(式中、mは患者の体重、cは患者の身体の比熱、(ΔT/Δt)は患者の体内温度の変化の時間速度、(ΔHc/Δt)はカテーテルから患者への熱送出の時間速度である)。75kgの体重と4186ジュール/℃−kgの比熱を有する患者を推定すると(人体の比熱と水の比熱が同じであると想定すると、実際の値は幾分異なる)、1時間(3600秒)当たり1℃の加熱速度では、約87ワット(1ワット=1ジュール/秒)の速度で患者に熱を伝達するカテーテルが必要となるであろう。しかし、この推定は、本発明のカテーテルと一緒に用いられる電源の望ましいサイズについての推定値としては低すぎるであろう。これは多くの理由で予測可能である。先ず、便宜上、患者の体内系が環境に対して失われた量に等しい熱量を生成すると推定した。低体温症患者の場合、これは明らかに該当しない。殆ど自明のこととして、偶発症候性低体温症は、ヒトの体内での熱生成能力が環境に対して失われる熱に圧倒されたときに発生する。カテーテルはこの差を埋め合わせなければならず、したがって、要求される電力レベルはその理由だけでも大きい必要がある。あるいは、低体温症を誘発させるためには、標的組織の温度を低下させるのに十分な熱を血液から除去するか、または全身低体温症の場合には、身体が生成する熱より多い熱を除去する必要があろう。熱を下げる場合、熱交換器の冷却に要する電力は、装置から環境への過剰な熱の放散を含めた冷却装置の効率に大いに依存する。

- 哺乳動物患者の解剖学的構造内に挿入可能なカテーテル装置であって、前記解剖学的構造を通って体液は患者の標的領域まで流れることが可能であり、前記カテーテル装置は標的領域の温度を変更させるために、体液を用いてその場での熱交換を果たすように機能し、前記カテーテル装置は、
基端および先端を備えた細長い可撓性カテーテルと、前記可撓性カテーテルの全長はその基端と先端との距離として定義されることと、
前記熱交換流体がその内部を循環可能な少なくとも1つの流体管腔と、
前記カテーテル上の第1位置に配置される熱交換器と、前記熱交換器は、前記熱交換器と熱交換すべく近接させた状態で流れる体液と前記熱交換器内を通って循環する熱交換流体との間で、熱を交換するように機能することと、前記第1位置は前記カテーテルの全長よりも短く延びることと、

から成る治療装置の群から選択される請求項17に記載のシステム。 - 前記体液流入口には弁が設けられ、前記弁は開放状態と閉鎖状態との間で作動可能であり、前記開放状態では体液の流れが前記体液流入口を通って前記体液誘導スリーブの中に入り、前記閉鎖状態では体液の流れが前記体液流入口を通って前記体液誘導スリーブから外へ出るのを防止する請求項1に記載のカテーテル装置。
- 前記カテーテルは先端シャフト部分を有し、前記先端シャフト部分は前記体液誘導スリーブよりも先端側で前記体液誘導スリーブ内部から延び、前記先端シャフト部分はその内部を通る中央管腔を備え、前記中央管腔は前記流れスペースと第1位置で流体の行き来が可能に連通し、前記通路と連通し、前記標的領域と第2位置で流体の行き来が可能に連通する請求項1に記載のカテーテル装置。
- 前記体液誘導スリーブは前記第1位置よりも先端側の前記先端シャフト周囲で封鎖される請求項20に記載のカテーテル装置。
- その場での熱交換を果たすために、哺乳動物患者の身体の流体含有部分に配置されるカテーテル装置であって、前記カテーテル装置は、
基端および先端を備えた細長い可撓性カテーテルと、前記可撓性カテーテルの全長はその基端と先端との距離として定義されることと、
前記可撓性カテーテルは患者に挿入される挿入部を備え、前記挿入部は前記先端から前記基端より短い地点まで延びることと、
