代謝障害、例えば2型糖尿病、肥満およびすべての関連する合併症は、主要な死因である。これらの障害は、過剰な栄養摂取および運動不足という欧米的生活様式に関連しており、これは世界のその他の地域でも増えつつある。2型糖尿病は、高い循環中の血糖、インスリンおよびコルチコステロイドレベルにより特徴付けられる消耗性疾患である。全世界を通じて、2型糖尿病の発病率は高くかつ伸びており、主要な死因、病因および医療費支出用途になりつつある(Amos et al., Diabetic Med. 14:S1-85, 1997(非特許文献1))。糖尿病(およびインスリン抵抗性状態)は、血中のブドウ糖レベルを上昇させる。長期間の高血糖は、血管および神経の損傷を引き起こし得る。
I. 序論
本明細書に提供されるのは、FGF-1およびその機能性バリアントを用いて代謝障害を処置するのに有用な方法および組成物である。本発明者らは、FGF-1が、血糖の正常化、インスリン感受性の増進、体脂肪率および総体重の減少、除脂肪率の増加ならびに脂肪肝(肝臓脂肪変性)の軽減を含む、迅速かつ持続的な効果を有することを示した。

線維芽細胞増殖因子(FGF)は、発達および成体組織の広範囲で発現される特別なポリペプチドホルモンのファミリーである(Baird et al., Cancer Cells, 3:239-243, 1991)。FGFは、血管新生、発達、細胞分裂促進、パターン形成、細胞増殖、細胞分化、代謝調節および組織損傷の修復を含む多くの生理学的機能において重要な役割を果たしている(McKeehan et al., Prog. Nucleic Acid Res. Mol. Biol. 59:135-176, 1998; Beenken and Mohammadi, 2009)。FGFファミリーは、現時点で、FGF-1からFGF-23の少なくとも23のメンバーで構成されている(Reuss et al., Cell Tissue Res. 313:139-157 (2003))。
本明細書中に記載されるFGF-1化合物は、代謝障害、例えば2型糖尿病、インスリン非感受性、耐糖能異常、代謝症候群、脂肪肝疾患、肥満およびそれらに関連する状態を処置するのに使用することができる。肥満への適用に関連して、FGF-1化合物はまた、個体において体脂肪の比率を減少および/または除脂肪量の比率を増加させるのに使用することもできる。FGF-1化合物を用いた処置から利益を享受することができる代謝障害に関連する状態には、高血圧(高血圧症)、心血管疾患、高血糖症、高尿酸血症および多嚢胞性卵巣症候群が含まれる。

本発明は、その必要のある対象において代謝障害を処置、予防および/または緩和する方法を提供する。処置過程は、対象の個々の特徴に基づき個体単位で最適となるよう決定される。処置は、対象に対して毎日、1日に2回、2日に1回、隔週、毎週、毎月または治療上有効な任意の適用可能な基準で実施することができる。処置は、単独でまたは少なくとも1つの他の治療剤、例えば同じ代謝障害または関連症候群を標的にする治療剤と組み合わせて実施することができる。追加の薬剤は、FGF-1化合物と同時に、異なる時点で、または全く異なる治療計画の中で投与することができる(例えば、FGF-1化合物を毎日投与し、追加の薬剤を毎週投与することができる)。
材料および方法
動物。使用した動物は、FGF-1-/-(Miller et al., 2000)、PPARγf/f/aP2-Creマウス(He et al., 2003)および>99% C57/B6の遺伝的背景の野生型同腹子対照であった。
核ホルモン受容体(NHR)の標的を同定するために、本発明者らは、全49のマウスNHRと経路特異的プロモーター・レポーターライブラリの大集団がコンビナトリアルなペアになっている検証済のcDNA発現ライブラリを含む「プロモーターオントロジー」スクリーンを使用した。このペアは、与えられた状況下での任意のNHRによる各遺伝的経路の転写調節の迅速な評価を容易にする。この高スループットのプロモータースクリーンを使用して、本発明者らは、NHRによる調節についてFGFファミリーのメンバーのプロモーターコンストラクトをスクリーンし、FGF-1をPPARγの直接的標的として同定した。より詳細には、本発明者らは、PPARγによるFGF-1の強力かつ特異的な転写調節を同定した。
