Description
本出願は、2009年11月30日出願の欧州特許出願第09075536.4号及び2009年11月30日出願の米国仮出願第61/283,154号の優先権をパリ条約に基づき主張する。
A)ウイルスが0.7以上の初生ひな(ニワトリ)の脳内病原性指数(ICPI)を有する。
B)複数の塩基性アミノ酸、F2タンパク質のC−末端及びF1タンパク質のN−末端である残基117のフェニルアラニンがウイルス中に(直接または演繹的に)確認されている。用語「複数の塩基性アミノ酸」とは、残基113と残基116との間の少なくとも3つのアルギニンまたはリジン残基を意味する。上述のようなアミノ酸残基の特徴的パターンを確認できない場合には、クローン化されたウイルスをICPI試験により特徴付けすることが必要となることがある。この定義では、アミノ酸残基は、F0遺伝子のヌクレオチド配列から演繹されたアミノ酸配列のN−末端から番号付けされ、113〜116は開裂部位から残基−4から残基−1に対応する。

当業者であれば、本明細書により、読む者の便宜のために本節にまとめた以下の本発明の目的は容易に確認できる。ただし、以下の列挙は本明細書に含まれる目的を限定するものではない。
1)インターフェロン非感受性であり、かつ1.2〜2.0のICPIを有するニューカッスル病ウイルスクローン。
2)インターフェロン非感受性であって、1.2〜1.5のICPIを有する目的1に記載のニューカッスル病ウイルスクローン。
3)配列番号1のDNAヌクレオチド配列を有する目的1に記載のニューカッスル病ウイルスクローン。
4)欧州細胞カルチャーコレクション(ECACC)に受入番号06112101として2006年11月21日に寄託された、目的1に記載のニューカッスル病ウイルスクローン。
5)腫瘍細胞死を誘導することによる癌の治療用薬剤の製造、または発生期あるいは残留癌細胞を破壊または転移巣の発達のリスクを低減する予防的生物学的癌治療剤のための目的1〜4の少なくともいずれかに記載のニューカッスル病ウイルスクローンの使用。
6)ヒト腫瘍細胞がp53陰性ヒト腫瘍細胞である、目的5に記載の使用。
7)ヒト腫瘍細胞が子宮頸部癌、卵巣癌、膀胱癌、腎癌、ウィルムス腫瘍、前立腺癌、肺癌(気管支癌を含む)、リンパ腫、白血病、中枢神経腫瘍(髄膜腫、髄芽腫、神経膠芽腫、星状細胞種、神経芽細胞腫を含む)、膵癌、皮膚癌(黒色腫を含む)、大腸癌、骨癌(一次及び転移性病巣)、乳癌、胃癌、食道癌、甲状腺癌、肉腫、中皮腫、頭頸部癌(口咽頭、鼻咽頭、副甲状腺を含む)、血液系腫瘍、外陰部、膣及び子宮内膜癌、睾丸癌、肛門直腸癌、肝及び肝外(胆管)癌、肉腫(ユーイング肉腫を含む)、眼癌(網膜芽細胞腫を含む)、胸腺癌、尿道癌、カルチノイド腫瘍及び副腎皮質癌から選択される、目的5に記載の使用。
8)投与経路が、静脈内、動脈内、腸内、腸管外、髄腔内、腹腔内、胸内、胸腔内、口腔内、舌下、頬粘膜、鼻腔内、嚢内、尿道内、直腸内、膣皮下、腫瘍内、腫瘍周囲、局所、筋肉内、気管支内、動脈内、頭蓋内及び/または外用適用である、目的5に記載の使用。
9)患者に投与する1回量が1×105〜1×1012ウイルス粒子、好ましくは1×106〜1×1010ウイルス粒子、最も好ましくは1×107〜1×109ウイルス粒子を含む、目的5に記載の使用。
10)1日に3回から1ヶ月に1回の頻度で、1×105〜1×1012ウイルス粒子相当またはその複数倍、好ましくは1×106〜1×1010ウイルス粒子相当またはその複数倍、最も好ましくは1×107〜1×109ウイルス粒子相当またはその複数倍を患者に投与する、目的5に記載の使用。
11)ウイルス療法を化学療法、放射線療法(α、βまたはγ線照射、X線照射、粒子照射など)、免疫治療または外科手術と組み合わせる、目的5に記載の使用。
12)ウイルス療法を化学療法、放射線療法(α、βまたはγ線照射、X線照射、粒子照射など)、免疫治療または外科手術の前、同時または後に行う、目的11に記載の使用。
13)インターフェロン感受性腫瘍性疾患、特に黒色腫、(非ホジキン)リンパ腫、白血病(慢性骨髄性白血病、有毛細胞白血病)、乳癌、膀胱癌、腎細胞癌、頭頸部癌、カルチノイド腫瘍、胆管癌、膵癌、多発性骨髄腫若しくはカポジ肉腫、または多発性硬化症、尖圭コンジローマ、肝炎、ヘルペス、リウマチ性関節炎、ベーチェット病、特発性肺疾患、アフタ性口内炎、重症悪性骨粗しょう症、頸部癌若しくはSARSなどの非腫瘍性インターフェロン感受性自己免疫、またはウイルス性状態の治療のための薬剤の製造における、目的1〜4の少なくともいずれかに記載のニューカッスル病ウイルスクローンの使用。
14)ニューカッスル病ウイルスを癌患者に投与することを有する、癌患者の痛みを低減する方法。
