担体:Amberchrom CG161c(ポリスチレンジビニルベンゼンコポリマ、Dow Chemical Company)、平均粒径120μm、平均細孔径15nm;アクセス可能な表面はポリマ(乾燥)1g当たり900m2。湿潤担体1ml当たりの乾燥重量は18%(w/v)。
ポリミキシンB(PMB):硫酸ポリミキシンB(Sigma Aldrich)
Albuflow(登録商標)フィルタ(ドイツ国Fresenius Medical Care社製)を用いて新鮮凍結血漿(クエン酸血漿)を分画化し、全血漿と共に−20℃で凍結した。

コンディショニングされた担体(Amberchrom CG161c:エチルビニルベンゼンジビニルベンゼンコポリマ(Dow Chemical Company、平均粒径120μm、平均細孔径15nm)に、様々な量のポリミキシンB(PMB)すなわち湿潤担体1g当たり0、5、10、15、および25mgを被覆する。これらをエンドトキシンバッチ試験でトリプリケート法により血清中のLPSの不活化について試験する。
PMBコーティング
様々なPMB濃度(湿潤担体1g当たり5mg、10mg、15mg、および25mg)の担体試料を作成する(上記実施例1のプロトコール参照)。PMB溶液(10mg/ml蒸留水)および50%懸濁液中の担体を121℃で30分間オートクレーブし、15mlのグライナーチューブで担体に次の通りPMBを被覆する(表2.2)。
8mlを測定する7本の採血管(凝固活性化用の血清ビーズ入りvacuette)をドナーから取り外す。血液が充填された管を30分間静置する。次いで凝固した血液を遠心分離して血清を得る(無菌三角フラスコ内で冷却する)。
エンドトキシン(LPS)溶液:
LPS:緑膿菌、L‐7018、Sigma社バッチ:128K4115、−70℃で保存、10-3g/ml(1mg/ml)で100μl
最低濃度の被覆PMBでも50%を超えるLPS不活化を達成することができた。結果を図9に示す(EU=エンドトキシン単位)。
3.1.目的
この試験の目的は、血漿中のポリミキシンB(PMB)濃度依存エンドトキシン活性化を決定することである(バッチ試験I)。さらに、このエンドトキシン活性化の結果サイトカイン分布が阻害される程度を調査する(バッチ試験II)。
5IUのヘパリンを添加した9本の血液試料採取管(各管が9mlを測定)をドナーから取り外す。血漿を遠心分離し、細胞ペレットをロールミキサで培養する。血漿にエンドトキシン(LPS)を添加し、バッチ試験Iに使用する:
LPS:緑膿菌(L‐7018、Sigma社バッチ:128K4115、−70℃、10-3g/ml(1mg/ml)で100μl)
LPS:大腸菌(L‐4130、Sigma社バッチ:110M4086M、?70℃、10-3g/ml(1mg/m)で100μl)
Charles Riverによるリムルス・アメボサイト・ライセート(Limulus Amebocyte Lysate)試験(LAL)を用いて、エンドトキシンをEU/mlの形式で測定した。
バッチ試験Iの後、LPSおよびPMBを添加した血漿を、比率1:1で血液ドナーから得た細胞濃縮液に戻す(表3.5参照)。サイトカインバッチ、バッチ試験Iからの試料を0(PMB無し)、250、500、および1000ng/mlのPMB濃度で使用した。対照として、LPSを含まず、1000ng/mlのPMBを含む試料を使用した。ロールミキサ(5回転/分)における37℃での4時間および12時間の培養時間後に、試料を採取し、遠心分離し、その後のサイトカイン定量化のために、50μlの血漿を−80℃で凍結した。サイトカインバッチのための試験データを表3.5に記載する。
エンドトキシンバッチ(バッチ試験I):
図10は、60分の培養後のPMB濃度(n=2)による血漿中の大腸菌由来のLPS(原LPS濃度:0.5ng/ml)の阻害を示す。
4時間培養後のLPS(大腸菌)添加血漿中のPMB濃度(PMB無し、250ng/ml、500ng/ml、および1000ng/ml;対照は1,000ng/mlでLPSを含まない)毎の血液細胞によるサイトカインTNF‐アルファ(図12)、IL‐1ベータ(図13)、IL‐6(図14)、およびIL‐8(図15)の分布を図3ないし図4に示す。バッチ試験IIの結果は、非常に低いPMB濃度でも、LPSの強い阻害(バッチ試験I参照)だけでなく、その後もサイトカイン分布の強い阻害が起きることをはっきりと示している。これは、重要なメディエータであるTNF‐アルファの阻害の場合に特に顕著である(図12)。
4.1 PMB血清中濃度100ng/ml血漿のボーラス投与
前提: 体重70kgで体重の60%がPMBの分布量の患者→42000ml分布量。
血漿1ml当たりPMB100ngのPMB血清中濃度が求められる→全部で4.2mgのPMBが必要である。
前提: 体重70kgで体重の60%がPMBの分布量の患者→42000ml分布量。
血漿1ml当たりPMB250ngのPMB血清中濃度が求められる→全部で10.5mgのPMBが必要である。
