Abstract
Description
MDSC表現型
ヒト血中MDSCを、次のように、フローサイトメトリーにより特徴付け、分類した。すなわち、Mo−MDSCがCD11b+CD33+HLA−DRlow/−CD14+CD15−、および、PMNーMDSCがCD11b+CD33+HLA−DR−CD14−CD15+であった。ラットの血中および脾臓MDSCを、フローサイトメトリーによりCD11b+MHCクラスII−NKRP1intとして(Mo−MDSC、PMN−MDSCは共に、この表現型に含まれる)特徴付け、分類した。マウスMDSCは、次のように特徴付けた。すなわち、Mo−MDSCがCD11b+MHCクラスII−Ly6ChighLy6G−、および、PMNーMDSCがCD11b+MHCクラスII−Ly6C+Ly6G+であった。
MDSCを前述の表現型に従って、フローサイトメトリーにより精製した。フローサイトメトリーにより分類した細胞の純度は、99%超であった。次に、新鮮精製MDSCを、96ウェル平底マイクロタイタープレートに細胞50x103個/ウェルで播種し、RPMI−1640培地(2mM L−グルタミン、100U/mLペニシリン、0.1mg/mLストレプトマイシン、10%熱不活性化ウシ胎児血清、1%非必須アミノ酸、5mM HEPES、1mMピルビン酸ナトリウムおよび1μM 2−メルカプトエタノールを補充)中、無関連対照抗体または10μg/mLの抗SIRPaモノクロナール抗体(ヒトMDSC用クローンSE7C2(Santa Cruz Biotechnology)およびSE5A5(Biolegend)またはラットMDSC用クローンED9(AbD Serotec))と共に2日間培養した。別の状況で、組換えGM−CSF(10ng/mL)も添加し、強制的にマクロファージ/樹状細胞を分化させた。2日後、上清を採取し、37℃で2mM EDTAと共に5分間インキュベートして、細胞を取り出した。次に、細胞を蛍光抗体で染色し、フローサイトメトリーによりそれらの表現型を特徴付けた。併行して、細胞を培養培地に再懸濁し、T−リンパ球増殖に対するそれらの免疫抑制機能を評価した。
平底96−ウェルプレートを、抗CD3抗体でコーティングした(0.5μg/mL;37℃で2時間)。3回繰返しで、ラットの脾臓細胞を細胞50x103個/ウェルで播種し、RPMI−1640培地(2mM L−グルタミン、100U/mLペニシリン、0.1mg/mLストレプトマイシン、10%熱不活性化ウシ胎児血清、1%非必須アミノ酸、5mM HEPES、1mMピルビン酸ナトリウムおよび1μM 2−メルカプトエタノールを補充)中、2μg/mLの抗CD28モノクロナール抗体と共に3日間培養した。培養0日に、異なる比率の新鮮単離MDSCをこれらのポリクロナール活性化Tリンパ球培養液に添加した。培養0日から、対照抗体または抗SIRPa抗体(SE7C2、Santa Cruz Biotechnology(登録商標)inc.またはSE5A5、Biolegend(登録商標)を10μg/mLで添加した。他のプレートでは、対照または抗SIRPa抗体の存在下で48時間維持したMo−MDSCを洗浄し、抗体を取り出し、新たな抗体を添加せずに、同様の抑制アッセイで異なる比率で添加した。3日目に、0.5μCi[3H]チミジン/ウェルを添加して増殖を測定した。