15)目的1〜4の少なくともいずれかに記載のニューカッスル病ウイルスクローンを癌患者に投与することを有する、目的14に記載の癌患者の痛みを低減する方法。
16)化学療法剤の投与の前、同時または投与の後にニューカッスル病ウイルスを癌患者に投与することを有する、化学療法剤による治療を受けた癌患者の化学療法による副作用を低減する方法。
17)化学療法剤の投与の前、同時または投与の後に、目的1〜4の少なくともいずれかに記載のニューカッスル病ウイルスクローンを癌患者に投与することを有する、目的16に記載の化学療法剤による治療を受けた癌患者の化学療法による副作用を低減する方法。
18)副作用が吐き気、嘔吐、脱毛、倦怠感、食欲不振、腸の疾患、食欲不振と体重変化から選択される、目的16または17に記載の化学療法剤による治療を受けた癌患者の化学療法による副作用を低減する方法。
19)放射線療法の前、同時または放射線療法の後に、ニューカッスル病ウイルスを癌患者に投与することを有する、放射線療法を受けた癌患者の放射線壊死を低減する方法。
20)放射線療法の前、同時または放射線療法の後に、目的1〜4の少なくともいずれかに記載のニューカッスル病ウイルスクローンを癌患者に投与することを有する、放射線療法を受けた癌患者の放射線壊死を低減する方法。
21)放射線療法の前、同時または放射線療法の後に、目的1〜4の少なくともいずれかに記載のニューカッスル病ウイルスクローンを癌患者に投与することを有する、放射線療法を受けた癌患者の放射線療法による急性または慢性続発症を含む副作用を低減する方法。
22)放射線療法による副作用が、倦怠感、食欲不振、皮膚の変化(または照射臓器に応じて他の急性症状)、下痢、失禁、排尿時痛、頻尿、嚥下困難、口腔乾燥、圧痛、潰瘍形成、咳、息切れ、咽頭痛、嗄声、脳組織または脊髄の放射線壊死、軟部組織壊死、骨放射線壊死、皮下線維症、委縮症、毛細血管拡張、慢性壊死、非吸収性潰瘍形成、狭窄形成、唾液形成を著しい低下、口腔内乾燥症、局所脊髄症、軟骨壊死、食道の潰瘍または狭窄形成、線維症、肺炎及び腸または直腸の慢性的炎症から選択される、目的21に記載の放射線療法を受けた癌患者の放射線療法による副作用を低減する方法。
23)ニューカッスル病ウイルスを癌患者に投与することを有する、激しい食欲不振、エネルギー損失、うつ病、無力症、吐き気または倦怠感を改善することによる癌患者の生活の質を向上する方法。
24)目的1〜4の少なくともいずれかに記載のニューカッスル病ウイルスクローンを癌患者に投与することを有する、目的23に記載の癌患者の生活の質を向上する方法。
25)ニューカッスル病ウイルスを癌患者に投与することを有する、進行癌患者の姑息的治療の方法。
26)目的1〜4の少なくともいずれかに記載のニューカッスル病ウイルスクローンを癌患者に投与することを有する、進行癌患者の姑息的治療の方法。
27)目的1〜4の少なくともいずれかに記載のニューカッスル病ウイルスクローンを有効成分として生理学的に許容される添加物と共に有する、癌の治療のための医薬組成物。
28)目的1〜4の少なくともいずれかに記載のニューカッスル病ウイルスクローンを有効成分として生理学的に許容される添加物と共に凍結乾燥状態で有する、癌の治療のための医薬組成物。
29)目的27または28に記載の医薬組成物及び化学療法剤を含有する医薬組成物を有する、目的5に記載の癌の治療のための薬剤キット。

31)I.精製純系ウイルスクローンを生成する(例えば、複数回のプラーク精製)、
II.該純系クローンを特定の病原体を含まない(SPF)ニワトリの卵へ播種する、
III.該SPF卵を培養する、
IV.該SPF卵を冷却する、
V.該SPF卵から尿膜液を採取する、
VI.必要に応じて濾過及び/または遠心分離により、該尿膜液から破砕物を除去する、
VII.該尿膜液を超遠心分離する、
ステップを有する、目的1〜4に記載のNDVウイルスクローンの作製方法。

33)精製及び凍結乾燥されたニューカッスル病ウイルス。
34)ニューカッスル病ウイルスが
a)インターフェロン非感受性であり、かつ1.2〜2.0のICPI、好ましくは1.2〜1.5のICPIを有する、または
b)配列番号1のDNAヌクレオチド配列を有する、または
c)欧州細胞カルチャーコレクション(ECACC)に受入番号06112101として2006年11月21日に寄託されたウイルスと同一のものである、目的33に記載のニューカッスル病ウイルス。
ゲノムの5箇所の重複部分を増幅した。3つの配列特異的プライマーをRTに使用し、ゲノムの94%を包含する内部領域の増幅のための3つのプライマー対により5箇所の重複部分を増幅した。