ボーラス投与により所望のPMB血清中濃度がセットされるやいなや、これは、上述の通り、本発明に係る灌流装置に関連付けられる分注手段を用いてPMB放出により維持される。
前提: 体重70kgの患者→PMBの分布量(体重の60%)、体液42000mlにPMB100ng/ml→分布量に4.2mgのPMB(2.1.1参照)。
以下の計算実施例は、PMBの患者クリアランスの他に、体外血液回路に配置された透析器(透析用フィルタ)のクリアランスおよび枯渇剤の担体のクリアランスをも考慮に入れている。計算実施例は既存のPMBの血清中濃度を前提とする。これは処置の開始前のボーラスの投与によってもたらされ、実施例4で記載した注射液をこの目的に使用することができる。
―PMBの透析クリアランス(CDial)は実験で決定することができ、血漿流量 および使用する透析用フィルタの種類にも依存する。特定の実施例では、これは60m l/分である。
―枯渇剤のPMBクリアランス(Cads)は使用する担体材料および濾液流量にも 依存する。特定の実施例ではこれは45ml/分である。
―PMBの患者クリアランスは、特定の実施例で、13.6のPMBの半減期から決 定された。それは36ml/分である。
=>透析および吸着による処置中、PMB0.84mg/時
6時間の体外処置により、以下の注入しなければならないPMB量がここから得られる。
6時間の透析および吸着による処置: 5.1mg
7.1 サイトカイン吸着バッチ試験
試験の説明:
異なる細孔径(30nmおよび15-20nm)の吸着体を、全血漿および分画血漿中のTNFα、IL‐6、およびIL‐10の吸着に関して試験する。
担体:ポリスチレンジビニルベンゼンコポリマ、CG300c(担体A)、CG161c(担体B)、Dow Chemical Group
担体A:粒径:120μm、細孔径:30nm
担体B:粒径:120μm、細孔径:15〜20nm
血漿:深温凍結された非分画クエン酸血漿(血液の遠心分離によって得られた新鮮凍結血漿)
分画クエン酸血漿:Albuflowフィルタ(ドイツ国Fresenius Medical Care)の使用により得られる。
下の表7.1によるサイトカイン添加(TNF‐α、IL‐6、IL‐10)。
R&D Systems社製の市販のELISAを用いてTNF‐α、IL‐6、およびIL‐10を定量した。
分画血漿における改善されたサイトカイン吸着を決定することができた。
同一の平均細孔径(15〜20nm)を持つが、3〜5μm、35μm、75μm、および120μmの異なる平均粒径を持つエチレンビニルベンゼンジビニルベンゼンコポリマ(担体)を、タンパク質Cに対する吸着特性に関して比較した。
この実施例で使用する同一平均細孔径および異なる平均粒径を持つエチレンビニルベンゼンジビニルベンゼンコポリマ(Amberchrom CG161、Rohm&Haas/Dow Chemical Company)を表8.1に記載する。
表8.1に記載された担体#2000、#1785、#1760、および#2004をバッチ試験で、タンパク質Cに対するそれらの吸着特性に関して試験し、相互に比較した。
図20は、個々の担体について時間の経過によるタンパク質Cの濃度(ヒト血漿中の生理的タンパク質C濃度に対する[%]単位で表記)を示す。曲線に基づいて、平均粒径に対するタンパク質C吸着の依存性は明確である。担体#2000および#1785では顕著なタンパク質Cの吸着が決定された。担体#2000の場合、タンパク質Cはわずか15分後に血漿から略完全に除去された。対照的に、担体#1760および#2004による血漿からのタンパク質Cの吸着の程度はずっと小さかった。担体#1760(60分の培養後)に観察された〜25%のタンパク質Cの減少は依然として、生理的関連量のタンパク質Cが血漿に残る範囲である。60分の培養後に、タンパク質Cの開始濃度に対しわずか8%という最低のタンパク質Cの吸着は、担体#2004に対して決定された。個々の担体によるタンパク質Cの吸着(タンパク質Cの開始濃度に対する%で表す)を表8.3に記載する。
9.1 担体
CG161c:
担体(Rohm&Haas/Dow Chemical Company;本書では以下、吸着体ともいう)は多孔性ポリスチレンジビニルベンゼンマトリックスから構成される。平均細孔径は15nmであり、平均粒径は120μmであり、アクセス可能な表面は吸着体(乾燥)1gにつき900m2である。湿潤吸着体1ml当たりの乾燥重量は18%(w/v)である。
担体(Rohm&Haas/Dow Chemical Company;本書では以下、吸着体ともいう)は、多孔性ポリスチレンジビニルベンゼンマトリックスから構成される。平均細孔径は30〜40nmであり、平均粒径は10μmであり、アクセス可能な表面は吸着体(乾燥)1gにつき500m2である。湿潤担体1ml当たりの乾燥重量は30%(w/v)である。
PMB溶液(Sigma Aldrich、10mg/ml蒸留水)を121℃で30分間オートクレーブし、次いで15mlのグライナーチューブ内でそれぞれの担体(CG161cまたはHPR10)にPMBを以下(表9.2)の通り、すなわち3mlの担体に7.5mlのPMB溶液を用いて、被覆させる。
