Abstract
Description
慣例的に、抗原特異的T細胞は、標的抗原に対して天然で特異的な末梢血液T細胞の選択的拡大によって生成されている。しかし大抵のがん抗原に対して特異的な多くのT細胞を選択および拡大させることは困難であり、不可能であることも多い。組み込みベクターでの遺伝子治療は、この問題への解決策を提供している:キメラ抗原受容体(CAR)のトランスジェニック発現は、任意の表面抗原に特異的な多くのT細胞が末梢血液T細胞の混合集団のex vivoウイルスベクター形質導入によって容易に生成できるようにする。
CD19は、B細胞分化の最初期に発現されるB細胞抗原であり、プラズマ細胞への最終B細胞分化でだけ失われる。それによりCD19は多発性骨髄腫を除くすべてのB細胞腫瘍において発現されている。それは、他の造血集団または非造血細胞では発現されず、したがってこの抗原を標的化することは、骨髄または非造血性器官に毒性をもたらさないはずである。有効なCD19 CAR T細胞治療はB細胞無形成を生じるが、結果として生じる低ガンマグロブリン血症はプールした免疫グロブリンで処置できることから、正常なB細胞コンパートメントの喪失はリンパ系悪性腫瘍を処置する場合には許容可能な毒性と考えられている。
CD19指向性CAR治療は、ALLにおいて最も有効であると考えられている。ALLにおける最初の研究は、Memorial Sloane Kettering(Brentjensら(2013年)Leukemia. Sci. Transl. Med. 5巻、177ra38)およびペンシルベニア大学のグループによって2013年春に発表された。近年の研究の最新報告が最近行われた(Maudeら(2014年)N. Engl. J. Med. 371巻、1507〜1517頁)。ここで25歳未満の患者25名およびこの年齢を超える5名が処置された。90%が1ヵ月で完全奏効を達成し、28名の内22名の評価可能例がMRD陰性状態を達成し、6ヵ月の無症候生存率は67%であった。患者15名は、研究後さらなる治療を受けなかった。
サイトカイン放出症候群(CRS)は、軽度から低血圧および呼吸不全を伴う多臓器不全までの範囲の一連の炎症性症状を包含する。ある程度のCRSは、CD19 CAR T細胞で処置された患者において一般的に生じる。近年のコホートにおいて処置された患者のおよそ30%(21/73)は、ある程度のCRSを示した(Daviliaら(2014年)上記;Leeら(2014年)上記;Kochenderfer(2014年)J. Clin. Oncol. Off. J. Am. Soc. Clin. Oncol.. doi:10.1200/JCO.2014.56.2025)。CRSは、ブリナツモマブ、CD19およびCD3の両方を認識する二重特異性組換え1本鎖抗体で処置された患者においても見られた。CRSは、典型的にはCAR T細胞注入の5〜21日後に生じる。
施設にわたるCD19 CAR研究における多数の患者は、失語症から鈍麻、せん妄および発作の多様な重症度で一過性の神経毒性を発症した(Daviliaら(2014年)上記)。これは、ALLを有する患者に限定されていると考えられ、同様の症候群がブリナツモマブ治療後に確認された。脳画像は正常であると考えられる。神経毒性は、高レベルの全身性サイトカインが血液脳関門を通過することを反映し得る。
持続的応答は、循環CAR形質導入T細胞のさらに高いピークレベル、およびB細胞無形成の持続期間と相関すると考えられる。CD19疾患を再発した患者を除いて、再発は一般に循環CAR T細胞の喪失および正常B細胞の回復と関連した。
したがって、上記の不利益と関連しない、CD19に対して向けられた代替的CARに対する必要がある。

キメラ抗原受容体(CAR)は、キメラT細胞受容体、人工T細胞受容体およびキメラ免疫受容体としても公知であり、免疫エフェクター細胞に任意の特異性をグラフトする操作された受容体である。古典的CARでは、モノクローナル抗体の特異性がT細胞にグラフトされる。CARコード核酸は、例えばレトロウイルスベクターを使用してT細胞に移入され得る。このように多数のがん特異的T細胞は養子細胞移入のために生成され得る。この手法の第I相臨床研究は有効性を示している。

本発明のCARは、膜に広がる膜貫通ドメインも含んでよい。それは、疎水性アルファヘリックスを含んでよい。膜貫通ドメインは、CD28由来であってよく、良好な受容体安定性をもたらす。

エンドドメインは、CARのシグナルの伝達部分である。抗原認識後、受容体がクラスター形成し、シグナルは細胞に伝達される。最も一般的に使用されるエンドドメイン構成成分は、3個のITAMを含有するCD3ゼータのものである。これは、抗原が結合した後にT細胞に活性化シグナルを伝達する。CD3ゼータは、完全に適格性の活性化シグナルは提供し得ず、追加的共刺激シグナル伝達が必要な場合がある。例えばCD28またはOX40もしくは41BB由来のエンドドメインは、増殖/生存シグナルを伝達するためにCD3ゼータと共に使用される場合がある、あるいは3種すべてが一緒に使用されてよい。