新規クローンMTH−68H/VBは、融合(F)タンパク質遺伝子、F2タンパク質のC−末端のタンパク質配列及びF1タンパク質のN−末端のヌクレオチド及び予測されるアミノ酸配列により特徴付けられる。アミノ酸残基は、F0遺伝子のヌクレオチド配列から演繹されるアミノ酸配列のN−末端から番号付けされ、113〜116は開裂部位から残基−4から残基−1に対応する。
本刊行物の著者らは、1968年には、ニューカッスル病ウイルス株が大きなプラークを作成する能力は、ニワトリでの毒性に関連していると信じていた。彼らは、ニワトリ胚線維芽細胞単層中で標準条件下におけるプラークサイズを比較した。短潜伏期性(高毒性)株は明らかに大きなプラークを形成した。亜病原性(中間毒性)株は小さなプラークのみを作成し、長潜伏期性(低毒性)株ではプラーク形成はほとんど見られなかった。
MTH−68H/VB株の親株は、著しく不均質な(1940年代に)英国に由来する「古代」NDV家禽類ワクチンであった。生成物の均質性を高め、不完全な粒子を除去するために、プラーク精製工程を複数回繰り返した。プラーク精製は、プラークの大きさ、形、外観に代表される所望の特徴を有する、純系ウイルスクローンを得るために当業者が通常使用する方法である(例えば、Massaabら,Plotkin及びMortimer編,Vaccines,Philadelphia:WB Saunders社,1994,78−801)。最初の部分精製で外見上均質なウイルス集団が得られる。CEFでの繁殖の間、「精製された」ウイルス系統により形成されたプラークはなお高い変動性を示した(図3)。ニワトリ線維芽細胞単層組織培養により繁殖したこのウイルス集団の、なお様々なプラークのいくつかから所望の特徴を有する1つのプラークを選択、分離し、さらに増殖させる。後日、この均質なウイルス集団を再度CEF内で増殖し、形及び外観に基づいて単一のプラークをさらに選択(第3の単離)するプラーク精製を再び行い、SPFニワトリ胚内で増殖する。厳格な品質コントロールの後、得られたウイルス懸濁液をMTH−68H/VBの新規マスターシードを作製するために使用した。
ニューカッスル病ウイルスは、脳内病原性指数(ICPI)により特徴付けられる異なる3つの病原性に分類される。
インターフェロン(IFN)は、ウイルス、寄生菌、腫瘍細胞などの攻撃に対する反応として、ほとんどの脊椎動物の免疫系細胞により作製される自然細胞シグナリングタンパク質である。インターフェロンは、ウイルス感染の主要な指標である二本鎖RNAの存在に反応して、多様な細胞により作製される。
主なNDV株を含むウイルスの多くがインターフェロンβ(IFNβ)に感受性である。IFNβは、感受性細胞でのインターフェロン感受性NDVの増殖を阻害する。IFN濃度の上昇に従って、インターフェロン感受性NDV株の細胞毒性効果が低減することが期待できる。
実験A.HEK293細胞を0日目に蒔き、20、200または2000U/mLのIFNβで0、1、2及び3日目に処理した。MTH−68H/VB感染を1日目に0.001または0.01のMOIで行い、細胞毒性を4日目に測定した。
実験B.実験条件は実験Aと同様であるが、細胞の感染は0.01または0.1のMOIで行った。
実験C.F11細胞を0日目に蒔き、2、10または20μg/mLの抗IFNβで0、1、2及び3日目に処理した。MTH−68H/VB感染を1日目に0.001または0.01のMOIで行い、細胞毒性を4日目に測定した。
実験D.実験条件は実験Cと同様であるが、細胞の感染は1または10のMOIで行った。抗IFNβの最終濃度は、2及び15μg/mLであった。
・IFNβは細胞増殖に影響しなかった(図6及び7)。
・IFNβはMTH−68H/VBの細胞毒性効果を阻害しない。
MTH−68H/VBの細胞毒性に対するIFNβの効果はHEK293細胞により検討し、二重に行った(実験A及びB)。両検討において、MTH−68H/VBは低MOI(1未満のMOI)でHEK293細胞に対して細胞毒性を示し、MTH−68H/VBはこの細胞系で複製を作製できることが示された(図8及び9、縦棒1−3)。結論として、本発明者らは、IFNβは、非常に高濃度のIFNβであっても、MTH−68H/VBの細胞毒性効果を阻害しないことを証明した(図9、縦棒4−12)。
次に、本発明者らは、F11一次ヒト線維芽細胞におけるMTH−68H/VBの細胞毒性効果に対するIFNβに対する抗体の効果を分析した。抗IFNβは2つの独立した実験の両者でF11細胞の増殖に影響を与えなかった(実験C及びD、図10及び11)。MTH−68H/VBの2つの異なる系統を使用して、本発明者らは、線維芽細胞に対する細胞毒性効果を検出できなかった(図12及び13、縦棒1−3)。低及び高濃度の抗IFNβ処理のいずれにおいても、MTH−68H/VBの細胞毒性効果を高めなかった(図12、縦棒4−12及び図13、縦棒4−9)。