カテーテル上の第1位置に配置される熱交換器と、前記熱交換器は、前記熱交換器と熱交換すべく近接させた状態にある体液と前記熱交換器内を通って循環する熱交換流体との間で、熱を交換するように機能することと、前記第1位置は前記カテーテルの全長よりも短く延び、前記熱交換器は湾曲され、その曲率の外径に沿った外表面を有すると共にその曲率の内径に沿った内表面を有し、前記外表面および前記内表面は実質的に異なる熱透過率を有することと、
を備えたカテーテル装置。 - 前記上側表面は前記下側表面よりも熱透過率が実質的に大きい請求項22に記載のカテーテル装置。
- 頭部に流れてくる血液が前記上側表面と熱を伝えるべく近接した状態で通過するように、ヒト患者の大動脈の湾曲に沿って配置される大きさおよび形状に形成される請求項23に記載のカテーテル装置。
- 患者の温度に制御可能に作用するシステムであって、前記システムは、
基端及び先端を備えた細長い可撓性カテーテルと、熱交換器と、を有するカテーテル装置と、前記熱交換器は、同熱交換器と熱交換すべく近接させた状態にある体液と同熱交換器内を通って循環する熱交換流体との間で、熱を交換するように機能することと、
患者からのデータを検知する複数のセンサと、前記センサの少なくとも一つからの前記データは体温を含むことと、
前記センサの少なくとも一つからのデータに応答し、目標とする体温を達成及び維持するために、前記センサからの信号を受け取り、前記カテーテル装置の運転を制御する制御装置と、
を備えたシステム。 - 熱交換ユニットをさらに備え、前記熱交換ユニットは前記熱交換流体と熱を交換するように機能する請求項25に記載のシステム。
- 前記熱交換ユニットは固体熱電冷却器からなる請求項26に記載のシステム。
- 前記目標パラメーターが温度であり、前記センサが温度センサであり、かつ前記制御装置が前記熱交換ユニットを作動させるように機能し得る請求項26に記載のシステム。
- 複数のカテーテル装置をさらに備え、前記制御装置が前記カテーテル装置の各々を制御するように機能し得る請求項25に記載のシステム。
- 少なくとも1つのカテーテル装置が第1位置において前記体液に熱を与え、少なくとも1つのカテーテル装置が第2位置において前記体液から熱を奪う請求項29に記載のシステム。
- 患者の温度に制御可能に作用するシステムであって、前記システムは、
各々が基端および先端を備えた細長い可撓性カテーテルと、前記カテーテル上の第1位置に配置された熱交換器と、を有する複数のカテーテル装置と、前記可撓性カテーテルの全長はその基端と先端との距離として定義され、前記可撓性カテーテルは患者の体内に挿入される挿入部を有し、前記挿入部は前記先端から前記基端より短い地点まで延び、前記熱交換器は、前記熱交換器の表面から延びる複数の熱交換フィンを有し、前記熱交換フィンは熱交換を高めるための増大された表面積を有し、前記熱交換器は、前記熱交換器と熱交換すべく近接させた状態にある体液と前記熱交換器内を通って循環する熱交換流体との間で、熱を交換するように機能することと、前記第1位置は前記カテーテルの全長よりも短く延びることと、
各々が患者からのデータを検知し、そのデータに応答して信号を生じる、複数のセンサと、
オペレーターが目標パラメーターを特定し得る手動入力装置と、
前記センサからの信号および前記目標パラメーターを受け取り、前記信号に応答し、かつ前記目標パラメーターに関連して前記カテーテル装置の各々の運転を制御する制御装置と、
を備えたシステム。 - 熱交換ユニットをさらに備え、前記熱交換ユニットが前記熱交換流体と熱を交換するように機能する請求項31に記載のシステム。
- 前記熱交換ユニットが固体熱電冷却器からなる請求項32に記載のシステム。
- 前記目標パラメーターが温度であり、前記センサが温度センサからなり、かつ前記制御装置が前記熱交換ユニットを作動させるように機能し得る請求項32に記載のシステム。
- 少なくとも1つのカテーテル装置が第1位置において前記体液に熱を与え、少なくとも1つのカテーテル装置が第2位置において前記体液から熱を奪う請求項31に記載のシステム。