7〜9週令の雄Lewis1W(RT1u)およびLewis1A(RT1a)コンジェニックラットを、Janvier(Savigny/orges,仏)から入手し、施設内のガイドラインに従って、特定の無病原体条件下の当方の動物施設で飼育した。既述(Dugast et al.,2008)のとおり、腎臓同種移植を実施した。一方の自然Lewis1W腎(右側)をLEW1Aで置換した。7日後、ドナー同種移植と対側腎切除を実施し、その後、レシピエントの生存は、前記同種移植片の適正な機能に頼った。これらのラットに、抗CD28抗体(ハイブリドーマJJ319)の0.3mg/日での7日間の腹腔内投与で治療し、これを移植当日に開始した。治療なしでは、移植から11日後に、移植片は拒絶された。100日以内に拒絶反応が起こらない場合、ラットは、免疫寛容ありと考えられた。次に、150日目に、免疫寛容レシピエントに、抗SIRPa抗体(クローンED9、AbDSerotec)500μgまたは無関連対照抗体(クロン3G8)を腹腔内投与した後、300μgを2回/週、21日間投与した。フローサイトメトリー分析用に血液サンプルを抜き取った。血清でクレアチニン血症および尿毒症をモニターした。クレアチニン血症または尿毒症いずれかが0日から2倍増加することで移植腎の機能障害が明らかになった時点で、ラットを屠殺した。
腎臓生検標本から凍結切片(10mm)を調製した。室温でアセトン固定を10分間行う前に、スライドを室温で1時間風乾した。切片を飽和溶液(5%ラット血清、2%正常ヤギ血清および4%BSA含有PBS)中で飽和し、必要な場合(すなわち、Foxp3染色)には、0.5%サポニンで透過処理した。切片を一次抗体と共に、続いて、蛍光二次抗体またはビオチニル化二次抗体により4℃で一晩インキュベートし、ABC Vectastainkit(Vector,Burlingame,CA)およびジアミノベンジリジン(DAB)で着色した後、エオジン−ヘマトキシリン染色した。標準または蛍光顕微鏡およびAxioVision画像ソフト(Carl Zeiss,Le Pecq,仏)を使ってスライドを分析した。
SYBR GreenおよびTaqMan PCRコア試薬を使ったApplied Biosystems Viia7システムで定量的リアルタイムPCRを実施した。RNAを、Trizol試薬中、腎臓組織均質液から抽出し、DNAおよびタンパク質からクロロホルムで分離し、イソプロパノールで沈殿させ、エタノールで洗浄し、乾燥後、無RNase水に再懸濁した。DNAse処理後、RNAを逆転写し、相補的DNA(cDNA)を得た。ハウスキーピング遺伝子HPRTと比較して、2(−dCt)法で相対遺伝子発現量を算出した。全サンプルを2回繰返しで分析した。対象遺伝子の発現量は、対照抗体(3G8)または抗SIRPa抗体(ED9)で治療された免疫寛容動物の間で比較した。
既述(Gauttier et al.,2014)のとおり、8週令C57B1/6J雄マウスが、腫瘍接種から4日および8日後、マウスに、ラット抗CD137 mAb(4−1BB mAb)100μgまたは抗マウスSIRPaモノクロナール抗体(クローンP84、Merck Millipore)300μg、または、両抗体、または、無関連対照抗体(クローン3G8)を3回/週、4週間(図17A)、または、抗PD−L1 mAb(クローン10F−9G2、BioXcell)200μgの腹腔内注射を行い、もしくは、両抗体(抗Sirpa+抗PDL1)を4週間与えた(図17B)。ソラフェニブ(Nexavar−Bayer)投与の治療マウスは、0日から28日まで、40mg/kgの強制経口投与100μLを毎日受けた(図10)。総生存率を分析した。白血球腫瘍浸潤を定量するため、また、PercollグラジェントおよびFACS分析により肝臓の非実質細胞を単離して特徴付けるために、腫瘍接種から14日後に、一部のマウスを屠殺した。
8週令C57B1/6J雄マウスが2x106個のB16−Ovaマウスメラノーマ細胞の脇腹への皮下注射を受けた。マウスは、腫瘍接種後0日から、300μgの無関連対照抗体(クローン3G8)または抗マウスSIRPaモノクロナール抗体(クローンP84)を3回/週、または、200μgの抗PD−L1 mAb(クローン10F−9G2、BioXCell)の2回/週腹腔内投与を受け、または、両抗体(抗Sirpaおよび抗PD−L1抗体)を4週間受けた。一部のマウスを、腫瘍接種から2週間目に屠殺し、フローサイトメトリーで腫瘍白血球浸潤を特徴付けた。総生存率を分析した。