8.1.異なる細胞系におけるMTH−68/H−VBの複製
8.1.1.MTH−68H/VBの細胞毒性効果を異なる一次及び樹立細胞系において20%胎児牛血清を使用し、インビトロ条件下で検討した。
本検討では、以下の細胞系を使用した。
・293N3S:アデノウイルス5型で形質転換したヒト胎児腎、浮遊培養での成長に適する(ATCCから購入)
・HeLa:子宮頸部腫瘍から樹立したヒト上皮細胞系(ATCCから購入)
・9L:樹立されたラット神経膠腫細胞系(ECACCから購入)
・H−一次ヒト線維芽細胞系(皮膚生検から樹立)
・V−一次ヒト線維芽細胞系(皮膚生検から樹立)
・A−一次ヒト線維芽細胞系(皮膚生検から樹立)
ウイルス複製の可能性をさらに検討するために、以下の細胞系を使用した。
・293:アデノウイルス5型で形質転換したヒト胎児腎(ATCCから購入)
・HeLa:子宮頸部腫瘍から樹立したヒト上皮細胞系(ATCCから購入)
・H−一次ヒト線維芽細胞系(皮膚生検から樹立)
・961107:NINSで樹立された一次ヒト線維性髄膜腫細胞
・960612:NINSで樹立された一次ヒト線維性髄膜腫細胞
・980128/2:NINSで樹立された一次ヒト多形膠芽腫細胞
この比較検討では、異なるNDV株の細胞毒性とMTH−68H/VBの細胞毒性とを比較した。本検討では、NDV株は以下のものである。
・H/W:Hertfordshire株のWeybridge系
・Mukteswar(Veterinarski Zavodからの「亜病原性」株、Subotica)
・LaSota:標準家禽類ワクチン株
・VP:「無毒性」標準ワクチン株
・293:アデノウイルス5型で形質転換したヒト胎児腎(ATCCから購入)
・HeLa:子宮頸部腫瘍から樹立したヒト上皮細胞系(ATCCから購入)
・H−一次ヒト線維芽細胞系(皮膚生検から樹立)
これらの実験では、10%胎児牛血清を細胞培養培地に加えた。この血清濃度は、一次線維芽細胞を除く全ての細胞系に最適であるが、一次ヒト線維芽細胞もこの培地中で成長することができる。10%の血清の存在では、MTH−68H/VBは、非常に高いMOI(10及び100、図16)以外は、線維芽細胞に対する毒性はわずかであった。
これらの実験でも、10%胎児牛血清を細胞培養培地に加えた。この血清濃度は全ての細胞系に最適である。10%の血清の存在では、MTH−68H/VBは、非常に低いMOIでも(図11)、HeLa細胞に対する毒性が高かった。MTH−68H/VBの細胞毒性効果は、特に低MOI(0.01及び0.1)でいずれの他の検討ウイルスよりも著しく高かった。
前記実験と同様に、10%の血清の存在では、MTH−68H/VBは、非常に低いMOIでも(図12)、293細胞に対する毒性が高かった。MTH−68H/VBの細胞毒性効果は、特に低MOI(0.01及び0.1)でいずれの他の検討ウイルスよりも著しく高かった。

5×103個の細胞を最終容量100μLで96ウェル培養皿に蒔き、その1日後に、異なる感染多重度(MOI:100/1,10/1,1/1,1/10及び1/100)でMTH−68H/VBにより形質転換した。72時間後にロッシュのWST−1キットにより細胞毒性効果を評価した。
他の癌治療方法は既に使い尽くされた癌進行期患者を単独で治療することができる治療剤として、ウイルス療法の腫瘍溶解性活性を観察すると同時に、化学療法または放射線療法との有効な組み合わせでのMTH−68H/VBの使用効果を、実験的にさらに調査した。本発明者らの実験では、患者治療の実際の経過を正確に描写することを試み、脳腫瘍を有する患者などの場合、具体的には神経膠腫をモデルとして使用して、実際の投与についての目安を得た。外科治療後の現在の標準的治療方法では、まず放射線療法で治療した後、適切な化学療法、すなわちBCNUまたはテモゾロミドが投与される。
検討の目的はMTH−68H/VB単独での、または他の治療方法との組み合わせによるインビボでの抗腫瘍効果を検討することである。MTH−68H/VB用量及び投与スケジュールの影響も調査した。

治療は2週間行った。
麻酔したマウスの担癌右肢を4GyのX線で照射した(THX−250治療用X線源、Medicor、ブダペスト、ハンガリー、線量率:1.003Gy/分)。鉛管で他の身体部分を遮蔽し、照射から保護した。
第1の実験では、やや大きな腫瘍が2つの異なる用量のMTH−68H/VB(1×107及び1×105ウイルス粒子/注射)を週1回または毎日腫瘍内注射することにより治療された(それぞれ図23及び24)。
MTH−68H/VB治療と局部腫瘍照射の組み合わせ効果の分析では、照射のみでは少数の動物の腫瘍成長のみを排除したと結論した。興味深いことに、MTH−68H/VB腫瘍内注射と局部腫瘍照射の組み合わせは全ての腫瘍の成長を排除した(図25及び26)。