本発明のCARは、CARがT細胞などの細胞内で発現されたときに、新生タンパク質が小胞体に、次いでそれが発現される細胞表面に向けられるようなシグナルペプチドを含んでもよい。

本発明のCARは、CD19結合ドメインを膜貫通ドメインに連結し、CD19結合ドメインをエンドドメインから空間的に離すスペーサー配列を含んでよい。可動性スペーサーは、CD19結合を可能にするようにCD19結合ドメインをさまざまな方向に方向付けることを可能にする。

本発明者らは、CAT19 scFvに基づくCD19 CARが、さらに低い毒性およびさらに良好な有効性を生じることができる特性を有することを見出した。
本発明の第2の態様は、本発明の第1の態様のCARをコードする核酸配列に関する。
本発明は、本発明による核酸配列を含むベクターも提供する。そのようなベクターは、核酸配列を宿主細胞に導入するために使用されてよく、それにより本発明の第1の態様による分子を発現および産生する。
本発明は、本発明による核酸を含む細胞も提供する。本発明は、本発明の第1の態様によるCARを細胞表面に発現する細胞を提供する。
本発明は、本発明の1つまたは複数のCAR発現細胞を薬学的に許容されるキャリア、希釈剤または賦形剤ならびに任意選択で1つまたは複数のさらなる薬学的に活性なポリペプチドおよび/または化合物と共に含有する医薬組成物にも関する。そのような製剤は、例えば静脈内注入のために好適な形態であってよい。
本発明のCAR発現細胞は、B細胞リンパ腫細胞などのがん細胞を死滅させることが可能であり得る。T細胞またはNK細胞などのCAR発現細胞は、患者自身の末梢血液から(第1のパーティー)、またはドナー末梢血液からの造血幹細胞移植の設定において(第2のパーティー)、または無関係のドナー由来の末梢血液から(第3のパーティー)のいずれかでex vivoで作製されてよい。代替的にCAR発現細胞は、誘導可能前駆細胞または胚性前駆細胞からT細胞などの細胞へのex vivo分化由来であってよい。これらの場合CAR細胞は、CARをコードするDNAまたはRNAをウイルスベクターでの形質導入、DNAまたはRNAのトランスフェクションを含む多数の手段の1つによって導入することによって生成される。
VHおよびVLのクローニングならびにCD19結合の実証
VHおよびVLをマウス抗CD19モノクローナル抗体からクローニングし、ヒトカッパ定常領域およびヒトIgG1定常領域にインフレームで融合した。これらのキメラ重鎖および軽鎖を発現ベクターにクローニングし、293T細胞にトランスフェクトするために使用した。次に産生された抗体をSupT1細胞(CD19陰性であるT細胞株)および、CD19陽性になるように操作されたSupT1細胞を染色するために使用した。この染色は、CD19の特異的結合を示している(図2)。
VH/VLがCD19を結合するscFvを形成できることの実証
次いで、クローニングされたVHおよびVLがscFvフォーマットにおいてCD19を結合できるかどうかを調査した。VHおよびVLをscFvとして2方向:VH−VLおよびVL−VHでクローニングし、ここで2個の可変領域は、(SGGGG)4からなるリンカーによって分けられた。これらのscFvを図3aに示すとおり切断型CD34と共発現される非シグナル伝達CARにクローニングした。簡潔にはこれは、シグナルペプチド、scFv、ヒトIgG1のヒンジCH2−CH3、CD8膜貫通ドメイン、CD8エンドドメインの最初の12残基、FMD−2AペプチドTeV、切断型ヒトCD34を含む。比較できるようにするために、fmc63由来scFvおよび別の抗CD19ハイブリドーマ4g7由来scFvを、VH−VLおよびVL−VHの両方向で同じフォーマットにクローニングした。

fmc36 CARに対するCAT19 CAR機能のin vitro比較
CAT scFvをHL方向で、Campana(Imaiら(2004年)Leuk. Off. J. Leuk. Soc, Am. Leuk, Res. Fund. UK 18巻:676〜684頁)によって設計されたCARスキャホールドにクローニングした。実際にfmc63 scFvをCAT scFvで置き換え、元のfmc63に基づくCARと比較した。このCARは、シグナルペプチド、scFv、CD8ストークスペーサーならびに膜貫通および41BBおよびゼータエンドドメインを含む。CAT CARおよびfmc63 CARのアミノ酸配列を下に示す:

このin vivoモデルについての実験設定の概要を図6(a)に示す。簡潔にはNSG(NOD scid gamma、NOD.Cg−Prkdcscid Il2rgtm1Wjl/SzJ)マウスは十分に免疫無防備状態であり、ヒト細胞株および初代ヒトT細胞の生着に許容的である。Raji細胞はバーキットリンパ腫由来のB細胞株である。これらの細胞は、NSGマウスの骨髄内に容易に生着し、高悪性度白血病様症候群を生じる。生物発光画像化(BLI)を使用した非侵襲性トラッキングを可能にするホタルルシフェラーゼを発現するようにRaji細胞を操作した。マウスに尾静脈注射を介してRaji.FLuc細胞2.5x105個を注射した。24時間後、NT初代ヒトT細胞またはfmc63 CARで形質導入したT細胞または、CAT19 CARで形質導入したT細胞のいずれかの4x106個を、尾静脈を介して投与した。腫瘍応答をBLIによって順次測定した。尾静脈血液を生着および血清サイトカインのために4日目に採取した。動物を11日目に選別し、CAR T細胞の持続性および腫瘍負荷について組織を研究した。さまざまなマウスコホートの10日目のBLI画像化を図6(b)に示す。広範囲の疾患がNT T細胞で処置したマウスの骨盤、脊椎、肋骨、頭蓋および脾臓において見られた一方で、わずかなシグナルがCAT19 CAR T細胞またはfmc63 CAR T細胞のいずれかを受けたマウスにおいて明らかである。さまざまなマウスコホートから経時的に平均化された定量的生物発光シグナルを対数目盛で図6(c)に示す。明白な差異がNT T細胞を受けたマウスとCAR T細胞を受けたマウスとの間のシグナル蓄積において見られる。シグナル蓄積の差異は、fmc63 CAR T細胞またはCAT19 CAR T細胞を受けたマウスにおいては見られない。最終的に屠殺後に各マウス由来骨髄のフローサイトメトリー分析を、腫瘍負荷を直接決定するために実施した。Raji細胞は、それらがヒトB細胞マーカーを発現することから、マウス造血細胞からおよび養子移入T細胞から容易に識別できる。わずかなRaji細胞がfmc63またはCAT19 CAR T細胞のいずれかを受けたマウスの骨髄において検出できた。
in vivo持続性CAR T細胞の特徴付け
上の動物モデルから、本発明者らは、両方の型のCAR T細胞がこれらのNSGマウスの骨髄および脾臓に生着したかを決定することを試みた。カウンティングビーズを用いた骨髄および脾臓のフローサイトメトリー分析は、CAR T細胞の絶対数の決定を可能にした。このデータを図7(a)および(b)に示す。fmc63 CAR T細胞またはCAT19 CAR T細胞で処置したマウス由来のマウスの脾臓におけるCAR T細胞の絶対数は同様であった。次に本発明者らは、これらのさまざまな組織中の疲弊T細胞の数に差異があるかどうかを決定することに進んだ。上の試料におけるPD1発現について共染色することによって、疲弊T細胞の数を決定できた。このデータを図7(c)および(d)に示す。驚くべきことに、fmc63 CARよりもCAT19 CARを用いた両方の組織コンパートメントにおいて、より少ない疲弊T細胞が存在した。

- 前記CD19結合ドメインが、scFvをVH−VL方向で含む、請求項1に記載のCAR。
- 前記CD19結合ドメインが、配列番号9として示される配列または少なくとも90%の配列同一性を有するそのバリアントを含む、請求項3に記載のCAR。
- 前記CD19結合ドメインが、ヒト抗体フレームワークにグラフトされた請求項1に定義された6個のCDRを含む、請求項1に記載のCAR。
- CD19結合ドメインおよび膜貫通ドメインが、スペーサーによって連結されている、前記請求項のいずれかに記載のCAR。
- 前記スペーサーが、以下:ヒトIgG1 Fcドメイン、IgG1ヒンジまたはCD8ストークの1つを含む、請求項6に記載のCAR。
- 前記スペーサーが、IgG1ヒンジまたはCD8ストークを含む、請求項7に記載のCAR。
- 細胞内T細胞シグナル伝達ドメインも含む、前記請求項のいずれかに記載のCAR。
- 前記細胞内T細胞シグナル伝達ドメインが、以下のエンドドメイン:CD28エンドドメイン、41BBエンドドメイン、OX40エンドドメインおよびCD3ゼータエンドドメインの1つまたは複数を含む、請求項9に記載のCAR。
- 前記細胞内T細胞シグナル伝達ドメインが、前記41BBエンドドメインおよび前記CD3ゼータエンドドメインを含む、請求項10に記載のCAR。

- 前記細胞内T細胞シグナル伝達ドメインが、以下のエンドドメイン:CD28エンドドメイン、OX40およびCD3ゼータエンドドメインのすべてを含む、請求項10に記載のCAR。

- 請求項19もしくは20に記載の細胞または請求項21に記載の細胞組成物を、薬学的に許容されるキャリア、希釈剤または賦形剤と共に含む医薬組成物。
- 請求項19もしくは請求項20に記載の細胞、請求項21に記載の細胞組成物または請求項24に記載の医薬組成物を被験体に投与するステップを含む、がんを処置するための方法。
- 前記被験体由来の細胞を、請求項18に記載のベクターでex vivoで形質導入するか、または請求項18に記載のベクターを該被験体由来の細胞にトランスフェクトし、次いでトランスフェクトした細胞を該被験体に戻し投与するステップを含む、請求項25に記載の方法。
