Abstract
Description

図6〜29は、顎部(42、44)が取る場合がある代替的形態の様々な実施例を示す。以下に説明する様々な実施例が器具(10)に容易に組み込まれる場合があり、また2極式RFエネルギーを組織に印加する場合があることを理解されたい。図6は、長手方向に延在するスロット(150)を有する例示的な顎部(142)を特に示す。本実施例では、スロット(150)は顎部(142)の中央部を長手方向に延在する。インサート(152)をスロット(150)内に摺動することによって、インサート(152)は顎部(142)に摺動可能に受容される。図7は、スロット(150)内に配置されたインサート(152)を備える顎部(142)を示す。上顎部(144)も、下顎部(142)と同様のスロット(151)を備える。第2のインサート(154)は、スロット(151)に適合するように構成される。図7に示すように、スロット(150、151)内に配置されたインサート(152、154)は、インサート(152、154)の間に締め付けられた組織(160)が斜方向に圧縮されるように成形及び配向される。本実施例では、インサート(152、154)は、顎部(142、144)に適合するダブテールを形成し、顎部(142、144)の後部から顎部(142、144)内に摺動可能である。当然ながら、ダブテール構成に対する任意の他の好適な代替を用いることもでき、かつ/又は所望の場合はインサート(152、154)を顎部(142、144)の遠位端から摺動して入れることもできる。
場合によっては、様々な顎部(42、44)の変形を介した締め付け圧力を用いて組織を切断する代わりに、移動式ナイフなどの代替的切断機構を用いて組織を切断する場合がある。図16〜17は、インサート(752、754)及びIビーム型ナイフ(770)を有する顎部(742、744)の例示的異形を示す。本実施例のインサート(752、754)は、本実施例のインサート(752、754)がそれぞれ「T」字型スロット(757、759)を画定することを除けば上記のインサート(652、654)と実質的に同様である。インサート(752)はプラスチック製電気絶縁性部分(753)、及び正温度係数(PTC)サーミスタポリマー部分(756)を含む。インサート(754)はプラスチック製電気絶縁性部分(755)及びPTCサーミスタポリマー部分(758)を含む。PTCサーミスタポリマー部分(756、758)が、プラスチック製電気絶縁性部分(753、755)に隣接した顎部(742、744)の「T」字型スロット(750、751)のそれぞれの中に部分的に延在する。
上述した顎部(42、44)の例示的変形は、顎部(42、44)の間に捕捉された組織を溶着する/封止する/凝固させるように操作可能である。場合によっては、顎部(42、44)の間に捕捉されていない組織を溶着する/封止する/凝固させることが望ましい場合がある。例えば、顎部(42、44)の何らかの異形を用いて組織を切断した後、Iビーム型ナイフ(770)を用いて組織を切断した後、及び/又は何らかの他の機構を用いて組織を切断した、若しくは他の方法で組織に処置を施した後、組織の一部分が出血し続ける場合がある。したがって、組織の出血している部分を顎部(42、44)の間に把持する必要なしに、顎部(42、44)のうちの1つ又は両方の外側部分をかかる組織の出血部分に押し付けて2極式RFエネルギーを印加して組織を封止する/凝固させることが望ましい場合がある。図20〜22は、2極式RFエネルギーを組織に印加するように操作可能である外側部分を有する単に例示に過ぎない顎部(42、44)の変形を示す。これらの実施例を以下でより詳細に説明するが、その他の実施例は本明細書における教示を考慮すれば当業者に明らかとなるであろう。
場合によっては、顎部の長さに沿って交互に配置された歯部を備える顎部を有することが望ましい場合がある。かかる構成は、顎部の間で組織を把持及び保持する能力を高める場合がある。かかる構成は、組織を溶着する/封止する/凝固させる能力も高める場合がある。更に又はあるいは、かかる構成は、ハンドル(12、14)を絞って組織を切断するのに必要な力を減少させることなどのように、顎部間の組織の切断を容易にする場合がある。図24〜26は、顎部(1142、1144)の長さに沿って交互に配置された歯部(1152、1154)を有する顎部(1142、1144)の例示的変形を示す。特に、歯部(1152)は、下顎部(1142)の中心に沿って延在する長手方向線の対向する側に非対称に配置される。歯部(1154)は、上顎部(1142)の中心に沿って延在する長手方向線の対向する側に非対称に配置される。一連の凹部(1156)が下顎部(1142)に沿って歯部(1152)を分離して格子型の模様を作り、一方で類似する一連の凹部(1158)は上顎部(1144)に沿って歯部(1154)を分離して、これも格子型の模様を作る。