神経膠腫は、通常外科的に治療され、その後放射線照射と化学療法を行うことが多い。使用される化学療法剤の中で、BCNU(ビス−クロロニトロソウレア、カルムスチン)に取って代わるテモゾロミド(テモダール)の投与は治療の標準的方法になりつつある。本研究の目的は、テモゾロミド化学療法などの化学療法や放射線療法にMTH−68H/VBウイルス療法を組み合わせた場合の抗腫瘍効果の検討であった。
インビトロで成長するGI261細胞を採取し、PBSで2回洗浄後、少量のPBSに懸濁した(1−2×107個/mL)。1−2×106個のGI261細胞を最終容量100μLのPBSにてC57BI/6雌マウスの右肢に移植して、皮下腫瘍を樹立した。GI261マウス神経膠腫細胞系を上述のようにダルベッコ変法イーグル最小必須培地(DME)中で培養した(T.Szatmari,K.Lumniczky,S.Desaknai,S.Trajcevski,EJ.Hidvegi,H.Hamada,G.Safrany.神経膠芽腫治療のためのマウス神経膠腫261腫瘍モデルの詳細な特徴付け,Cancer Science,97,546−553,2006)。
腫瘍の治療は、腫瘍細胞の移植後7日目に開始した。下記の方法を様々な組み合わせで適用した。
1.局所腫瘍にMTH−68H/VB(最終容量50μLで1×107ウイルス粒子/注射)の注射を毎日2週間(計10回注射、5回注射/週)。併用プロトコルでは、放射線照射及び/またはテモダール(100mgテモゾロミド/カプセル、シェーリング社、Kenilworth、NJ 07033)治療後直ちにMTH−68H/VB注射を行った。カプセルは滅菌ブレードで開け、中身を2.5mLのジメチルスルホキシドに音波処理により懸濁させた。音波処理後、均質な懸濁液の最終容量をPBSで20mLに調整し、最終濃度が5mg/mLのテモダールを得た。この溶液を4℃で3〜5日間保存した。
2.腹腔(ip)テモダール注射(100mgテモダール/体重1kg)連続3日間。
3.放射線照射療法を連続3日間行った。麻酔下のマウスの担癌右肢を2GyのX線で照射した(THX−250治療用X線源、Medicor、ブダペスト、ハンガリー、線量率:1.003Gy/分)。放射線照射をテモダール治療と併用した場合には、照射は化学療法の1時間後に行った。

第1の実験では、小さな腫瘍を治療した。PBSの腫瘍内注射による偽の治療では、非治療担癌マウスと同様な腫瘍成長が見られた(図25)。
腫瘍体積が小さい場合には、MTH−68H/VB注射液の腫瘍内注射により腫瘍の進行が遅くなった。
本研究では、BCNUの抗腫瘍効果に対する、MTH−68H/VB治療及び/または放射線照射の効果について検討した。BCNU(有効成分:カルムスチン、一般名:BCNU、BiCNU、カルムスチン、分類:アルキル化剤、ニトロソウレア)。BCNUは長年にわたって一次脳腫瘍の単剤化学療法剤として使用され、多形膠芽腫に対する標準的な化学療法において30年以上にわたって重要な役割を担ってきた。
1.局所腫瘍にMTH−68H/VB(最終容量50μLで1×107ウイルス粒子/注射)の注射を毎日2週間(計10回注射、5回注射/週)。併用プロトコルでは、放射線照射及び/またはBCNU治療後直ちにMTH−68H/VB注射を行った。
2.腹腔内(ip)BCNU治療。
3.放射線照射療法を連続3日間行った。麻酔下のマウスの担癌右肢を2GyのX線で照射した。腫瘍放射線照射をBCNU治療と併用した場合には、照射は化学療法の1時間後に行った。

本発明者らのデータによれば、MTH−68H/VB処理は、局所腫瘍放射性照射及び化学療法と組み合わせることにより効果的となる。放射線照射療法と組み合わせてMTH−68H/VBを使用することの大きな利点は、ウイルス療法としてのMTH68/H/VBは、長期間にわたって投与されても、非毒性であることである。
本発明に記載のNDVクローンを腫瘍性疾患の治療に使用してもよい。治療は、以下のように定義される。
1.腫瘍の退縮。退縮は、理学的検査または公知の造影法により客観的に測定することができる、占拠性病変を含む腫瘍の大きさの減少だけでなく、その他、限定的ではない例として、白血病などの血液疾患における悪性細胞の減少や、転移性疾患の兆候の減少、と定義することができる。
2.MTH−68/HVBウイルスクローンと他の癌治療法との同時適用。他の癌治療法として、それぞれの抗腫瘍効果を相乗的に増強するために、化学療法や放射線照射が挙げられるが、これらに限定されない。
3.MTH−68/H−VBウイルスクローンと他の負の副作用を減らすための癌関連方法との同時適用。副作用としては、吐き気、嘔吐、脱毛、倦怠感、食欲不振、放射線壊死などが挙げられるが、これらに限定されない。
