Description

本発明のさらなる特徴と利点、および本発明の各種実施形態の構成について、図面を参照し以下に詳細を説明する。
本発明の各種態様は、リモートプレコンディショニングを行う、安全で信頼性のある方法を提供するシステムに関する。本システムは、医療専門家が定めた治療プロトコルを実施可能であり、医療専門家による管理を最小限にまたは不要にしたものである。本システムの実施形態は、被験者を保護する機能と、治療プロトコルへの準拠をモニタする機能とを含む。
学研究を支援したりすることが挙げられるが、これらに限られない。

図に示す実施形態において、カフは一端にラチェット機構を備えたバンドとして構成される。バンドは使用の際、自身の自由端がラチェット機構に通されて被験者の肢に巻かれる。このような実施形態において、アクチュエータは、ラチェット機構を介してバンドの自由端をさらに引っ張ると肢周囲でカフを引き戻す機構か、あるいは、バンドを緩めるようにラチェット機構を開放して肢からカフを解除する機構を備えてもよい。本発明の態様はこれに限らず、止血帯機構など別の機構も可能である。

Claims (22)
- リモートプレコンディショニングを行うシステムであって、
被験者の肢周囲で収縮するように構成されたカフと、
前記カフに接続され、作動時には前記カフを被験者の肢周囲で収縮させて肢の血流を減少させるアクチュエータと、
複数の治療周期を含む治療プロトコルに従って前記アクチュエータを制御するコントローラとを備え、各治療周期が、
前記アクチュエータが、肢の血流を閉塞するよう被験者の肢周囲の前記カフを収縮期圧より高い圧力になるまで収縮させる、カフ作動と、
前記アクチュエータが、肢の血流を閉塞するよう肢周囲のカフを収縮期圧よりも高い設定値に維持し、、かつ少なくとも5秒間継続する虚血期間と、
前記アクチュエータが、肢の血流を許容するよう前記カフを緩める圧力解除と、
肢の血流を許容するよう前記カフを緩んだ状態で肢周囲に保持し、かつ少なくとも一分間以上継続する再潅流期間とを含む、システム。 - 前記治療プロトコルは、収縮期圧の識別をさらに含むことを特徴とする請求項1に記載のシステム。
- 収縮期圧の識別は、最初の治療周期の前に行われることを特徴とする請求項2に記載のシステム。
- 前記収縮期圧の識別は、複数の治療周期それぞれで行われることを特徴とする請求項2に記載のシステム。
- 前記虚血期間は、カフによって与えられる圧力を前記設定値で維持するアクチュエータによるさらなる作動を含むことを特徴とする請求項1に記載のシステム。
- 前記設定値は被験者の収縮期圧よりも高く、約10mmHgであることを特徴とする請求項5に記載のシステム。
- 虚血期の設定値は被験者の収縮期圧よりも高く、約20mmHgであることを特徴とする請求項5に記載のシステム。
- 前記設定値は被験者の収縮期圧よりも高く、約30mmHgであることを特徴とする請求項5に記載のシステム。
- 肢の再潅流を識別する変換器をさらに備え、
前記コントローラは虚血期に変換器から信号を受信し、再潅流が起こっていないことを確認することを特徴とする請求項1に記載のシステム。 - 前記コントローラは再潅流期に変換器から信号を受信し、肢の再潅流を確認することを特徴とする請求項9に記載のシステム。
- 前記変換器は、光プレチスモグラフ変換器、超音波流量変換器、温度変換器、赤外線検知器、近赤外線変換器のうちから選択されることを特徴とする請求項9に記載のシステム。
- 安全基準が満たされたときに肢の血流を許容するように構成された安全解除部をさらに備えたことを特徴とする請求項1に記載のシステム。
- 前記安全基準は、システムの電源異常、システムの出力スパイク、手動解除スイッチの起動のうちから選択される一つまたは複数の基準を含むことを特徴とする請求項12に記載のシステム。
- 前記治療プロトコルは、再プログラム可能に構成されることを特徴とする請求項1に記載のシステム。
- 被験者が利用可能であり、被験者が治療プロトコルの開始および終了を制御可能なユーザインターフェースをさらに備えた請求項1に記載のシステム。
- 前記コントローラは、被験者による治療プロトコルの変更を防ぐ複数の保護手段を含むことを特徴とする請求項14に記載のシステム。
- 被験者によるシステムの使用について記録するデータロギング手段をさらに含むことを特徴とする請求項1に記載のシステム。
- 治療周期は、治療プロトコル中に5回繰り返されることを特徴とする請求項1に記載のシステム。
- 前記虚血期間は、治療プロトコルの最初の治療周期において1分未満であり、
前記虚血期間は、前記治療プロトコル中の次の治療周期において1分を超えることを特徴とする請求項1に記載のシステム。 - 前記カフは、空気圧膨張式のカフであり、