MDSCは、SIRPaを発現する。
本発明者は、先に、ラットMDSC(単球および顆粒球)がその表面に高レベルのSIRPaを発現する、と述べた(Dugast et al.,2008)。ここで、図1は、マウスMo−MDSCおよびPMN−MDSCも、成熟骨髄性細胞(CD11b+クラスII+)と同様のレベルで、その表面にSIRPaを発現することを示している。対照的に、ヒトでは、SIRPaは、Mo−MDSCだけに成熟骨髄性細胞(CD11b+HLA−DR+)と同様のレベルで発現されるが、PMN−MDSCは、SIRPaを発現しない。
SIRPaが標的となり、そのCD47との相互作用に拮抗するモノクロナール抗体が、インビトロで、48時間に亘ってMo−MDSCの生存に影響しないことを、我々は初めて観察した。対照的に、Mo−MDSCは、新鮮単離Mo−MDSCや対照抗体と48時間の間培養したMo−MDSCと比較してその表現型を改変した(図2および3)。MDSCは、これまでに、マクロファージや腫瘍関連マクロファージ(TAM)、樹状細胞または顆粒球に分化すると述べられているが、ここでは、我々は、抗SIRPa mAb分化Mo−MDSCがこれらの表現型を示さないことを観察した。対照的に、GM−CSFの存在下でのインキュベーションは、既述の表現型への分化を誘導した(図4)。明らかに、ラット種では、抗SIRPa処理MDSCは、高レベルのMHCクラスII分子もCD68も獲得せず、代わりに、CD4マーカーの獲得を失った。ヒトでは、これらの細胞は、CD11c発現を失い、HLA−DRを獲得しなかった。予想外なことに、我々は、これらの細胞が高レベルのNK−特異性マーカー、特にCD161およびCD49b、ならびに、成熟細胞(CD44h,CD103,CD80,CD86)のマーカーを発現することを観察した。それらは、リンパ系の他のマーカー(CD25、CD28、CD2)も発現し、これらの細胞が抗SIRPaモノクロナール抗体によって(非骨髄性)成熟リンパ系細胞に分化されていることが確認された。さらに、抗SIRPaが非骨髄性細胞において分化を誘導することを確認するため、MDSCをGM−CSFおよび抗SIRPaモノクロナール抗体の両方と共に培養した。GM−CSF単独は、成熟MHCクラスII+骨髄性細胞(すなわち、樹状細胞およびマクロファージ)においてMDSCの分化を誘導するが、我々は、GM−CSFが、抗SIRPa抗体と結合すると、効果を示さず、抗SIRPa抗体が誘導する新規の分化経路を阻止しないことを観察した(図4)。これらの結果は、抗SIRPaモノクロナール抗体が、エフェクターNK様(CD161+)表現型を有する細胞へのMDSCの分化を誘導すること、これが、それらの従来の分化経路を圧倒することを実証した。最後に、抗SIRPaモノクロナール抗体の作用メカニズムを確認するために、初めに、MDSCの免疫抑制機能の無効化能を評価した。抗SIRPaモノクロナール抗体は、新鮮単離MDSCの抑制機能を改変しなかったが、我々は、MDSCが、インビトロで抗SIRPa抗体によって分化されたMDSCが、T−リンパ球増殖の抑制能を喪失してしまうことを認めた(図5)。要約すると、抗SIRPaモノクロナール抗体は、Mo−MDSCの非抑制エフェクターNK様リンパ系細胞への分化を誘導した。