4.癌による痛みの軽減。痛みとして、特に転移性疾患または占拠性病変による痛みが挙げられるが、これらに限定されない。痛みの軽減により、コデインやモルフィネなどの鎮痛薬の必要量を下げることができる。
5.従来の治療方法を全て受けた末期癌患者の治療。ウイルス療法により、腫瘍の退縮が二次的に期待でき、それによる延命が期待できる場合がある。
6.腫瘍退縮の直接的証拠が得られない場合においても、癌関連症状を軽減することにより癌患者または末期癌患者の生活の質を改良する。癌関連症状は、例えば、倦怠感、痛み、食欲不振(極端な場合、悪液質として現れる)、気力減退、幸福感の減退、性欲減退、と定義してもよい。
I.精製純系ウイルスクローンを生成する(例えば、複数回のプラーク精製)。
II.該純系クローンを特定の病原体を含まない(SPF)ニワトリの卵へ播種する。
III.該SPF卵を培養する。
IV.該SPF卵を冷却する。
V.該SPF卵から尿膜液を採取する。
VI.必要に応じて濾過及び/または遠心分離により、該尿膜液から破砕物を除去する。
VII.該尿膜液を超遠心分離する。
VIII.製剤して各容器に詰める。
IX.完成した産物を凍結真空乾燥及び凍結乾燥を行う。
X.品質管理試験、クローンの中和及び細胞系、動物モデル、PCRの使用を含む方法により可能である。
MTH68H/VBマスターシードの調製
・−70℃の冷凍庫から再構成したマスターシードの一定量を取り出し、融解する。融解したウイルス懸濁液をさらに希釈し、各卵が1.0mL当たり103.0EID50、好ましくは105.0EID50の力価の、少なくとも0.1mLの希釈ウイルス懸濁液で接種されたことを確認する(EID50は本技術分野で公知の方法に従って決定する)。
・9〜11日齢、好ましくは10日齢のSPFニワトリ胚の尿膜腔に0.1mLの希釈ウイルスシードを常法に従って接種する。
・接種の24時間後に卵に光を当てて調べ、死んだ胚を廃棄する。
・接種の4日後に、胚をインキュベーターから取り出し、少なくとも2時間、好ましくは一晩冷やす。
・尿膜液を卵から滅菌容器、好ましくは遠心分離可能な滅菌容器に回収する。
・滅菌性を検査した後、ウイルス懸濁液を集め、遠心分離により予備精製する。
・超遠心分離により濃縮とさらに精製を行う。
・滅菌容器に一定量を調製することにより、1回の凍結乾燥に適切な最大量を調製できる。
・次の工程まで−70℃で保存する。
・次の工程は、好ましくは1.0mL当たり109.2EID50の力価及び保護剤を有する精製ウイルス懸濁液からなる凍結乾燥バルクの作製である。
・保護剤と混合したウイルスを凍結乾燥する。
3年前に第IV期乳癌と診断された年配の女性患者は、その後の数年にわたって局部放射線療法とともに数サイクルの複数の化学療法を受け、脊椎、大腿骨、骨盤に見られた転移病巣を含む広範囲に広がった有痛性の骨への転移による痛みに対する経口鎮痛剤が処方されていた。突然、臨床症状が悪化し、食欲不振、体重減少、衰弱により寝たきりとなり、起き上がれず、咳がおさまらず、重篤な呼吸困難により継続的酸素補給が必要となった。患者は転移性肺障害と診断され、いずれの腫瘍学的治療ももはや効かないと判断された。患者はホスピスケアを勧められ、延命の僅かな期待をもって酸素とモルヒネを処方した。患者にMTH−68/H/VB治療を開始した。毎日6回まで、1バイアルに108ウイルス粒子を含有するバイアルを1回に1バイアル鼻腔用スプレーを使用して点鼻した。ウイルス療法開始1週間で、咳が少なくなり、最終的には完全に寛解した。患者は、起き上がって、1回の投与分108ウイルス粒子の投与を吸入器を使って断続的に受けることができるまでに一度は回復した。最終的には、介護者の助けを借りて歩行できるまでになり、酸素補給も必要なくなった。食欲が回復し、喜んで食事をし、体重も増えた。転移による骨の痛みに対する鎮痛剤の処方量も少なくなった。1ヶ月後、患者はX線を再度開始し、肺の転移巣が消失していることが示された。2ヶ月後、担当医は、追加の化学療法を再開することを検討し、低用量の経口化学療法治療を開始した。上述のウイルス療法は108ウイルス粒子の投与を1日2〜3回に減らして点鼻または噴霧器を使用して継続した。患者は食欲もよく、吐き気や倦怠感もなかった。歩行も可能で、理学療法も再開し、生活の質も向上した。転移骨病巣による痛みや不快感を感じることもなくなり、全ての鎮痛剤の投与を取りやめた。骨のスキャンでは骨の転移病巣は消えていなかったが、進行を示す兆候は認められなかった。患者は1年後、脳卒中で突然亡くなった。患者はクマジン(進行転移条件により凝固亢進のリスクが高まる)を4年前に第IV期と最初に診断された時から継続して服用していたが、最近その予防的用量の投与を止めていた。