次に、本発明者らは、FGF-1ノックアウト(KO)マウスを用いて、インビボでのFGF-1消失の帰結を決定した。FGF-1 KOマウスを、創傷の治癒および心血管系の変化に関して調査した。これらのマウスもFGF-1/FGF2二重KOマウスもどちらも、正常な給餌条件下では有意な表現型を示さなかった(Miller et al., 2000)。PPARγを介するFGF-1の調節の役割を研究するため、FGF-1 KOおよび野生型同腹子に高脂肪食(HFD)を与えた。HFD誘導性の体重増加については差が観察されなかったが(図4A)、FGF-1 KOマウスはより小さなWATおよびより大きな脂肪変性性の肝臓を有しており、このことは、FGF-1 KOマウスはそれらの脂肪組織量を増加させることに失敗し、代わりに脂肪を肝臓に移動させることを示唆している(図4B、C)。同時に、FGF-1 KOマウスは、野生型同腹子と比較して、ブドウ糖およびインスリンの空腹時レベルの増加ならびにインスリン抵抗性の増進を示すことが、それぞれ、ブドウ糖負荷およびインスリン負荷試験(GTT、ITT)により実証された(表1および2、図4D-F)。正常な膵島の形態、組織学およびブドウ糖刺激に対するインスリン分泌により示されたように、膵臓の機能については明らかな異常は観察されなかった。しかし、膵臓あたりの膵島の数は、わずかに増加していた(図4H)。
FGFは、FGFR-1から-4で示される4つの同種の高親和性チロシンキナーゼ受容体を通じてシグナルを伝達し、それによりMAPK(ERK1/2)およびPI3K/AKT経路を含む複数のシグナル伝達経路の下流活性化をもたらす。これらの経路は、インスリンに応答してグリコーゲン合成を調節するグリコーゲンシンターゼキナーゼ3(GSK-3)の活性化およびブドウ糖輸送担体GLUT4の転位を含む、インスリン/ブドウ糖シグナル伝達経路の要素の調節を行う(Cho et al., 2001)。これらのシグナル伝達経路の完全性(integrity)を調査するため、本発明者らは、HFD給餌FGF-1 KOおよび野生型マウスのWAT、BAT、肝臓および筋肉内のその重要な成分の発現を決定した(図5)。興味深いことに、本発明者らは、HFD処置FGF-1 KOマウスのWATにおけるAKT(およびそれよりも程度は低いがGSK3β)の全体レベルが、WTマウスと比較して減少していることを見出した。対照的に、肝臓、BATまたは筋肉におけるAKTレベルは正常であり、4つすべての組織におけるERK1/2レベルも正常であった。
FGF-1は、3T3-L1脂肪細胞においてGLUT1の発現を誘導し、ロシグリタゾンと相乗的に作用する。FGF-1は、マウス3T3-L1脂肪細胞において、長期的処置の後にブドウ糖輸送担体1(Glut1)の発現を誘導し(図6)、ob/obマウスにおける食後血糖値を低下させる。この結果は、FGF-1が糖尿病および肥満の処置のための治療法として使用できることを示している。
8週齢のオスob/obマウスを、組換えマウスFGF-1(0.5 mg/kg/日、s.c.、250μl中)、ロシグリタゾン(TZD、5 mg/kg/日、p.o.、300μl中)またはビヒクル対照(皮下ビヒクル対照0.9 % NaCl、250μl/マウス;経口ビヒクル対照0.5 % CMC、300μl/マウス)で処置した。第3日および第6日の処置1時間後に標準的なプロトコルを用いて血糖を測定した(図7A)。
FGF-1の血糖レベルに対する急性的効果を検証するため、用量応答曲線(図8)ならびに皮下(図9)および静脈内(図10)投与後の経時変化を調査した。結果は、FGF-1がob/obマウスのブドウ糖レベルの劇的な用量依存的低下をもたらすことを示している。皮下投薬は数時間規模で効果的であり、ブドウ糖レベルの有意な低下は少なくとも2日間持続する(図9)。図10は、静脈内投与がブドウ糖レベルに対してより長時間持続する効果を示し、0.2 mg/kg体重の用量が少なくとも1週間の間の血糖の有意な低下をもたらすことを示している。
ob/obマウスにおけるFGF-1の長期処置の代謝的効果を調査するため、8週齢のオスob/obマウスを36日の期間、ビヒクルまたは組換えマウスFGF-1(0.5 mg/kg/3日、s.c.)で処置した。この間、ブドウ糖レベル、食物摂取および身体組成をモニターした。図11は、ブドウ糖レベルが試験した第1時点(第2日)までに正常化され、残りの試験期間の間これが安定的に維持されることを示している。
図12は、FGF-1の投与が最初にob/obマウスの食物摂取を減少させることを示している。食物摂取は約2週間以内に正常通りに戻るが、図13に示されるように、FGF-1処置ob/obマウスの体重は未処置ob/obマウスのそれよりも低い状態を維持する。この図13に示される体重減少は、FGF-1が迅速かつ持続的な体重減少を達成するのに使用できることを示している。
