとして被包された(encapsulated)安定化水溶液、
を含み、
前記安定化水溶液は、第2水性緩衝液、およびそこに溶解した安定化したビンクリスチンを含み、
前記第2水性緩衝液は、前記リポソーム相の外に輸送することができ、前記安定化水溶液中に正電荷を持つ溶質またはヒドロニウムイオンを残す、少なくとも1つの溶質を有する塩を含み、前記正電荷を持つ溶質またはヒドロニウムイオンは前記ビンクリスチンを安定化し、且つ、
前記連続した水相および前記安定化水溶液は、少なくとも2pH単位のpH差を有する、
即時使用可能なビンクリスチン組成物。 - 前記第2水性緩衝液がアンモニウム塩を含む、請求項1に記載の組成物。
- 前記第1水性緩衝液がリン酸緩衝液を含む、請求項1に記載の組成物。