デュプリケート法で、Enviro‐genie内で0.5mlの担体懸濁液を各々4.5mlクエン酸血漿=10%(v/v)を用いて37℃で60分培養する。次いで担体を遠心分離し、上澄みを使用してELISAによりPMBを定量する(中国Beijing Kwinbon Biotechnology Co.,Ltd.社製ポリミキシンELISA)。
結果(図21、図22、図23、および図24参照)から、血漿中のポリミキシンの脱着率は利用可能な担体表面に非常に大きく依存することが明らかである。これは、ポリミキシンの脱着が、1m2当たりの疎水結合ポリミキシンの量に依存することを意味する。これは、以下の計算表(表9.4.1および表9.4.2)からも明らかである。
担体(以下、吸着体ともいう)からのPMBの脱着による処置中の血漿中のPMB濃度を正確に決定するために、それぞれの吸着体についてインビトロ脱着実験(実施例9で実施した通り)を行う必要がある。実験で得たデータに基づいて、担体のコーティングの程度によって(吸着体1g当たりのPMBの量)によって、血漿中の脱着およびしたがって血漿中のPMB濃度を非常に正確に調整することが可能である(図21ないし図24参照)。実施例1に示す通り、分画血漿中の脱着率はより低くすることができ、したがって別々に決定することができる。
体外血液回路でPMB被覆吸着体HPR10を使用することによって、0.8μg/mlの血漿中PMB濃度を得る。予備試験(図23参照)のおかげで、吸着体1g当たりの被覆PMBの量と血漿中の脱着PMBの量との間の相関を記述する関数を実験によって決定することができる。この場合、それは次の通りである。

体外血液回路でPMB被覆吸着体CG161cを使用することによって、0.8μg/mlの血漿中PMB濃度を得る。予備試験(図24参照)のおかげで、吸着体1g当たりの被覆PMBの量と分画血漿中の脱着PMBの量との間の相関を記述する関数を実験によって決定することができる。この場合、それは次の通りである。

この試験は、血漿中の平衡反応(ポリミキシン(B)の吸着および脱着)が迅速かつ安定的であることを実証するように意図されている。
10.1 担体
HPR10: 担体HPR10(Rohm&Haas/Dow Chemical Company;以下、吸着体ともいう)は、多孔性ポリスチレンジビニルベンゼンマトリックスから構成される。平均細孔径は30〜40nmであり、平均粒径は10μmであり、アクセス可能な表面は担体(乾燥)1gにつき500m2である。湿潤担体1ml当たりの乾燥重量は30%(w/v)である。
PMB溶液(Sigma、10mg/ml蒸留水)を121℃で30分間オートクレーブし、次いで15mlのグライナーチューブ内で担体(HPR10)にPMBを以下(表10.2)の通り、すなわち3mlの担体に7.5mlのPMB溶液を用いて、被覆させる。
5IUのヘパリン入り血漿に5ng/mlのLPS(L‐7018緑膿菌、Sigma社、バッチ:128K4115)を添加する。トリプリケート法で、1%PMB被覆担体をLPS添加血漿と共に(30μlの担体+2970μlのLPS添加血漿)、オーバヘッドシェーカ内で37℃で培養し、次いでLAL分析用の試料を(5分、15分、および60分)間隔で採取した。
血漿中の脱着PMBの平衡濃度に非常に迅速に到達する。5分後のLPS不活化は培養60分後と略同一である(図25参照)。
11.1 試験の説明
非被覆吸着体CG161cと比較してPMB被覆吸着体CG161cがサイトカインの吸着に適している程度を、10%(v/v)バッチ試験で試験した。緑膿菌由来の5ng/mlエンドトキシン(LPS)も追加した。
担体:Amberchrom CG161(平均粒径120μm、平均細孔径15nm)
PMBによるコーティング:
PMB溶液(Sigma Aldrich、10mg/ml蒸留水)および50%懸濁液中の担体を15mlのグライナーチューブ内でPMBで次の通り(表11.2.1)被覆する。
トリプリケート法:各々の場合に1mlの吸着体(湿潤)+9mlのスパイク
15mlのグライナーチューブをEnviro‐Genieで25/50rpm、37℃で、60分間振動させる。
保存液を1:10で血漿(新鮮な凍結血漿、血漿ドナーセンターRetz)中に希釈する(1:10;保存液5μL+血漿45μL)。使用したサイトカインに対する血漿スパイク(100mL)の最終濃度を下の表11.2.2に示す。
緑膿菌:L‐7018、Sigma社、バッチ:128K4115、−70℃、10-3g/ml(1mg/ml)で100μl。
LPSを5ng/mlの最終濃度でバッチに使用する。
→100mlの血漿中に50μlの10-5溶液(表11.2.3および表11.2.4参照)
Biorad社製のLuminex装置(抗体ベース)を用いてサイトカイン分析を実行する。
結果を図26に示す。この図から、ポリミキシンBによる担体表面の吸着性コーティングがサイトカインの吸着に影響しないことが明瞭に分かる。