図16は、FGF-1投与(0.5 mg/kg/3日、s.c.)の4週間後に実施したブドウ糖負荷試験の結果を示している。FGF-1処置ob/obマウスは、未処置マウスよりも効率的にブドウ糖を減少させた。FGF-1処置マウスはまた、インスリン感受性の増進を示すことが、ITTにおけるブドウ糖のより迅速なクリアランスにより示された(図17)。血清脂質レベル(トリグリセリド、遊離脂肪酸およびコレステロール)は、2つのグループ間で類似していた(図18)。これらの試験は、上記の通りに行った。
36日の処置期間の後の肝臓組織の分析は、FGF-1処置ob/obマウスの肝臓がそれらの未処置対応動物よりもずっと健康的であることを明らかにした。図19は、未処置(A)および処置(B)マウスのH & E染色組織を示している。未処置の肝臓は、有意な脂肪変性(脂肪蓄積および損傷)を示しているのに対して、FGF-1処置マウス由来の肝臓は、脂肪変性の程度がそれよりずっと低いこと、およびあるとしても、炎症がほとんどないことを示している。さらに、肝グリコーゲンレベルは、FGF-1処置マウスにおいてずっと高く、このことは、適切なブドウ糖処理およびインスリン応答を示している(図20)。
FGF-1の効果が投与経路に依存するかどうかを決定するため、本発明者らは、皮下、腹腔内および静脈内送達した0.5 mg/kg体重のFGF-1に応じたob/obマウスの血糖レベルを試験した。対照としてPBS注射を使用した。図22は、FGF-1の急性的効果が3つすべての注射方法でほぼ同じであることを示している。本発明者らは次に、ブドウ糖正常化効果の持続期間について静脈内および皮下注射を比較した。図23に示されるように、静脈内投与したFGF-1は、試験期間の、少なくとも60時間の間、安定的なブドウ糖レベルを与えた。図10のデータは、静脈内注射の効果がずっと長い持続性(少なくとも1週間)を有することを示している。
ob/obモデルは、非常に重度の糖尿病疾患を表すものとみなされている。それよりも重症度の低い糖尿病/代謝障害モデルに対するFGF-1の効果を調査するため、本発明者らは、db/dbマウスおよび食事誘導性の肥満マウスにおける血糖レベルを試験した。図24および25は、0.5 mg/kgのFGF-1の皮下投与が両方の系において血糖レベルを低下させるのに効果的であったことを示している。このデータは、FGF-1が異なる原因から発生したブドウ糖レベルを正常化し異なる原因から発生した代謝障害を処置するのに使用できることを示している。
図26は、皮下投与された同一用量のhrFGF-1がob/obマウスにおいてブドウ糖レベルを効果的に低下させることができることを示している。ヒト組換えFGF-1はすでに臨床的に使用されているので、それを使用して代謝障害を処置する本発明の方法は、処置に至るまでの規制上の道程が短くてすむ。
上述の通り、FGFファミリーの因子は、FGFRファミリーの受容体のメンバーに対して異なる特異性で結合する。FGF-1は、FGFR1およびFGFR4に優先的に結合し、これらの受容体を発現する細胞中へと内在化することができる。他のFGFタンパク質がFGF-1と類似の代謝的効果を有するかどうかを決定するために、本発明者らは、FGF-2、FGF-9およびFGF-10で処置(0.5 mg/kg s.c.)したob/obマウスにおける血糖を試験した。このFGFタンパク質の組み合わせは、多種のFGFRに結合する。図27に示される結果は、観察された代謝的効果にはFGF-1の固有の受容体結合およびシグナル伝達特性が必要であることを実証している。
Claims (20)
- FGF-1化合物を含む、個体の代謝障害を処置するための薬学的組成物。
- FGF-1化合物が、FGF-1の機能性フラグメントである、請求項1記載の組成物。
- FGF-1化合物が、FGF-1の機能性アナログである、請求項1または2記載の組成物。
- 静脈内投与用に製剤化されている、前記請求項のいずれか一項記載の組成物。
- 皮下投与用に製剤化されている、請求項1〜3のいずれか一項記載の組成物。
- 代謝障害が、血糖上昇、耐糖能障害、インスリン抵抗性、II型糖尿病、肥満、体脂肪率上昇、および脂肪肝疾患から選択される、前記請求項のいずれか一項記載の組成物。
- 組成物中のFGF-1化合物の用量が、体重1kgあたり0.01〜1 mgのFGF-1に相当する、前記請求項のいずれか一項記載の組成物。
- 個体の代謝障害を処置する方法であって、個体にFGF-1化合物を投与し、それによって代謝障害を処置する工程を含む、方法。
- 代謝障害が、血糖上昇、耐糖能障害、インスリン抵抗性、II型糖尿病、肥満、体脂肪率上昇、および脂肪肝疾患から選択される、請求項8記載の方法。