図1〜3に関して上述したように、顎部(42、44)の間に締め付けられた組織にRFエネルギーを伝達するために、単一のボタン(16)を用いて、電極表面を選択的に活性化する場合がある。一部のかかる異形では、ボタン(16)を作動をしたとき、単一のボタン(16)は、RFエネルギーを一組の作業パラメータ(例えば周波数、振幅など)で単にオンにする。ボタン(16)が解放されると、RFエネルギーは単にオフになる。場合によっては、実施される特定の作業に基づいて、RFエネルギーに2組以上のパラメータを提供することが望ましい場合がある。例えば、組織溶着作業は、RFエネルギーパラメータの第1の組み合わせ(例えばより低い振幅など)を必要とする場合があり、一方で組織切断作業はRFエネルギーパラメータの第2の組み合わせ(例えばより高い振幅など)を必要とする場合がある。そのため、RFエネルギーを当面の作業に最も適したエネルギーパラメータの組み合わせで選択的に活性化するように操作可能である、1つ以上の使用者入力機構を提供することが望ましい場合がある。かかる使用者入力機構の例示目的に過ぎない幾つかの実施例を以下でより詳細に説明するが、更なる実施例は本明細書における教示を考慮すれば当業者に明らかとなるであろう。以下の教示は上記の教示のいずれとも様々な順列で容易に組み合わせることが可能であることを理解されたい。

(12) 前記第1の顎部の遠位端に歯部を更に備え、前記第2の顎部が前記歯部を受容するように構成されたネストを備える、実施態様11に記載の機器。
(13) 前記第1の顎部の外側部が、鈍的切開を実施するように構成された少なくとも1つの突出部を備える、実施態様1に記載の機器。
(14) 移動式切断部材を更に備え、前記移動式切断部材が前記第1の顎部と前記第2の顎部との間で締め付けられた組織を切断するように操作可能である、実施態様1に記載の機器。
(15) 切断部材ロックアウト機構を更に備え、前記切断部材ロックアウト機構が前記第1の電極表面及び前記第2の電極表面の活性化状態に基づいて前記切断部材の移動を選択的に防止するように操作可能である、実施態様14に記載の機器。
(17) 前記旋回部材と一体化した活性化ボタンを更に備え、前記活性化ボタンが2極式RFエネルギーで前記第1の電極表面及び前記第2の電極表面を活性化するように操作可能である、実施態様16に記載の機器。
(18) 機器であって、
(a)第1の顎部であって、前記第1の顎部が第1の電極表面を含む、第1の顎部と、
(b)前記第1の顎部と旋回可能に連結された第2の顎部であって、前記第2の顎部が第2の電極表面を備える、第2の顎部と、
(c)前記第1の顎部及び前記第2の顎部を通って平行移動するように構成されたナイフと、
(d)前記第1の顎部及び前記第2の顎部と連絡しているハンドル部分であって、前記ハンドル部分が前記第1の顎部を前記第2の顎部に対して閉じるように操作可能である、ハンドル部分と、
(e)前記ハンドル部分上に配置されたトリガであって、前記トリガが前記第1の顎部及び前記第2の顎部に通電するように構成され、前記トリガが前記第1の顎部及び前記第2の顎部を通して前記ナイフを前進させるように更に構成された、トリガと、
(f)前記顎部の通電状態に基づいて前記ナイフの前進を選択的に防止するように操作可能なロックアウト機構と、を備える機器。
(19) 前記ロックアウト機構が前記ナイフと関連付けられたノッチを備え、前記トリガが前記ノッチと係合するようにばね付勢され、それによって前記ナイフの前進を防止する、実施態様18に記載の機器。
(20) 機器であって、
(a)第1の顎部であって、前記第1の顎部が導電性部分及び絶縁性インサートを含み、前記絶縁性インサートが前記導電性部分の一部を露出させて第1の電極表面を提供するように構成された、第1の顎部と、
(b)前記第1の顎部と旋回可能に連結した第2の顎部であって、前記第1の顎部が導電性部分及び絶縁性インサートを含み、前記絶縁性インサートが前記導電性部分の一部を露出させて第2の電極表面を提供するように構成され、前記第1の顎部の絶縁性表面が前記第2の電極表面と接触するように配置され、前記第2の顎部の絶縁性表面が前記第1の電極表面と接触するように配置される、第2の顎部と、
(c)前記顎部を閉鎖位置に運んで組織を把持させるように操作可能なハンドルアセンブリと、を備える機器。
Claims (2)
- 機器であって、
(a)第1の顎部であって、前記第1の顎部が第1の電極表面を含む、第1の顎部と、
(b)前記第1の顎部と旋回可能に連結された第2の顎部であって、前記第2の顎部が第2の電極表面を備える、第2の顎部と、
(c)前記第1の顎部及び前記第2の顎部を通って平行移動するように構成されたナイフと、
(d)前記第1の顎部及び前記第2の顎部と連絡しているハンドル部分であって、前記ハンドル部分が前記第1の顎部を前記第2の顎部に対して閉じるように操作可能である、ハンドル部分と、
(e)前記ハンドル部分上に配置されたトリガであって、前記トリガが前記第1の顎部及び前記第2の顎部に通電するように構成され、前記トリガが前記第1の顎部及び前記第2の顎部を通して前記ナイフを前進させるように更に構成された、トリガと、
(f)前記顎部の通電状態に基づいて前記ナイフの前進を選択的に防止するように操作可能なロックアウト機構と、を備える機器。 - 前記ロックアウト機構が前記ナイフと関連付けられたノッチを備え、前記トリガが前記ノッチと係合するようにばね付勢され、それによって前記ナイフの前進を防止する、請求項1に記載の機器。