患者は膀胱癌と診断された中年男性である。病理により、筋肉壁を侵している高悪性度移行上皮癌であることが確認され、重篤なリンパ節転移も認められた。患者は第IV期と診断された。手術後、患者は組み合わせ化学療法の積極的な治療方式を受けたが、危険なほどに低WBCを含む重篤な副作用のため、化学療法の全行程を行うことはできなかった。数ヶ月後、画像診断により、新たに手術不可能な骨盤腔内腫瘤が発見された。患者は余命2年以下と告げられた。患者は化学療法プロトコルによる緩和療法を希望した。化学療法開始後、患者は同時に、1日1回108ウイルス粒子を静脈内投与するMTH−68/H/VB補助ウイルス療法を開始した。患者は直ぐに化学療法による副作法の全てにおいて目覚ましい低減を認め、化学療法治療の間中、セイリングや乗馬を含む以前の活発なライフスタイルを再び行うことができた。患者は、処方された化学療法治療方式を、不快感を最少に抑えながら、完了することができた。その完了後、CTによる追跡調査では、骨盤腔内腫瘤の大きさが小さくなっていることが認められ、1年後にはほとんど転移性疾患の兆候は見られなかった。患者はMTH−68/H/VBウイルス療法を中止した。1年以上の後、患者は再び骨盤腔内腫瘤がCTにより発見され、その時に化学療法のコースと上述と同様のMTH−68/H/VBウイルス療法を補助として再開した。患者は化学療法による副作用を経験することはなかった。化学療法治療の完了後、患者はMTH−68/H/VBウイルス療法を1日当たり107.4ウイルス粒子の舌下投与に切り替えて続け、無症状のままである。
口腔の扁平上皮癌と診断された中年女性患者である。局所の発症のみで転移巣はないと判断され、第二期と診断された。患者は切除術を受け、化学療法は受けなかったが、手術後に集中的な放射線照射治療を受けた。放射線照射治療後に、味覚異常、潰瘍形成、痛み及び出血があり、粘膜炎の症状により、おしゃべりが影響を受け、飲食、経口剤の投与に影響が出たため、ストローの使用のみとなり、体重減が甚だしかった。患者の症状は6ヶ月以上続き、軽減されなかった。一過性急性症状であることを期待していた症状は、放射線壊死による慢性的な障害であることが明らかになった。患者はまた、抜歯が激しく美観が大きく損なわれた。患者は、1日2回、各108.7MTH−68/H/VBウイルス粒子をうがい薬として局所適用による口腔治療と、1日1回の108.7MTH−68/H/VBウイルス粒子の鼻腔内スプレーによる投与を開始した。毎日の治療の1週間後、患者の症状が緩和し始め、1ヶ月後には、不快感を感じることはなくなり、出血、痛みもなく、経口栄養の摂取が可能になった。粘膜は治癒したが、患者は不可逆的粘膜損傷により、治療後の歯科インプラントはまだ考えられなかったが、痛みや不快感は感じなくなった。
大きな脳腫瘤を有する12歳男子である。減量手術により腫瘍を広範囲にわたって取り除いた。病理診断は多形神経膠芽腫であった。患者は強力な焦点放射線照射治療単位全部を受けた。患者の容態は安定し、治療を終了した。9ヶ月後、患者は持続性頭痛を訴え、MRIにより腫瘍の再発が認められた。集中的な化学療法を開始したが、治療中に腫瘍がどんどんと大きくなった。患者は臨床的に急速に衰弱し続け、その後の5ヶ月で末梢的な衰弱の増加及び失語症とともに、寝たきりとなった。患者の余命は短いと告げられ、全ての治療を中止したが、大きくなった脳浮腫の姑息的治療としてステロイドの静脈内注射だけを行った。MRIの再調査によって、活動性腫瘍との判断に対して、同様なイメージが放射線壊死においても見られ、その区別が難しいので、イメージは実際には慢性放射線壊死を反映している可能性があると考えられた。頭蓋骨の骨構造の接近している領域での放射線壊死の広がりと空間占有特性のために、脳腫瘍の壊死性炎後の放射線壊死は致死的なことがあるが、患者の臨床症状及び病変の位置のために、生検することができなかった。患者は毎日3回まで107.4MTH−68/H/VBウイルス粒子の静脈内投与を開始した。患者の臨床症状が改善され、「腫瘍」が治療の6ヶ月で小さくなり始め、1年後には完全に消失した。患者は、噴霧器を使って週に1回、用量108.0ウイルス粒子のウイルス療法の維持スケジュールを続けた。GBMに対して、放射線壊死の存在を証明することはできないが、患者の劇的な、急速な改善及び表面上の回復により、癌専門医は最初のGBM診断に疑問を感じ、症状を放射線壊死の反映であると真剣に考えている。
