Abstract
Description
本出願は、米国特許法第119条に基づき、2007年3月29日に出願された米国仮特許出願第60/920,561号、2007年9月24日に出願された米国仮特許出願第60/994,732号、及び2007年10月2日に出願された米国仮特許出願第60/997,099号の優先権を主張するものであり、これらの開示内容は参照により本明細書に組み込まれる。
本発明は、概して、1つの管腔から別の管腔への再入、例えば、これに限定されるわけではないが、血管の管腔外又は内膜下空間から血管の真腔への再入のためのデバイス及び方法に関する。
経皮管腔外再疎通時にガイドワイヤーの操作によって真腔に再入できない場合、真腔再入(true lumen reentry)デバイスを用いねばならない。現在、市場には2つの特殊設計された再入デバイスがある。
本発明は、例えば、限定されるわけではないが、PTA(経皮経腔血管形成術)、PTCA(経皮経腔冠動脈形成術)及び血管、動脈、軟組織若しくは他の任意のヒト組織の2つの隣接層間におけるカテーテルの他の任意の経皮的若しくは非経皮的留置における真腔再入のような、多くの適用を有する。
Claims (17)
- 遠位端に配置されたバルーン(11)を含むカテーテル(10)を含むデバイス(1)であって、前記カテーテル(10)が、ガイドワイヤー(27)を通過させるための前記バルーン(11)を貫く第一の管腔(12)と、前記バルーン(11)の膨張及び収縮のための第二の管腔(13)とで形成され、前記カテーテル(10)及び前記バルーン(11)が、身体管腔(8)の壁部の2層(3、4)間への進入のためのサイズ及び形状であり、前記層(3、4)間に配置されたときの前記バルーン(11)の拡張と同時に、前記バルーン(11)を通過する前記第一の管腔(12)の遠位部(26)が、前記バルーン(11)の近位にある前記第一の管腔(12)の部分に対して屈曲し、前記身体管腔(8)方向に向けられる、デバイス(1)。
- 前記バルーン(11)が反転遠位ネック部(11D)を含む、請求項1に記載のデバイス(1)。
- 前記第一の管腔(12)を通過し、且つ前記身体管腔(8)に進入するために少なくとも1つの層(3、4)を穿孔するように適合されたガイドワイヤー(27)を更に含む、請求項1に記載のデバイス(1)。
- 前記第一の管腔(12)に配置されたハイポチューブ針(14)と、前記針(14)の内部空洞(16)を通過するガイドワイヤー(27)とを更に含み、前記針(14)の遠位端(15)が、前記身体管腔(8)に進入するために少なくとも1つの層(3、4)を穿孔するように適合された、請求項1に記載のデバイス(1)。
- 前記バルーン(11)の直径が前記身体管腔(8)の直径を超えない、請求項1に記載のデバイス(1)。
- 前記バルーン(11)の長さが前記バルーン(11)の直径を超えない、請求項1に記載のデバイス(1)。
- 前記カテーテル(10)が、前記バルーン(11)が配置された遠位部(26)の近位にある旋回部(18)により形成される、請求項1に記載のデバイス(1)。
- 前記旋回部(18)が、前記カテーテル(10)の他の部分より薄い壁厚を有する脆弱部を含む、請求項7に記載のデバイス(1)。
- 前記旋回部(18)が、可撓性材料で作られている部分を含む、請求項7に記載のデバイス(1)。
- 前記旋回部(18)が継ぎ手を含む、請求項7に記載のデバイス(1)。
- 前記旋回部(18)が、前記カテーテル(10)の他の部分より柔軟で屈曲させるのがより容易な部分を含む、請求項7に記載のデバイス(1)。
- 前記カテーテル(10)がシース(17)に摺動的に配置され、前記カテーテル(10)の遠位部(16)が、前記シース(17)の遠位端との接合部において屈曲可能である、請求項1に記載のデバイス(1)。
- 前記バルーン(11)の直径が、前記身体管腔(8)の直径に等しく、又は前記身体管腔(8)の直径よりわずかに大きい、請求項1に記載のデバイス(1)。
- 前記バルーン(11)の近位にある前記カテーテル(32)に装着され、且つ前記カテーテル(32)において形成された第三の管腔(42)を通じて膨張可能な第二のバルーン(40)を更に含む、請求項1に記載のデバイス(30)。
- 遠位端に配置されたバルーンを含み、且つガイドワイヤーを通過させるための前記バルーンを貫く第一の管腔と、前記バルーンの膨張及び収縮のための第二の管腔とで形成されたカテーテルを、身体管腔の壁部の2層間に導入するステップと、
前記バルーンを通過する前記第一の管腔の遠位部が、前記バルーンの近位にある前記第一の管腔の部分に対して屈曲し、前記身体管腔方向に向けられるように前記バルーンを膨張させるステップと
を含む、方法。 - ガイドワイヤーを前記第一の管腔内に通すステップと、前記身体管腔に進入するために前記層の1つを穿孔するステップとを更に含む、請求項15に記載の方法。
- 前記カテーテルが、前記身体管腔における閉塞部に隣接する前記身体管腔の壁部の2層間を通過し、まず、前記ガイドワイヤー上の前記カテーテルを前記閉塞部の近位にある前記身体管腔内に導入するステップと、前記閉塞部の近位端の近傍にて前記ガイドワイヤーにより前記身体管腔の前記壁部を切開するステップと、前記ガイドワイヤーを前記第一の管腔内に通すステップと、前記閉塞部の遠位にある前記身体管腔に再入するために前記層の1つを穿孔するステップとを更に含む、請求項16に記載の方法。