我々は、先に、ラットにおいて抗CD28モノクロナール投与により誘導された同種移植腎が、Mo−MDSCの蓄積によって維持される、と述べた(Dilek et al.,2012;Dugast et al.,2008)。抗SIRPaモノクロナール抗体が、マクロファージ貪食作用の改善によるその腫瘍除去への効果とは無関係に、MDSC持続性免疫抑制を無効化できることを確認するために、免疫寛容同種移植腎レシピエントを抗SIRPaモノクロナール抗体または無関連対照抗体で治療した。免疫寛容レシピエントが抗SIRPa投与から2〜3ヶ月以内に同種移植片を拒絶することを認めたが、対照抗体によると、移植片の機能は安定を保った(図6)。これらの動物の末梢血免疫表現型決定を分析すると、我々のインビトロの観察が確認された。平均10日間の抗SIRPa抗体での治療後、MDSCの有意な減少を我々は観察したので、これらの動物の末梢血免疫表現型決定分析は、我々のインビトロ観察を確認した。同様に、NK細胞は、SIRPaを発現しなかったが、平均10日間の抗SIRPa抗体での治療後、NK系(CD161+)細胞の有意な増加を認めた。外植片の組織検査は、T−リンパ球介在の予想される急性細胞拒絶反応を認めなかった。対照的に、免疫寛容レシピエントの移植片に元々存在するT−リンパ球浸潤が、抗SIRPa抗体投与後、拒絶反応動物の移植片ではるかに顕著でなくなることを、我々は認めた(図8)。このモデルの末梢MDSC蓄積が、移植片調節性T細胞の蓄積と関連することを、我々は先に述べた(Dilek et al.,2012)。本明細書に、我々は、調節性T細胞は抗SIRPa投与レシピエントの移植片ではほとんど検出不能であるので、抗SIRPa抗体治療がこれらの細胞を間接的にも調節する、と述べた(図9)。さらに、骨髄性(CD11b/c)細胞浸潤は、群間で同様であったが、マクロファージなどの成熟骨髄性細胞は、抗SIRPa投与レシピエントの移植片で一層豊富であった。さらに重要な点として、NK(CD161+)細胞は、免疫寛容レシピエントの移植片ではほとんど検出不能であったが、我々は、抗SIRPa投与レシピエントに有意な移植片浸潤を観察し、インビトロ試験およびインビボの末梢観察において、抗SIRPa抗体がMDSCとNK細胞の両方を調節することを確認した。
肝細胞癌およびメラノーマのマウスモデルに抗SIRPaモノクロナール抗体を投与した。肝細胞癌マウスモデル(Hepa1.6)は、肝臓への腫瘍細胞系接種から2週間以内に死亡を誘導する侵襲性癌モデルである。このモデルでは、承認済みケアの化学療法標準品(例、ソラフェニブ)は、マウスの平均60%を救済した(図10)。この厳密なモデルで、単剤療法における抗SIRPaモノクロナール抗体治療は、マウスを著しく保護し、ソラフェニブと同様な効果を有した。興味深いことに、2週間の抗SIRPa抗体による治療後、腫瘍浸潤が著しく増強された(特にT−リンパ球)が、我々は、骨髄性細胞内でのMDSCの有意な減少を認めた(図11c)。さらに、対照マウス(図12)に比較して、これらの腫瘍へのNK(CD161+)細胞浸潤、特に成熟NK細胞(CD11b+CD27−)の蓄積の有意な増加も認めた(図13)。事実、CD27+CD11b−表現型が細胞を傷害できず、低レベルサイトカインを産生する未成熟NK細胞に相当し、CD11b+CD27+表現型がサイトカインを産生するが、細胞傷害性が低いエフェクターNK細胞に相当し、CD11b+CD27−表現型が成熟した、細胞傷害性の高いNK細胞に相当することは既述された(Desbois et al.,2012)。別の腫瘍モデルでこれらの結果を確認するために、B16メラノーママウスモデルを使用した。同様に、抗SIRPaモノクロナール抗体で2週間治療されたマウスの腫瘍から抽出した白血球もMo−MDSCの腫瘍内減少(図15B)およびNK(CD161+)細胞の蓄積も示した(無関連抗体条件の細胞の4.23%から抗Sirpa治療動物の12.4%(図14))。図15Aは、4週間の間、メラノーマを接種され、抗PD−L1で治療された、または、抗SIRPaで治療された、または、両方で治療された動物の総生存率を表す。単一分子の治療に比較して、複合抗体は、相乗効果を示した。