扁桃周囲瘤を有する少年である。生検によりB細胞非ホジキンリンパ腫と診断された。患者に転移病巣はなく、第I−II期と判断された。患者は、4ヶ月の積極的な多剤化学療法治療を受けるとともに、局部的な放射線照射治療を受けた。退院の3週間後に新たな唾液腺腫瘍が現れ、中咽頭瘤が再発した。この時点で医師は患者の家族に長期生存の可能性は低いことを告げた。患者は積極的な多剤静脈内投与による化学療法の複数サイクルを開始し、同時に、毎日1回108ウイルス粒子を噴霧器により吸入するMTH−68/H/VBウイルス療法を化学療法治療とともに1年間にわたって行った。多剤及び積極的な高用量化学療法治療を数回の入院時に行った。注目すべきは、小児病棟の同じ宿命を有する他の子供とは正反対に、通常の吐き気、食欲不振、倦怠感を引き起こすことなく処方された積極的な化学療法剤治療を受けられたことである。患者の複数回の入院を通して、身体的な活発さを維持し、非常に陽気で、食欲もあった。治療は成功し、患者は回復段階のままであった。
Claims (16)
- インターフェロン非感受性であって、1.2〜2.0のICPIを有するニューカッスル病ウイルスクローン。
- インターフェロン非感受性であって、1.2〜1.5のICPIを有する請求項1に記載のニューカッスル病ウイルスクローン。
- 配列番号1のDNAヌクレオチド配列を有する請求項1に記載のニューカッスル病ウイルスクローン。
- 欧州細胞カルチャーコレクション(ECACC)に受入番号06112101として2006年11月21日に寄託された、請求項1に記載のニューカッスル病ウイルスクローン。
- 腫瘍細胞死を誘導することによる癌の治療用薬剤の製造、または発生期あるいは残留癌細胞を破壊または転移巣の発達のリスクを低減する予防的生物学的癌治療のための、請求項1〜4の少なくともいずれか一項に記載のニューカッスル病ウイルスクローンの使用。
- ウイルス療法を化学療法、放射線療法(α、βまたはγ線照射、X線照射、粒子照射など)、免疫治療または外科手術と組み合わせる、請求項5に記載の使用。
- インターフェロン感受性腫瘍性疾患、特に黒色腫、(非ホジキン)リンパ腫、白血病(慢性骨髄性白血病、有毛細胞白血病)、乳癌、膀胱癌、腎細胞癌、頭頸部癌、カルチノイド腫瘍、胆管癌、膵癌、多発性骨髄腫若しくはカポジ肉腫、ならびに多発性硬化症、尖圭コンジローマ、肝炎、ヘルペス、リウマチ性関節炎、ベーチェット病、特発性肺疾患、アフタ性口内炎、重症悪性骨粗しょう症、頸部癌若しくはSARSなどの非腫瘍性インターフェロン感受性自己免疫およびウイルス性状態の治療のための薬剤の製造における、請求項1〜4の少なくともいずれか一項に記載のニューカッスル病ウイルスクローンの使用。
- 請求項1〜4の少なくともいずれか一項に記載のニューカッスル病ウイルスクローンを癌患者に投与することを有する、癌患者の痛みを低減する方法。
- 化学療法剤の投与の前、同時または投与の後に、請求項1〜4の少なくともいずれか一項に記載のニューカッスル病ウイルスクローンを癌患者に投与することを有する、化学療法剤による治療を受けた癌患者の化学療法による副作用を低減する方法。
- 放射線療法の前、同時または放射線療法の後に、請求項1〜4の少なくともいずれか一項に記載のニューカッスル病ウイルスクローンを癌患者に投与することを有する、放射線療法を受けた癌患者の放射線壊死を低減する方法。
- 放射線療法の前、同時または放射線療法の後に、請求項1〜4の少なくともいずれか一項に記載のニューカッスル病ウイルスクローンを癌患者に投与することを有する、放射線療法を受けた癌患者の放射線療法による急性または慢性続発症を含む副作用を低減する方法。
- 請求項1〜4の少なくともいずれか一項に記載のニューカッスル病ウイルスクローンを癌患者に投与することを有する、癌患者の生活の質を向上する方法。
- 請求項1〜4の少なくともいずれか一項に記載のニューカッスル病ウイルスクローンを有効成分として生理学的に許容される添加物と共に有する、癌の治療のための医薬組成物。
- I.精製純系ウイルスクローンを生成する(例えば、複数回のプラーク精製)、
II.該純系クローンを特定の病原体を含まない(SPF)ニワトリの卵へ播種する、
III.該SPF卵を培養する、
IV.該SPF卵を冷却する、
V.該SPF卵から尿膜液を採取する、
VI.必要に応じて濾過及び/または遠心分離により、該尿膜液から破砕物を除去する、
VII.該尿膜液を超遠心分離する、
VIII.製剤して各容器に詰める、
IX.完成した産物を凍結真空乾燥及び凍結乾燥を行う、
ステップを有する、請求項1〜4のいずれか一項に記載のNDVウイルスクローンの作製方法。 - 精製及び凍結乾燥されたニューカッスル病ウイルス。
- 腫瘍細胞死を誘導することによる癌治療のための薬剤、または
発生期または残留癌細胞を破壊し、または転移巣の発達のリスクを低減する予防的生物学的癌治療剤の
の製造における、1.4以上のICPIを有するニューカッスル病ウイルスの使用。