図17Aは、肝臓癌を接種され、4週間の間、抗CD137、抗Sirpaまたは両方で治療された動物の総生存率を表す。抗Sirpa治療動物の30%は、接種後20日超生存した。この結果は、動物が抗CD137抗体を受けた場合に得られた結果に匹敵する。興味深いことに、抗Sirp+抗CD137の複合抗体を受けた動物は100%が生存した。これは、各分子単独で得られる結果に比較して、2分子の強力な相乗効果を示すものである。

Claims (19)
- 骨髄由来抑制細胞(MDSC)の非抑制細胞への分化によって改善または予防できる任意の病態の治療に使用するための、シグナル調節タンパク質アルファ(SIRPa)とそのリガンドのうちの少なくとも1つとの間の相互作用を遮断する化合物。
- 前記化合物がSIRPaとCD47との間の相互作用を遮断する、請求項1に記載の使用のための請求項1に記載の化合物。
- 前記化合物がポリペプチドである、請求項1に記載の使用のための請求項1または2に記載の化合物。
- 前記化合物がペプチド拮抗剤である、請求項1に記載の使用のための請求項1〜3のいずれか1項に記載の化合物。
- 前記化合物が抗体である、請求項1に記載の使用のための請求項1〜4のいずれか1項に記載の化合物。
- 前記化合物が抗SIRPa抗体である、請求項1に記載の使用のための請求項1〜5のいずれか1項に記載の化合物。
- 前記MDSCが単球MDSC(Mo−MDSC)である、請求項1に記載の使用のための請求項1〜6のいずれか1項に記載の化合物。
- 前記非抑制細胞が非抑制リンパ系細胞である、請求項1または7に記載の使用のための請求項1〜6のいずれか1項に記載の化合物。
- 前記非抑制細胞が、MHCクラスIIに陰性で、ナチュラルキラー(NK)細胞の少なくとも1つのマーカーに陽性である、請求項1、7または8に記載の使用のための請求項1〜6のいずれか1項に記載の化合物。
- 前記非抑制細胞が、エフェクターリンパ系細胞である、請求項1および7〜9のいずれか1項に記載の使用のための請求項1〜6のいずれか1項に記載の化合物。
- 前記病態が癌、感染症、外傷、自己免疫疾患、ワクチン接種、慢性炎症疾患または移植片機能障害である、請求項1および7〜10のいずれか1項に記載の使用のための請求項1〜6のいずれか1項に記載の化合物。
- 前記病態が固形癌である、請求項1および7〜11のいずれか1項に記載の使用のための請求項1〜6のいずれか1項に記載の化合物。
- 前記病態が転移癌である、請求項1および7〜12のいずれか1項に記載の使用のための請求項1〜6のいずれか1項に記載の化合物。
- 前記化合物が第2治療薬に併用される、請求項1および7〜13のいずれか1項に記載の使用のための請求項1〜6のいずれか1項に記載の化合物。
- 前記第2治療薬が化学療法剤、放射線療法、手術、免疫療法剤、抗生物質およびプロバイオティックスから成る群から選択される、請求項14に記載の使用のための請求項1〜6のいずれか1項に記載の化合物。
- 前記第2治療薬が治療ワクチンおよび免疫チェックポイント遮断剤または賦活剤から成る群から選択される免疫療法剤である、請求項15に記載の使用のための請求項1〜6のいずれか1項に記載の化合物。
- 前記第2治療薬が抗PDL1、抗PD1、抗CTLA4および抗CD137から成る群から選択される免疫チェックポイント遮断剤または賦活剤である、請求項16に記載の使用のための請求項1〜6のいずれか1項に記載の化合物。
- 請求項1〜6のいずれか1項に記載の化合物による治療効果を決定する方法であって、前記化合物によって治療される患者からのサンプル中で、CD161に陽性であり、HLA−DRに陰性である非抑制細胞の存在を測定することを含む、方法。
- 前記サンプルが血液サンプル、組織サンプル、腫瘍からのサンプル、または滑液サンプルである、請求項18に記載の方法。
